1.Fibrinogen/C reactive protein ratio as a parameter for diagnosis of disseminated intravascular coagulation in patients with sepsis.

Objective To determine if the fibrinogen/C reactive protein (CRP) ratio could be used in sepsis patients as a predictor for disseminated intravascular coagulation.Methods A total of 61 patients with sepsis admitted to emergency intensive care unit (EICU) were enrolled.C reactive protein (CRP) and variables of coagulation such as Platelet (PLT),prothrombin time (PT),activated partial thrombopastin time (APTT),fibrinogen and D-dimer were analyzed during 24 hours after admission to EICU.APACHE Ⅱ score,ISTH score and JAAM score were evaluated.According to DIC score (JAAM score and ISTH score),the patients were divided into two groups:overt DIC and no overt DIC.Data were expressed in mean ±standard deviation and were statistically analyzed by SPSS 17.0.The differences of CRP,PLT,PT,APTT,fibrinogen and D-dimer of patients in each group were analyzed by independent samples t test.The receiver-operating characteristics (ROC) analysis was performed to predict the diagnostic power of the variables fibrinogen,CRP,and fibrinogen/C reactive protein (CRP) ratio.In addition,the odds ratios (OR) were estimated by chi-square test and the 95% confidence intervals were calculated.Results In overt DIC group fibrinogen/C reactive protein (CRP) ratio was significantly lower.The receiver-operating characteristic analysis showed for the fibrinogen/C reactive protein (CRP) ratio (area under the curve:ISTH criteria 0.691 and JAAM criteria 0.814) significantly better discriminative power than for fibrinogen (area under the curve:ISTH criteria 0.608 and JAAM criteria 0.429) and CRP (area under the curve:ISTH criteria 0.561 and JAAM criteria 0.788).The odds ratio for fibrinogen/C reactive protein (CRP) ratio (< 45) was 11.14 and CRP (> 70 mg/L) was 8.05.Conclusions We suggest that the fibrinogen/C reactive protein (CRP)ratio could be a diagnostic factor for the occurrence of disseminated intravascular coagulation in sepsis patients.

Primary ciliay dyskinesia (PCD)is a kind of monogenic disorder.Young patients often present recurrent respiratory tract infection, even progress to bronchiectasis, and some children are combined with situs inversus.The pathogenesis is closely related to the abnormality of cilia structure and/or function.So far, over 40 genes that are associated with PCD has been confirmed in humans.About 70% of the cases are correlated with these genes.Although the relationship between genotype and phenotype is not fully clear, some genetic defects and clinical manifestations might have certain relevance.The severity of the clinical phenotype may also be related to a specific genotype.With the maturity of gene sequencing technology and the reduction of the cost, gene sequencing can also be applied in clinical practices to make up for the shortcomings of traditional diagnostic methods.
3.Application of serum procalcitonin test in diagnosis of neonatal bacterial infectious diseases

Objective To investigate the diagnostic value of procalcitonin(PCT) in bacterial infectious diseases for newborns . Methods Using enzyme‐linked immune fluorescence analysis technology to determine PCT of 90 samples of neonatal serum .The Observation group(48 cases) and control group(42 cases) were grouped according to the gold standard which were microbial cul‐ture result of sterile body fluid and clinical diagnosis .Recorded the values of PCT ,WBC ,CRP and IL‐6 detected in the same period , then calculate the sensitivity and specificity ,and draw the ROC curve to compare sensitivity and specificity between PCT and the other infectious indicators .Results The sensitivity and specificity of PCT were 87 .50% and 85 .71% respectively ,the positive pre‐dictive value(PPV) and negative predictive value(NPV)of it were 87 .50% and 85 .71% .However ,the sensitivity and specificity of CRP were 81 .25% and 83 .33% ,the PPV and NPV of it were 84 .78% and 79 .54% .The sensitivity and specificity of WBC were 68 .75% and 57 .14% ,the PPV and NPV of it were 64 .70% and 61 .53% .The sensitivity and specificity of IL‐6 were 93 .75% and 71 .43% ,the PPV and NPV of it were 78 .95% and 90 .91% .In addition to that ,the area under the ROC of PCT ,CRP ,IL‐6 and WBC ,which were 0 .859 ,0 .852 ,0 .803 and 0 .647 respectively .Conclusion Compared with the other 3 infectious indicators ,PCT has high sensitivity and specificity in the diagnosis of neonatal bacterial infection diseases .Moreover ,the detection method of PCT is simple and feasible ,PCT could also provide laboratory basis for the diagnosis .
4.Significance and expression of zinc finger protein 139 and matrix metalloproteinase 7 in endometrial adenocarcinoma

Objective To observe the expression of ZNF139 and MMP-7 in endometrial adenocarcinoma tissues and discuss the correlation with clinical pathological significances.Methods In 71 patients diagnosed as endometrial adenocarcinoma,71 cancer tissues(adenocarcinoma group) and 71 adjacent normal tiuues(adjacent cancer group) were collected in operations.Immunohistochemical methods was used to detect the expression of ZNF139 and MMP-7 in the two groups,the relationship between the two genes and communication with clinicopathological indexes were discussed.Western blot and qRT-PCR were used to detected the expression of ZNF139 and MMP-7 at protein and mRNA level.Results The positive rates of ZNF139 was 66.20% in adenocarcinoma group,higher than in adjacent cancer group(21.42%,P=0.016);the positive rates of MMP-7 was 74.65% in adenocarcinoma group,higher than7 than in adjacent cancer group(26.76%,P=0.012).The expression of ZNF139 and MMP-7 was correlated with lymph node metastasis,muscle invasion and FIGO stage(P=0.028,0.031;P=0.004,0.016;P=0.008,0.011).Spearman rank correlation analysis showed there was positive correlation between ZNF139 and MMP-7 in adenocarcinoma tissues(r=0.716,P=0.039).Western blot showed that ZNF139 and MMP-7 expression in adenocarcinoma group were higher than in adjacent cancer group(t=6.92,7.34;P<0.01);qRT-PCR showed that ZNF139 mRNA and MMP-7 mRNA expression in adenocarcinoma group were higher than in adjacent cancer group(t=4.27,4.06;P<0.05).Conclusion ZNF139 and MMP-7 are correlated with malignant pathological characteristics in endometrial adenocarcinoma,they may modulate the malignant behavior together.

Bronchiectasis is a kind of chronic lung disease which threatens the health of children in China.It can affect the lung function,life quality and development of the patients.There are varied causes of bronchiectasis,and some of the causes have the special intervention.So the etiology diagnosis of bronchiectasis is important.The aims of the treatment of bronchiectasis is removing the causes,releasing the symptoms,preventing the acute exacerbation,sustaining the lung function and life quality.It needs a long tern combine therapy.
6.Effects of psychological factors on quality of life in aged patients with non-erosive reflux disease under maintenance treatment of proton pump inhibitors

Objective To explore the effects of psychological disorders on qualityoflife of elderly patients with non-erosive reflux disease (NERD) under maintenance treatment of proton pump inhibitors (PPIs). Methods A total of 76 elderly patients with NERD treated by proton pump inhibitors (maintenance treatment of Esomeprazole over half a year) were enrolled. They were classified into symptom group aged (66.5±8.2) years (n=35) and symptom group aged(68.2+6.9)years (n=41)according to reflux symptoms,with 20 healthy people aged(65.4 ±8.8)years as control.The subjects completed the questioniaires on psychological status including self-rating anxiety scale (SAS) and self-rating depression scale (SDS) and their quality of life was evaluated by gastroesophageal heath related quality-of-life instrument(GERD -HRQL) and medical outcomes study short form(SF-36). Results Scores of SAS and SDS were significantly higher in symptom group (54.9±6.2,57.9±8.6) than in symptomless(37.9±8.7,42.7±7.9) and health control group (38.9± 7.2,43.1 ± 8.9),all (P<0.05),but there was no difference between symptomless and health control group.The rates of anxiety and depression in symptom group (51.4%,57.1%) were increased as compared with symptomless (12.2%,9.8%)and health control group(0.0%,5.0%).Scores of GERD HRQL in symptom group were higher than in symptomlcss group (21.4± 5.0 vs.4.5±2.2,t=19.76,P<0.01)while the indexes such as body pain,vitality,social function,affection,mental and whole health in score of SF-36 besides physiology function were lower in symptom group than in symptomless and control groups (P<0.05 or P<0.01 ).Only the significant differences in mental and whole health of SF-36 were found between symptomless and control groups (P<0.01).Correlation analysis revealed that SAS and SDS scores were positively correlated with scores of GERD-HRQL in symptom group(P<0.01) and negatively with all dimensions of SF-36 but physical function (P< 0.05). Conclusions Most of elderly symptomatic patients with NERD treated by PPI are in psychological disorders which results in lower quality of life.
7.Expression of NF-Kappa B and human telomerase reverse transcriptase in gastric carcinogenesis

Purpose:To investigate the variant expression of human belomerase reverse transcriptase (hTERT) and NF ?B p65 protein and their relationships in gastric carcinogenesis.Methods:A total of 41 primary gastric cancers, 15 cases dysplasia, 23 cases intestinal metaplasia and 10 cases normal gastric mucosa were taken into this study. Expression of hTERTmRNT, hTERT protein and p65 protein were determined by in situ hybridization and immunohistochemistry. 8.Effects of insulin treatment on intracellular lipid metabolism in liver of diabetic rats.

The effects and the mechanism of insulin treatment on intracellular lipid metabolism in liver of diabetic rats were evaluated. Type 2 diabetic rats were induced by injecting the streptozotocin (25 mg/kg) and fat rich food. According to the results of oral glucose tolerance test (OGTT) and glucose-induced insulin secretion test (IRT), the rats were divided into two groups: untreated group (UT) and insulin-treated group (IT). Normal rats (NC) served as controls. The treatment with either Humulin N (4--6 U/kg every day), or saline lasted for 4 weeks. Body weight, OGTT, IRT, blood lipids, intracellular lipids in liver, hepatic fatty acid oxidation and the activity of fatty acid synthase (FAS) were detected. The change of liver histology was observed. The insulin sensitivity index (ISI) was applied to assess the status of insulin resistance. The results showed that as compared with NC group, the plasma and hepatic intracellular Triglyceride (TG), total cholesterol (TC) and free fatty acids (FFAs) were increased significantly in UT group (P < 0.05), and lipid droplets could be seen dispersedly in the liver specimens, the hepatic fatty acid oxidation was increased markedly (P < 0.05), while the fatty acid synthase activity decreased (P < 0.05). Insulin treatment resulted in a further accumulation of lipids in liver by 55.7%, 19.87% and 22.2% increase in TG, TC, FFAs respectively. The size of hepatocytes was enlarged and the cells were filled with fat drops. Plasma lipids showed little decrease and still significantly higher than those in NC group after the insulin treatment. Meanwhile, insulin treatment was companied by 20% decrease in the rate of fatty acid oxidation and 31% increase in hepatic FAS activity compared to UT group. It was concluded that treatment with insulin on type 2 diabetic rat increases hepatic intracellular lipid accumulation by inhibiting hepatic fatty acid oxidation and activating FAS.
9.Effects of isocorydione on cell proliferation in SiHa human cervi-cal carcinoma cell lines

Objective:This paper aimed to investigate the effects of isocorydinone on cell proliferation in SiHa human cervical carcinoma cell lines. Methods:Different concentrations of isocorydione (100, 200, 400, 800, and 1200 μmol/L) were used to treat SiHa human cervical carcinoma cells in vitro for 24, 48, and 72 h. Methyl thiazolyl tetrazolium (MTT) assays were conducted to determine the inhibitory action of isocorydione. Flow cytometry was performed to detect the cell cycle in SiHa human cervical carcinoma cells af-ter treatment with 400 μmol/L isocorydione. Hoechst 33342 staining was used to observe the micro-morphological changes of SiHa cell nucleus after the treatment. The expression of Bcl-2, Bax, and caspase-3 proteins in cervical carcinoma SiHa cell lines was determined using western blot analysis. Results: MTT assays showed that isocorydione inhibits the proliferation of SiHa cells in a dose- and time-dependent manner (P<0.05). The flow cytometry results showed that SiHa cervical carcinoma cells treated with different concen-trations of isocorydione exhibited increased cell cycle. Compared with the control group, Hoechst 33342 staining showed that SiHa cells became narrow, with nuclear pyknosis and fragmentation, and formed an apoptotic body after treatment with 400 μmol/L isocoryd-ione for 48 h. Furthermore, western blot analysis proved that isocorydione significantly inhibited the proliferation of SiHa cell lines, and the expression of Bax protein was increased. By contrast, the expression of Bcl-2 protein decreased gradually. Consequently, the ra-tio of Bax/Bcl-2 increased, as well as the expression of caspase-3 protein. Conclusion:Isocorydione exhibited an overt inhibitory ac-tion on SiHa cells. Isocorydione promoted the occurrence of cell apoptosis, which may be associated with related proteins of mitochon-drial apoptotic pathway.
10.Pharmaceutical Care for Pediatric Child with Disseminated Cryptococcosis

Objective To explore the approaches and significance of pharmaceutical care for pediatric child with disseminated cryptococcosis.Methods Medical records were established for a disseminated cryptococcosis pediatric child.Upon analyzing drug use,pharmaceutical care was provided for drug use.Results The child geting better,was discharged from hospital.Conclusion Tothrough participating in the clinical practice,clinical pharmacists can assist the clinicians to make effective and reasonable dosage regime as well as providing pharmaceutical service for patients.
