1.Contrivance and clinical application of anatomical dynamic hip lock plate(ADHLP)

[Objective]To investigate the effects of self-contrive antomical dynamic hip lock plate(ADHLP) in fixation of intertrochanteric fracture.[Method]Seventy-three cases (male 52,female 21) of intertrochanteric fracture were treated by internal fixation with ADHLP.The age were from 19~92y,average 54.7y.Traumatic causes:traffic accident,32 cases;pedesteian tumble,21 cases;fall from height,9 cases;other reasons,11 cases. According to the evans' classification:type Ⅰ,42 cases;type Ⅱ,17 cases;type Ⅲ,9 cases;type Ⅳ,5cases.[Result]Operating time averaged 60 minutes(ranged from 40~90 minutes). Blood loss in operation:average 200 ml (ranged from 100~400 ml).Complication was not found,(such as:coxa vara;femoral head concis;rotation displacement;infection;re-fracture;lock plate broken or loosening,et al.).All cases were followed-up from 6 to 48 months,average 36 months.The function of all hips were satisfactory.The excellent and good rate was 98.6%.[Conclusion]ADHLP in fixation of intertrochanteric fracture is accord with the demands of biomechanism,The indication is widespread,applicable for osteoporotic patients or old patients mespecial.This operation is simple and reliable,with little invasive injury and less blood losing.
2.The clinical research Chuanxiong-Chatiao powder treating migraine

Objective To observe the clinical effect of Chuanxiong-Chatiao San in treatment hemicrania. Methods All patients were randomly recruited into a control group (75cases) and a treatment group (78 cases). The control group was treated with Gastrodin injection, Ligustrazine injection and Mailuoning injection; on this basis, the treatment group was treated with Chuanxiong-Chatiao powder. Both groups were treated for 14 days with a 7 days interval. Results The excellent rate and effective rate were 91.03% and 96.08% in the treatment group respectively; and 70.66% and 94.67% in the control group respectively. The difference between the two groups was significant (χ2= 12.7143,P<0.01) . Conclusion Chuanxiong-Chatiao powder combined with western medicine is effective in treating hemicrania.
3.Optimal position of locking compression plate for proximal humeral fractures:choice of lateral deltoid splitting approach?

BACKGROUND:Classical deltopectoral approach leads to a great injury to soft tissues. It is difficult to lay the steel plate on the lateral side. The steel plate is frequently laid on the anterolateral side of humerus. Fixed position is not ideal.
OBJECTIVE:To evaluate the safety and feasibility of lateral deltoid-splitting approach in treatment of proximal humeral fractures.
METHODS:From August 2008 to August 2013, 114 patients with fracture of surgical neck of humerus, who were treated in the Department of Orthopedics, Yan’an Hospital Affiliated to Kunming Medical University in China, were enrol ed in this study. They were treated with locking compression plate, and assigned to deltoid splitting approach group and deltopectoral approach group (n=57). Surgical trauma, fracture healing, curative effects, functional recovery and Constant shoulder score were compared between the two groups.
RESULTS AND CONCLUSION:Al patients were fol owed up for at least 8 months. Compared with the deltopectoral approach group, the proximal humeral fractures were healed as scheduled in the deltoid splitting approach group, and the anteflexion, abduction, shoulder lifting of the shoulder joint, and sensation of lateral skin of the upper arm were not impacted. No significant difference in Constant shoulder score was detectable between the two groups (P>0.05), which suggested that deltoid splitting approach for treating the proximal humeral fractures was safe and feasible. In addition, in accordance with the Neer’s classification system, the excellent and good rate in the deltoid splitting approach group (91%) was significantly higher than that in the deltopectoral approach group (75%) (P<0.05). Therefore, the deltoid splitting approach can be used as the first choice for the treatment of proximal humeral fractures.
4.Research on antiproliferative effect of flavones isolated from Laggera pterodonta.

OBJECTIVETo research the cytotoxicity and in vitro antiproliferative effect of the six flavone compounds extracted from Laggera pterodonta.
METHODThe cytotoxicity on the normal cells and antiproliferative effect on tumor cells were tested by MTT assay, and then the preliminary structure-activity relationship was analysed. The phase distribution of the cell cycle and apoptosis rate were analyzed by flow cytometry.
RESULTThe results of MTT assay showed 5,7,3',4'-tetramethoxy-3-hydroxyflavone and chrysosplenetin B inhibited growth of A549 and Hela cells significantly with a dose dependent mode, while exhibited low cytotoxicity to the two normal cells, Vero and EVC304. Both compounds contain ortho-phenolic methoxyl moietys in their structures. Flow cytometry analysis revealed that Hela cells treated with increasing quantities of chrysosplenetin B increased the percentage of cells in the G2/M phase, and Hela and A549 cells treated with increasing quantites of the 5,7,3',4'-tetramethoxy-3-hydroxyflavone and chrysosplenetin B increased the apoptosis rates.
CONCLUSIONThe 5,7,3',4'-tetramethoxy-3-hydroxyflavone and chrysosplenetin B extracted from L. pterodonta showed high antiproliferative effect on cancer cells with low cytotoxicity on normal cells, and took the effects on A549 and Hela cells through the hold-up of the G2/M phase of the cell cycle and induction of the apoptosis.

5.Chemical constituents of Laggera pterodonta.

OBJECTIVETo study the chemical constituents of Laggera pterodonta.
METHODThe ethanol extract of L. pterodonta was isolated by column chromatogramphy on silica gel, ODS, and Sephadex LH-20 to afford compounds. The structures of the obtained compounds were identified by chemical reactions and spectroscopic analysis.
RESULTNineteen compounds were separated and identified to be pterodondiol (1), ilicic acid (2), artemitin (3), chrysosplenetin B (4), 3,5-dihydroxy-3',4',6,7-tetramethoxyflavone (5), chrysosplenol D (6), 5,6,4'-trihydroxy-3,7-dimethoxyflavone (7), quercetin (8), tamarixetin (9), patuletin (10), quercetin-3-O-beta-D-galactopyranoside (11), patuletin-3-O-beta-D-glucopyranoside (12), helichrysoside (13), 4,5,7-trihydroxy-6-methoxyflavone-3-O-beta-D-rutinoside (14), kaempferol-3-O-beta-D-glucopyranoside (15), stigmasterol (16), stigmasterol 3-O-beta-D-glucopyranoside (17), 2-hydroxy-benzoic acid (18), beta-sitosterol (19).
CONCLUSIONCompounds 5, 7, 9-15, and 17-18 were isolated from this plant for the first time. The 13C-NMR data of compound 7 is reported for the first time.

6.Content determination and chemometric analysis of 8 components in Smilax glabra

OBJECTIVE To provide reference for Smilax glabra quality control. METHODS UPLC method was established to simultaneously determine the contents of 5-O-caffeoylshikimic acid, neoastilbin, astilbin, neoisoastilbin, isoastilbin, engelitin, resveratrol and isoengelitin in 20 batches of S. glabra from different areas (No. S1-S20). The quality evaluation of 20 batches of samples was performed by chemometrics; the differential biomarkers that affected the quality of S. glabra were screened. RESULTS The measured 8 components had good linear relationship within the range of measured concentration (r≥0.999 6). RSDs of precision, repeatability and stability tests (24 h) were all lower than 2.00% (n=6). The average recoveries varied between 97.60% and 106.40% (RSDs were all lower than 2.00%, n=6). Cluster analysis showed that the samples produced in Zhejiang (S1-S5) and Jiangxi (S6-S10) were clustered into one category; the samples produced in Hunan (S11-S15) were clustered into one category; the samples produced in Yunnan (S16-S20) were clustered into one category. Principal component analysis showed that the first two principal components could represent 85.60% information of 8 components in S. glabra. Partial least squares-discriminant analysis showed that variable importance projection values of 5-O-caffeoylshikimic acid, astilbin, isoastilbin and neoastilbin were all greater than 1. CONCLUSIONS There are differences in the contents of the above 8 components in S. glabra from different origins; 5-O-caffeoylshikimic acid, astilbin, isoastilbin and neoastilbin may be differential markers affecting the quality of S. glabra.
