1.Investigation about the use of antibiotics for type Ⅰ incision thyroid surgery

Objective To investigate the use of antibiotics for type Ⅰ incision thyroid surgery of our hospital,so as to provide guidance for reasonable clinical use of antibiotics.Methods A total of 149 discharged patients of Xinhua Hospital,who underwent the type Ⅰ incision thyroid surgery from Jun 2015 to Jun 2016,were enrolled in the study.There were 117 cases underwent unilateral thyroid surgery,and there were 32 cases underwent bilateral thyroid surgery.According to the operation condition,the application of antibiotics in perioperative period,the number of days in hospital,the total cost of hospitalization,the infection situation and so on were analyzed.Results There were 69 cases(58.97%) who were perioperative prophylactic use of antibiotics,of which unilateral surgery 117 cases,and there were 20 cases(62.50%) who were perioperative prophylactic use of antibiotics,of which bilateral surgery 32 cases.The hospitalization days and hospitalization cost of the antibiotics used group were higher than those of the antibiotics unused group.All the cases were well stage wound healing,and one case occurred nosocomial infection.Conclusion It is not worthy for the prophylactic use of antibiotics for the type Ⅰ clean incision thyroid surgery to prevent the nosocomial infection,furthermore,the hospitalization days and hospitalization cost of patients increased.
2.Clinical observation of Entecavir combined Liuweiwuling Pian in the treatment of chronic hepatitis B patients with HBeAg+

Objective To explore the curative effect and safety of of Entecavir ( ETV) combined Liuweiwul-ing Pian in the treatment of chronic hepatitis B patients with HBeAg +.Methods 124 patients according to the ran-dom number table were divided into the two groups ,which respectively used ETV combined Liuweiwuling Pian treat-ment and ETV.The liver function and index of viral replication of the two groups were compared .Results The effec-tiveness of the observation group was 91.94%,which of the control group was 82.26%,the difference between the two groups had statistical significance(Ridit=32.251,P=0.000);Since 24 week treatment,HBV DNA of the observa-tion group was lower than the control group,the difference was statistically significant (t =2.365,4.595,all P<0.05);After treatment,ALT,AST of the observation group decreasing degree was higher than the control group ,the difference was statistically significant (t=78.512,62.145,all P<0.05);after treatment,the ALT rate,effective rate and bounce rate in the two groups was statistically significant differences (χ2 =25.621,46.241,11.512, all P<0.05);After treatment,the HBV DNA and HBeAg positive in two groups was below cut-off test cases,differences was statistically significant(χ2 =17.265,21.264,all P<0.05).Conclusion Entecavir combined Liuweiwuling Pian in the treatment of chronic hepatitis B patients with HBeAg +can reduce liver fibrosis , and has obvious detoxification and anti-viral.

3-phosphoinositide dependent kinase-1 (PDK1) has been shown to be a critical regulator of the PI3K-Akt pathway.PDK1 can activate Akt and participates in the activation of PI3K-Akt signaling pathway to promote tumor development,invasion and metastasis.At present,it has been found that PDK1 is highly expressed in head and neck cancer,multiple myeloma,pancreatic cancer,esophageal cancer,colon cancer and other malignant tumors.Thereby inhibiting PDK1 overexpression may provide a new breakthrough for the treatment of malignant tumor.At present,many kinds of PDK1 inhibitors have been put into production,which plays an important role in tumor therapy.
4.Pathogenesis and prevention of cerebral vasospasm after subarachnoid hemorrhage

5.Neuroprotective effects of peroxisome proliferators-acfivated receptor γ ligand thiazolidinediones in cerebral ischemia

Thiazolidinodiones are a synthetic ligand of peroxisome proliferator-activated receptor γ(PPARγ).PPARγhas certain protective effects for several types of tissues.The research covers the areas of glucose,lipid metabolism,cancer and atherosclerosis,etc.At present,the roles of thiazolidinediones in central nervous system diseases have received much attention,such as cerebral infarction,multiple sclerosis,and Alzheimer's disease.This article reviews the effects of PPARγ ligand thiazolidinediones on inhibiting the proliferation of glial cells,promoting angiogenesis,and antagonizing neuronal apoptosis after cerebral ischemia.
6.A MODIFIED METHOD OF DETERMINING NK ACTIVITY IN HUMAN PERIPHERAL BLOOD:WHOLE BLOOD ASSAY

With use of ~(125)I-udR release test,a significant natural killer activityagainst K_(562) target cells has been determined in human peripheral wholeblood(WB).r There was a gradual decrease in NK activity as rise inthe dilution or WB.In comparison with peripheral monouclear cells(PBMC)oneself,the NK activity lowered distinctly at both 1:4 and 1:8 dilution ofWB(p0.4).It seems that neither the red blood cells nor thePlasma ffeect on NK activity of WB,The NK activity in WB as well as inPBMO was improved by incubation with human leukocyte interferon.TheNK activity of WB and PBMC in patients with hepatocellular carcinomawas signiticantly lower than in normal controls.The results showed tahtNK activity may be determined by using properly diluted WB instead orPBMC,and the assay can be easily performed in clinical practice.
7.Extracellular matrix-related molecule on fundus neovascularization

Neovascularization is the main cause of the vision loss of the patients sufferring from proliferative diabetic retinopathy,retinopathy of prematurity and age-related macular degeneration.It is proved that collagen and elastin in the extracellular matrix contribute to the choroidal and retinal neovascularization in vitro and in vivo.Among the extracellular matrix-related adhesion molecules,integrin α5β1 could enhance the cell adhesion and hyperplasy,while its inhibitors could restrain the choroidal and retinal neovascularization in vivo,so are the inhibitors of the integrin α V β3 and α V β5.Selectins and intercellular adhesion molecule-1 mainly affect the neovascularization as the medium between the endothelial cells and the leucocytes.It is demonstrated that the extracellular matrix degradationrelated serine proteinases (mainly urokinase-type plasminogen activator ) /matrix metalloproteinases (mainly MMP-2 and MMP-9)also could induce the choroidal and retinal neovascularization in vitro and in vivo.Furthermore,type-1 plasminogen activator inhibitor and tissue inhibitor of metalloproteinase could prevent that and the further study of the extracellular matrix-related molecules would bring out new insights and methods for the precaution and treatement of the ocular neovascularization.
8.Determination of Cinnamaldehyde in Cortex Cinnamomi and Shentai Plaster by HPLC

Objective: To develop an assay for the determination of cinnamaldehyde in Cortex Cinnamomi and Shentai Plaster. Methods: Hypersil C 18 column (200?4.6mm, 5?m) was used. The mobile phase consisted of a mixture of methanol acetonitrile water tetrahydrofuran (25∶15∶55∶5) with a flow rate of 0.8mL?min -1 . The detection wavelength was at 285 nm. Results: The calibration curve was linear in the range of 32~320ng for cinnamaldehyde ( r =0.9995). The average recoveries of cinnamaldehyde in Cortex Cinnamomi and Shentai Plaster were 97.8% (RSD=3.0%) and 92.2% (RSD=2.9%), respectively. Conclusion: This HPLC method is simple, rapid and accurate, and it can be used in the control of product quality and preparation process.
9.Arthroscopic one-stage reconstruction of anterior and posterior cruciate ligaments with LARS artificial ligaments

Objective To evaluate the efficacy of arthroscopic one-stage reconstruction of anterior and posterior cruciate ligaments with LARS artificial ligaments. Methods From October 2009 to December 2011, 8 patients with combined injury to anterior and posterior cruciate ligaments received arthroscopic one-stage reconstruction with LARS artificial ligaments. They were 7 men and one woman, aged from 26 to 52 years ( average, 35. 3 years ) . The time from injury to operation ranged from 17 to 21 days, averaging 19 days. Pre-operative and intraoperative inspections showed complete tear of both anterior and posterior cruciate ligaments. Inter national Knee Documentation Committee ( IKDC ) scoring was used at 6 months postoperation to grade the knee movement of the patients; Lysholm scoring was used at final follow-ups to evaluate the knee function. Results The operating time for this series ranged from 85 to 102 minutes ( average, 93 minutes ) . All the patients were followed up for 6 to 24 months ( average, 12 months ) . Wound infection at upper tibia occurred in one patient, which responded to debridement and screwing 4 weeks after operation. All the patients got rid of instability of the knee joint, and gained normal knee extension and fine functional recovery. At final follow-ups, the range of motion of the knee ranged from 100° to 125° ( average, 108. 4°) . Follow-ups revealed no breakage or loosening of the ligaments. By the IKDC scoring, of the 8 patients who had been graded all as D before opera-tion, one was improved into A and 7 into B. The Lysholm scores at final follow-ups averaged 86. 1+4. 8, sig-nificantly higher than the postoperative value ( 18. 7+3. 1 ) ( P < 0. 05 ) . Conclusion Arthroscopic one-stage reconstruction of anterior and posterior cruciate ligaments with LARS artificial ligaments can restore the stability of knee joint and obtain satisfactory short-term results.
10.Application of FMEA ia medical risk management in America

Doing a good job of guarding against medical risks can effectively improve medical safety and reduce waste of medical resources. In recent years, marked results have been attained in the US in studies on the use of FMEA, a prospective quality analysis tool, to minimize medical risks. Similar studies, though not many, have been made in some other countries, such as Italy and Australia. No reports, however, have been published at home on studies in this field. The paper gives an account of the use and theoretical studies of FMEA in medical risk management in the US, illustrates cases wherein it was used to lower risks in prescribing drugs to patients and ensure the safe use of hospital software and hardware, and puts forward issues that shouldn't be ignored in using FMEA so as to arouse the attention of domestic hospital management departments.
