1. Establishment of a HEK-293 cell line stably expressing human TLR5 and NF-κB responsive reporter

Objective To construct the lentivirus plasmids of human Toll like receptor-5(TLR5)and nuclear factor(NF)-κB signaling pathway responsive luciferase and green fluorescent protein(GFP)reporters, and establish a HEK-293 cell line stably expressing human TLR5 and NF-κB responsive reporters. Methods The TLR5 and 5×NF-κB-binding sites genes were cloned to lentiviral expressing plasmid pWSLV-puro and pWSLV-nanluc-GFP, respectively. The recombinant plasmids, pWSLV-hTLR5-puro and pWSLV-5×NF-κB-nanluc-GFP were respectively transfected with packaging plasmid into HEK293T cells in order to package lentivirus. The lentivirus was used to coinfect HEK-293 cells, and the positive clones stably expressing TLR5 and NF-κB responsive reporter (HEK293-N-T)were generated using colony-purification in present of puromycin and fluorescence activated cell sorting. The cells expressing TLR5 were confirmed by Western blot and immunofluorescence. Additionally, the cells expressing NF-κB responsive reporter were confirmed by fluorescence microscopy and luciferase quantitative assay. Results The results of recombinant plasmids digestion identification and the sequencing of cDNA showed that the sequences inserted into the plasmid and the expected sequences of hTLR5 and 5×NF-κB-binding sites genes were completely consistent. It was confirmed that the hTLR5 and 5×NF-κB-binding sites genes were correctly inserted into the vectors, and that the recombinant plasmids, pWSLV-hTLR5-puro and pWSLV-5×NF-κB-nanluc-GFP, were successfully constructed. Additionally, Western blot and immunofluorescence analysis showed that TLR5 were stably expressed in a high level in the HEK293-N-T cells and located in the cell membranes. Moreover, the results of fluorescence microscopy and luciferase quantitative assay indicated that the NF-κB responsive reporter could reflect the activation of NF-κB signaling in a dose-dependent manner. Conclusion The HEK-293 cell line stably expressing human TLR5 and NF-κB responsive reporter has been successfully established and the HEK293-N-T cells can be used for the screening and evaluation of drugs target to TLR5.

The connotation and norms of herbal preparation in the past dynasties have been improved gradually, and have been included in the code system since the Tang Dynasty. The process of legislative evolution is closely related to the fate and development of TCM. In the era of full-scale rejuvenation of Chinese medicine, with the development of Chinese medicine industry to 4.0, the perfection of the standards and standard system with the construction of quality standardization of Chinese medicine as the core, especially under the Pharmacopoeia system, is the important support and guarantee for the promotion of the whole industry, which is also an important part of the rejuvenation of the entire Chinese medicine industry.

3.Characteristics of Schizophrenia Patients' Homicide Behaviors and Their Correlations with Criminal Capacity.

OBJECTIVES:
To explore the characteristics of schizophrenia patients' homicide behaviors and the influences of the assessments of criminal capacity.
METHODS:
Indicators such as demographic and clinical data, characteristics of criminal behaviors and criminal capacity from the suspects whom were diagnosed by forensic psychiatry as schizophrenia (n=110) and normal mental (n=70) with homicide behavior, were collected by self-made investigation form and compared. The influences of the assessments of criminal capacity on the suspects diagnosed as schizophrenia were also analyzed using logistic regression analysis.
RESULTS:
There were no significant statistical differences between the schizophrenic group and the normal mental group concerning age, gender, education and marital status (P>0.05). There were significant statistical differences between the two groups concerning thought disorder, emotion state and social function before crime (P<0.05) and there were significant statistical differences in some characteristics of the case such as aggressive history (P<0.05), cue, trigger, plan, criminal incentives, object of crime, circumstance cognition and self-protection (P<0.05). Multivariate logistic regression analysis suggested that thought disorder, emotion state, social function, criminal incentives, plan and self-protection before crime of the schizophrenic group were positively correlated with the criminal capacity (P<0.05).
CONCLUSIONS
The relevant influences of psychopathology and crime characteristics should be considered comprehensively for improving the accuracy of the criminal capacity evaluation on the suspects diagnosed as schizophrenia with homicide behavior.

4.Clinical observation on acupuncture at Zhaohai (KI 6) and Shenmai (BL 62) for treatment of insomnia.

OBJECTIVETo observe therapeutic effect of acupuncture at Zhaohai (KI 6) and Shenmai (BL 62) on insomnia.
METHODSSeventy-eight cases of insomnia were randomly divided into a treatment group of 40 cases and a control group of 38 cases. The treatment group were treated with acupuncture at Zhaohai (KI 6) using reinforcing method and at Shenmai (BL 62) using reducing method, combined with acupuncture at acupoints selected according to syndrome differentiation. The control group were treated with acupuncture at acupoints selected according to syndrome differentiation. Their therapeutic effects were compared.
RESULTSThe cured rate and the total effective rate were 62.5% and 97.5% in the treatment group, and 31.6% and 68.4% in the control group, respectively, with significant difference between the two groups (P < 0.01).
CONCLUSIONAcupuncture at Zhaohai (KI 6) and Shenmai (BL 62) has a better therapeutic effect on insomnia.

5. Risk factors for incidence of white matter lesion in patients with ischemic cerebrovascular diseases: A retrospective analysis of 417 cases

Objective To study the risk factors of white matter lesions (WMLs), especially on the correlation of WMLs incidence with those well known risk factors for cerebrovascular disease (hypertension, hyperglycemia, hyperlipemia and smoking). Methods Four hundred and seventeen consecutive inpatients in Neurology Department of our hospital were enrolled who had undergone brain MRI. Detailed clinical history, relevant data and the Fazekas scores of WMLs were collected. Logistic regression was used to analyze the independent risk factors. Results Age (P=0.000), homocysteine level (P=0.010), grade of hypertension (P=0.007), patients with diabetes for less than 10 years (P=0.033) and smoking more than 40 years (P=0.003) were correlated with WMLs. In the group of patients with ischemic cerebrovascular disease, the independent risk factors of WMLs are Age (P=0.006), smoking (P=0.000) and grade of hypertension (P=0.028). Conclusions Age, homocysteine level and grade of hypertension are independently correlated with WMLs. Age, smoking and grade of hypertension are the independent risk factors for the patients with WMLs and ischemic cerebrovascular disease.

7.Clinical effect of Jinlong capsules combined with Western medicine treatment in treatment of primary liver cancer: A Meta-analysis

ObjectiveTo investigate the clinical effect and adverse events of Jinlong capsules combined with Western medicine treatment in the treatment of advanced primary liver cancer. MethodsPubMed, The Cochrane library, CNKI, Wanfang Data, CBM, and VIP were searched for randomized controlled trials (RCTs) on Jinlong capsules combined with Western medicine treatment in the treatment of advanced primary liver cancer published up to May 2019. The patients treated with Jinlong capsules combined with Western medicine treatment were enrolled as intervention group, and those treated with Western medicine treatment alone were enrolled as control group. Data extraction and quality assessment were performed, and RevMan 5.3 was used to perform the Meta-analysis with a fixed or random effects model. ResultsA total of 26 RCTs were included, with a total of 2318 patients (1175 patients in the intervention group and 1143 patients in the control group). The results of the Meta-analysis showed that compared with Western medicine treatment alone, Jinlong capsules combined with Western medicine treatment had significant increases in response rate (relative risk [RR]=1.36, 95% confidence interval [CI]: 125-1.49, P<0.001), 1-year survival rate (RR=1.29, 95%CI: 1.14-1.47, P<0.001), Karnofsky Performance Scale score (RR=137, 95%CI: 1.19-1.59, P<0.001), peripheral blood CD3+ T lymphocytes (mean difference [MD]=11.18, 95%CI: 268-1968, P=0.01), CD4+ T lymphocytes (MD=6.43, 95%CI: 3.82-9.05, P<0.001), and CD4+/CD8+ ratio (MD=0.31, 95%CI: 026-0.36, P<0.001), as well as significant reductions in rate of progression (RR=0.41, 95%CI: 0.29-0.57, P<0001), rate of leucopenia (RR=0.65, 95%CI: 0.57-0.75, P<0.001), incidence rate of gastrointestinal reactions (RR=0.72, 95%CI: 059-088, P=0.001), and peripheral blood CD8+ T lymphocytes (MD=-4.68, 95% CI: -5.84 to -3.51, P<0001). ConclusionJinlong capsules combined with Western medicine treatment has a marked clinical effect in the treatment of advanced primary liver cancer with few adverse events and thus holds promise for clinical application.
8.Research on Ethical Connotation and Its Practice of Rehabilitation Medicine

Objective:To construct an ethical system to guide the practice of rehabilitation in China based on the ethical principles of health services, in conjunction with the environment and ethical concepts of rehabilitation services in Chinese community. Methods:To review the ethical historiography of rehabilitation medicine, analyze the ethical setting of rehabilitation medicine, and summarize the practice of ethical conducts of rehabilitation medicine in national rehabilitation institutions. Results:The development of ethics in rehabilitation medicine was systematically analyzed, the principles and methods of ethical setting in rehabilitation institutions were elaborated, and the methods of ethical setting and practice in national-level rehabilitation institutions were explored. Conclusion:The connotation and methods of ethical setting in rehabilitation and the path and methods of practicing ethical guidelines in national-level rehabilitation institutions were systematically discussed.
9. Transvaginal four-dimensional hysterosalpingo-contrast sonography in evaluation on patency of fallopian tubes in infertile patients

Objective: To explore the application value of four-dimensional hysterosalpingo-contrast sonography (4D-HyCoSy) with transvaginal ultrasound in evaluation of fallopian tube patency in infertile patients. Methods Clinical data of 112 infertile patients who underwent 4D-HyCoSy and laparoscopic chromopertubation using methylene blue (LC) were retrospectively analyzed. The sensitivity, specificity, positive predictive value, negative predictive value, positive likelihood ratio, negative likelihood ratio and AUC of 4D-HyCoSy in diagnosis of tubal obstruction were calculated. Then the consistency between 4D-HyCoSy and LC in diagnosis of fallopian tube patency was evaluated. Results: A total of 218 fallopian tubes were examined in 112 infertile patients. Taken LC results as gold standards, the coincidence rates of 4D-HyCoSy in diagnosis of tubal patency, partially obstructed and completely obstructed were 88.64% (39/44), 72.09% (62/86) and 89.77% (79/88), respectively, and the overall coincidence rate was 82.57% (180/218). The sensitivity, specificity, positive predictive value, negative predictive value, positive likelihood ratio and negative likelihood ratio of 4D-HyCoSy in diagnosis of tubal obstruction were 80.61% (79/98), 92.50% (111/120), 89.77% (79/88), 85.38% (111/130), 10.75 and 0.19, respectively, AUC was 0.87 (P<0.01). 4D-HyCoSy had higher consistency with LC in diagnosis of tubal patency (Kappa=0.74). Conclusion: 4D-HyCoSy is consistent with LC in diagnosis of fallopian tube patency, which has high specificity in diagnosing tubal obstruction.
10. Clinical Outcome of Endoscopic Submucosal Dissection for Large Protruding Colorectal Tumor

Background: Endoscopic submucosal dissection (ESD) is the first choice for treating laterally spreading colorectal tumor. However, the effect of ESD on large protruding colorectal tumor is not ideal. Aims: To evaluate the features of large protruding colorectal tumor and the clinical outcome of ESD. Methods: The clinical data of 36 patients with protruding colorectal tumor with diameter ≥20 mm that undergone ESD from November 2016 to December 2019 at Beijing Hospital were retrospectively analyzed. The clinical, endoscopic, surgical and pathological features were summarized, and the risk factors of ESD were analyzed. Results: The average diameter of 36 lesions was (25.44±5.57) mm. All lesions were located in left hemicolon. The percentage of en bloc resection was 75.0%, and the percentage of R0 resection was 72.2%. ESD procedure was discontinued in 5 patients (13.9%) and were converted to surgery. The rate of curative resection was 55.6%. Non-curative resection was found in 11 patients, and of them 7 patients accepted surgery later. The overall rate of surgery was 33.3%. The en bloc resection rate (0 vs. 87.1%, P=0.003) and R0 resection rate (20.0% vs. 80.6%, P=0.008) were significantly decreased in muscle-retracting sign group than in non-muscle-retracting sign group. There were significant differences in en bloc resection rate, R0 resection rate and curative resection rate among the non-submucosal fibrosis (F0), mild fibrosis (F1) and severe fibrosis (F2) groups (P<0.001, P=0.000, P=0.010). Conclusions: The ESD procedure of large protruding colorectal tumors ≥20 mm is technically difficult. The rates of the en bloc resection and R0 resection are low, and the curative resection rate is even more lower. Muscle-retracting sign, severe submucosal fibrosis are the important factors that affecting the outcome of ESD operation.
