1.Application of negative pressure drainage on nasal septum reconstructomy by endoscopy

Objective To study the application of negative pressure drainage on nasal septum reconstructomy by endoscopy.Methods Totally 80 patientswith nasal septum deviation in our hospital from May 2014 to March 2015 were randomly divided into the observation group and the control group.All patients were given nasal septum reconstructomy by endoscopy,and patients of the observation group were given negative pressure drainage after surgery while the patients in control group were given traditional nasal cavity filling postoperatively.Observed the subjective symptoms,degree of disease and complications of patients 12 hours and 24 hours after surgery.And the clinical curative effect of the two groups were compared and analyzed 1 week after the surgery.Results The subjective symptoms of the observation group was obvi-ously better than the control group 24 hours after the surgery with statistically significant difference (P <0.05).And the degree of disease in observation group had significantly improved,and the nasal mucosa edema ratio of level 0 was 40%,which was significantly higher than 10% in the control group (P <0.05).The total effective rate was 97.50% in the observation group 1 week after surgery and it was 80% in the control group,and there were significant difference between the two groups (P <0.05).The incidence of complications in the observation group was 2.50%,which was significantly less than 20% in the control group (P <0.05).Conclusion The application of negative pres-sure drainage on nasal septum reconstructomy by endoscopy could effectively relieve the pain of patients and reduce complications,and its prognosis effect is better.
2.Exploration of the regulation site of HIF-1α in Notch signaling pathway in the hippocampus of rats with status ;epilepticus

Objective To study the expression of hypoxia inducible factor-1 (HIF-1 ) and Notch signaling pathway downstream gene HES 1 in the hippocampus of pubertal rats with status epilepsy (SE), and to explore the regulation site of HIF-1αin Notch signaling pathway. Methods One hundred and seventy-six 21-day-old SD rats were randomly divided into control group (NS group), pentetrazole (PTZ)-induced SE group (PTZ group), and Notch signaling pathway speciifc inhibitor (DAPT) intervention group (DAPT group). In PTZ group PTZ was intraperitoneally injected to build SE model and in NS group normal saline was injected as control. The intraperitoneal injection of diazepam was used to terminate SE seizures. After successful modeling, the bilateral hippocampuses were isolated after the rats were sacriifced at 0.5, 1, 2, 4 and 8 h, respectively, and RT-PCR was performed to detect the mRNA expression of HES 1 and HIF-1α. The Western Blot was performed to detect protein expression in hippocampuses which were collected at 2 , 4 , 8 , 12 , and 24 h after successful modeling. DAPT group received intraperitoneal injection of DAPT 30 min before the start of molding, then the hippocampuses were isolated at 2 and 8 h after successful modeling. RT-PCR was performed to detect the mRNA expression of HES 1 and HIF-1αat 2 h, and Western blot was performed to detect protein expression at 8 h. Results At each time point after SE, the expression of mRNA of HES 1 and HIF-1αand the expression of protein were higher than the same time point of NS group (P0 . 05 ). Compared with the same time point of PTZ group, the mRNA expression of HES 1 and HIF-1αand the expression of protein of DAPT group were obviously reduced (P0 . 05 ). Conclusion HES 1 gene may be the regulatory site of HIF-1 expression in Notch signaling pathway in the hippocampus of puberty rats with SE.
3.Efficacy of nasal packing, septal suture technique and vacuum sealing drainage after nasal septum surgery.

OBJECTIVE:
The objective of this study was to evaluate the effect of nasal packing, septal suture technique and vacuum sealing drainage (VSD) after septoplasty.
METHOD:
Ninety patients of nasal septal deviation in Combination with outfracture of the inferior turbinates who had received septoplasty were selected in this study. The patients were allocated into three groups, with thirty in each: for packing group, marcel materials were used for nasal packing after septoplasty; for suturing group, septal suture technique was performed after septoplasty; for VSD group, one drainage tube was used for negative pressure sucking after septoplasty without nasal packing. Postoperative signs and symptoms were compared between three groups. The comfort degree assessment included headache and nasal obstruction were evaluated by using visual analogue scale (VAS) at the 12th hour and 24 hour after operation. The edema in nasal cavity, hemorrhage. abscess,adhesive and healing rates after operation were compared among three groups.
RESULT:
The VAS score of headache and nasal obstruction and the severity of patient's conditions were significantly less in septal suture group and VSD group than that in packing group at the 12th and 24th hour after operation. The mucosa edema of nasal cavity was significantly slighter in septal suture group and VSD group than that in packing group at the third day after operation. The healing rates and number of complications are better in septal suture group and VSD group than those in packing group at the 7th day after operation. There were no hemorrhage or abscess in VSD group.
CONCLUSION
Septal suture technique and VSD after septoplasty can significantly relieve the distress of patients and reduce the healing time of mucosa in nasal cavity without increasing the risk of complications.

4.Effects of exogenous interleukin-10 on STAT3 protein activation in rats with allergic rhinitis

Objective To investigate the effect of exogenous interleukin-10 (IL-10) on STAT3 protein activation in rats with allergic rhinitis (AR).Methods60 SD rats were randomLy divided into AR model group, IL-10 group and control group, with 20 in each group.AR group and IL-10 group were treated with conventional ovalbumin (OVA) combined with aluminum hydroxide sensitization method.The negative control group was treated with saline instead of 48 h after the last challenge.The levels of IL-6, TGF-β and P-STAT3 in nasal mucosa and serum IgE were measured by ELISA kit.The expression of IL-6, TGF-β and P-STAT3 in nasal mucosa was detected by Western-blot Detection.ResultsAR group exhibited severe clinical manifestations of AR, such as friction around, tears flowing, the signs of which in IL-10 group were significantly lower than that in the control group (P<0.05).Compared with the control group, the levels of serum IgE and IL-6, TGF-β and P-STAT3 in the nasal mucosa were significantly higher than those in the control group (P<0.05).After treatment with IL-10, the levels of TGF-β and P-STAT3 were significantly increased (P<0.05).Western-blot showed that compared with the control group IL-6, TGF-β and P-STAT3 in nasal tissues of AR group and IL-10 group were significantly increased, while the expression level of IL-10 group was lower than that of AR group.ConclusionTreatment of AR with exogenous IL-10 can significantly relieve the clinical manifestation of AR, inhibit STAT3 activation immune response, and achieve the purpose of treatment of AR in some extent.
5.Bacteriological analysis of persistent rhinosinusitis after endoscopic sinus surgery.

OBJECTIVE:
To investigate the bacterial characteristics of persistent rhinosinusitis after functional endoscopic sinus surgery (FESS).
METHOD:
Twenty patients with nasal septum deviation, 30 patients with chronic rhinosinusitis (CRS) and 20 patients with persistent rhinosinusitis, were selected to take discharges from middle meatus during the operation. Bacteria culture and drug susceptibility of the discharges were compared between three groups.
RESULT:
There were 13, 15 and 15 isolates detected in nasal septum deviation group, CRS group and persistent rhinosinusitis group. There was no significant difference among the three groups at the detection rate of Gram-positive bacteria. But there was significant difference between the persistent rhinosinusitis group and the other two groups at the detection rate of Gram-negative bacteria. The detection rate of antibiotic-resistant bacteria were significantly higher in persistent rhinosinusitis group than in CRS group.
CONCLUSION
Aerobic bacteria can live in nasal cavity. Bacteria infection is one of the etiological factors of persistent rhinosinusitis after FESS. Gram-negative bacteria and antibiotic resistant bacteria are increased in patients with persistent rhinosinusitis. To treat the persistent rhinosinusitis after surgery, the antibiotics should be reasonably used according to the bacteria culture and the drug susceptibility.

6.The therapeutic effect of frequency spectrum equipment of the rat epilepsy model and the influence on the pia microcirculatory blood flow volume

The epilepsy symptom and the pia microcirculatory blood flow volume (PMBFV) were investigated in epilepsy rat model induced by strychnine and the changes after treatment by using faradized frequency spectrum therapeutic equipment were observed. The epilepsy symptom was improved and the PMBFV increased after treatment. The convulsion seizure time was delayed, the seizure frequency and convulsion time were reduced, and the epilepsy even was not appeared in some cases. Results suggested that this improvement might be related to the change of PMBFV.
7.Exogenous IL-10 affects the expression of certain cytokines by Th cells and Th17 cells in rats with al-lergic rhinitis

Objective To investigate the expression of IFN-γ, IL-5, IL-17 and TGF-βby Th cells and Th17 cells in rats with allergic rhinitis upon the intervention of IL-10.Methods SD rats were ran-domly divided into three groups including allergic rhinitis ( AR ) group, IL-10 treated group and control group (n=10).Rats in AR group and IL-10 treated group were sensitized by injection of ovalbumin (OVA) and aluminum hydroxide on the 1st, the 7th and the 14th days.The rats treated with equal volume of saline were set up as the control.The corresponding interventions ( OVA, OVA and IL-10, saline) were respec-tively given to rats in each group on the 21th day for 7 consecutive days.The clinical manifestations in rats were observed within 30 minutes after each administration.Serum samples were collected at 48 hours after the last challenge for the detection of IgE and OVA-sIgE.ELISA and Western blot assay were performed to detect IFN-γ, IL-5, IL-17 and TGF-βin nasal mucosa samples.Results Some characteristic symptoms of AR were observed in rats from AR group and IL-10 treated group.Compared with IL-10 treated rats, rats in AR group showed severe clinical symptoms such as constant rubbing and tearing of the eyes (P<0.05).The levels of IgE and OVA-sIgE in serum samples and the levels of IFN-γ, IL-5, IL-17 and TGF-βin nasal tis-sues were significantly increased in rats with RA (P<0.05), but were reduced with IL-10 intervention (P<0.05).Conclusion Exogenous IL-10 could be used to treat AR by reducing the expression of IFN-γ, IL-5, IL-17 and TGF-βin nasal tissues.
8.Preliminary experience of endovascular recanalization of chronic symptomatic internal carotid artery occlusion: a report of four cases

Objective To summarize the preliminary experience of endovascular recanalization in treating chronic symptomatic internal carotid artery occlusion.Methods Four patients with chronic symptomatic internal carotid artery occlusion,admitted to and underwent endovascular recanalization in our hospital from August 2013 to August 2014,were chosen in our study; their clinical data were retrospectively analyzed.Results Four patients were successfully opened the internal carotid arteries;cerebral CT angiography showed that all arteries were unobstructed.One appeared intra-operative iatrogenic internal carotid artery cavernous sinus fistula,and successful occlusion of the fistula with internal carotid artery patency was achieved after one week.Three months after the operation,two showed unobstructed internal carotid artery by DSA and the other stated no abnormalities during the telephone follow up.Conclusion Endovascular recanalization is a safe and effective treatment method for chronic symptomatic internal carotid artery occlusion.
