1.Controversies and focus on surgical treatment of pancreatic cancer

Pancreatic cancer is a common disease with a poor prognosis. Despite recent advance in the field of diagnos-tic technique, surgical resection and adjuvant therapy for pan-creatic cancer, the overall 5-year survival rate is still less than 5%. This is due to its aggressive growth behavior, early local invasion and metastasis, and resistance to chemotherapy and radiation therapy. Surgical treatment is still regard as the only chance for curing pancreatic cancer. Many new strategies in the surgical treatment of pancreatic cancer, including extended lymphadeneetomy, vascular resection, the use of laparoscopy, surgery for metastastic or recurrent disease, and neoadjuvant therapy, are currently under debate. In this review, we discuss the current status of surgical treatment for pancreatic cancer, and highlight the controversies and focus.

Objective To investigate the effects of lipopolysaccharide(LPS) on the expression of hypoxia inducible factor-1?(HIF-1?) and its target gene glucose transpoter-1(GLUT-1)in human monocyte lines THP-1. Methods THP-1 cells were stimulated with 1 ?g/mL LPS for 0,2,4,6 or 8 h.The expression of HIF-1? protein of THP-1 cells was detected by Western blotting,and RT-PCR was employed to detect the expression of HIF-1? mRNA and GLUT-1 mRNA.THP-1 cells were exposed to different concentrations of LPS(0,0.01,0.1 and 1 ?g/mL) for 6 h,and the expression of HIF-1? protein of THP-1 cells was detected by Western blotting. Objective To screen melanocortin 4 receptor (Mc4R) gene mutation by direct DNA sequencing in order to explore the mutation situation of Mc4R gene in simple obese children in Nanjing.Methods One hundred and five simple obese children(obesity group) and 127 healthy children(healthy control group) were examined for mutations of Mc4R gene.Body mass index(BMI)cutoff points for overweight and obesity adopted Chinese children and adolescents,recommended by China Working Group of Obesity,and all children had no other hereditary and metabolic abnormality.Touch-down PCR was performed to amplify the full length Mc4R gene,then direct DNA sequencing was used to analyze the Mc4R gene.The differences of biochemical index levels between obesity group and healthy control group were analyzed ,including alanine transaminase,aspartate transaminas,total protein,albumin,globulin,albumin/globulin,triglyceride,cholesterol,high density lipoprotein,cortisol,insulin and C peptide.Results There were significant differences of biochemical index levels between obesity group and healthy control group,including triglyceride,insulin level after dining 2 h,C peptide and BMI(Pa
5.Clinical and pathological analysis of 34 cases with exaggerated placental site

Objective To summarize the clinical and pathological features of exaggerated placental site (EPS),explore its pathogenesis regularity,diagnosis and treatment strategies.Methods The clinical data related to 34 patients with EPS were analyzed retrospectively.Results In 34 patients,11 patients performed full-term cesarean section,2 patients performed normal vaginal delivery,the other 21 patients had abortion.Thirty-one patients had pregnancy history.Fifteen patients performed hysterectomy,13 patients performed dilatation and curettage,6 patients performed exploratory hysteroscopy and lesions resection.All the patients survived after treatment.Conclusions The patients can not be diagnosed in the antepartum and intrapartum,but can be diagnosed relying on the pathological diagnosis.When it is ineffective to stop bleeding after delivery or abortion by conventional treatment,we should consider the possibility of EPS.Timely perform hysterectomy is theeffective method to stop bleeding,can save the life of patients.If bleeding is not much,curettage or exploratory hysteroscopy can get a significant effective treatment and avoid hysterectomy.
6.DNA typing of HLA-B antigens by DNA chip technique

Objective To discuss the value of medium resolution typing method for HLA-B antigens of Northern Chinese by DNA chip technique.Methods The chip was made with specific medium distinguish typing probes designed according to gene frequency of HLA-B alleles from Northern Chinese.Unsymmetrical PCR was used to amplify HLA-B exon 2 and 3, then labeled PCR products and hybridize with probes on the chip.Certified the typing of HLA-B by analysed and scanned the signals of hybridize through a set of computer software.Results HLA-B alleles were successfully typed in 30 clinical samples. This medium distinguish probes were able to discern 42 HLA-B alleles from the scope of HLA-B 7~83. Using it we can distinguish B14,73 and 82 three new alleles contrast to PCR-SSP methods.Conclusions DNA typing of HLA-B by chip was proven to be a high-resolution and high-specificity method. It is able to check out the multitudinous samples in one DNA chip and it is more suitable for clinical application.
7.The Meta-analysis on efficacy and safety of Eszopiclon and Alprazolam in the treatment of insomnia

Objective To make a systematic evaluation of the efficacy and safety of Eszopiclon and Alprazolam in the treatment of insomnia.Methods By searching the PubMed,Cochrance Library,CNKI, WANFANG,and VIP database,we studied the literature published between 2005 to 2016 on the efficacy and safety of Eszopiclon and Alprazolam in the treatment of insomnia.We collected the randomized controlled trials (RCTs), evaluated the quality of methods and then analyzed the data using RevMan5.3 software.Results A total of 18 RCTs were included,involving 2088 patients.According to Meta-analysis,Eszopiclon group had a significantly higher total efficacy rate [RR=1.07,95% CI (1.02,1.11),P=0.003],lower severity of adverse reactions [MD=-0.43,95% CI(-0.75,-0.12),P=0.008]and incidence of adverse reactions [RR=0.46,95% CI(0.32, 0.66),P<0.0001]than Alprazolam group.However,the two groups did not significantly differ in sleep quality scores after 4-week teatment [MD=-0.05,95% CI(-0.22,0.12),P=0.54].Conclusion Eszopiclon is more effective on insomnia than Alprazolam,with better safety,and it deserves wide clinical use.

9.Expression and correlation of Lewis y antigen and mucin 1 in ovarian epithelial tumor

Objective To investigate the expression and the clinical significance of Lewis y antigen and Mucin 1 (MUC1),as well as to evaluate the correlation between them in epithelial ovarian tumor.Methods The expression of Lewis y antigen and MUC1 in 60 cases of epithelial ovarian malignant tumors,30 cases of borderline ovarian tumors,30 cases of benign ovarian tumors and 20 cases of normal ovarian tissues were detected by immunohistochemical staining.The relationship between Lewis y antigen and MUC1,and their relationship with biology characteristic of ovarian carcinoma were analyzed.An immunofluorescence double labeling methods was performed to detect the correlation between Lewis y antigen and MUC1.Results In malignant epithelial ovarian tumors,the positive rates of Lewis y antigen was 88.33%,which was significantly higher than the positive rates in borderline(60.00%,x2 =9.6405,P <0.01) and benign ovarian tumors(33.33%,x2 =28.8095,P <0.01) and normal ovarian samples (0,x2 =52.3457,P < 0.01).The positive rates of Lewis y antigen had nothing to do with the clinical pathological parameters of ovarian tumor,but the expression intensity of Lewis yantigen was increased with the development of the malignant degree(P < 0.05).The positive rates of MUC1 in malignant epithelial ovarian tumors was also significantly higher than that in borderline,benign ovarian tumors and normal ovarian samples (86.67% vs 53.33%,30.00%,25.00%,x2 =12.0321,29.4064,27.8464 ; P <0.01).And the expression intensity of MUC1 also increased with the development of clinical stage(P <0.01),but had nothing to do with the lymph node metastasis and histological grade(P > 0.05).In ovarian cancer,both Lewis y antigen and MUC1 were highly expressed,and their expression levels were positively correlated (r =0.707,P <0.01),and Lewis y antigen colocalized with MUC1.Conclusion Both Lewis y antigen and MUC1 are associated with the occurrence and development of ovarian cancer.Lewis y antigen and MUC1 might be a sigh of biological behavior in ovarian cancers,and this study provides theoretical evidence of ovarian cancer biological treatment.
10.Recurrent laryngeal nerve injury during total thyroidectomy

Objective To investigate the mechanism of recurrent laryngeal nerve injury during total thyroidectomy.Methods The clinical data of 36 patients suffering from recurrent laryngeal nerve injury in total thyroidectomy from 2003 to 2014 was analyzed retrospectively.Results 21 cases had temporary recurrent laryngeal nerve injury while 16 cases had permanent injury.The cases of injury included giant thyroid goitre,with variation of recurrent laryngeal nerve,undergoing secondary surgery,with tumor invasion and complicating thyroiditis.During the first surgery,the incidence of temporary recurrent laryngeal nerve injury was 0.43%,and the incidence of permanent damage was 0.27%.The temporary and permanent injury incidence of secondary surgery after initial subtotal thyroidectomy was 4.59% and 4.59%,respectively.For patients with grade 3 thyroid gland,that was 1.81% and 0.30%,respectively and 0.51%,0.72% respectively in malignant cases.In cases complicating thyroiditis temporary recurrent laryngeal nerve injury occurred in 1.01%.Conclusions There is increased risk of recurrent laryngeal nerve injury in giant goiter cases undergoing secondary operations.Hence it is suggested that initial surgical procedure be hemithyroidectomy or total thyroidectomy
