Objective To explore the effects of recombined human growth hormon-enhanced early enteral nutrition on healing of the patients after operation for sereve abdominal trauma.Methods 60 patients with severe abdominal trauma were randomly divided into PN(n=20),EN(n=20) and EN+ rhGH(n=20) groups.Nutrient status and immune function of 3 groups were detected on the postoperative day 1,7 and 10.Results EN+ rhGH group showed more effective process than EN and PN groups in improving postoperative nutrient status,immune function and nitrogen balance,whose indexs except CD8 changed significantly on the postoperative day 7 and 10 than that in PN group(P

Overnutrition during the early postnatal life,a critical time window for growth and development,may induce metabolic syndrome later in life,including overweight/obesity and insulin resistance.The important target organs of insulin,such as liver,adipose tissue,skeletal muscle,and central nervous systems show insulin resistance.The involved mechanisms include abnormality of insulin signal pathway,increment of free fatty acid and some adipocytokines,oxidative stress,maladjustment of orexigenic and anorexigenic neuron,modifications of the hypothalamic-pituitary-adrenal glucocorticoid axis as well as epigenetic,etc.Hence,overnutrition should be avoided during the early postnatal life,so as to decrease the risk of developing long-term insulin resistance.

Objective:To express hTRAIL_ 41~281 protein using E.coli and refold it into a functional form.Methods:The expression of protein was induced by IPTG,protein was purified by Ni-NTA chromatography column,and refolded by dialysis ,protein purified was determined by SDS-PAGE and Western blot.Antitumor activity of hTRAIL_ 41~281 protein was measured by DNA fragmentation electropherogram.Results:Results of SDS-PAGE and Western blot proved that the 30.5 kD protein purified were hTRAIL_ 41~281 protein,the purity of protein was more than 90%.After refolding,DNA fragmentation electropherogram showed that hTRAIL_ 41~281 protein had good antitumor activity.Conclusion:hTRAIL_ 41~281 protein with antitumor activity was successfully expressed with E.coli and purified by Ni-NTA chromatography column,refolded by dialysis.
4.Analysis of Correlation Factors of Locally Recurrent Gastric Carcinoma.

Objective To discuss the correlation factors of locally recurrent gastric carcinoma. Methods Clinical, pathological and periodical follow-up datas of 50 cases with locally recurrent gastric carcinoma after radical resection from 1991 to 2005 were studied retrospectively, logistic regression analysis was used to disscuse the possible correlation factors of locally recurrent gastric carcinoma. Results 66.0% of 50 cases had recurrenced within 2 years after operations; Tumor pathological stages, positive lymph nodes and radical resection extent were independent correlation factors of locally recurrent gastric carcinoma. Conclusions Most recurrences would happen within 2 years after radical resection for gastric carcinoma; Early diagnosis of reccurrent gastric carcinoma depends on periodic follow-up observation especially gastroscope; Tumor pathological stages, positive lymph nodes and radical resection extent are independent correlation factors of locally recurrent gastric carcinoma.
5.Diagnosis and Treatment of Gastroparasis Syndrome After Abdominal Operation

Objective To explore the diagnosis and treatment of gastroparasis syndrome after abdominal operation. Methods Clinical data of 25 cases with gastroparasis syndrome after abdominal operation from Jan 1995 to June 2006 were analysed retrospectively. Results Gastrectomy and nongastrectomy amounted to 16 and 9 cases( portal-azygous vein disconnection in 4 cases, panceaticoduodenectomy in 1 case, partial small intestinectomy and adhesions lysis in 1 case, radical excision of colon carcinoma in 2 cases, choledocholithotomy in 1 case). Gastroparasis syndrome characterized by upper abdominal distension, nausea, vomiting after clipping the pipe or large quantity of gastric drainage after operations. Gastrointestinal series and gastroscopy confirmed functional delayed gastric emptying. All cases recoverd through non-operative therapy including fasting, continuous gastrointestinal decompression, total parenteral nutrition or enteral nutrition, adn administration of gastro-intestinal dynamic medicine. Conclusions Gastroparasis syndrome is the common complication after abdomianl operation, gastrointestinal series, gastroscopy are main diagnostic methods, conservative medication obtains good effect.
6.Research of Early Using Glutamine-enriched Enteral Nutrition in Short Period on Patients with Gastric Neoplasms After Total Gastrictomy

Methods 36 patients after total gastriecomy were randomly divided into EN(n=18) and GLN(n=18) group. Compared the index after total gastrectomy including nutrient status, immune function and lassitude degree in the 1st, 3rd, 7th day. Results GLN group showed more effective process than EN group in improving postoperative nutrient status, immune function, nitrogen balance and lassitude degree. Objective To analyze blood-CSF barrier and cerebrospinal intrathecal IgG synthesis in patients of multiple sclerosis, and explore the Protis software, IgG index and IgG synthesis rate in the and evaluation for cerebrospinal intrathecal IgG synthesis and blood-CSF barrier dysfunction. Methods 134 patients with neurological diseases were divided into threes groups, including multiple sclerosis group (34 cases), disease control group(80 cases) and control group(20 cases). IgG and ALB concentration of CSF and blood was detected by BN-Ⅱ. The oligoclonal IgG was detected by immunofixation electrophoresis. The relationship between Protis software results and IgG index or IgG synthesis rate was calculated and analyzed. Results The function of blood-brain barrier was almost normal in multiple sclerosis group,QALB was 4.5×10-3[(3.1-7.6)×10-3], there was no significance compared with control group (D=5.25, P>0.05). there were 31 patients with oligaclonal bands in cerebrospinal fluids in multiple sclerosis group (34 patients) with the positive rate of 91.2%. In multiple sclerosis group the intrethecal IgGIF, IgG index and IgG synthesis rate of 24 hours were 31.25% (11.65%-71.45 %), 0.93% (0.80%-1.04%), 24.25 mg/24 h(15.25-46.15 mg/24 h) ,the results were all higher than that in control group (D=175.5, 112.5,103.4,P<0.05). And in multiple sclerosis group there was no significant difference among the intrethecal IgGIF, IgG index and IgG synthesis rate of 24 hours, the positive rate of oligaclonal bands (P>0.05). In disease control group(80 patients) QALB Was 35.2×10-3[(18.5-55.5)×10-3] ,the result was significantly higher than control group (D=102.7, P<0.05), the oligaclonal bands were all negative, IgG index and IgG synthesis rate of 24 hours were 0.75(0.69-0.82) ,44.29 mg/24 h(20.35-65.98 mg/24 h) were both higher than control group(D=85.6,98.5,P<0.05). In disease control group the positive rate of intrathecal IgGIF, IgG index and IgG synthesis rate of 24 hours were 0 (0/80), 40.0% (32/80), 72.5% (58/80) respectively, IgG index and its positive rate both increased alone with the lesion degree of blood-brain barrier increasing (P<0.05). And there was similar trend in the IgG synthesis rate of 24 hours. There were significant correlations among intrathecal IgGIF, IgG index and IgG synthesis rate of 24 hours in multiple sclerosis group (r=0.788, 0.695 ,P<0.05). However there was no correlation among intrathecal IgGIF, IgG index and IgG synthesis rate of 24 hours in disease control group (r=0.000,P>0.05). Conclusions The Protis software can be used to analyze the intrathecal IgGIF and reflect the actual status of intrathecal IgG synthesis more accurately. Protis software has great application value for clinical diagnosis for nervous system disease.
9.Assessment of coronary artery disease with the nitroglycerin augmented ~(99m)Tc-MIBI and ~(201)Tl dual-isotopic myocardial perfusion imaging

Objective To assess the clinical value of dobutamine 201Tl stress-redistribution/nitroglycerin augmented ~(99m)Tc-metho~(99m)Tc-xyisobutylisonitrile (MIBI) gated myocardial perfusion imaging (MPI) in patients with coronary artery disease (CAD). Methods A total of 132 patients with suspected CAD underwent dobutamine 201Tl stress-redistribution/nitroglycerin augmented MIBI gated MPI and coronary arteriography (CAG) within two weeks after MPI. Percutaneous coronary interventions (PCI) were performed in 23 patients with myocardial infraction. The regional wall movement was assessed with echocardiography before and within three months after PCI. Results Taking stenosis ≥50% as standard of CAD, the sensitivity, specificity and accuracy of dual-isotopic SPECT imaging in diagnosing CAD was 93.41%, 87.80% and 91.67%, respectively. The sensitivity, specificity and accuracy in diagnosing left anterior descending artery (LAD) stenosis was 93.85%, 91.04% and 92.42%, for left circumflex artery (LCX) was 86.79%, 89.87% and 88.64%, while for right coronary artery (RCA) was 81.25%, 82.14% and 81.82%, respectively. Of all 207 myocardium segments in 23 patients with myocardial infarction, radioactivity defect was found in 113 segments on the redistributed ~(201)Tl images, and radiofilling was found in 52 (52/113) segments on the nitroglycerin augmented ~(99m)Tc-MIBI images, whereas 61 segments had no radiofilling. Taking regional wall movement improvement as the criterion of viable myocardium, the sensitivity, specificity and accuracy of identifying viable myocardium with nitroglycerin augmented ~(99m)Tc-MIBI imaging was 87.23%, 83.33% and 84.96%. Conclusion Dobutamine ~(201)Tl stress-redistribution/nitroglycerin augmented ~(99m)Tc-MIBI gated MPI is able to effectively detect CAD and viable myocardium and helpful in diagnosis and treatment of CAD.

This paper summarizes the features of web sites that would be useful to infectious diseases physicians by exploring the Internet through search engine including Google,Baidu and Yahoo.Meanwhile,suggestions from professional forums,web sites and publications are also taken into consideration.Nine Comprehensive sites containing three categories and more of microbial pathogens,nine special sites for parasites,four special sites for fungi,two special sites for viruses and two special sites for bacteria are collected.Subjective navigation for each site is given.Features of these sites,including laboratory images,clinical images and number of images are also described.
