1.Diagnostic Value of Spiral CT Scanning and CT Virtual Endoscopy for Gastric Carcinoma

Objective To evaluate spiral CT scanning and CT virtual endoscopy(CTVE) in the clinical diagnosis of gastric carcinoma.Methods Spiral CT scan was performed in 28 patients with pathologically-proved gastric cancer . The CT scan parameters were : collimating width 5 mm , Pitch 1 . The volumetric data were acquired using a spiral CT scanner and transferred to computer workstation . CTVE was performed by navigator smooth function . The results were compared with the findings of fiberoptic endoscopy and/or surgery .Results Spiral CT scan could demonstrate the location ,size,extension of lesions and the relation with organs around .In 28 patients,CT findings included omentum involvement(n=7),pancreatic ivolvement (n=4),hepatic involvement (n=3),lymph node metastasis(n=3). CTVE could clearly demonstrate the three-dimensional image of normal wall stomach and the lesions , size and extension of lesions . The appearances of the inner lesions and normal wall of stomach on CTVE were similar to those on fiberoptic endoscopy and/or surgery . The accuracy in detecting lesion by CTVE was 92.8% in 28 patients . Conclusion CTVE combination with spiral CT scan is of great value in clinical diagnosis of gastric carcinoma .
2.Diagnostic Value of Spiral CT and 3D Reconstruction in Pulmonary Tuberculosis

Objective To evaluate the diagnostic value of spiral CT and all kinds of 3D reconstruction in pulmonary tuberculosis .Methods 145 cases of pulmonary tuberculosis confirmed by clinical study and histopathology were analyzed retrospectively. All cases weregiven to helical scan and different cases were given to different 3D reconstruction (CTVE, MPR and CPR) .Results Spiral CT (HRCTespecially) could find many signs of pulmonary tuberculosis, such as centrilobular distribution nodules, tree-in-bud, cavitations,tuberculous pneumonia, miliary nodules, branchogenic dissemination, endobranchial tuberculosis, lymph node enlargement and calcification. The accuracy of spiral CT in the diagnosis of pulmonary tuberculosis was 96.6%, The accuracy of spiral CT with 3Dreconstruction in the diagnosis of pulmonary tuberculosis was 99.31%.Conclusion Pulmonary tuberculosis can be diagnosed accurately by spiral CT with all kinds of 3D reconstruction.

Objective To evaluate the correlation of epicardial adipose tissue volume (EATV)with coronary plaques in patients with a coronary artery calcium score of zero.Methods 183 patients with a coronary artery calcium score of zero were selected.They were divided into plaque group and control group according to the findings of CT coronary angiography.Independent t test was used to analyze the difference of EATV between two groups.Results ①EATV was significant higher in plaque group than that in control group (P <0.05).②EATV was non-significant higher in plaque group than that in control group for female individuals (P >0.05), while it was significant higher in plaque group than that in control group for male individuals (P <0.05).③EATV was significant higher in plaque group than that in control group for the individuals with age< 50 years (P <0.05 ),meanwhile it was significant higher in plaque group than that in control group in age≥50 years(P <0.05).Conclusion EATV is correlated with coronary plaques in male patients with a coronary artery calcium score of zero while there is no correlation with female patients.EATV is correlated with coronary plaques in different age patients with a coronary artery calcium score of zero.
4.Application of pathological diagnosis by rapid paraffin sections for biopsy in the diagnosis and treatment of cervical diseases

Objective To evaluate the application of pathological diagnosis by rapid paraffin sections in the diagnosis and treatment of cervical diseases. Methods A total of 176 cases from our hospital between September 2009 and January 2010 with abnormal cervical cancer screening (including abnormal cytology result and high-risk HPV continuous positive) were randomly divided into 2 groups. Eighty-seven cases of them whose biopsy were got by Belinson forceps under the direction of colposcopy with rapid paraffin sections by ultrasonic histopathological rapid processor and BT transparent agents were selected as group A, while 89 cases with conventional paraffin sections were selected as group B. The production time and quality for paraffin sections were analyzed in the two groups. Those diagnosed as cervical intraepithelial neoplasia (CIN)Ⅱ or even worse and some special patients with CIN Ⅰ in the two groups received surgery, including loop electrosurgical procedure (LEEP) ,cold knife conization (CKC),.hysterectomy or radical hysterectomy.Tissue obtained after surgery was sent for routine pathological examination. If the results of postoperative routine pathological examination were inconsistent with the rapid or routine biopsy pathological examination,the heavier results were regard as the final diagnoses. The pathological results and diagnose accordance rates were recorded and compared between group A and group B. Results The quality of sections in two groups were all satisfied or basically satisfied to meet the diagnostic requirements. There were statistically significant difference in average production time between group A and B (40 minutes vs 24 hours, P < 0. 05). Thirty patients in group A and 32 patients in group B received surgery. The coincidence rate of biopsy pathological results and final diagnoses were 93% (28/30) for group A and 91% (29/32) for group B, in which there were not statistically significant difference (P > 0. 05). Conclusion Bapid paraffin sections technology is safe, accurate and economical for rapid pathological diagnosis of cervical diseases, which is worthy for being widely used in hospitals.
5.The relationship between the second victim experience and support and organizational support in clinical nurses: the mediating role of psychological capital

Objective:To explore the status quo of the second victim experience and support in clinical nurses, and analyze the mediating effect of psychological capital between organizational support and the second victim experience and support.Methods:From March to April 2020, totally 264 clinical nurses from 2 Class Ⅲ Grade A hospitals in Zhejiang Province were selected by convenient sampling and investigated with the general information questionnaire for nurses, Nurse Psychological Capital Questionnaire, Organization Support Scale and Second Victim Experience and Support Scale. Totally 264 questionnaires were distributed, and 259 valid questionnaires were collected, accounting for an effective recovery rate of 98.11%. SPSS 19.0 was used for statistical analyses.Results:The total scores of second victim experience and support, psychological capital and organizational support of the 259 clinical nurses were (32.73±8.15) , (83.49±9.74) and (40.23±9.88) , respectively. Correlation analysis showed that the total score of organizational support of clinical nurses was negatively correlated with second victim experience and support ( r=-0.404, P<0.01) , and the total score of positively correlated with psychological capital ( r=0.417, P<0.01) , and the total score of psychological capital was negatively correlated with second victim experience and support ( r=-0.365, P<0.01) . The mediating effect analysis revealed that the mediating effect of psychological capital between organizational support and the second victim experience and support was significant ( P<0.01) , and the mediating effect accounted for 40.29% of the total effect. Conclusions:The second victim experience and support level of clinical nurses is relatively high. Psychological capital can account for some relationship between organizational support and the second victim experience and support. The second victim experience and support levels in clinical nurses with a high organizational support after facing adverse events in patients are low, which has a direct negative effect. Medical institutions should establish an organizational support system centered on improving the negative state of the second victim, the non-punitive management system for adverse events as well as the responsibilities and content of the professional psychological counseling team, in order to effectively respond to the adverse effects of patients' adverse events on clinical nurses as the second victim, help them adopt active coping methods, and improve their psychological capital levels, thereby improving the quality of clinical nursing services.
