2.Clinical characteristics of Mycoplasma pneumoniae pneumonia with different imaging changes in children

Objective To explore the clinical characteristics of Mycoplasma pneumoniae pneumonia (MPP) with different imaging changes in children.Methods Hospitalized patients with MPP during September 2012 to August 2013 in Beijing New Century Children's Hospital were retrospectively reviewed.The data including clinical features,laboratory tests,treatment and prognosis were collected.The patients were divided into 3 groups according to the radiological findings,including interstitial infiltration (group 1),intralobular infiltration (group 2) and segmental or larger infiltration (group 3).The clinical data were compared among the 3 groups.Results The percentages of patients in the 3 groups were 18.8% (36/191 cases) in group 1,39.8% (76/19 cases) in group 2 and 41.4% (79/191 cases) in group 3,respectively.In 144 cases of MPP patients older than 3 years old,48.6% (70/144 cases) were segmental or larger infiltration.The incidence of higher fever,hypoxemia and elevated C-reaction protein and lactate dehydrogenase in group 3 [50.6% (40/79 cases),87.3% (69/79 cases),78.5% (62/79 cases),29.1% (23/79 cases)] were higher than those in group 1 [5.6% (2/36 cases),61.1% (22/36 cases),19.4% (7/36 cases),0] and group 2 [10.5% (8/76 cases),67.1% (51/76 cases),14.5% (11/76 cases),3.9% (3/76 cases)],and the differences were significant(all P < 0.01).Patients in group 3 had higher risk of cardiac and/or liver impairment,refractory MPP,and treated by glucocorticoids and bronchoalveolar lavage [41.8 % (33/79 cases),29.1% (23/79 cases),94.9 % (75/79 cases),50.6% (40/79 cases)].The hospital stay was (9.48 ±3.26) d in group 1,(9.24 ±2.97) d in group 2,and (12.09 ±3.01) d in group 3,respectively,and the difference was significant among 3 groups (F =19.348,P =0.005),the hospital stay in group 3 was longer than that in the other 2 groups (t =4.210,5.931,all P =0.000),while there was no difference between group 1 and group 2 (P > 0.05).Patients in group 1 with wheeze were more co-mmon,and were treated by Azithromycin earlier than that in group 3 [(5.08 ± 3.43) d vs.(4.16 ± 2.20) d],the difference was significant(t =2.498,P =0.014),while the incidence of hypoxemia was common than that in group 2 (x2 =3.176,P =0.012).Conclusions MPP patients with segmental or larger infiltration are older than patients in the other groups,and present with severe complications,higher inflammatory factors,longer hospital stay,and higher risk for refractory MPP.Glucocorticoids and bronchoalveolar lavage are usually used in these patients.Patients with interstitial infiltration are prone to manifest with wheeze and hypoxemia,and its diagnosis and treatment by Azithromycin are usually delayed.Therefore,pediatrician should pay more attention to MPP patients with different imaging changes,in order to diagnose and treat the patients timely.
3.Repeated injection of mitoxantrone containing thermosensitive liposomes in rat induced ABC phenomenon.

To investigate whether accelerated blood clearance (ABC) phenomenon could be induced after repeated injection of mitoxantrone thermosensitive liposomes, LC-MS/MS and enzyme linked immunosorbent assay (ELISA) were used to measure the concentration of mitoxantrone and the anti-polyethylene glycol (PEG) IgM levels in rat plasma, separately. The drug was rapidly cleared away after the second administration. The anti-PEG IgM was detected after the first dose which was neutralized quickly after the second dose. It is proved that repeated administration of mitoxantrone thermosensitive liposomes in rat caused the ABC phenomenon.

4.Twisting acupuncture and sham acupuncture at Taichong (LR 3) in patients with essential hyper- tension: an fMRI study.

OBJECTIVEWith functional magnetic resonance imaging (fMRI), cerebral function imaging of acupuncture and sham acupuncture at Taichong (LR 3) in patients with essential hypertension (EH) was observed to analyze central mechanism of Taichong (LR 3) to treat EH.
METHODSEighteen EH patients were divided into a twisting acupuncture group (10 cases) and a sham acupuncture group (8 cases), which were treated with twisting manipulation and sham acu- puncture at right Taichong (LR 3), respectively. The treatments were both given with 30-second manipulation and 30- second interval for totally 5 min. During the acupuncture, fMRI was adopted to scan the cerebral function imaging of EH patients on resting state, and the acquired data were analyzed with SPM2 module in Matlab software.
RESULTSAcupunc- ture at Taichong (LR 3) in EH patients mainly increased opposite-side oxygen consumption, which generally activated left anterior cingulated gyrus (BA 32), left inferior parietal lobule (BA 40), left inferior temporal gyrus (BA 19), left middle temporal gyrus (BA 37) and right anterior central gyrus (BA 6). The activated areas were related to motor, vision and cognition of emotion. Stimulation region of sham acupuncture mainly included right anterior cingulated gyrus (BA 24), left inferior parietal lobule (BA 40) and left inferior frontal gyrus (BA 7).
CONCLUSIONThe central mechanism of Tai- chong (LR 3) on antihypertensive effect and symptom improvement is not in thalamus and medulla that are traditionally believed to control blood pressure. Taichong (LR 3) could activate anterior cingulated gyrus to regulate parasympathetic nerve and lower blood pressure, and through anterior cingulated gyrus, the connection with the surrounding areas is strengthened to improve the cognitive impairment caused by long-term hypertension.

5.A pedigree of multiple endocrine neoplasia type 1 with the proband initially presenting adrenal cortical carcinoma

[Summary]_ This is a pedigree of multiple endocrine neoplasia type 1(MEN1). The proband pursuit medical assistance because of hypertension and weakness. Adrenal cortical carcinoma with possible Cushing's syndrome was diagnosed after a series of tests. During this process, the proband was found to have hypercalcemia, and he was diagnosed as primary hyperparathyroidism. Adrenal carcinoma plus primary hyperparathyroidism suggested MEN1, which was confirmed by MEN1 gene 400_401insC mutation. Pedigree investigation found six additional patients, including one with high parathyroid hormone level and two without clinical evidence of any MEN1 diseases. The proband died of metastatic malignancy 7 months after diagnosis while the other 3 patients with clinically confirmed MEN1 tumor responded well to surgery, including one with adrenal cortical carcinoma.
6.Effect of sirolimus on tumor necrosis factor α-induced lipid accumulation in HepG2 cells

Objective To investigate the effect of sirolimus on tumor necrosis factor alpha (TNF-o)-induced lipid accumulation in HepG2 cells.Methods Palmitate acid (PA)-loaded HepG2 cells were divided into three groups:Control group(0.2% BSA + 0.16 mmol · mL-1 PA + 25 ng· mL-1 TNF-o),model group(0.2% BSA +0.16 mmol · mL-1 PA +25 ng · mL-1TNF-α),experimental group(0.2% BSA + 0.16 mmol · mL-1 PA +25 ng · mL-1 TNF-α + 10 ng · mL-1 sirolimus).After 24 h treatment,lipid accumulation in the HepG2 cells was visualized using oil red O staining.The phosphorylation of mammalian target of rapamycin (mTOR)and its downstream translation regulator including P70 ribosomal protein S6 kinase (P70S6K) in HepG2 cells were detected by Western blotting.The mRNA expression of sterol regulatory element binding protein 1 (SREBP1),acetyl CoA carboxylase(ACC),fatty acid synthetase(FAS) were detected by real-time PCR.Results TNF-o-induced lipid accumulation was significantly inhibited by sirolimus in HepG2 cells.The TNF-α-induced phosphorylation of mTOR and its downstream translation regulator P70S6K were also reduced by sirolimus:the relative levels of p-mTOR protein in control group,model group and experimental group were 1.00 ± 0.20,3.11 ± 0.60,2.38 ± 0.50,respectively;the relative levels of p-P70S6K protein in that three groups were 1.00 ±0.30,3.67 ±0.60,1.62 ±0.50,respectively.The differences between model group and control group or experimental group and model group were statistically significant (all P < 0.05).Additionally,the mRNA expression of SREBP1,ACC and FAS were downregulated by sirolimus:the mRNA levels of FAS in control group,model group andexperimental group were 1.04 ± 0.32,2.85 ±0.90,1.68 ± 0.38,respectively;the mRNA levels of ACC in that three groups were 1.04 ± 0.30,3.23-±1.33,2.07 ± 0.52,respectively;the mRNA levels of SREBP1 in that three groups were 1.01 ± 0.16,3.85 ± 1.30,2.82 ±0.57,respectively.The differences between model group and control group or experimental group and model group were statistically significant (all P < 0.05).Conclusion Sirolimus ameolirates TNF-α-induced lipid accumulation in HepG2 cells through inhibiting the mTOR signaling pathway.
7.Effect of traditional Chinese herbs combined with low dose human menopausal gonadotropin applied in frozen-thawed embryo transfer.

OBJECTIVETo assess embryo implantation rate (IR) and pregnancy rate (PR) in women who received Bushen Wengong Decoction (BSWGD), a Chinese herbal formula, combined with low dose of human menopausal gonadotropin (hMG) prior to frozen-thawed embryo transfer (FET).
METHODSA total of 262 subjects (674 transferred embryos) who received FET were analyzed retrospectively. In them, 122 women were under 30 years old, 106 between 30 - 35 years and 32 over 35 years. The 85 subjects with normal ovulation were assigned to Group A, the natural menstruation cycling group, on whom no pre-transfer treatment was applied. The other 177 subjects with abnormal ovulation were assigned to Group B, and subdivided, according to the pre-transfer treatment they received, into three groups, Group B1 (50 cases) received BSWGD, Group B2 (58 cases) received hMG and Group B3 (69 cases) received both BSWGD and low dose hMG. The IR and PR of FET in the four groups were compared, and the effect of the embryo cryotime on PR of FET were compared also. Besides, the influencing factors to FET were analyzed.
RESULTSIR and PR were significantly higher in all age sects of Group B3 than those in Group A, showing significant difference (P< 0.05). IR and PR in subjects in age sects of <30 years and > 35 years in group B3 were significantly higher than those in Group B1 ( P<0.05), but no significant difference was shown in the two parameters between Group B 2 and Group B3 (P>0.05). PR in the subjects who received embryos with cryo-time of > 200 days was significantly lower than that in those with cryo-time of < 100 days ( P<0.05). Embryo cryo-time, endometrial thickness, use of BSWGD and use of hMG were of significance in FET ( P 0.05).
CONCLUSIONA programmed cycle of BSWGD combined with low dose of hMG could improve the embryo IR and PR of FET. Embryo cryo-time, endometrial thickness, and the use of BSWGD and hMG are of significance for FET.

9.Buccal musculomucosal flap for reconstruction of wide vermilion and orbicularis oils muscle defect.

OBJECTIVETo investigate the reconstruction of wide vermilion and orbicularis oris muscle defect with satisfactory outcome of aesthetics, sensation, and function.
METHODSThe buccal musculomucosal flap based on the anterior buccal branches of the facial artery was used to reconstruct wide defect of vermilion and orbicularis oris muscle on upper or lower lip.
RESULTS7 patients were treated. 5 cases had no postoperative complication. Partial mucosal necrosis on the tip of the flaps happened in 2 cases, but the underlying muscle survived and was re-mucosalized spontaneously. No other complication was observed. The sensation of cold, heat and touch could be detected on the first postoperative day. Electromyographic and electron microscopic studies confirmed innervation of the muscle in the flap.
CONCLUSIONSThe buccal musculomucosal flap is a reliable reconstruction option for wide defect of vermilion and orbicularis oris muscle which can' t be reconstructed with conventional method. Satisfactory aesthetic and functional results can be achieved with the buccal musculomucosal flap.

10.Investigation of the sense of competence and depression status among neonatal fathers

Objective To comprehend the sense of competence and depression status for neonatal father and explore the relationship between them in order to provide evidence for the improvement of psychological and physical health. Methods A total of 180 3 days neonatal father had been chosen by convenience sampling method in 5 three-level hospitals in Xi′an. We utilized Edinburgh Postnatal Depression Scale and the Chinese version of the Parenting Sense of Competence Scale ( PSCS) to survey neonatal father by questionnaire and analyzed neonatal father′s sense of competence and depression by Spearman correlation analysis. Results The average score of neonatal father′s PSCS acquired (69. 74 ± 8. 40) while there were 35 (21. 08%) father felt depression. Parenting sense of competence had negative correlation with postpartum depression (r= -0. 510,-0. 507,-0. 326,respectively;P<0. 01). Conclusions The depression of neonatal father is one issue neglected which could impact on parenting sense of competence.
