1.Technology of breast cancer screening:current situation and prospect

Breast cancer is the first malignant tumor of women, and the incidence is still rising. Although the overall survival rate of breast cancer is high, the difference in prognosis is significant with different stage. Finding the patients at early stage and commencement of therapy timely by breast cancer screening is the key to reduce the incidence and improve the survival rate. At present, clinical breast examination, ultrasonography, mammography and MRI are still the most common methods for breast cancer screening, but in view of the specific technology and specific population it remains controversial as to which techniques are the best, especially the value of ultrasonography still needs to be confirmed in randomized clinical trials. With the constant improvement of technologies and programs, the breast cancer screening work will be more standardized and optimized.
2.Suppression of Delayed Type Hypersensitivity Response to DNP_6OVA in Mouse by UV --Irradiated Urocanic Acid

In order to investigate the effects of cis-UCA on cell -mediated immunity, we established a murine model of DTH response to DNP6OVA which is indicated by the increasing in the ear thickness of the mouse after resensitization, compared with those of the mouse injected with 200 g cis - UCA and trans -UCA intraperitoneally (i. p. ). The average of the increasing in ear thickness of each group was 9. 1 1. 13 10-2 mm (n=8, the number of ears) for the group administrated with cis-UCA; 17. 45 0. 88 10 - 2 mm (n= 8) for the group administrated with trans-UCA ; 16. 43 + 1. 83 10 - 2 mm(n= 7) for control group. It is suggested that there are immunosuppression of DTH response to DNP6OVA in mouse administrated with ets-UCA and no effecs of trans-UCA on this response.
3.Influence of RNA interference-induced repression of p53 expression on senescence in human skin fibroblasts

Objective To establish a cell line with repressed expression of p53 by transfecting a plasmid construct expressing short hairpin RNA(shRNA)targeting p53 into human skin fibroblasts(HSFs),and to evaluate the effect of repression of p53 expression on the senescence in HSFs.Methods The eukaryotic expressing plasmid pGCsi-p53 containing shRNA targeting p53 gene was transfected into HSFs with lipofectamine.Subsequently,the cells were selected by G418,and resistant cell clones were chosen and expanded.Reverse transcription-PCR and real time fluorescence-based quanitative PCR were performed to determine the expression of p53 gene,and Western blot to detect the expression of p53 protein in HSFs.The senescence in HSFs was evaluated by SA β-gal staining,and cell proliferation by methyl thiazolyl tetrazolium(MTT)assay.Results A HSF clone with repressed expression of p53 was established successfully.The expressions of p53 mRNA and protein were downregulated in transfected HSFs compared with untransfected HSFs(0.09 ± 0.03 vs.0.32 ± 0.04,0.11 ± 0.04 vs.0.84 ± 0.05,both P < 0.01).The percentage of senescent cells was 13.47% ± 1.01% in the transfected HSFs,significantly lower than that in untransfected HSFs(18.10% ± 0.66%,P < 0.05).As MTT assay showed,the proliferation was accelerated in transfected HSFs compared with untransfected HSFs(P < 0.05).Conclusions The repression of p53 expression decelerates the senescence in HSFs,but promotes the proliferation of HSFs.
4.Clinical significance of the determination of serum cortisol and adrenocorticotropic hormone for sepsis in children

Objective To dynamic monitoring the changes of serum cortisol,adrenocorticotropic hormone (ACTH) in children with sepsis and servere sepsis,and to assess it's relationship with clinical features.Methods Thirty children with sepsis were enrolled in this study from Sep 2010 to Jan 2011 in our hospital,and were divided into severe sepsis group (n =10) and sepsis group (n =20).The serum cortisol and ACTH were analysed in acute and recovery period respectively in both two groups,and compared with 10 non-sepsis children as control group.Results In the acute period,the level of serum cortisol in severe sepsis group [ (33.97 ±6.83) μg/ml ] was significantly higher than those in sepsis group [ (26.30 ± 10.43) μg/ml]and control group [ ( 19.06 ± 6.91 ) μg/ml ] ( P < 0.01 ) ; the level of serum ACTH in severe sepsis group [ (25.47 ± 9.41 ) pg/ml ] was significantly higher than those in sepsis group [ ( 15.52 ± 5.85 ) pg/ml ] and control group [ (9.16 ± 3.02 ) pg/ml ] ( P < 0.01 ).Conclusion Stress response exists in the early period of sepsis,and the levels of serum cortisol and ACTH are increased significantly.The levels of serum cortisol and ACTH are positive correlation with the severity of diseases,and they could be used as serious index and a marker for evaluating prognosis in children with sepsis.
5.Preliminary evaluation of posterior dynamic lumbar stabilization in lumbar degenerative disease

[Objective] To evaluate the early effect of posterior dynamic lumbar stabilization in lumbar degenerative disease.[Methods]The clinical outcomes of 31 patients with lumbar degenerative disease treated by posterior decompression with Wallis posterior dynamic lumbar stabilization implant or combined with posterior lumbar fusion were retrospectively studied,and assessed with visual analogue scale(VAS)and spinal operative standard of Chinese Medical Association.The early effect and complications associated with Wallis posterior dynamic lumbar stabilization were recorded.[Results]The operative procedure of Wallis posterior dynamic lumbar stabilization implant was easy and less invasive.The VAS scores were 7.9?2.0,2.6?1.2 and 1.7?0.8 at one day preoperatively,two week postoperatively and final follow-up,respectively.The good to excellent result was 94.4% at the lastest follow-up.No compliction related with Wallis posterior dynamic lumbar stabilization was found.[Conclusion]It is easy and safe to use Wallis posterior dynamic lumbar stabilization in treatment of degenerative lumbar disease,and the early effect is good.The Wallis system provides an alternative for treatment of lumbar degenerative disease.
6.Impact of health management on treatment of rheumatoid arthritis combined with osteoporosis

Objective To explore the role of health management in the treatment of rheumatoid arthritis combined with osteoporosis.Methods A total of 140 women with postmenopausal osteoporosis and rheumatoid arthritis were enrolled in this 12-month randomized controlled study and randomly assigned to receive drug therapy ( group A,n =70 ) or drug therapy + health management ( group B,n =70 ).Bone mineral density (BMD) of the forearm,lumbar spine and hip was measured by using dual-X-ray absorptiometry at baseline and 12 months after the intervention.The rates of fractures were calculated at 12months.Results BMD of the forearm,lumbar spine and hip of both groups were significantly increased at 12 months (group A:0.50 ± 0.13,0.78 ± 0.15 and 0.68 ± 0.14,respectively,all P < 0.05; group B:0.54± 0.14,0.80 ± 0.12 and 0.73 ± 0.15,respectively,all P < 0.05 ).The elevation of BMD and therapy compliance of group B were significantly higher than those of group A ( P < 0.05 ),while the rate of fracture was relatively lower.Conclusions Health management can significantly improve patient compliance and medication efficacy.
7.Application of Phadiatop UniCAP in detection in children with allergic diseases

Objective To evaluate a blood test,UniCAP system(screening IgE to food or inhalant allergens,eosinophil cationic protein,ECP)in differentiation the capability of lgE-mediated allergic diseases in children. Methods 26 atopic dermatitis,33 asthma/bronchiolitis,20 allergic purpura, 15 Kawasaki disease patients and 20 normal children were tested total lgE,lgE to food or inhalant allergens,ECP by UniCAP system and compared.Resuits Fx5E positive rate are 73.1%,24.2%,25.0%,13.3%in atopic dermatitis,asthma,allergic purpura,Kawasaki disease respectively,and the Phadiatop positive rate are 23.1%、69.7%、10.0%、6.6%.The serum levels of ECP、TIgE of patients were higher than the mormal controls,with the highest level in asthma, and lowest in Kawasaki disease,the differences were significant(P<0.05).Conclusions Food orinhalant allergens plays an important role in the inflammatory process of asthma,atopic dermatitis,and some role in allergic purpura,Kawasaki disease.Food allergens are more important to atopic dermatitis.and inhalant allergens are more important to asthma.
8.Relationship between nonalcoholic fatty liver and apolipoprotein E gene polymorphism in undergraduate students of Shanghai

Objective To explore the relationship between nonalcoholic fatty liver (NFALD)and apolipoprotein E (ApoE)gene polymorphism in Shanghai undergraduate students. Methods A cross-sectional survey was performed in 4 148 students from Shanghai Tongji University. Quesgionary, physical measurement, ultrasonography and biochemical parameters were used to diagnose NFALD. In patients with NAFLD and 200 healthy subjects (served as control), ApoE genotypes were determined by PCR-RFLP. Results (1)Totally 398 subjects were diagnosed as NAFLD (prevalence 9.6%). Body mass index, waist-to-hip ratio, triglycerides, total cholesterol, alanine transaminase, aspartate aminotransferase and γ-glutamyltransferase in NAFLD group were higher than those in control, which were related to the degree of NAFLD (P<0.01). (2)ApoE genotypes distributions were significantly different between two groups (χ2=10.52, P<0.01). In NAFLD,E4/4 genotype frequency (5.5%)increased, and E3/3 (61.8%)decreased . (3)Unconditional logistic regression showed that E4/4 were closely associated with lipid profiles. Conclusion ApoE point mutations 112 and 158 may cause different dyslipidosis. E4/4 genotype seems to be one of the genetic predisposition factors of NAFLD.
9.Dopa responsive dystonia

Objective To explore the clinical feature and treatment method of the dopa responsive dystonia (DRD).Methods To observe the clinical manifestation,auxiliary test and the response on the treatment of levodopa in 6 patients from 4 families.Results 1 patient for childhood onset and 2 patients for adolescent onset with DRD, first symptom showed leg dystonia and toe walking with difficulty, whereas 3 patients of adult onset showed tremor and rigidity. Babinski sign was presented in 3 cases. Diurnal fluctuation in symptom severity occurred in all cases. All patients had obvious response to small dose levodopa in 1~6 days. In 2 cases, the effect was maintained for 7 years, without increasing the intake of levodopa.Conclusion The clinical feature of DRD was more remarkable,the therapeutic effectiveness of levodopa was rapid,sustained and obvious.
10.Tumor-targeting carrier equipped with cell-penetrating peptides

Equipping tumor-targeting carrier with cell-penetrating peptide with high transduction efficacy has become a trend. According to the fusion modes and tumor-targeting mechanisms, cell-penetrating peptides can be divided into five categories: first, self-targeting cell-penetrating peptides; second, fusion carriers made of targeting ligands and cell-penetrating peptides; third, cell-penetrating peptide-modified nano-carrier; fourth, tumor-microenvironment-targeting cell-penetrating peptides; fifth, other special cell-penetrating peptides. Fusion carriers, which can not be effectively released into the cytoplasm after transducted into target cells, can be further modified to increase their efficacy. In all, cell-penetrating peptide introduced into tumor-targeting carrier should provide a new era for anti-tumor pharmacy research and cancer therapy.
