1.Clinical application and progress of tissue-engineered trachea

BACKGROUND: Tissue-engineered trachea serving as a tracheal transplant, can not only reduce the expression of antigen cells, suppress immunoreaction, but also in advance conduct vascularization of blood vessels and regeneration culture of tissue cells in vitro. OBJECTIVE: To summarize the commonly used methods for tracheal transplantation, to introduce the latest clinical transplantation method of tissue-engineered trachea, and to provide reference evidence for carrying out such operations. METHODS: Taking tissue engineering, trachea, transplantation in English as search terms, Pubrned database from January 1990 to May 2009, ScienceDirect database from January 1990 to May 2009, Springer database from January 1990 to May 2009 was retrieved; Taking tissue engineering, trachea, transplantation in Chinese as search terms, CNKI database from January 1990 to May 2009 was researched. Literatures were limited to English and Chinese languages. Inclusive criteria: clinical studies and animal experiments related to the tracheal transplantation; exclusive criteria: duplicate documents. RESULTS AND CONCLUSION: Totally 270 literatures were screened out by computer, accordingto inclusion and exclusion criteria, 32 documents of which were involved for analysis. There is an increasing cases with irreversible injury of tracheal resulted from various reasons and in need of transplantation during clinical work, and the material source of tracheal transplantation is always the main problem to limited the clinical use. Constructing tissue-engineered trachea in vitro is considered to be a good way for repairing trachea. This paper will review the origin and development of tracheal transplantation and tissue-engineered trachea, summarize the common method of allotransplantation, autotransplantation and biomaterial transplantation, as well as the research of constructing bio-scaffold, choosing seed cells and clinical applied development of tissue-engineered trachea, and emphatically introduce the first case of successful artificial tracheal transplantation by combining the method of allotransplantation and culturing the seed cell without the immune cells of donor.

Immunodeficient animals are animal models that are suitable for medical research. Beige-Nude mice are deficient in both T lymphocytes and natual killer (NK) cells simultaneously. The T cell activity and NK cell activity are much lower in Beige-Nude mice. They were bred for cancer research at first. Researchers have studied the immunologic characteristics of Beige-Nude mice vastly and deeply since 1970s. So this expands its application in medical research.
3.The comprehensive application of conservative treatment for adenomyosis

Objective:To investigate the clinical effects of high-intensity focused ultrasound (HIFU), gonadotropin-releasing hormone agonist (GnRH-a), and levonorgestrel intrauterine extended-release system (LNG-IUS) on adenomyosis (AM) alone or in combination.Methods:The clinical and follow-up data of 338 AM patients who received conservative treatment in the First Affiliated Hospital of Xinjiang Medical University from July 2018 to March 2019 were analyzed retrospectively. The treatment related parameters, dysmenorrhea (VAS) score, menstrual volume (PBAC) score, uterine volume, recurrence rate of dysmenorrhea, hemoglobin (Hb) and carbohydrate antigen 125 (CA125) levels at 3, 6, and 12 months after treatment were compared among HIFU treatment (H group), GnRH-a treatment (G group), LNG-IUS treatment (L group), HIFU combined with GnRH-a treatment (H+ G group), HIFU combined with LNG-IUS treatment (H+ L group), HIFU combined with GnRH-a and LNG-IUS treatment (H+ G+ L group).Results:There was no significant difference in sound power, treatment time and ablation rate among the four HIFU groups ( P>0.05). The dysmenorrhea score, menstrual volume score, and CA125 levels in the 6 groups were significantly lower than before treatment ( P<0.05). The hemoglobin level of H group, H+ G group, H+ L group, H+ G+ L group continued to increase after treatment ( P<0.05). The VAS scores of the H+ G, H+ L, and H+ G+ L groups were significantly lower than those of the H and L groups at 3 and 6 months after treatment ( P<0.05). G group and H+ G group alleviated 100% of dysmenorrhea after 3 months of treatment, and the curative effect decreased afterwards. The recurrence rate of dysmenorrhea was 19.44%, 30.16%, 26.67%, 7.30%, 8.10%, 3.77% in H group, G group, L group, H+ G group, H+ L group, and H+ G+ L group, respectively ( P<0.05). The PBAC scores in the H+ G, H+ L, and H+ G+ L groups were significantly lower than those in the H, L, and G groups at 6 and 12 months after treatment ( P<0.05). The uterus volum in group L before treatment was smaller than that of the other groups ( P<0.05), and the uterus volume increased slightly in 3 months, 6 months, and 12 months after treatment than before treatment, but with no statistically significant difference ( P>0.05). The uterine volume of G, H+ G, and H+ G+ L groups were smaller than that of H and G groups at 3 months after treatment, and the uterine volume of group G at 12 months after treatment was larger than that at 3 and 6 months after treatment, but still smaller than before treatment ( P<0.05). The uterine volume, dysmenorrhea score, and menstrual volume score of the H+ G+ L group at 12 months after treatment were [(157.33±35.96)cm 3, (1.07±0.82)point, (69.57±17.24)point], which were all smaller than those of the other groups ( P<0.05). Conclusions:HIFU, GnRH-a, LNG-IUS alone or combined treatment of AM are safe and effective, but comprehensive treatment can improve clinical efficacy, delay and reduce recurrence and reduce the incidence of total hysterectomy.

Besides improving the design of intraocular lens, perfecting the technique of surgery and examining far and near visual acuity, we should stress the objective evaluation including contrast sensitivity, glare sensitivity, accommodate ability, binocular function and wavefront aberation for better visual results.
7.A comparative study of clinical efficacy between off-pump and conventional coronary artery bypass grafting in treating the patients with coronary heart disease and its impact on brain natriuretic peptide

Objective To investigate the clinical efficacy of off-pump coronary artery bypass grafting (OPCABG) comparing with the conventional coronary artery bypass grafting (CCABG) for treating the patients with coronary heart disease (CHD) and its impact on brain natriuretic peptide (BNP).Methods One hundred and twenty-two patients undergoing elective coronary artery bypass grafting were divided into CCABG group and OPCABG group according to the surgical method.The operative condition and postoperative clinical data,postoperative complications and death of the two groups were observed.The level of BNP was detected before surgery,immediately after surgery,postoperative 6,24,72 h and 1 week.Results The operative time,postoperative mechanical ventilation time,ICU monitoring time,24 h after drainage and blood transfusion,hospital stay in OPCABG group was(210.08 ± 60.02) min,(9.01 ± 2.57) h,(32.08 ±9.17) h,(343.43 ± 98.12) ml,(341.75 ±97.64) ml,(9.70 ±2.77) d,significantly lower than those in CCABG group [(309.38 ± 88.39) min,( 15.25 ±4.36) h,(45.14 ± 12.90) h,(530.24 ± 151.50) ml,(752.90 ± 215.11 ) ml,( 15.44 ± 4.41 ) d] ( P < 0.05 ).The incidence of postoperative complication of OPCABG group and CCABG group was 15.9%(10/63) and 47.5%(28/59),there was significant difference ( χ2 =14.172,P < 0.01).The mortality rate of OPCABG group and CCABG group was 1.6%(1/63) and 8.5%(5/59),there was no significant difference ( x2 =3.091,P > 0.05 ).The level of BNP in CCABG group before surgery,immediately after surgery,postoperative 6 h was (104.54 ±29.87),(114.74 ±32.36),( 129.10 ± 36.15 ) ng/L,and in OPCABG group was ( 103.46 ± 29.56 ),( 109.49 ± 31.28 ),( 126.42 ± 36.12 )ng/L respectively,there was no significant difference (P > 0.05).The level of BNP in CCABG group postoperative 24,72 h and 1 week [(335.57 ± 95.83 ),(429.98 ± 122.85 ),(350.92 ± 100.26) ng/L] were significantly higher than those in OPCABG group [(241.22 ± 68.92 ),( 317.49 ± 90.71 ),(256.86 ± 73.39)ng/L] (P < 0.05).The levels of BNP in both groups postoperative 24,72 h and 1 week were significantly higher than those before surgery (P< 0.05).Conclusion The OPCABG surgery is safe and effective,and has certain advantages for maintenance of cardiac function.

Objective To study the preparation of volatile oils obtained from asarum and lily magnolia complexed with ?-cyclodextrin and hydroxypropyl-?-cyclodextrin in Xuetie Dingchuan plaster,and to study its impacts of transdermal absorption.Methods The volatile oil was complexed with ?-cyclodextrin and hydroxypropyl-?-cyclodextrin.GC method was used in the determination of volatile oils complexed with ?-cyclodextrin and hydroxypropyl-?-cyclodextrin to investigate the impacts of transdermal absorption.Results The accumulation transmission of the volatile oils complexed with ?-cyclodextrin was superior to that with hydroxypropyl-?-cyclodextrin.Conclusion The inclusion of volatile oils with ?-cyclodextrin can enhance drug penetration via skin.
10.Significance of the mutation of c-kit gene in the clinical prognosis of gastrointestinal stromal tumor

Objective To investigate the significance of the mutation of c-kit gene in the development,(clinical) pathology and prognosis of gastrointestinal stromal tumors(GISTs).Methods The technique of(PCR-SSCP) was used to detect ckit gene mutation in 106 cases of GIST,and 57 of the cases were(followed-up).Results The c-kit gene mutation rate in the whole group was 45.3%(48/106),and(3.4)%(2/41) in benign GIST,and 70.8%(46/65) in malignant GIST.The 1-,3 and 5-year survival rates and median survival time of the mutation positive group were all markedlly lower than those of the(mutation) negative group.A comparison of the parameters that can differentiate the benign GISTs from malignant ones such as size,histological necrosis,aggressive grade,nucleus mitotic count and nuclear discrepancy,showed significant difference between the of postive and negative gene mutation(P
