1.HOW TO EXPLORE THE HIGH EFFECTIVE PROVED RECIPE OF CHINESE MEDICINE IN INTEGRATING TRADITIONAL AND WESTERN MEDICINE

with the aim of how to explore the high effective proved recipes of chinese medicine, our research laboratory tried to propose that one may find out the corresponding diagnosis in term of western medicine as precisely as possible on the basis of the leading symptoms or leading signs which could be afforded by the case report on ideal cure with traditional medicine, and then correlate this diagnosis regularly with the high effective treatment. Therefore, not only the rich data about such diagnosis may be used to extend knowledge of the disease under study, but also a high effective recipe specific to the recorded disease can be obtained at first, then tested in practice. This paper describes two practical examples to illustrate the analytic Processes. One was to explore both acupuncture method and medical prescription for curing the rheumatoid arthritis complicated by subcutaneous nodules, and the other to identify the yam medical drink for treating the phosphaturia coming from the constitutional anomaly.
2.SDS-PAGE for foot proteins of Semisulcospira in Zhejiang Province

Objective To understand the biochemical characters of the intra-species of Semisulcospira and importance of paragonimiasis transmission. Methods The foot proteins of six species of Semisulcospira in Zhejiang Province were studied and compared by SDS-PAGE. Results 17-20 protein bands were found with molecular weight 27-162 kDa. Two strong bands,93 kDa and 52 kDa, were the same among the six species of Semisulcospira. The similitude coefficient was 0.74-0.85. Conclusion Two strong bands,93 kDa and 52 kDa, would be the protein markers of Semisulcospira, which may be related to the transmission of paragonimiasis.[
3.CONTRIBUTIONS TO THE SKELETAL MUSCLE LESION AND ITS REPARATION

The skeletal muscle materials sectioned from 20 cases of patients were histologically examined, who had all died of Kerschen disease. In addition to the simple atrophy and the hyaline degeneration which are generally regarded as lesions often found by autopsy of Kerschen disease, special types of muscle damage and reparation were noticed and described in the musculus gastrocnemius in 6 cases out of them, namely the myofibrillar dissociation of the muscle fibers and the connective tissue growth extending from the periphery to the internal part of the individual muscle fiber. The dissociated and even the necrotic muscle fibers could be distinctly limited within the area of the foci, while beyond the edge of the foci muscle fibers still remained almost unchanged. In the case of reparation the mesenchymatous elements in some region displayed their active action of ground substance production on the surface of the damaged fibers. But it was more noteworthy that a rare phenomenon happened: connective tissue growth got increased from outside into inside of the individual muscle fibers and at the same time it substituted for the position of the lost muscular substance. Thus in the cross section of a single complete muscle fiber muscular mass and connective tissue including its blood capillaries did stand side by side with an obvious contrast after the staining. This evidence naturally leads to the suggestion that a process of sarcolemmolysis may occur and allow the continuity of the connective tissue between the exterior and interior of a single muscle fiber. The disbanding of myofibrils within focus can also be explained by lysis of the sarcolemma and of its invagination, the transverse tubule system. In fact, it has not to be suggested, but was virtually revealed in electron microscopy, that sarcolemma of the damaged muscle fiber was lost. In place of the lost myofibrils a filamentous network was produced, which contained perhaps collagen stainable with aniline blue in Mallory's trichrome methode used for light microscopy. Theory of metaplasia about the reparation after the skeletal muscle lesions is however not supported by our observations. Besides, no regular correlation can be established between these special pathological changes of skeletal muscle and the Kerschen disease.
4.Changes of melatonin and cellular immunological function in children with febrile seizures and its clinical significance

Objective To investigate the changes of melatonin and cellular immunological function in children with febrile seizures and its clinical significance. Methods 50 children, including 23 cases with complex febrile seizure (CFS) and 27 cases with simple febrile seizure (SFS) , and 25 cases with upper respiratory infections children selected as control group were enrolled in this study. Serum melato- nin was measured by enzyme-linked immunosorbent assay (ELISA) and cellular immunological function was measured by flow eytomcter. Results The levels of serum melatonin in the 3 groups of CFS, SFS, control were(14. 91±2. 61) ng/L, (20. 72±2. 54) ng/L, (23.93± 2. Ol) ng/L, respectively. The melatonin levels in CFS children were significantly decreased than that in control group and SFS children (P <0. O1). CD3 + ,CD4 +, the ratio of CD4 + /CD8 + and CD8 + in CFS group were significantly decreased than that in control group and SFS group (P <0.01). The ratio of CD4 +/CD8 + in SFS group was significantly decreased than that in control group (P <0.05), but CD3 + ,CD4 + and CD8 + had no statistics significance among these groups(P >0. 05). The serum rnelatonin level were positive related withdecreaseddegreeofCD3+,CD4+ andtberatioofCD4+ /CDS+ (r≥0. 472, P <0.05). Conclusion The disorder cfcellular immunological function was possible related with the loss of serum melatonin, and the loss of serum melatonin maybe one of the reasons for febrile seizures relapse and brain injured.

6.A Study of Correlation Between Serum Leptin and Insulin Resistance in Patients with Type 2 Diabetes

Objective To study the association of sex, body mass index and insulin resistance with serum leptin in patients with type 2 diabetes. Methods one hundred and eight patients were divided into two groups according to BMI: diabetic obese group and diabetic normal BMI group.Their weight and height were measured. Fasting plasma glucose(FPG),fasting true insulin(FTI),postprandial true insulin(PTI),and fasting serum leptin(LEP) were also measured. ISI=1/(FPG?FTI) as insulin sensitivity index were used to analyse the relation between leptin and insulin resistance. Results ⑴The serum leptin,body mass index,FTI and PTI were significantly higher in diabetic obese group than diabetic normal BMI group. ISI was significantly lower in diabetic obese group than diabetic normal BMI group. ⑵Female serum leptin levels were significantly higher than male serum leptin levels in diabetic obese group and diabetic normal BMI group. ⑶Fasting leptin was correlated with sex. Leptin was positively corrected with body mass index,FTI and was negatively corrected with ISI. Serum leptin wasn't correlated with FPG. Conclusin Serum leptin level is associated with sex,BMI and insulin resistance in patients with type 2 diabetes.
7.Clinical analysis of valproic acid combined with all-trans retinoic acid in the treatment of myelodysplastic syndrome

Objective To evaluate the efficacy and safety of valproic acid combined with all-trans retinoic acid(ATRA) in the treatment of myelodysplastic syndrome. Methods Twenty-two patients with MDS consisting of 4 RA, 1 RARS, 10 RCMD, 1 RCMD-RS, 3 RAEB-1 and 3 RAEB-2 were analyzed. The initial dose of valproic acid was 0.6 g/d, then increased to 1.2 g/d after one week; all-trans retinoic acid was administered at 30 mg/d every other week after taken VPA one week later. All patients maintained treatment for 3 months unless severe side effect was found or disease progression. Bone marrow examination was taken every 4 weeks. Results The response rate was 27.3 % (6 cases) according to International Working Group (IWG) criteria, no patient achieved completely remission(CR), 2 cases achieved partial remission(PR), 4 case achieved hematologic improvement(HI), 9 cases was stable and 7 cases was failure. Most of these cases had slight adverse events and could be tolerated. Conclusion Valproic acid combined with all-trans retinoic acid in the treatment of patients with myelodysplastic syndrome was efficient and side effect was tolerable.
8.Velocity vector imaging in the evaluation of mechanical vasomotor

Velocity vector imaging (VVI) is a promising and relatively new noninvasive method of assessing wall movement and flexible features of vessels,being able to qualitatively, quantitatively, accurately and intuitively assess mechanical characteristics of vascular wall movement. VVI can reflect changes of characteristics of flexibility and movement of carotid artery in early atherosclerosis and plaque rotation angle. It provides a new way for the study of unstable plaque. The research and current status of clinical application of VVI were reviewed in this article.
9.Study on relationship between serum insulin-like growth factor 1 level and children with tourette syndrome

Objective To explore the relationships between serum insulin-like growth factor 1 (IGF-1) level and children with tourette syndrome, and the significance of IGF-1 level for the diagnosis of tourette syndrome.Methods Enzyme-linked immunosorbent assay (ELISA) was adopted to detect the serum level of IGF-1 and S100B of 53 children patients with tourette syndrome and 53 children with physical examination.The receiver operating characteristic (ROC) curves were used to determine the cut-off value and assess the sensitivity,specificity and diagnostic efficiency.Results The concentration of IGF-1 was significantly lower in the children with tourette syndrome group than in normal group ((41.51±4.23) μg/L vs.(69.04±5.26) μg/ L,t=29.693,P<0.01),and the concentration of S100B was significantly higher in the children with tourette syndrome group than in normal group((129.48±9.54) ng/L vs.(81.37±8.42) ng/L,t =27.526,P<0.01) .When 59.28 and 93.35 ng/mL ware used as cut-off value, the sensitivity, specificity and area under curve for IGF-1 in diagnosis of children with tourette syndrome were 89.6%, 96.4% and 0.931, respectively.The sensitivity,specificity and area under curve for S100B in diagnosis of children with tourette syndrome were 94.1%, 91.3% and 0.969, respectively.And the diagnostic efficiency of IGF-1 was similar to that of S100B.Conclusion IGF-1 may play an important role in the development of tourette syndrome.Detection of IGF-1 in the serum is helpful in the diagnosis of children with tourette syndrome.
10.Value of 2 dimension color Doppler ultrasound in surgical and interventional treatment of carotid artery stenosis

Objective:To systematically assess the value of 2 dimension color Doppler ultrasound(2D-CDUS) in diagnosis,surgical treatment and interventional therapy of carotid artery stenosis. Methods: Twenty seven carotid stenosis patients were examined before and after operation with 2D-CDUS. The morphology and hemodynamic of carotid artery were detected. Results: Pre-operative examination showed that there were 17 cases with stenosis degree being 50%-70%, 2 cases 71%-79%, 5 cases 80%-90%, 3 cases 90%-99%. Twelve cases were operated on with carotid endarterectomy(CEA), 15 cases with percutaneous transluminal angioplasty and stent(PTAS). After operation all the vessels with CEA were patent, colored well and blood velocity was close to normal. Residual intima was observed floating in the blood vessel in only one patient. In the patients treated by PTAS, the location and courser of the stents were normal,and the vessels were patent. Conclusion: 2D-CDUS is convenient to use and non-invasive, which can assess extracranial carotid stenosis accurately before operation, assess the curative effect and detect the complications after operation.
