1.Clinical Observation on Treatment of 288 Cases of Correlative Gastric Diseases Infected by Pyloric Helicobacterium with Kang Wei Granules

Kang Wei Granules which can strengthen the spleen, replenish Qi, clear away heat and remove dampness was used to treat 288 cases of correlative gastric disease infected by pyloric helicobacterium. Seventy-four cases were treated with triple therapy (Bismuth being the mainly ingradient) as control group. Results indicated that the treatment group was significantly superior to the control group in improvment of main symptoms, especially for the types of deficiency of spleen and stamach, and internal accumulation of dampnes-heat (P 0. 05). It is suggested that Kang Wei Granules has a better clinical therapeutic effect for treatment of correlative gastric disease infected by pyloric helicobacterium.
2.Efficiency of the ratio of axial length to corneal curvature radius in the evaluation of myopia in children and adolescents

AIM: To evaluate the value of the ratio of axial length to corneal radius of curvature(AL/CR)in the diagnosis of myopia in children and adolescents.METHODS: The refraction and ocular biometric parameters of 2 182 cases of children and adolescents(4 364 eyes)who initially visited the optometry clinic of the Second People's Hospital of Beihai from January 2022 to December 2023 were collected and analyzed.RESULTS: The receiver operating characteristic curve(ROC curve)showed that AL/CR had a higher area under the curve(AUC=0.925, 95%CI: 0.917-0.933)in diagnosing myopia. When AL/CR was 3.053, the Youden index was the largest. As the threshold of AL/CR decreased, the sensitivity of myopia diagnosis increased, while the specificity decreased. Compared with screening myopia, the sensitivity of AL/CR=3.053 in diagnosing myopia was low, but the specificity was high, especially in the cases of hyperopia and pre-myopia.CONCLUSION: The accuracy of AL/CR in diagnosing myopia is superior to that of axial length and average corneal curvature, with higher specificity. As the threshold of AL/CR decreases, the sensitivity of diagnosing myopia increases, but the specificity decreases. In cases of hyperopia and pre-myopia, the specificity of AL/CR in diagnosing myopia is higher than that of screening myopia.
3.Investigation and analysis of refraction state of primary school students in Haicheng district, Beihai city

AIM: To understand the prevalence of refractive state of primary school students in Haicheng district, Beihai city.
METHODS: Cross-sectional study and cluster sampling were used. A total of 30 716 students aged 6 to 14 from 15 primary schools in Haicheng district, Beihai city, who had taken physical examination from October 2019 to January 2020, were selected for visual acuity and pupil diopter. SPSS17.0 statistical software was used for data analysis.
RESULTS: In the eyes of the 15 primary schools monitored in this study, 30 716 cases had mild myopia(29.62%), 0.50D < astigmatism < 2.00D(25.43%), metropia(21.05%)and mild hyperopia(15.85%). With the growth of age, the proportion of emmetropia and farsightedness declined as awhole. The proportion of myopia is on the rise. A total of 1 894 cases of wearing glasses 3 788 eyes. The rate of poor vision was 23.30%, and the rate of qualified correction of frame glasses was 64.57%. Compared with the left eye, the difference was statistically significant, and the P50 of the right eye was more negative than that of the left eye. The equivalent spherical mirror was compared between boys and girls. The difference was statistically significant, and the P50 of equivalent spherical mirror of girls was more negative than that of boys.
CONCLUSION: With the growth of age, the proportion of mild hyperopia and squarely eyes decreases year by year; The proportion of myopia increases year by year; Among the students with poor eyesight, the number of those who regularly wear glasses is small; However, after wearing glasses, there are still some students whose glasses are not up to standard. The refractive state of the right eye is more negative than that of the left eye; Female students tend to be more negative than male students in the refractive state.
4.K-mean cluster analysis for incisal jaw morphology of normal occlusion subjects among different vertical facial skeletal types.

OBJECTIVETo study the incisal jaw morphology of subjects with normal occlusion and to discuss the relationship between different vertical facial skeletal types and different incisal jaw cluster types.
METHODSAfter studied by radiographic cephalometry, the incisal jaw morphology of 169 subjects with normal occlusion in early permanent dentition were analyzed by K-mean cluster analysis.
RESULTSMorphology of maxillary or mandibular incisal jaws could be divided into four types separately, including division I, division II, division III and division N. In maxilla or in mandible, the incisal jaw morphology of division I was characterized by wideness and shortness. In contrast, division II was characterized by thinness and length. As for division III and division IV, both of two were intermediate types between division I and division II . In maxilla, it was found that division II was the majority (86.49%) in the high-angle subjects, division III was the majority (47.67%) in the average-angle subjects, and division I and III were the majority (82.61%) in the low-angle subjects. In mandible, it was found that division II was the majority (51.35%) in the high-angle subjects, division IV was the majority (48.84%) in the average-angle subjects, and division III and IV were the majority (69.57%) in the low-angle subjects.
CONCLUSIONOwing to the obvious characteristics of morphology in each cluster types and in each vertical facial skeletal types, different clinic considerations must be put forward when we want to move the incisor.

5.Comparison of clinical effects between locking intramedullary nail fixation and skeletal traction in treating tibiofibula fractures.

OBJECTIVETo compare and evaluate clinical effects between locking intramedullary nail fixation and skeletal traction in order to provide the best therapeutic choice for tibiofibula fractures.
METHODSFrom November 1989 to December 2000, 108 patients with tibiofibula fractures (group A) were treated with skeletal traction, there was 76 males and 32 females; the age was from 29 to 66 years with an average of (40.3 +/- 9.6) years; according to AO classification standard,48 cases were type A, 46 type B, 14 type C, of which the standard typing Gustio, type I in 15 cases, type II in 13, type III A in 2. And from January 2001 to July 2007, 57 patients with tibiofibula fractures (group B) were treated with locking intramedullary nail fixation,there was 40 males and 17 females,the age was from 21 to 69 years with an average of (38.1 +/- 8.9) years; according to AO classification standard, 27 cases were type A, 22 type B, 8 type C, of which the standard typing Gustio, type I in 7 cases, type II in 6, type III A in 1. Four indexes were compared, including the time of fracture healing, the time of achieved 90 degrees genuflection, the time of off-bed weight-bearing and complication.
RESULTSComparison of fracture healing time: group A was (19.0 +/- 0.6) weeks and group B was (14.0 +/- 0.5) weeks; the time of achieved 90 degrees genuflection in group A was (92.0 +/- 8.2) d and in group B was (39.0 +/- 6.1) d; the time of off-bed weight-bearing in group A was (96.0 +/- 6.3) d and in group B was (38.0 +/- 6.5) d. Complication occurred in 42 cases in group A and 6 in group B. There was significantly difference in 4 items between two groups (P < 0.01). The method of locking intramedullary nail fixation was better than skeletal traction in treating tibiofibula fractures.
CONCLUSIONTreatment of tibiaofibula fractures with locking intramedullary nail fixation can obtain satisfactory effect, which has less complication, the fracture heals and weigh-loading can be achieved early.

6. Long non-coding RNA in malignant melanoma

Malignant melanoma is the leading cause of skin cancer-related deaths. However, the current understanding of molecular mechanism for malignant melanoma remains extremely limited, and the therapeutic methods of malignant melanoma need to be broken urgently. Long non-coding RNA (LncRNA) regulates gene expression as RNA molecules at many levels, including epigenetic regulation, transcriptional regulation and post- transcriptional regulation. In recent years, with the in-depth research of LncRNA, it is showed that LncRNA molecules have a strong correlation with the occurrence of many major human diseases, including malignant melanoma. Many LncRNAs associated with malignant melanoma have been found, and the molecular mechanisms of some LncRNAs involving the proliferation, invasion and metastasis of malignant melanoma cells have been gradually discovered. So the research of LncRNAs on the early diagnosis and treatment of malignant melanoma as well as the development of target drugs has become a hotspot. In this article, the research progress in LncRNAs associated with malignant melanoma is emphatically reviewed, and their potential value in clinical application for malignant melanoma is preliminarily discussed.
7.Construction, expression and immune response of eukaryotic plasmid secreting SNCG.

Hsc70-SNCG fusion protein cDNA fragment containing signal peptide sequence of Igkappa, MMP9, and P37 was inserted into the vector pVAX1 to construct recombinant plasmid pVAX-Igkappa-Hsc70-SNCG, pVAX-MMP9-Hsc70-SNCG, and pVAX-P37-Hsc70-SNCG. Three eukaryotic vectors were constructed and verified by restriction enzyme digestion and sequencing. After transfection with recombination plasmids in QM-7 cells, the transient expression and secretion of three fusion proteins were detected by ELISA and Western Blot. The results suggested that Hsc70-SNCG carrying three different signal peptides could be expressed and secreted by transfected cells, and three signal peptides effectively directed secretion of fusion protein by QM-7 cells. BALB/c mice were immunized by three plasmids using gene gun system. The serum levels of anti-SNCG antibodies in mice were measured by ELISA. The results showed that three secreted plasmids could stimulate humoral immune responses to SNCG in mice, which depended on the secreted expression levels induced by signal peptides.

8.Predictive value of combination detection of tissue Pgp1 expression and preoperative serum CEA level for colorectal cancer.

OBJECTIVETo explore the predictive value of combination detection of Pgp1 expression in cancer tissue and serum CEA level for the prognosis of colorectal cancer (CRC) patients.
METHODSClinicopathological data, complete 5-year follow-up data and CRC tissue samples of 153 CRC patients with stage I( to II( tumor undergoing radical operation in our department from January 2004 to August 2006 were retrospectively collected. Immunohistochemical staining was used to detect the expression level of Pgp1. The combined evaluation of staining intensity and positive cell percentage was performed to determine the expression level of Pgp1. Pgp1 staining (-) and (+) was defined as low expression; and staining (++) and (+++) as high expression. Electrochemiluminescence immunoassay was used to detect the level of serum CEA. CEA > 5 μg/L was defined as positive. χand Fisher's exact test were performed to analyze the association of Pgp1 expression with CEA level and clinicopathological variables. Moreover, Kaplan-Meier method was used to analyze the survival. Univariate and multivariate Cox proportional hazard regression models were used to evaluate the roles of Pgp1 expression combined with serum CEA level in prognosis prediction.
RESULTSOf 153 patients, 105 were males and 48 females with mean age of 59 (27 to 90) years; 41 cases were rectal cancer, and 112 cases colon cancer; 23 patients were TNM stage I( tumor, and 130 patients stage II( tumor; median follow-up time was 64 months; 30 cases were dead. Positive rate of Pgp1 expression in colorectal cancer tissues was 66.0%(101/153). The expression of Pgp1 was associated with gender, tumor location, and survival during the follow-up (all P<0.05). The preoperative positive rate of serum CEA was 28.1% (43/153). The preoperative serum CEA level was associated with tumor recurrence and survival (all P<0.05). Kaplan-Meier analysis showed the overall 5-year survival rate was 81.7%. The 5-year survival rate of patients with high expression of Pgp1 was 88.1%, which was significantly higher than 69.2% of those with low expression of Pgp1(P=0.003). The 5-year survival rate of patients with preoperative positive serum CEA was 72.1%, which was significantly lower than 86.1% of those with preoperative negative serum CEA(P=0.023). Furthermore, the 5-year survival rate of patients with negative Pgp1 plus positive CEA was 66.7%, which was significantly lower than 91.0% of those with positive Pgp1 plus negative CEA(P=0.002). Univariate analysis showed that gender, Pgp1 expression level, preoperative serum CEA level, and Pgp1 combined with CEA were significantly associated with the prognosis of patients(all P<0.05). Multivariate analysis showed that Pgp1 expression was an independent prognostic factor of CRC [HR(95%CI:1.261 to 64.224), P=0.028].
CONCLUSIONSLow expression of Pgp1 in cancer tissue indicates poor prognosis in patients with stage I( and II( tumor. Combination detection of Pgp1 expression and serum CEA can be applied to predict the prognosis of patients with stage I( and II( colorectal cancer.

9.Expression of NF-E1b in colorectal cancer tissues and its clinical significance.

OBJECTIVETo explore the expression of NF-E1b in colorectal cancer tissues and its association with various clinicopathological parameters and prognosis of the patients.
METHODSClinicopathological and follow-up data of 168 colorectal cancer patients undergoing radical operation at Department of Gastrointestinal Surgery, Peking University Cancer Hospital and Institute from 2005 to 2012 were retrospectively analyzed, including 96 males and 72 females, with mean age of (57.8±11.2) years. The expression of NF-E1b protein was detected in samples of 168 resected colorectal cancer tissues and 45 adjacent non-cancerous tissues by immunohistochemistry. The expression rates of NF-E1b were compared among different clinicopathological features. Moreover, the association between NF-E1b expression and prognosis was analyzed.
RESULTSThe expression of NF-E1b protein located mainly in cytoplasm. Positive rate of NF-E1b expression in adjacent non-cancerous tissues was 17.8% (8/45), which was obviously lower than 67.9%(114/168) of cancer tissues with significant difference (χ(2)=36.376, P=0.000). Clinicopathological parameters analysis suggested that the expression level of NF-E1b in cancer tissues was associated with age (χ(2)=4.862, P=0.030), TNM staging (χ(2)=10.969, P=0.002), lymph node metastasis (χ(2)=7.390, P=0.008) and distal metastasis (χ(2)=17.887, P=0.000). The median follow-up time was 23(1-77) months. The overall 5-year survival of this cohort was 33.3%. Colorectal cancer patients with high levels of NF-E1b expression showed a worse overall survival compared with those with low levels of NF-E1b expression (18.4% vs. 56.6%, P=0.000). Univariate Cox regression analysis showed that tumor location (P=0.034), tumor size (P=0.003), TNM staging (P=0.000), depth of tumor invasion (P=0.009), lymph node metastasis (P=0.000), distant metastasis (P=0.000) and NF-E1b expression level (P=0.001) were associated with the prognosis of colorectal cancer patients. Multivariate Cox regression analysis revealed that tumor diameter >4 cm (HR=2.193,95% CI:1.334 to 3.603, P=0.002), distant metastasis (HR=2.064, 95% CI:1.160 to 3.672, P=0.014) and high NF-E1b expression (HR=1.994,95% CI:1.068 to 3.724, P=0.030) were independent risk factors of predicting poor prognosis of colorectal cancer patients.
CONCLUSIONSNF-E1b expression up-regulates in colorectal cancer tissues. High expression of NF-E1b is associated with poor prognosis of colorectal cancer patients. NF-E1b may serve as a potential target of the treatment for colorectal cancer.

10.Multi-center randomized double-blind controlled study on children's anorexia (spleen-stomach disharmony) treated with Child Compound Endothelium Corneum.

Child Compound Endothelium Corneum(CCEC)has the effects in invigorating the spleen and appetizing the appetite, and dissolving the accumulation of food. The recent studies have proved that it could improve gastrointestinal motility, restore physiological gastrointestinal peristalsis, increase gastrointestinal digestive motility, and enhance appetite. This trial aimed to evaluate its clinical efficacy and safety in the treatment of children's anorexia(spleen-stomach disharmony). A total of 240 children with anorexia in line with the inclusion and exclusion criteria were selected and randomly divided into experimental group and control group, with 120 in each group. Patients in the experimental group took CCEC and Erpixing Granules simulant. Patients in the control group took Erpi-xing Granules and CCEC simulant. After 21 days of treatment, there was no statistical difference in the recovery rate of anorexia, reduced food intake, eating time, weight change, traditional Chinese medicine syndrome effect, single symptom effect, and trace element Zn recovery rate between the two groups. Based on the non-inferiority test, the experimental group was not inferior to the control group in efficacy. How-ever, the effect of CCEC in reducing appetite in children with anorexia was better than that of control drugs(P<0.05). There was no statistical difference in the incidence of adverse events and adverse reactions between the two groups during the trial. This experiment confirmed the efficacy and safety of CCEC in the treatment of children's anorexia(spleen-stomach disharmony), with a safety and re-liability in clinical application. In addition, it was a better choice for children with anorexia who were mainly manifested by reduced appetite. Meanwhile, compared with granule, chewable tablets were more convenient to take in clinic. Therefore, the efficacy and safety of CCEC for the treatment of children's anorexia(spleen-stomach disharmony) were not inferior to those of Erpixing Granules, with a safety and reliability in clnic. However, due to the small sample size of this trial, the efficacy results only show a trend. It is suggested to further carry out a large-sample-size clinical study to define the clinical advantages of CCEC.
