Chinese Journal of Digestive Endoscopy 2001;0(01):-
Objective To study the clinical characteristics in patients with peptic ulcer (PU) in different nationalities and regions. Methods Retrospective analysis on the clinical data of 2473 cases of PU encountered in our hospital within past 15 years. Results The detecting rate of PU is 12.53% ; whith the ratio of males and females 3.67:1 inwhich the detecting rates of Han and Hui nationalities are 13.42% and 10.66% respectively, Among them 44.4% were duodenal ulcer,50.63% were gastric ulcer, the ratio being 0.88:1. Conclusion The detecting rate of peptic ulcer in Han nationality is hihger than in Hui nationality , gastric ulcer occurs more often than duodenal ulcer. Episodic attacks occur mostly in spring and autumn . As to the location of ulcer in duodenal ulcer they are most on anterior wall and greater curvature of bulb; in gastric ulcer they are most on gastric angul and antrum , the peak age of incidence is 21 to 50. Bleeding is the most common complication in peptic ulcer patients.

AIM: To observe the influence of Ganoderma lucidum spores on the cytokine IL-1? and c-Fos in the brain tissue of epilepsy rats. METHODS: The IL-1? level was examined by radioimmunoassay and the c-Fos expression was measured by immunohistochemical assay. RESULTS: Comparing the Ganoderma lucidum spores group with the epilepsy model group: c-Fos positive cell count in brain cortex and hippocampus in treatment group was obviously reduced. IL-1? level in brain tissue was also reduced obviously. CONCLUSION: Ganoderma lucidum spores effectively reduces cytokine IL-1? in brain tissue of epilepsy rats, improves the immunity dysfunction and plays a role in anti-epilepsy through suppressing c-Fos expression in epilepsy rats brain tissue and blocking of LRG.

AIM: To study the mechanism of diabetic cardiomyopathy and abnormality of oxygen free radicals. METHODS: The contents of myocardial cytosolic cytochrome C, mitochondria cytochrome C, mitochondrial calcium, NO, MDA and the activity of SOD and NOS were determined in diabetic rats induced by STZ. The pathological changes were observed under transmission electron microscope. RESULTS: Compared to the normal and ganoderma group, the levels of mitochondrial NO, iNOS, MDA, calcium and plasma Cyt-C in rat myocardium were higher (P<0.05), while mitochondrial Cyt-C and SOD were lowered in model group (P<0.05). The bouncary indistinct, disorganization, a focal loss of muscular fibril, myocardium mitochondria swelling, pulmonary vascular endothelial cellular swelling and obstructed lumen of the capillary were also observed under transmission electronic microscope. CONCLUSION: The findings indicate that oxyradical and lipid peroxidation might be associated with the damage of myocardial mitochondria in NIDDM rats. Cyt-C and mitochondrial calcium is also involved in the process.

OBJECTIVETo study the effects of super painkiller on the sexual function of male Wistar rats with diabetes.
METHODSThirty-two Wistar rats at 3-5 months of age were divided into two groups at random: 22 in the diabetes group, and 10 in the control group. After 72 hours, the former was further divided into 2 subgroups: non-treatment and super painkiller treatment. In 5 weeks, blood glucose was determined. After the apomorphines experiment, the rats were killed, blood taken from the vein, homogenate prepared from the isolated testis tissue and the level of NO and NOS in the serum and tissue homogenate surveyed, separately.
RESULTSCompared with the control group, serum NO and NOS of the diabetic rats dropped sharply. Compared with the non-treatment group, the serum NO and NOS of the super painkiller treatment group were very high and the difference was significant (P < 0.01). Apomorphine injection showed that the times of penis erection of the treated rats were more than those of the non-treatment group (P < 0.01) , but the difference was not significant compared with the control (P > 0.05).
CONCLUSIONSuper painkiller has sure effect of reducing blood glucose, with a certain curative value for sexual and reproductive malfunction of diabetic rats.

Objective:To explore the new mode of screening and intervention for obstructive sleep apnea (OSA) in civil servants and its influencing factors. Methods:From September to December 2017, 1 241 civil servants who underwent annual physical examination in the outpatient department of a civil servant unit in Guangdong province were enrolled. They were screened for high-risk patients with OSA by Berlin questionnaire, and then those high-risk patients would receive type 3 home sleep testing (HST). Patients diagnosed with OSA were given free continuous positive airway pressure (CPAP) intervention and follow-up. The HST acceptance of high-risk OSA patients, OSA prevalence, CPAP initial treatment response rate and adherence trend and influencing factors were analyzed among the civil servants. Results:A total of 1036 civil servants completed the Berlin Questionnaire screening, of which 22.0% (228/1 036) were positive for the Berlin Questionnaire and were considered to be at high risk for OSA. A total of 228 high-risk OSA patients underwent free HST screening, and 32.5% (74/228) refused sleep monitoring. 154 people received sleep monitoring, 103 people were eventually diagnosed with OSA, of which 41 were mild (40.2%), 35 were moderate (33.3%), and 27 were severe (26.5%). The estimated prevalence of OSA among civil servants was 9.9% (103/1 036). All OSA patients were provided with free auto-CPAP treatment, and only 55.3% (57/103) received initial CPAP treatment. Multivariate logistic regression analysis showed that the CPAP treatment response rate was positively correlated with the severity of OSA ( OR=5.65, 95% CI: 1.007―31.693); it was negatively correlated with the general health status score of the 36-Item Short Form of the Medical Outcomes Survey (SF-36, OR=0.968, 95% CI: 0.938―0.998).Self-determined behavioral interventions and self-perceptions that treatment not needed were the reasons for not receiving treatment. In the first week of initial CPAP treatment follow-up, 70.2% (40/57) patients had good adherence ≥4 h/night, and the median adherence was 5.0(4.0, 6.0) h/night. The adherence of 17 cases (29.8%) was less than 4 h/night, and the median adherence was 0 (0, 2.0) h/night. Univariate analysis showed that those with difficulty falling asleep, anxiety, depression, and adverse reactions to CPAP (nasal mask discomfort and suffocation) had worse adherence. The long-term adherence to CPAP treatment gradually declined, and by the 2-year follow-up period, only 22.0% of patients had good adherence. Conclusions:Even with free sleep screening and disease intervention mode, the acceptance of sleep monitoring and CPAP treatment is still low, and the short-term and long-term adherence to CPAP is poor. Epworth sleepiness Score, hypertension, and disease cognition affected the acceptance of sleep monitoring. Psycho psychological factors and adverse reactions to CPAP affect patient compliance.

OBJECTIVE To study the improvement effects and mechanism of gentian iridoids (GI)on lipid accumulation and inflammation in non -alcoholic fatty liver disease (NAFLD)model. METHODS Human hepatoma HepG 2 cells induced by free fatty acids(0.5 mmol/L)were selected as NAFLD cell models . The effects of GI at different concentrations (0.125,0.25,0.5,1,2,4 mg/mL)on HepG 2 cell viability were investigated . HepG2 cells were divided into control group ,model group and low ,medium and high concentration groups of GI (0.25,0.5 and 1 mg/mL). Except for control group ,0.5 mmol/L free fatty acid was added in other groups for modeling .After 24 h of culture ,the formation of lipid droplets was observed in HepG 2 cells;the content of triglyceride(TG)was detected . The levels of aspartate aminotransferase (AST),alanine aminotransferase (ALT)and inflammation indexes [interleukin-1 β(IL-1 β),IL-6,tumor necrosis factor α(TNF-α)] in cell supernatant were determined . The mRNA expressions of sterol -regulatory element binding protein (SREBP-1c)and fatty acid synthase (FAS)as well as the expression of nuclear factor -κB(NF-κB)related proteins [NF-κB inhibitor protein α(IκBα),NF-κB,phosphorylated NF -κB(p-NF-κB)] were also detected . RESULTS After treated with different concentrations of GI ,the cell viability of HepG 2 cells was improved significantly(P<0.01). Compared with model group ,the number of red lipid droplets in the cells of each concentration group of GI was significantly reduced ,the nuclear atrophy was not obvious ,and the size was normal ;the content of TG (except for low concentration group ),and the levels of ALT ,AST,IL-1β,IL-6(except for low concentration group )and TNF - α in cell supernatant were all decreased significantly ;mRNA expression of SREBP -1c and FAS and the phosphorylation level of NF -κB were also decreased significantly (P<0.05 or P<0.01);the protein expression of I κBα was increased significantly (P<0.01). CONCLUSIONS GI can reduce the lipid accumulation and inflammatory response of NAFLD cell model induced by free fatty acids, and its mechanism may be related to the inhibition of SREBP-1c/FAS and NF -κB signaling pathway .
