2.Anesthetic management for clipping giant intracranial basilar artery aneurysm under deep hypothermic circulatory arrest

Objective To study the performance of deep hypothermic circulatory arrest and the anesthestic management for giant intracranial basilar artery aneurysm. Methods Seven patients undergoing resection of giant intracranial basilar artery aneurysm were included in the study. By monitoring cerebral oximetry (rSO_2), body temperature and electrocardiogram (ECG), brain and other vital organs were guaranteed to maintain oxygen supply/demand balance in perioperative management, especially during the circulatory arrest period under deep hypothermia. The measured parameters, the outcome of patients and the complications were observed and recorded. Results Even in profound hypothermic condition, rSO_2 was degraded significantly during circulatory arrest, and recovered after in circuit. Low flow perfusion maintained cerebral oxygen supply/demand balance better than circulatory arrest. In all 7 cases, cerebral anoxia caused by circulation arrest after operation were not observed. Conclusion Deep hypothermic circulation arrest could be applied effectively in the resection of giant intracranial basilar artery aneurysm with brain protective measure and monitoring. To improve the outcomes of the patients, avoiding cerebral anoxia and of local vessel injuries is important.

4.Experimental study of rabbit liver with contrast gray-scale imaging-doses and effects

Objective To study the relationship between doses of ultrasound contrast agent(UCA) and contrast-enhancement effects of gray-scale imaging in rabbit liver. Methods Ultrasound contrast agent SonoVue TM in eight different doses ( 0.02 - 0.16 ml/kg) was applied respectively in five normal rabbits in randomized order as a bolus in a marginal ear vein followed by a 1.5 ml saline flush. Corresponding parameters of the time-intensity curve: time to enhancement(ET),time to peak intensity(PIT),peak signal intensity(PSI),mean transit time(MTT) and area under the curve(AUC) were measured using low mechanical index contrast gray-scale imaging and Wash-in/Wash-out time intensity curve software package. Results Along with an increase of the SonoVue TM dose,the MTT and the AUC increased exponentially,and the ET decreased exponentially. The PSI of portal vein and liver parenchyma increased only in the dose range from 0.02 ml/kg to 0.10 ml/kg ( r = 0.89 and 0.92 ,respectively),and decreased with a further increase of the UCA dose. A possible explanation for this effect is the bubble saturation and attenuation. No significant correlation was found between the UCA dose and the PIT( r = -0.35 and 0.22 ,respectively). Conclusions The parameters of time-intensity curve are influenced intensely by different UCA dose. It is important to grasp the relationship between the UCA doses and contrast-enhancement effects for microcirculatory quantification. The 0.10 ml/kg appears to be the optimal dose for tissue perfusion studying because it significantly improved videointensity and duration of enhancement and did not result in the bubble saturation and attenuation.
5.Surveillance of Antimicrobial Resistance of Commonly Encountered Nonfermentative Gram-negative bacilli

OBJECTIVE To investigate the isolation and antimicrobial resistance of nonfermentative Gram-negative bacilli(NFGNB) in order to provide laboratory evidence for clinically selecting antibiotics.METHODS All isolates were identified by routine procedure and DADE BEHRING MicroScan auto-system,and K-B paper disk diffusion in accordance with NCCLS standards was employed for drug sensitivity tests.RESULTS There were 273 strains of NFGNB isolated from Feb 2005 to Aug 2006,mainly from sputum,middle urine and secretion. The main NFGNB were Pseudomonas aeruginosa,Acinetobacter baumannii and Stenotrophomonas maltophilia.The resistance rates of P.aeruginosa and A.baumannii to imipenem/cilastatin,cefoperazone/sulbactam and cefepime were less than 13.6%,11.9% and 25.4%;to amikacin and piperacillin/tazobactam were 31.1% to 37.8%.The resistance rate of S.maltophilia was the highest among the NFGNB.Its resistance rates to imipenem/cilastatin showed 100%,to cefoperazone/sulbactam,levofloxacin and compound sulfamethoxazole were less than 14.3%.CONCLUSIONS The NFGNB are the important pathogens causing clinical infection,especially P.aeruginosa,A.baumannii and S.maltophilia.There are significant differences in antimicrobial resistance among different NFGNB.
6.Clinical profile and characteristics of serum GP73 in cirrhotic patients with chronic hepatitis B infections

Objective To explore clinical profile and significance of serum golgi protein 73 (GP73) in cirrhotic patients with chronic hepatitis B virus (HBV) infection .Methods A total of 109 cirrhotic patients with HBV infection were included ,in which 60 patients were compensated cirrhosis ,and 49 patients were decompensated cirrhosis .The receiver operating characteristic curve (ROC) analysis were used to evaluate the clinical significance of GP73 for diagnosing decompensated cirrhosis in cirrhotic popula-tion .Results Serum GP73 level in cirrhotic patients was correlated with alanine aminotransferase (ALT ) and HBV DNA ,but neg-atively correlated with albumin levels .Serum GP73 levels in patients with decompensated cirrhosis was (215 .9 ± 96 .56)ng/mL , which was significant higher than (113 .7 ± 68 .95)ng/mL in patients with compensated cirrhosis .The area under ROC was 0 .82 (95% CI :0 .74 ~ 0 .90 ,P< 0 .05) .The sensitivity and specificity of GP73 diagnosing decompensated cirrhosis in cirrhotic population with chronic HBV infection were 77 .55% and 75 .00% respectively ,when the cut-off was set at 140 .5 ng/mL .Conclusion Serum GP73 level is related with liver injury and HBV replication .GP73 might be an useful marker for diagnosing decompensated cirrhosis in cirrhotic population with chronic HBV infections .

ObjectiveTo analyze the distribution of complications of patients in different age in post-anesthesia care unit (PACU).Methods2679 patients in PACU were divided into three age group, and complications were observed and compared among each group.ResultsThere were statistics difference of recovery-time and incidence of complications among each group, except myopalmus.ConclusionIt is important to understand the difference of incidence of complications in different age, which may need different kind of nursing.
