1.Role of MiR-20a and its Diagnostic Value in Gastric Cancer

Gastric cancer is one of the most common malignancies worldwide. Early diagnosis and intervention is the key to improve the prognosis of patients with gastric cancer. Expression of miR-20a is up-regulated in the circulating blood of gastric cancer patients,cancer tissues and cell lines,and has been demonstrated to be closely related to the clinicopathological characteristics and prognosis of gastric cancer. MiR-20a promotes the proliferation of gastric cancer cells,regulates the self-renewal of gastric cancer stem cells,and induces chemoresistance via a variety of mechanisms. Detection of serum/ plasma miR-20a alone or in combination with other miRNAs is helpful for the early diagnosis and prediction of progression,relapse and prognosis of gastric cancer. MiR-20a has the potential to be used as a novel and noninvasive biomarker for gastric cancer.
2.FHIT alteration in human esophageal, gastric and colorectal carcinomas

Objective To investigate the status of fragile histidine triad (FHIT) gene in human esophageal, gastric and colorectal carcinomas. Methods Ninety six samples of digestive tract cancer (including 21 esophageal carcinomas, 43 gastric carcinomas and 32 colorectal carcinomas) tissues and their adjacent non carcinoma tissues and 18 samples of normal tissue were examined by nested RT PCR for FHIT gene alteration. Results Aberrant transcripts were observed in 33.3% esophageal cancers, 51.7% gastric cancers and 31.3% colorectal cancers. In the adjacent esophageal,gastric and colorectal non carcinoma tissues the rate of aberrant transcripts were 4.8%,20.9% and 9.4%, respectively ( P
3.Construction and implementation on clinical medicine modular integration curriculum system

Wuhan University built up a new modular integrated curriculum in clinical medicine oriented for post competence,by learning from the successful international experience and the course of vertical and horizontal integration.The curriculum focused on five modules (including humanities and vocational course,courses of integrated medicine,clinical practice and thinking,innovation awareness and ability,prevention and health promotion program) and the eight integrated curriculum (including human structure,organization and function,cell,molecule and gene,reaction,nerve injury science,pathogen biology,CPPT,clinical skills).During the curriculum implementation,Wuhan University vigorously promoted the group discussion teaching,cultivated high-quality core teaching team,and effectively carried out curriculum construction and teaching evaluation,which effectively improved the level and quality of medical personnel training.
4.Effects of psychological factors on quality of life in aged patients with non-erosive reflux disease under maintenance treatment of proton pump inhibitors

Objective To explore the effects of psychological disorders on qualityoflife of elderly patients with non-erosive reflux disease (NERD) under maintenance treatment of proton pump inhibitors (PPIs). Methods A total of 76 elderly patients with NERD treated by proton pump inhibitors (maintenance treatment of Esomeprazole over half a year) were enrolled. They were classified into symptom group aged (66.5±8.2) years (n=35) and symptom group aged(68.2+6.9)years (n=41)according to reflux symptoms,with 20 healthy people aged(65.4 ±8.8)years as control.The subjects completed the questioniaires on psychological status including self-rating anxiety scale (SAS) and self-rating depression scale (SDS) and their quality of life was evaluated by gastroesophageal heath related quality-of-life instrument(GERD -HRQL) and medical outcomes study short form(SF-36). Results Scores of SAS and SDS were significantly higher in symptom group (54.9±6.2,57.9±8.6) than in symptomless(37.9±8.7,42.7±7.9) and health control group (38.9± 7.2,43.1 ± 8.9),all (P<0.05),but there was no difference between symptomless and health control group.The rates of anxiety and depression in symptom group (51.4%,57.1%) were increased as compared with symptomless (12.2%,9.8%)and health control group(0.0%,5.0%).Scores of GERD HRQL in symptom group were higher than in symptomlcss group (21.4± 5.0 vs.4.5±2.2,t=19.76,P<0.01)while the indexes such as body pain,vitality,social function,affection,mental and whole health in score of SF-36 besides physiology function were lower in symptom group than in symptomless and control groups (P<0.05 or P<0.01 ).Only the significant differences in mental and whole health of SF-36 were found between symptomless and control groups (P<0.01).Correlation analysis revealed that SAS and SDS scores were positively correlated with scores of GERD-HRQL in symptom group(P<0.01) and negatively with all dimensions of SF-36 but physical function (P< 0.05). Conclusions Most of elderly symptomatic patients with NERD treated by PPI are in psychological disorders which results in lower quality of life.
5.Expression of NF-Kappa B and human telomerase reverse transcriptase in gastric carcinogenesis

Purpose:To investigate the variant expression of human belomerase reverse transcriptase (hTERT) and NF ?B p65 protein and their relationships in gastric carcinogenesis.Methods:A total of 41 primary gastric cancers, 15 cases dysplasia, 23 cases intestinal metaplasia and 10 cases normal gastric mucosa were taken into this study. Expression of hTERTmRNT, hTERT protein and p65 protein were determined by in situ hybridization and immunohistochemistry. Results:①Among normal mucosa intestinal metaplasia dysplasia carcinoma,p65 expression were 0?34.18%(8/23)?53.33%(8/15)?60.98%(25/41),hTERTmRNA 10.00%(1/10)?39.13%(9.23)?66.67%(10/15)?85.37%(35/41) and hTERT protein 0?30.43%(7/23)?60.00%(9/15)?78 05%(32/41),respectively. 6.Study on Early Predictive Value of Clinical Indicators for Acute Kidney Injury in Patients with Acute Pancreatitis

Background:Acute kidney injury(AKI)is one of the early serious complications of acute pancreatitis(AP). Early identification and intervention have important clinical significance. Aims:To investigate the early predictive value of clinical indicators found within 48 hours after admission for AKI in patients with AP. Methods:A retrospective analysis of 205 AP patients from January 2014 to December 2014 at Renmin Hospital of Wuhan University was performed. AP patients were divided into AKI group and non-AKI group according to the KDIGO standard,the demographic characteristics,history of chronic diseases and indicators defined within 48 hours after admission such as Ranson score,BISAP score and biochemical indices were compared. The early predictive value of clinical indicators for AKI was evaluted,and strength of association between clinical indicators and AKI was assessed. Results:Compared with non-AKI group,Ranson score, BISAP score,D-dimer,PCT,blood glucose,triacylglycerol were significantly increased in AKI group(P < 0. 05),and calcium was significantly decreased(P < 0. 05). The accuracies of D-dimer,PCT,calcium were high for early prediction of AKI,and the AUC were 0. 881,0. 803 and 0. 782,respectively. Logistic regression analysis showed that D-dimer, PCT,calcium had marked correlation with AKI(OR all > 7)(P < 0. 05). Conclusions:D-dimer,PCT and calcium are effective and economical clinical indicators for the early prediction of AKI in patients with AP.
7.Serum Biochemical Markers for Early Prediction of Severity of Acute Pancreatitis

Background:As the incidence of acute pancreatitis( AP)is increasing and the mortality remains high,early identification and early intervention of patients at risk of severe acute pancreatitis(SAP)is of great clinical significance. Aims:To investigate the value of commonly used AP-related serum biochemical markers for early prediction of severity of AP. Methods:Retrospective analysis was performed in 205 AP patients in Renmin Hospital of Wuhan University from Jan. 2014 to Dec. 2014,and 92 healthy subjects were served as controls. Patients with AP were divided into mild(n = 92), moderately severe(n = 72)and severe AP(n = 41)groups. The demographic characteristics,history of chronic diseases, serum biochemical markers within 48 hours after admission,Ranson score,BISAP score and other related clinical data were collected and compared between the four groups. The diagnostic performance of these indices in predicting SAP was evaluated by ROC curve,and the correlation of these indices with SAP was assessed by Logistic regression analysis. Results:There were significant differences in Ranson score,BISAP score,D-dimer,blood calcium,procalcitonin(PCT), CRP,hypersensitive CRP(HS-CRP),triacylglycerol and blood glucose between the four groups(P < 0. 05). The area under curve(AUC)of blood calcium was 0. 838 for early prediction of SAP,which was second to that of Ranson score and BISAP score. The accuracies of PCT and D-dimer were acceptable but inferior to blood calcium,while the accuracies of other indices were considerably low. Logistic regression analysis revealed that the odds ratio of risk for SAP of Ranson score,PCT and HS-CRP were all greater than 1 000,while that of blood calcium and D-dimer were 0. 013 and 19. 479, respectively. Conclusions:Of the AP-related serum biochemical markers,blood calcium,D-dimer and PCT are the preferred early predictors for severity of AP.
