1.Clinical effect of self made Jianpi Yiqi Xiaoyu decoction combined with FMC chemotherapy for treatment of elderly patients with moderate to advanced gastric cancer: results of a randomized controlled study

Objective To discuss the clinical effect of self made Jianpi Yiqi Xiaoyu decoction combined with FMC chemotherapy for treatment of elderly patients with moderate to advanced gastric cancer and its impact on their quality of life.Methods One hundred and two elderly patients with moderate to advanced gastric cancer accompanied by the Chinese medicine dialectical syndrome belonging to deficiency of spleen and stomach qi and blood stasis admitted to Zhengzhou Traditional Chinese Medicine Hospital from January 2013 to January 2016 were enrolled,they were divided into observation group and control group according to random number table method and 2 patients who missed the follow-up in each group were excluded,resulting in 49 patients in each group;the patients in the control group were treated with FMC chemotherapy [including mitomycin 15 mg/m2,cisplatin 60 mg/m2 and fluorouracil 1000 mg/m2,slow intravenous drip (drop) after dilution with 60 mL 0.9% sodium chloride injection];the patients in observation group were treated with FMC chemotherapy combined with Jianpi Yiqi Xiaoyu decoction (consisting of astragali 30 g,codonopsis pilosula 20 g,atractylodes macrocephala 15 g,Chinese yam 15 g,hoelen 15 g,ligustici 10 g,moutan cortex 10 g,aurantii 10 g,rehmannia dride rhizome 10 g,caulis dendrobii 10 g,500 mL water was added to make a decoction,boiled and concentrated to 200 mL decoction,100 mL taken in the morning and another 100 mL in the evening orally in one day).The clinical efficacy,the change of quality of life and incidence of adverse reactions were compared between the two groups.Results The proportion of patients whose quality of life improved in the observation group was significantly higher than that in the control group [55.10% (27/49) vs.16.33% (8/49),P < 0.05].The total effective rate was obviously higher in the observation group than that in the control group [51.02% (25/49) vs.22.45% (11/49),P < 0.05];the incidence of reduction of hemoglobin [30.61% (15/49) vs.63.27% (31/49)],aleucocytosis [28.57% (14/49) vs.67.35% (33/49)],thrombocytopenia [28.57% (14/49) vs.69.39% (34/49)],nausea and vomiting [44.90% (22/49) vs.71.43% (35/49)],diarrhea [32.65% (16/49) vs.63.27% (31/49)] and neurotoxicity [34.69% (17/49) vs.55.10% (27/49)] were significantly lower in observation group than those in the control group (all P < 0.05).Conclusions Self made Jianpi Yiqi Xiaoyu decoction combined with FMC chemotherapy can significantly improve the clinical efficacy of theelderly patients with moderate to advanced gastric cancer.It can improve the patients' quality of life and reduce the incidence of adverse reaction of chemotherapy.

Objective To observe the clinical efficacy and safety of 5% imiquimod cream in topical treatment of anogenital warts. Methods A randomized, double-blind, parallel placebo-controlled clinical study was conducted. Patients with anogenital warts were instructed to apply the test drug topically and then clean the drug with water 6 ~ 8 hours later, three times a week for 8 weeks. Patients whose warts cleared completely were followed up for one month to determine recurrence rates. Results Two hundred and thirty-one patients with anogenital warts were enrolled in this trial. One hundred and sixteen patients were randomly selected to receive 5% imiquimod cream; and the other receive placebo cream. 3.Insulin sensitivity and beta cell function in female systemic lupus erythematosus patients

Objective To investigate insulin sensitivity and beta cell function in female systemic lupus erythematosus (SLE) patients with different glucose tolerances. Methods Insulin sensitivity and beta cell function were compared between SLE patients and non-SLE subjects in the states of normal glucose tolerance (NGT), impaired glucose tolerance (IGT)and diabetes mellitus (DM) respectively.Furthermore, risk factors for insulin sensitivity and beta cell function in SLE patients were analysed by linear regression. Results In NGT state, insulin sensitivity and beta cell function of newly diagnosed SLE patients without glucocorticoids treatment were not significantly different from those of normal control group ( P <0. 05). Compared with newly diagnosed SLE patients without glucocorticoids treatment and normal control group, HOMA insulin resistance index (HOMA-IR) , In (HOMA-β), In (early phase insulin secretion index, EISI ) and In ( late phase insulin secretion index, LISI ) of SLE patients with glucocorticoids treatment were significantly higher( 1.91 ± 1.04 vs 0. 81 ±0. 75,0. 94 ±0. 27;5.05 ±0. 65 vs 4. 01 ±0. 63,4. 23 ±0.47;3. 14±0.81 vs 2.42 ±0.39,2.50±0.65;2.30 ±0.55 vs 1.62 ±0.57,1.56 ±0.43;P <0.05),while In ( Matsuda index, MI ) was significantly lower ( 4. 53 ± 0. 54 vs 5. 27 ± 0. 68,5. 18 ± 0. 38; P <0. 05). In IGT and DM state, HOMA-IR (2. 84 ± 1. 87 vs 1.82 ± 1.22, 3. 18 ±2. 29 vs 2. 94 ±2. 26) and In (HOMA-β) (5. 18 ±0. 93 vs 4. 06 ±0. 58, 3. 99 ± 1.04 vs 3.43 ±0. 83) were significantly higher in SLE patients with glucocorticoids treatment than those of non-SLE subjects ( P < 0. 05 ) respectively. BMI and In (daily glucocorticords doses) were independent risk factors for insulin sensitivity, and age, the SLE disease activity index(SLEDAI) and In(daily glucocorticords doses) were related factors beta cell function.Conclusion In NGT, IGT and DM state,SLE female patients with glucocorticoids treatment have reduced insulin sensitivity and increased beta cell function, these changes are related to the use of glucocorticoids.
