1.Doppler hemorrhoid artery ligation combined with lauromacrogal sclerosis injection in treatment of hemorrhoids

ObjectiveTo study the clinical value of minimally invasive treatment of the doppler-guided hemorrhoid artery ligation with lauromacrogal sclerosis injection in treatment of hemorrhoids.MethodsA retrospective analysis was made on treatment of 32 cases with hemorrhoids using doppler-guided hemorrhoid artery ligation combined with lauromacrogal sclerosis injection.ResultsAccording to the branch of the Chinese Medical Association by the Surgery the clinical diagnosis and Treatment Guidelines of Hemorrhoids (draft),the Ⅰ degree hemorrhoid was found in 1 case,Ⅱ degree hemorrhoids in 18 cases,Ⅲ degree hemorrhoids in 13 cases.Recovery was found in 32 cases.The longest follow-up time was 30 weeks.No relapse was found. ConclusionsDoppler-guided hemorrhoid artery ligation combined with lauromacrogal case sclerosis injection is a low aggressive minimally invasive method to treat hemorrhoid and has small trauma,definite effect,less pain,fewer complications,and short course and other advantages,but the longterm outcome needs further observation.
3.Association of plasma cystatin C and intima-media thickness of carotid artery in patients with early type 2 diabetes mellitus

Ninty-eight patients with newly diagnosed type 2 diabetes mellitus were divided into atherosclerosis(AS) group and non-AS group.Fasting plasma cystatin C level( CysC ) was determined.The results showed that there existed a significant correlation between CysC level and the number of carotid arteries plaque ( r =0.432,P<0.01 ).CysC was an independent risk factor( OR =2.21,95% CI 1.88-3.02 ) of carotid artery intimamedia thickening in patients with type 2 diabetes.
4.Clinical efficient of encircling constriction of superficial femoral vein in the treatment of primary deep venous insufficiency on a report of 187 cases

ObjectiveTo evaluate the therapeutic effect of encircling constriction of superficial femoral vein in the treatment of primary deep venous insuficiency(PDVI).MethodsFrom Jan.2004 to Jun.2011,187 patients( 196 lower extremities)with primary deep venous insufficiency were treated with encircling constriction of venous wall at the first pair of superficial femoral venous valve pulse superficial varicose stripping.Results The 183 limbs in 176 patients were followed-up from 7 months to 8 years,with a mean period for 63.5 months.Ninety-four point one percent of the patients were followed up (176/187).Clinical symptomatic improvement achieved in 87.4% ( 160/183 ) of the lower extremities.There was 2.2% ( 4/183 ) of recurrence of the varicose vein,9.3% ( 17/183 ) of light edema,8% ( 2/25 ) of recurrence of ulcer.ConclusionEncircling constriction of superficial femoral vein is an effective method in the treatment of primary deep venous valve insufficiency.
5.An Analyses of X-ray Diagnosis of 13 Cases of Congenital Pulmonary Agenesis and Hypoplasia in Children

Objective:To evaluate the efficacy of imaging diagnisis of congenital pulmonary agenesis and hypoplasia in children.Methods:The 13 cases of congenital pulmonary agenesis and hypoplasia were veritfied by bronchcscopy and operation or pathology the characteristics of the X-ray,CT and bronchography were analysed.Results:the X-ray features included:(1)capacity decreased and density increased of diseased lung;(2)small hilar pulmonis;(3)mediastinum shifted to the affected side.Conclusion:Radiography is a good procedure in diagnosis of the most cases of congenital pulmonary agenesis and hypoplasia.
6.Application of Intelligent Narcotic Drug Management Cabinet in the Narcotic Drug Management in the Outpatient Pharmacy of Our Hospital

OBJECTIVE:To promote the informationization and standardization of narcotic drug management so as to enhance the level of pharmacy management. METHODS:The function of intelligent narcotic drug management cabinet and its application in our hospital were introduced. The application effects were evaluated by comparing the indexes in 1 month(or 1 quarter)before and after the adoption of the cabinet. RESULTS:The intelligent narcotic drug management cabinet used in our hospital had such func-tions as login,getting drugs,adding drugs,expiration date management,inventory query,update,shifting of duty,record and da-ta processing. After the adoption,the inventory decreased by 35.2%,turnover rate increased by 53.4%;the time of applying for and registering narcotic drugs by the keeper of secondary warehouse respectively shortened by 84.1%(13.2 min vs. 2.1 min) and 88.9%(10.8 min vs. 1.2 min);and the incidence of external errors reduced from 3 times to once,an decrease of 66.7%. Mean-while,high integration of space improved the overall image of the pharmacy and the automatic pharmacy was constructed on a cer-tain scale and in a systematic manner. CONCLUSIONS:The application of intelligent narcotic drug management cabinet to the out-patient pharmacy can reduce the inventory cost,increase work efficiency and the safety of drug use,and make the narcotic drug management more standard,scientific and sophisticated.
7.Protective effects of Acanthopanax senticousus saponins on cortical neuronal ischemia-hypoxia injury

Objective To explore the protective effects of Acanthopanax senticousus saponins (ASS) on ischemia-hypoxia injury of cortical neuron. Methods The models of cortical neuron damage induced by hypoxia-ischemia (HI) and glutamate (Glu) were established on cultured embryonic cortical neurons. The neurons were randomly divided into HI group, Glu group, ASS group and control group. ASS (50 mg/L) was added into the ASS group before and during neuron damage. The rate of neuronal apoptosis was measured by flow cytometer, nitric oxide (NO) content was determined by Nitrate reductive assay and neuron survival was measured by MTT assay and the release of LDH. Morphologic change of neurons was observed under electron microscopy.Results (1) The cortical neuron survival decreased time-dependently in HI group and reached peak at 8h after hypoxia-ischemia. Glutamate leaded the cortical neuron survival decreasing time-dependently. 8.Regulatory effect of cytokines on promoter activity of human transforming growth factor-?_1 gene

Objective:To study the effect of the cytokines, tumor necrosis factor-?(TNF-?), interferon-?(IFN-?), interferon-?(IFN-?), platelet-derived growth factor BB(PDGF-BB), basic fibroblast growth factor(bFGF),on promoter activity of human transforming growth factor-?1(TGF-?1) gene.Methods:A construct of phTGF2.14 containing sequence from -1 328 bp to +812 bp of human TGF-?1 gene,linking with chloramphenicol acetyltransferase(CAT) as reporter gene,was transiently transfected into rat hepatic stellate cell line nFSC. The cells were subsequently treated with TNF-?,IFN-?,IFN-?,PDGF-BB,bFGF, and the CAT activity was assessed 48 hours after stimulation with each cytokines.Results:TNF-? of 10 ng/ml can increased the CAT activity of phTGF2.14 to 3.24 fold compared to control. IFN-? and IFN-? at 1 000 U/ml decreased CAT activity to (42?12)% and (58?6)% of control respectively.PDGF-BB,bFGF of 10 ng/ml had no effect on promoter activity of human TGF-?1 gene;Combined application of IFN-?,IFN-? and TNF-?,the promoter TGF-?1 gene were 1.32 and 1.46 fold compared with control,respectively.Conclusion:These data indicate that TNF-? can increase the promoter activity of human TGF-?1 gene, but IFN-?,IFN? can downregulate the CAT activites of phTGF2.14, and IFN-?,IFN-? can interdict the upregulate effect of TNF-? on phTGF2.14. We did not find PDGF-BB,bFGF have any effect on TGF-?1 promoter. These provided an essential evidence for study the interaction mechanism of cytokines in fibrogenisis diseases.
9.Dynamic analysis of the transverse arch of the foot and its relationship with the hallux valgus

Objective To study the spatial change of transverse arch in the metatarsal head during a normal walking cycle, demonstrate the significance of transverse arch in normal walking and the relationship with hallux valgus. Methods A modeling gait cycle was obtained under the C- armed machine, the coronal spatial change of the transverse arch of the metatarsal head were recorded using video tape. On computer work station, the images from video tape were transformed to mpeg files. A model of the transverse arch was created for the analysis of the relationship between the transverse arch index Q and sinking time; between Q and I andⅡ intermetatarsal angle;Ⅰ andⅣ intermetatarsal angle; sinking time and body mass. Results There was a transverse arch formed by the 5 metatarsal heads. In normal walking, during standing phase to toe- off phase, the 2 and 3 metatarsal heads approach the ground resulting in sinking of the transverse arch. The transverse arch index had an adverse relationship with the hallux valgus angle. The time of the transverse arch sinking decrease accompanying with the transverse index Q. Conclusion The transverse arch of metatarsals played an important role during normal gait cycle. During normal walking, when the center of pressure moved forward, the force acting on the 2 and 3 metatarsal was increasing. So, the metatarsal heads drop down resulting in sinking of the transverse arch of the metatarsal head. In hallux valgus patients, the time of transverse arch sinking was earlier than normal and duration was longer. The total force acting on the 2 and (or) 3 metatarsal increased, which may caused disorder of forefoot.
10.Clinical features and follow-up study of 49 elderly patients with pituitary adenomas

Objective To study the clinical features and therapeutic effects of pituitary adenomas in elderly patients. Methods The clinical materials including main clinical manifestations, diagnostic methods and outcome of treatments of 49 elderly patients with the pituitary adenomas from 1987 to1998 were analyzed retrospectively. Results It was showed from the data that the average duration of illness was 4 4 years; and 46(93 8%) cases were with visual deterioration, 20 (40 8%) with headache, and 16(32 7%) with endocrine abnormality. The diameter of tumors was more than 3 cm in 25 (51 0%), and the non functioning adenomas were found in 27 (55 1%). Operation was undergone through trans sphenoidal (61 2%), subfrontal (32 7%) and extensive subfrontal extradural approaches(6 1%), respectively. Among the 49 cases, tumor was totally or subtotally removed in 38 cases (77 6%), large partially or partially removed in 11 cases (22 4%). The results of operation through the trans sphenoidal approach were superior to other routes All patients were long term followed up for an average of 50 5 months. Visual disturbances were improved in 27 of 46(58 7%). A total of 43 patients could live by themselves Tumor disappearance was observed in 30 cases and recurrence in five cases. Five cases showed no changes in tumor size. 28 cases underwent radiotherapy after surgery and the occurrence rate of hypopituitarism after radiotherapy turned to be 50%. Total tumor control rate was calculated as 93%. Conclusions Macroadenomas and non functional pituitary adenomas happened most frequently in elderly patients and the main clinical symptoms was visual deterioration. Trans sphenoidal route for microsurgery was believed to be the first choice of operation. Tumor recurrence can be controlled or delayed by postoperative radiotherapy, but severe complication of the visual injury and pituitary dysfunction should be paid much attention.
