1.The effect of saponin isolated from stems and leaves of ginseng (SLG) on immunologic function in mice

The effects of SLG on the immune function of normal and immune-suppressed mice were investigated. The results showed that SLG (50mg/kg, im) could correct the suppressive effect of cyclophosphamide, dexamethasone or prednisolone on spleen and. thymus weight, serum specific antibody level and lymphocyte transformation. SLG could also improve the phagocytic activity of peritoneal macrophages and enhance peripheral blood ANAE+-lymphocyte ratio of mice. No obvious effect of SLG on the normal mice were observed except that it could increase the antibody level of normal mice. The results implicate that SLG may have some immunomodulatory effect.
2.A study of risk factors for contralateral hip fracture within 2 years following primary hip fracture surgery in elderly patients

Objective:To study the risk factors for contralateral hip fracture within 2 years after primary hip fracture surgery in elderly patients.Methods:A retrospective study was conducted of the 1,962 elderly patients who had been surgically treated for hip fractures at Department of Orthopaedic Trauma, Beijing Jishuitan Hospital from May 2015 to April 2018. They were 573 males and 1,389 females; the median age of primary hip fracture was 81 (75, 86) years. They were divided into 2 groups according to whether a contralateral hip fracture occurred or not within 2 years after primary hip surgery: 134 patients in the contralateral fracture group and 1,828 ones in the contralateral fracture-free group. The duration from primary hip fracture to secondary contralateral fracture was recorded. The χ2 test and Mann-Whitney U test were used to compare between the 2 groups their gender, age, walking ability, laboratory indexes upon admission, concomitant internal diseases, Charlson comorbidity index (CCI), duration from admission to surgery, duration from admission to discharge and complications during follow-up. The risk factors for contralateral hip fracture within 2 years after primary hip surgery were determined by the Cox's proportional hazard regression model. Results:In the elderly patients with hip fracture, the 2-year cumulative incidence of secondary contralateral hip fracture was 6.83%(134/1,962) and the median duration from primary surgery to secondary contralateral hip fracture was 365 (189, 611) d. The risk factors for contralateral hip fracture were female ( RR=2.081, 95% CI: 1.351 to 3.207, P=0.001), concomitant peripheral vascular disease ( RR=5.876, 95% CI: 2.922 to 11.818, P< 0.001), concomitant chronic obstructive pulmonary disease ( RR=3.750, 95% CI: 1.897 to 7.413, P< 0.001), progressively higher CCI ( RR=1.363, 95% CI: 1.223 to 1.519, P<0.001), complicated pneumonia ( RR=3.606, 95% CI: 2.054 to 6.332, P<0.001), complicated urinary infection ( RR=7.670, 95% CI: 4.441 to 13.248, P<0.001), and complicated deep venous thrombosis (DVT) ( RR=7.389, 95% CI: 3.992 to 13.677, P<0.001). Conclusions:The risk factors for contralateral hip fracture within 2 years after primary hip fracture surgery in elderly patients may be female, concomitant peripheral vascular disease and chronic obstructive pulmonary disease, progressively higher CCI, and complicated pneumonia, urinary infection and DVT.
3.Case control study on anatomical locking plate and ordinary steel plate for the treatment of closed calcaneal fracture with SandersⅡand Ⅲ.

OBJECTIVE:
To compare clinical efficacy between anatomical locking plate (ALP) and ordinary steel plate (OSP) in treating closed calcaneal fractures with SandersⅡ and Ⅲ.
METHODS:
From May 2016 to May 2018, 68 patients with closed Sanders typeⅡ and Ⅲ calcaneal fractures were retrospectively analyzed, and were divided into anatomical locking plate group (ALP group) and ordinary steel plate group (OSP group) according to two kinds of plate fixation, and 34 patients in each group. In ALP group, there were 21 males and 13 females aged from 20 to 63 years old with average of (35.16±8.45) years old; 14 patients were typeⅡand 20 patients were type Ⅲaccording to Sanders classification;treated with ALP. In OSP group, there were 20 males and 14 females aged from 19 to 63 years old with average of (35.05±8.39) years old;19 patients were typeⅡand 15 patients were type Ⅲ according to Sanders classification;treated with OSP. Operative time, intraoperative blood loss and complications between two groups were observed and compared;preoperative and postoperative Böhler angle and gissane angle were also compared;American Orthopaedic Foot & Ankle Society (AOFAS) ankle and hind foot scores, foot and ankle disability index (FADI) scores were applied to evaluate clinical effect.
RESULTS:
All patients were followed up from 11 to 14 months with an average of (12.06±0.81) months. There were no statistical differences in opertive time, intraoperative blood loss, incision infection and refracture rate in complications between two groups (
CONCLUSION
Compared with OSP, ALP in treating SandersⅡ and Ⅲ calcaneal fractures could achieve better therapeutic effect, avoid screw loosening, reduce complications, and improve limb function in further.

4.Body composition of upper limbs for hemiplegic patients after isokinetic strength training

ObjectiveTo study the changes of body composition indexes reflecting muscle condition for stroke hemiplegic patients after isokinetic strength training. MethodsFrom January, 2021 to April, 2022, 30 stroke inpatients with hemiplegia in Beichen Hospital of Nankai University were randomly divided into control group (n = 15) and isokinetic group (n = 15). Both groups accepted routine rehabilitation training, while the isokinetic group accepted isokinetic strength training of shoulder and elbow on the affected side in addition. All patients were tested the lean body mass, affected upper arm muscle circumference, affected upper limb muscle mass, edema index, skeletal muscle index before, and two weeks and four weeks after training. ResultsCompared with the control group, the edema index decreased in isokinetic group after training (t > 2.536, P < 0.05); the affected upper limb muscle mass increased more in the isokinetic group four weeks after training (t = -2.750, P = 0.010). ConclusionIsokinetic training can reduce edema and muscular disuse atrophy of the affected side for stroke patients with hemiplegia.
5.Effect of early acupuncture on cognitive function in patients with vascular dementia after cerebral infarction.

OBJECTIVE:
To observe the clinical efficacy of early acupuncture for vascular dementia (VD) after cerebral infarction, and explore its possible mechanism.
METHODS:
A total of 120 patients with VD after cerebral infarction were randomized into an acupuncture combined with medication group (60 cases, 1 case dropped off) and a western medication group (60 cases, 1 case dropped off). In the western medication group, oxiracetam capsules were given orally, 2 capsules each time, 3 times a day. On the basis of the treatment as the western medication group,
RESULTS:
The total effective rate was 89.8% (53/59) in the acupuncture combined with medication group, which was superior to 76.3% (45/59) in the western medication group (
CONCLUSION
Early acupuncture could improve cognitive function and activities of daily living in patients with VD after cerebral infarction, its mechanism may be related to improving the blood flow velocity of MCA, promoting blood circulation, and improving cerebral perfusion.

6.Comparison of Diagnostic Performance Between PI-RADS v2.1 and PI-RADS v2 for Prostate Cancer: A Meta-analysis

Objective To compare the diagnostic performance of PI-RADS v2.1 and PI-RADS v2 in the detection of clinically significant prostate cancer(csPCa) by Meta-analysis. Methods The major biomedical databases were searched (CNKI, CBM, Medline, and Embase) with the keywords "PIRADS v2.1" or "PI-RADS v2.1". The Quality Assessment of Diagnostic Accuracy Studies Tool v2 (QUADAS-2) was used to evaluate literature quality. Meta-analysis was performed using STATA17.0 and ReMan5.4 software. Forest plots were used to represent the sensitivity and specificity of PI-RADS v2.1 and PI-RADS v2 for each study. Sensitivity, specificity, positive likelihood ratio, negative likelihood ratio, and diagnostic odds ratio were combined, and diagnostic performance was evaluated using asummary receiver operating characteristic curve (SROC). Subgroup analysis was performed on three covariables: tumor location, threshold, and the nationality of authors. Results A total of 12 studies were included, involving 3 158 patients and 3 243 lesions. Forall zones and the whole gland, PI-RADS v2.1 had a larger area under the SROC curve (AUC) for csPCa performance, compared with PI-RADS v2. Subgroup analysis: PI-RADS v2.1 also had a larger area under the SROC (AUC) to detect transitional zone csPCa. Different diagnostic thresholds: when a score of 4 was used for the threshold, PI-RADS v2.1 had the maximum area under SROC (AUC) for csPCa performance detection. Author nationality: Researches of PI-RADS v2.1 in Chinese authors had the largest area under the SROC (AUC) in detecting csPCa performance. Conclusion Compared with PI-RADS v2, the diagnostic performance of PI-RADS v2.1 in detecting csPCa is not obviously improved and overall specificity is still low.

9.The predictive role of the expression ratio of androgen receptor splice variant-7 and androgen receptor in the prognosis of prostate cancer treated by castration therapy

Objective To evaluate the prognostic value of the ratio of AR and AR-V7 expression in prostate cancer treated by castration therapy.Methods Immunohistochemical staining was performed in biopsy specimen of 136 prostate cancer patients received hormone therapy in Tongji Hospital of Huazhong University of Science and Technology from January 2010 to December 2015.Expression was determined using modified H score method.Patients aged from 53-96,the median age was 71,median tPSA value at diagnosed was 110.00 ng/ml(2.61-4 003.4 ng/ml),median fPSA value was 14.62 ng/ml(0.12-640.19 ng/ml),median PSA density was 1.15 ng/(ml · cm3) [0.02-62.63 ng/(ml · cm3)].Among these,88 (64.7%)patients were diagnosed Gleason score≥8,39(28.7%) patients with Gleason score 7,while 7 (6.6%) patients Gleason score <7.There were 54(39.7%) patients diagnosed T4 stage,57(41.9%)patients T3 stage and 25 (18.4%) patients Tx stage;62 (45.6%) patients were diagnosed N 1 stage,46 (33.8%) N0 stage and 28(20.6%) patients Nx stage;97(71.3%) patients were diagnosed M1 stage,30(22.1%) M0 stage,9 (6.6%) patients Mx stage.Cause-specific Cox regression and Kaplan-Meier Analysis were used to analyze the prognosis risk.Results The median follow-up time was 44 months,ranged 15-71 months.During the surveillance,the disease progression-free survival time ranged from 5-59 month,median 19 months.The overall survival time ranged from 12-61 months,median 31 months.Among these,79(58.1%) patients were AR positive and 26(19.1%) patients were AR-V7 positive,while AR and AR-V7 expression had no significant correlation (Spearman-test r =0.042,P =0.629).The AR-V7 positive patients showed significantly lower CRPC progression free survival (10.8 months vs.25.0 months,P < 0.001) and much lower overall survival (20.3 months vs.42.8 months,P < 0.001).The high AR-V7/AR expression ratio group showed significantly lower CRPC progression free survival (12.0 months vs.24.8 months,P < 0.001) and much lower overall survival (22.5 months vs.42.8 months,P < 0.001).In univariate Cox regression analyses,Gleason score at diagnosis,T stage,tPSA,PSA density and high AR-V7/AR expression ratio could predict the prognosis of hormonal therapy.While in multivariate Cox regression analyses,T stage(HR =2.597,95% CI 1.351-4.995,P =0.004) and high AR-V7/AR expression ratio(HR =5.788,95% CI 2.530-13.242,P < 0.001) could effectively and independently predict the prognosis of hormonal therapy.Conclusion High AR-V7/AR HS ratio is the independent predictor of the prognosis of prostate cancer hormone therapy.AR-V7 positive and high AR-V7/AR HS ratio patients may have shorter PFS and overall survival time than AR-V7 negative and low AR-V7/AR HS ratio patients.
10.Research progress on the relationship between high fluoride exposure and cardiovascular system injury

High fluoride exposure induces not only skeletal damage such as dental fluorosis and skeletal fluorosis, but also injury on the cardiovascular system. In this article, we elaborated on the effects of high fluoride exposure on cardiovascular diseases such as heart injury, vascular sclerosis, hypertension and hyperlipidemia by reviewing domestic and foreign literatures in the last decade. We analyzed the damage effects and mechanisms of high fluoride exposure from the perspectives of population epidemiology, animal experiments and in vitro cell experiments, and proposed that prospective cohort studies on the effects of high fluoride exposure on cardiovascular system damage should be further strengthened. This paper provides a scientific basis and new ideas for the prevention and treatment of endemic fluorosis and cardiovascular diseases.
