4.The influence of clinicopathologic factors and operative method on the prognosis of gallbladder carcinoma

Objective To analyze the clinicopathologic factors related to the prognosis of gallbladder carcinoma.Methods A retrospective analysis was conducted on 61 cases of advanced gallbladder carcinoma who were operated on from 2001 to 2006 in Nanjing First Hospital.Results According to the 6th UICC/AJCC TNM classification and staging of gallbladder carcinoma,of the 61 patients,8 patients were stage IB,22 were IIA,11 were IIB,14 were III and 6 were IV.Palliative cholecystectomy and/or bile duct drainage procedures were performed in 24 patients;removal of gallbladder plus regional lymph nodes and fatty tissue dissections were performed in 14 cases;radical resection was performed in 7 patients;removal of gallbladder and part of liver in combination with resection of bile duct(radical operatian) was performed in 13 patients;1 patient had extensive radical operation including pancreaticoduodenectomy.In 37 patients,18 had positive lymph node metastasis.Fifty-two patients were followed up,41(67.2%) patients were alive at 6 months after operation,26(42.6%)were alive at 12 months,10(16.4%) were alive at 24 months,and 7 cases were alive at 36 months,1 patient is still alive 5 years after operation;51 patients died within 2 years after operation.The symptoms of recurrence in 31 patients included progressing jaundice(21 cases),metastasis in liver(14 cases),and refractory ascites(12 cases).Five patients died of non-bile duct disease.Conclusions The prognosis of gallbladder cancer has a close relationship with the clinical stage and method of resection.
6.Effects of Alcohol Extract of Herba Epimedii on the Release of NO from Human Umbilical Vein Endothelial Cells in Vitro

To investigate the mechanism of Herba Epimedii alcoho l extract for imp otence, Giess reagent was used to observe the effect of the alcohol extract of H erba Epimedii in mice and its effect on release of NO fro m human umbilical vein endothelial cells in vitro was also studied. The results showed that alcoh ol extract of Herba Epimdii had no effect on the release of NO. But the serum co llected from the mice after the medication of alcohol extract of Herba Epimedii for 120min and 180min promoted the production of NO, and the reaction reached th e peak at 120 min. It is indicated that the promotion of NO release from endothe lial cells may be one of the mechanisms of Herba Epimedii for impotence.
7.Review of the results of different operative procedures for old acromioclavicular dislocation

Objective To compare the results of different operative methods for old acromioclavicular dislocation. Methods Forty three patients with old acromioclavicular dislocation were treated with three different operative methods: 1)open reduction plus Kirschner wire fixation (9 cases); 2)open reduction plus reconstruction of acromioclavicular ligament (14 cases) and 3)open reduction plus modified Dewar s operation (20 cases). After surgery, the subjective symptoms of the arm, muscular contraction power, function of the shoulder joint and space of acromioclavicular joint in 43 cases were followed up for an average 4.8 years. Results The excellent results following three operative methods were seen in 33% , 50% and 74% respectively. There was no significant difference between Kirschner wire fixation and reconstruction of acromioclavicular ligament (P > 0.05). Modified Dewar's operation was better compared with Kirschner wire fixation and reconstruction of acromioclavicular ligament (P< 0.05). Conclusion A satisfactory surgical procedure for treatment of old acromioclavicular dislocation should accomplish the following points: removal of scar tissue and intra articular cartilaginous fragments, reconstruction of joint stability and effective internal fixation until complete healing of the ligament structures.
8.Performance of registration regulation policies on biosimilars in Japan and its enlightenment

Objective:This paper intends to provide reference for consummation of biosimilar registration poli-cies in China. Methods:This paper evaluates the performance of Japanese registration regulation policies on biosimi-lars by analyzing the procedure and reviewing the features of biosimilars. Results and Conclusions:According to the findings of the present investigation, the availability and affordability of biosimilars have been improved, and the bio-medicine industry has achieved great development. On the other hand, it's recommended that the guideline for the de-velopment and evaluation of biosimilars should be amended on the principle of appropriate simplification, which means to simplify the research and development procedure and exempt partial application dossier. Besides, the regis-tration procedures should be adopted continuously to ensure the stability of biosimilars' R&D and registration.
9.Caudatin combined with Gefitinib reversing HGF induced non-small cell lung cancer to EGFR-TKI acquired drug resistance

Objective To investigate the effect and underlying mechanism of Caudatin combined with Gefitinib on Gefitinib resistance induced by HGF in PC-9.Methods Model of EGFR-TKIs resistance in PC-9 cells was induced by exogenous HGF and co-cultured with MRC-5.Caudatin was tested as a drug resistant modulator to reverse the resistance of Gefitinib in PC-9 cells induced by HGF by MTT assay.Western blot was performed to observe the mechanism of Caudatin combined with Gefitinib reversing the resistance of PC-9 induced by HGF.Results The resistance of gefitinib to PC-9 was induced by exogenous HGF and co-cultured with MRC-5 which could reduce relative inhibitory rate ( P<0.05 ) .Neither caudatin ( 0-32 μM ) or Gefitinib (1μM) alone could significantly inhibit proliferation of PC-9 in the presence of HGF, which could be inhibited in a dose-dependent manner by Caudatin combined with Gefitinib ( P<0.05 ); Caudatin combined with Gefitinib down-regulated the phosphorylation levels of Met and PI3K/Akt simultaneously (P<0.05).Conclusion Caudatin could reverse the drug resistance of Gefitinib in PC-9 induced by HGF, the mechanism of which may be related to the inhibition of Met/PI3K/AKT pathway.
10.Percutaneous Transhepatic Insertion of Metalic Stent and Arterial Chemoemblization for Malignent Biliary Obstruction

Objective:To vealuate the effect of percutaneous transhepatic insertion of biliary setents(PTIBS) and arterial chemoenbolization for treatment of malignent biliary obstruction(MBO).Methods:Twelve patients with MBO were treated with PTIBS,following by arterial chemoenbolization in two weeks,one time each months.Results:Fourteen stents were placed in 12 cases.The total serum bilirubin level declined from 534?120 umol/L to 45?23 umol/L in 11 patients.The survival rate of 6 and 12 monthes were 83.3% and 50% respectively.Two cases were reobsturcted after 5 months of first stent implacemnt and were retreated by insertion of second stents.The total thirty-five times arterial chemoembolization was performed in 12 cases with remarkable effect.Conclusion:PTIBS combined arterial chemoembolization was an safe ;effective interventional treatment for MBO. [
