1.Influence of Jieyu Huoxue Decoction on rehabilitation of patients with depression after cerebral infarction

OBJECTIVE: To study the influence of Jieyu Huoxue Decoction on rehabilitation of patients with depression after cerebral infarction. METHODS: Fifty four patients suffered from the depression after acute cerebral infarction were randomly divided into three groups: depression control group, fluoxetine treated group and Jieyu Huoxue Decoction treated group. Eighteen patients of cerebral infarction without depression were enrolled in the no depression control group randomly. The Zung self-rating depression scale (SDS), the modified Edinburgh-Scandinavia stroke scale (MESSS) and the activities of daily living (ADL) were evaluated before treatment, 30 and 60 days after treatment. RESULTS: After 30 and 60 days of treatment, SDS evaluation of the Jieyu Huoxue Decoction treated group showed significant difference as compared with the condition before treatment and that of the depression control group (P < 0.05 or P < 0.01), MESSS evaluation and ADL evaluation also showed significant difference as compared with the condition before treatment and that of the depression control group (P < 0.05 or P < 0.01); there was no obvious difference between the fluoxetine treated group and the Jieyu Huoxue Decoction treated group, but the Jieyu Huoxue Decoction treated group showed fewer symptoms and less side effect. CONCLUSION: Jieyu Huoxue Decoction can not only relieve depression after cerebral infarction, but also improve neurological functions.
2.Influence of sequential enteral nutrition on nutritional status and prognosis in patients with acute cerebral infarction

Objective To explore the influence of the sequential enteral nutrition(SEN) on nutritional status and prognosis in patients with acute cerebral infarction(ACI).Methods 80 ACI patients complicated with dysphagia were divided into SEN group and control group(40 cases in each group).In the first three days patients in SEN group were treated with nasal feeding short-peptide enteral nutrition[84~126(kJ/kg.d) ],than,which would be replaced with nasal feeding integral-protein enteral nutrition from the forth day.Patients in the control group were provided with ordinary fluid food by nasal feeding.Nutrition support for both groups was at last more than 10 d.In the 1 d,7 d and 14 d after treatment,data of both groups concerned with triceps skin fold(TSF),mean upper arm muscle circumference of triceps(MAMC),serum albumen(Alb),serum prealbum(PAB),heamoglobin(Hb),incidence of infectious complications,National Institutes of Health Stroke Scale(NIHSS) score and Barthel index(BI) were examined.Results Nutritional indexes(TSF,MAMC,Alb,PAB,Hb) in the SEN group were significantly higher than those in the control group(P
3.Preparation and measurement of magnetic adriamycin-verapamil-albumin nanoparticles

Objective To prepare magnetic adriamycin-verapamil-albumin nanoparticles(MAVANs) and measure the physical property of the MAVANs,such as magnetism,drug loading(DL)and size of nanoparticles in order to direct its further experimental study on animals and its application in clinical practice.Methods Commercial cottonseed oil was refined,then mixed with adriamycin,verapamil,human albumin and Fe3O4 magnetic powder according to appropriate proportion,and MAVANs was prepared by using a heat denaturation technique.The size and shape of nanoparticles were determined by electron transmission micro-photography.The magnetic response and structure of the MAVANs were analyzed by upside-down microscope.The standard curve was plotted.The drug loading of verapamil in the MAVANs was quantified by high performance liguid chromatography.The drug loading of adriamycin was quantified by a standard spectrophotonuorometry.Results The nanoparticles featured homogenous size,regular and spherical shape,and good magnetic response.The mean diameter of nanoparticles was 0.5?m.The loading amount of verapamil was 0.8%.The drug loading of adriamycin was 1.25%.Conclusions The heat denaturation technique is feasible to prepare the nanoparticles which can load two kinds of drug with good physical property.The process is simple and the testing condition is easy to control.
4.Detection of macrophage migration inhibitory factor in peripheral blood mononuclear cells, sera and skin tissue fluid of patients with vitiligo vulgaris

ObjectiveTo detect the expression levels of macrophage migration inhibitory factor (MIF) in peripheral blood mononuclear cells (PBMCs),sera and skin tissue fluid from patients with vitiligo vulgaris,and to investigate their clinical significance.MethodsThirty-nine patients with vitiligo vulgaris and 31 age- and sex-matched normal human controls were recruited in this study.Real-time reverse transcription-PCR was employed to assess the expressions of MIF mRNA in PBMCs,enzyme-linked immunosorbent assay (ELISA) to detect the concentrations of MIF in sera and skin tissue fluid from these subjects.ResultsPatients with vitiligo vulgaris showed a significantly higher level of MIF mRNA in PBMCs (6.70 (2.64 - 8.65) vs.1.67 (1.24 - 2.45),Z=5.895,P< 0.05),MIF protein in sera (32.76 (10.67 - 40.98) μg/L vs.7.89 (6.13 - 9.54) μg/L,Z=5.936,P < 0.05 ) and skin tissue fluid ( 167.80 ( 107.40 - 219.60) μg/L vs.42.44 (32.29 - 49.74) μg/L,Z =4.715,P < 0.05) compared with the normal human controls.The expression levels of MIF mRNA in PBMCs,and MIF protein in sera and skin tissue fluid were also higher in patients with progressive vitiligo than in those with stable vitiligo (7.89 (3.89 - 9.12) vs.5.62 (2.23 - 7.29),Z=2.213,P< 0.05; 37.80 (29.50 - 45.70) μg/L vs.22.70 (9.36 - 37.78) μg/L,Z=2.141,P< 0.05; 211.50 (131.70 - 248.75) μg/L vs.144.65 (89.13 - 167.30) μg/L,Z =2.100,P < 0.05).In addition,the vitiligo area severity index (VASI) score was positively correlated with the expression levels of MIF mRNA in PBMCs (r =0.486,P < 0.05)and MIF protein in sera (r =0.453,P < 0.05).ConclusionMIF might play a certain role in the pathogenesis of vitiligo vulgaris.

Objective:To study the reasons and preventive measure of the effect of human urinary kallikrein on blood pressure in the treatment of acute cerebral infarction.Method:Documents about the effect of human urinary kallikrein on blood pressure of pre-clinical study and the phaseⅠ-Ⅲclinical trials were collected and analyzed in combination with the related information of the phaseⅣclinical trial from our hospital.Result:The rate and concentration of medication in combination with ACEI were the reasons for the effect of human urinary kallikrein on blood pressure.Conclusion:Controlling the rate and concentration of medication and forbidding the combined use of ACEI from the 72hr before the medication and the 24hr after it can prevent hypotension caused by human urinary kallikrein.
6.The clinical effect of treatment of severe carbon monoxide poisoning with Naoxingjing injection combined with naloxone

Objective To investigate the clinical effect of treatment of severe carbon monoxide poisoning with Naoxingjing injection combined with naloxone.Methods Emergency department of affiliated theorem clinical college, hospital 70 patients diagnosed with severe carbon monoxide poisoning from emergency department in June 2014 to December 2016, were randomly divided into observation group and control group 35 cases.In the control group were treated with naloxone treatment, observation group were treated based on the use Naoxingjing injection treatment.The two groups were observed and recorded recovery time, mortality, the incidence of delayed encephalopathy, Glasgow Coma Score(GCS)and serum levels of IL-1β.Results The patients in the observation group total effective rate of 97.14% was significantly higher than 77.14%, and the difference was statistically significant(P<0.05);the recovery time was observed in patients(2.25 ± 0.88)h, significantly shorter than the control group(3.46 ± 1.10)h, and the difference was statistically significant(P<0.05);and follow-up observation group mortality rates were delayed encephalopathy(1.43%, 8.57%)than the control group(17.14%, 25.71%), and the difference was statistically significant(P<0.05);④ observation group were GCS score was (12.77±1.89), significantly better than the control group, and the difference was statistically significant(P<0.05);⑤ observed in patients IL-1β is(65.41±9.93)ng / L was significantly lower than the control group(89.86±10.74)ng / L, and the difference was statistically significant(P<0.05).Conclusion Naoxingjing injection combined with naloxone treatment of severe carbon monoxide poisoning exact clinical effect, can effectively reduce patient recovery time, reduce mortality and morbidity.
7.Effect of Resuscitating Acupuncture and Scalp Acupuncture on Neurological Function and High Sensitivity C Reactive Protein in Patients with Acute Cerebral Infarction

Objective To investigate the clinical effect of resuscitating acupuncture and scalp acupuncture on neurological function and high sensitivity C reactive protein (hs-CRP) in serum in patients with acute cerebral infarction. Methods 90 patients with acute cerebral infarction were divided into three groups: drug, routine acupuncture and resuscitating acupuncture and scalp acupuncture groups in random. They were evaluated with NIH Stroke Scale (NIHSS) , Modified Rankin Scale (MRS) 0, 7, 14 d after treatment, and measured with the levels of hs-CRP. Results There were significant improvements for NIHSS and MRS scores in the resuscitating acupuncture and scalp acupuncture group compared with those of the others, as well as the levels of hs-CRP. Conclusion Resuscitating acupuncture and scalp acupuncture can reduce the level of hs-CRP and improve the neurological functions in patients with acute cerebral infarction.
8.Protective effects of Dextran-40 on fatal jellyfish stings

Objective To investigate the protective effects of Dextran-40 on fatal jellyfish stings at whole animal and cellular levels.MethodsFirstly, using the fatal jellyfish envenomation syndrome (acute jellyfish envenomation syndrome, AJES and delayed jellyfish envenomation syndrome, DJES) models established earlier by ourselves, we surveyed the effects of Dextran-40 on the survival rate of AJES mice, on the circulatory function of AJES rats and on the serum biochemical indexes of DJES rats.In addition, the protective effects of Dextran-40 on cardiomyocytes against the damage induced by tentacle extract (TE) from the jellyfish Cyanea capillata were studied at the cellular level by laser scanning confocal microscopy.ResultsAt the whole animal level, Dextran-40 at 0.8g/kg significantly improved the survival rate of AJES mice, and at 0.6g/kg greatly inhibited the drop of blood pressure of AJES rats.For the DJES rats, Dextran-40 at 0.6g/kg had a significant protective effect on the liver function indexes (ALT, AST, A/G) and myocardial enzymes (CK, LDH).At the cellular level, Dextran-40 5μg/mL greatly inhibited the TE-induced increase in intracellular Ca2+ content and the death of cardiomyocytes.ConclusionThe protective effects of Dextran-40 on fatal jellyfish stings may be related to its ability to stabilize blood pressure or block the TE-induced pore formation in the cardiomyocytes.Considering that the clinical safety of Dextran-40, we strongly recommend it as a first-aid medicine for jellyfish stings.
9.Evaluation on a modified Ziehl-Neelsen stainin the diagnosis of tuberculous meningitis

Objective Toevaluatea modified Ziehl-Neelsen(Z-N) stain in the diagnosis of tuberculous meningitis. Methods Cerebrospinal fluid specimens from 35 patients were stained by using the modified Ziehl-Neelsen staining. Re-sults The positive rate was 94.29% in 35 patients with tuberculous meningitisand the intracellular acid-fast bacilli was detected in 53.40%of all specimens. One case was stained positive in 15 patients with non-tuberculous meningitis. Con-clusion The modified Ziehl-Neelsen stain not only significantly improves the detection rates of tuberculous meningitisbut alsois able to identify intracellular M.tuberculosisin cerebrospinal fluidspecimen.Thus, the modified Z-N stain can be a convenient tool for diagnosing tuberculous meningitis.
10.Preparation of ibuprofen/EC-PVP sustained-release composite particles by supercritical CO2 anti-solvent technology.

Ibuprofen/ethyl-cellulose (EC)-polyvinylpyrrolidone (PVP) sustained-release composite particles were prepared by using supercritical CO2 anti-solvent technology. With drug loading as the main evaluation index, orthogonal experimental design was used to optimize the preparation process of EC-PVP/ibuprofen composite particles. The experiments such as encapsulation efficiency, particle size distribution, electron microscope analysis, infrared spectrum (IR), differential scanning calorimetry (DSC) and in vitro dissolution were used to analyze the optimal process combination. The orthogonal experimental optimization process conditions were set as follows: crystallization temperature 40 degrees C, crystallization pressure 12 MPa, PVP concentration 4 mgmL(-1), and CO2 velocity 3.5 Lmin(-1). Under the optimal conditions, the drug loading and encapsulation efficiency of ibuprofen/EC-PVP composite particles were 12.14% and 52.21%, and the average particle size of the particles was 27.621 microm. IR and DSC analysis showed that PVP might complex with EC. The experiments of in vitro dissolution showed that ibuprofen/EC-PVP composite particles had good sustained-release effect. Experiment results showed that, ibuprofen/EC-PVP sustained-release composite particles can be prepared by supercritical CO2 anti-solvent technology.
