1.Progress of gastric cancer organoids in basic research and clinical application

Gastric cancer ranks fifth in global cancer mortality,which is highly aggressive and heterogeneous.How-ever,the research and treatment of gastric cancer is hindered by short of research models that may characterize the developmental properties of gastric cancer.Gastric cancer organoid is a multicellular three-dimensional structure de-veloped in vitro,which can mimic the structure and function of native gastric cancer.Gastric cancer organoids have great application potential and development prospects in establishing gastric cancer research models,mimicking the tumor microenvironment,high-throughput screening of drugs,discovering new therapeutic targets,predicting clinical therapeutic responses,and guiding individualized treatment.In this paper,the progress of gastric cancer or-ganoids in basic research and clinical application is reviewed aiming for promoting the progress of preclinical re-search and supporting the clinical treatment of gastric cancer.
2.Association between HULC gene locus rs7763881 polymorphism and recurrence and metastasis after radical resection in hepatocellular carcinoma

Objective:To investigate the association between the expression of long non-coding RNA genes and the HULC rs7763881 polymorphism, recurrence, and metastasis after radical resection in patients with hepatocellular carcinoma (HCC).Methods:Paraffin tissue samples were selected from 426 cases diagnosed with HCC between January 2004 to January 2012. The expression of different genotypes of HULC gene locus rs7763881 in paraffin tissues was detected by PCR, and the association between different genotype expressions and clinical case characteristics of HCC [gender, age, TNM stage, alpha-fetoprotein, tumor maximum diameter (cm), vascular invasion, tumor capsule, tumor grade] was analyzed. Cox proportional risk regression model was used to analyze the correlation between different genotypes and clinicopathological features, prognosis, and recurrence. Survival analysis between different genotypes was performed using the Kaplan–Meier method for a parallel log-rank test.Results:There were 27 (6.3%) cases in the whole group who lost to follow-up. A total of 399 (93.7%) specimens were included in the study, and 105 (26.3%), 211 (52.9%) and 83 (20.8%) were included in the rs77638881 AA, AC, and CC genotypes, respectively. Kaplan-Meier curve showed that the postoperative overall survival and recurrence-free survival rate were significantly higher in patients with the AA than AC/CC genotype ( P < 0.05). Univariate analysis showed that the AC/CC genotype was closely related to tumor vascular invasion and recurrence or metastasis of HCC ( P < 0.05). Cox multivariate analysis results showed that patients with the AA genotype were taken as references, and the results showed that the risk of recurrence and metastasis in patients with the CA/CC genotype increased to varying degrees, with statistical significance ( P < 0.05). Conclusion:The rs7763881 polymorphic loci located on the HULC gene are closely related to HCC recurrence and metastasis after radical resection. Thus, it may be an indicator for evaluating HCC recurrence and metastasis.
3.Randomized, Double-blind, Single-simulation, Placebo-controlled, Multicenter Clinical Trial of An'erning Granules in Reducing Use of Antibiotics for Children's Community-acquired Pneumonia

ObjectiveTo evaluate the efficacy and safety of An'erning granules in the treatment of community-acquired pneumonia in children. MethodA randomized, double-blind, single-simulation, placebo-controlled trial was designed in this study. The children were randomly assigned into an observation group (An'erning granules combined with ceftriaxone sodium) and a control group (An'erning granules placebo combined with ceftriaxone sodium) according to the ratio of 2∶1. The disease cure rate was taken as the main indicator of efficacy, and the safety of An'erning granules was observed. ResultA total of 206 children (137 in the observation group and 69 in the control group) were included in this study. Before treatment, the age, sex, body height, body weight, diagnosis time of pneumonia, and symptom and sign scores had no significant differences between the two groups. After 8 days of continuous medication, the observation group[70.80%(97/137)] had higher cure rate than the control group[56.52%(39/69)](χ2=4.17,P<0.05) and total effective rate of chest X-ray [97.98%(97/99)] than the control group[86.27%(44/51)] (χ2=12.98,P<0.01). The observation group was superior to the control group in the alleviation of TCM syndrome under the condition of 0-3 g dose stratification on day 3 of medication (P<0.01). The recovery time, time to complete fever abatement, time to fever abatement and expectoration alleviation, rate of conversion to severe case, and reduction in the frequency of antibiotic use showed no significant differences between the two groups. In terms of safety, 13 and 7 adverse events occurred in the observation group and control group, respectively, which were relieved or disappeared after drug withdrawal or symptomatic treatment and showed no significant difference between the two groups. ConclusionIntravenous drip of ceftriaxone sodium combined with An'erning granules is effective in the treatment of community-acquired pneumonia in children. It can accelerate the absorption of pulmonary inflammation, alleviate the clinical symptoms in a short time for young children or the children with mild symptoms, and is safe in clinical application.
4.Biomechanical Analysis on Different Methods of Growing Rod Fixation for Early Onset Scoliosis

Objective To study the effects of selecting different fixing segments on biomechanical characteristics of the spine during the treatment of early onset scoliosis(EOS) by growing rod technique. Methods By using finite element method, four spine models (C6-S1 segments) fixed by growing rod were established: preoperative model, T1-L4 fixed model, T2-L4 fixed model, T3-L4 fixed model. Then 7 N·m torque load and 1 rad angular displacement load were applied on superior surface of C6 segment to analyze biomechanical characteristics for adjacent structure of the fixed vertebral body. Results The whole spinal range of motion (ROM) decreased significantly after operation. When the T2-L4 segment was fixed, the spine activity was mostly restricted, the compensatory activity of adjacent vertebrae increased significantly, and the intervertebral disc stress increased significantly. Conclusions When T2 segment is selected as the upper instrumented segment, the risk of proximal junctional kyphosis and spinal degeneration is the highest, which should be avoided in clinical surgery.
5.An exploration of the training and evaluation systems of medical undergraduates in the context of tutorial system

Objective To investigate the training process of clinical practice ability in medical undergraduates in the context of tutorial system,as well as new clinical training models and evaluation methods.Methods A total of 40 medical undergraduates who entered the stage of clinical practice were randomly divided into tutorial group and control group,with 20 undergraduates in each group.A diversified evaluation system was used to perform a comprehensive evaluation of clinical practice ability of medical undergraduates at different stages,with a combination of professional theory examination,Mini-Clinical Evaluation Exercise,Direct Observation of Procedural Skills,and clinical case report.All the data were analyzed by SPSS 23.0,t test was used for statistical analysis.Results Compared with the control group after the internship ended,the tutorial group had significantly higher scores of clinical professional theory (86.24 ± 2.93),clinical practice ability (7.53 ± 0.48),clinical operation skills (4.35 ± 0.47),and clinical thinking ability (6.31 ± 0.52) (all P<0.05).Conclusion The application of the tutorial training mode during the internship of medical undergraduates can improve their comprehensive qualities such as professional quality,medical theory,and practical skills.The diversified evaluation system can evaluate the teaching effect and reflect the cultivation of comprehensive quality among students.
6.The sensitization pattern of Blomia tropicalis in allergic respiratory disease in Guangzhou

Objective To investigate the sensitization pattern of Blomia tropicalis in patients with aller-gic respiratory disease and the association between Blomia tropicalis with allergic rhinitis and / or asthma. Meth-ods 330 allergic rhinitis and / or asthma patients sensitized with mites were selected in Guangzhou,and sIgE lev-el in the serum of all recruited patients of Blomia tropical,Dermatophagoide pteronyssinus and Dermatophagoide fa-rina was measured. Results The sensitization rate of Blomia tropicalis was significantly lower than that of Derma-tophagoide pteronyssinus and Dermatophagoide farina(P < 0.001,both). 80.0% Patients were sensitized with both three mites,and only 0.3% patients were of monosensitization to Blomia tropicalis. The sIgE of these three mites were significantly positive correlated with each other(P < 0.001),Dermatophagoide pteronyssinus and Derma-tophagoide farina have strong correlation(r = 0.906),Blomia tropicalis has moderate correlation with Dermatopha-goide pteronyssinus and Dermatophagoide farina(r = 0.540 and r = 0.512,respectively). With the increase of Blomia tropicalis sIgE,the severe sensitization rate(sIgE level:class 5~6)of patients sensitized combined Der-matophagoide pteronyssinus or Dermatophagoide farina significantly increased(P < 0.001). The sIgE level of Der-matophagoide pteronyssinus and Dermatophagoide farina in allergic asthma and rhinitis patients were significantly higher than that of patients with rhinitis or asthma alone(P < 0.001),and the sIgE level of Blomia tropicalis in pa-tients with both allergic asthma and rhinitis or with asthma alone were also significantly higher than that of patients with rhinitis alone(P = 0.006 and P = 0.020,respectively). Conclusion The Blomia tropicalis sensitized pa-tients usually sensitize together with Dermatophagoide pteronyssinus or Dermatophagoide farina,and the degree of Blomia tropicalis sensitized in asthma patients are higher.

8.Features of human cytomegalovirus UL146 gene in asymptomatic children

Objective To investigate the polymorphisms of human cytomegalovirus ( HCMV ) UL146 gene in asymptomatic children. Methods Urine samples were collected from 47 asymptomatic chil-dren who were positive for HCMV DNA. PCR was performed to amplify the open reading frame ( ORF) of UL146 gene. Positive bands were sequenced and variations in UL146 gene were analyzed by using bioinfor-matics software. Results Seventeen samples were successfully amplified and sequenced. Variations spread all over the sequence of UL146 gene and the variability in nucleotide and amino acid sequences ranged from 0% to 42. 5% and 0% to 67. 7% respectively. Compared with the Towne strain, there was diversity in sig-nal sequence and C-terminal region. Phylogenetic analysis indicated that UL146 in the 17 asymptomatic chil-dren belonged to four genotypes, which were G1, G8, G9 and G11. Forms of post-translational modification varied greatly among the four genotypes, while the important functional region of ELRCXC chemokine was highly conservative. Secondary structure prediction showed that random-coli conformation was the predomi-nant structure of active proteins. Isoelectric point ( PI) and molecular weight ( MW) were dissimilar among the four genotypes. Conclusion HCMV UL146 gene in asymptomatic children was hypervariable in both nucleotide sequence and amino acid structure. However, the important functional region was highly con-served. The predominant genotypes of UL146 in these children were G1, G8, G9 and G11, and the geno-type distribution in them showed no significant difference with previous findings in children with symptomatic HCMV infection.

Objective To analyze the correlation among serum total IgE(TIgE),blood eosinophil(EOS) counts,the degree of sensitization for skin prick test(SPT),and fractional exhaled nitric oxide(FeNO)in chil-dren with asthma and allergic rhinitis. Methods We collected 121 children with asthma and allergic rhinitis andtheir serum TIgE,blood EOS,FeNO,and SPT for 14 common allergens. Results There were 75.2%patients sensitizing to at least two allergens.TIgE levels in poly-sensitized subjects were higher than that of mono-sensitized subjects(P < 0.05)The number of SPT sensitized allergens was positively correlated with the TIgE levels(P <0.05)FeNO levels were correlated with TIgE levels and EOS percentage(P < 0.05). TIgE levels in subjects with class 2 or 3 in SPT for house dust mite increased significantly ,compared with class 0 or 1. Conclusion There were some correlations among the four detections ,providing important reference for clinical diagnosis and treatment of respiratory allergic disease
10.Distributions of sIgE and tIgE in patents with allergic diseases in Guangdong

Objective To investigate the types and distributions of allergens, and the responsiveness to these allergens SIgE and tIgE as related to allergic disorders in patients with allergies in Guangdong. Methods Serum samples were obtained from patients with allergic disorders (n = 7 144) who visited our hospital between 2009 and 2014. The sera were subjected to analysis of 15 common allergens. Results The positive rates of sIgE and tIgE were 62.4%and 54.6%, respectively. Dermatophagoides pteronyssinus(Der p) had the highest prevalence of aeroallergen-specific IgE and milk the highest prevalence of food allergen-specific IgE. Other aeroallergens and food allergens produced mild responses except Der p and Der f. The sensitization peak of Der p and Der f appeared at the age of 10 to 12 years. The sensitization peak of milk appeared at the age of less than 3 years and that of eggs did at the aged of 4 to 6 years. The averaged tIgE positive rate went up with the increase in the number of allergen sensitization. Conclusion Der p, Der f, milk and eggs are major sensitizers responsible for common allergic disorders in Guangdong. Knowledge concerning allergen characteristics at various age groups may be helpful for early diagnosis and intervention for allergies.
