Objective To investigate the occurrence, progress and conversion of hypotony in anterior proliferative vitreoretinopathy (aPVR), and to provide knowledge about how to prevent and treat it. Methods Animal models of chronic hypotony by aPVR were made with cultured homologous dermal fibroblasts on pigmented rabbits. The intraocular pressure (IOP) and ultrasound biomicroscopy(UBM) examination were taken preoperatively and on days 7,14, 28 and 56 postoperatively. 2.ESTABLISHMENT OF AN EXPERIMENTAL RETINAL DETACHMENT AND REATTACHMENT MODEL USING MICROPIPETTING IN CATS

To establish a model of experimental retinal detachment and reattachment, extracapsular lens extraction and vitrectomy were performed in 78 adult cats. After three weeks, a glass micropipette with a flat tip (diameter 150~200?m) was inserted between the neural retina and retinal pigment epithelium (RPE) monolayer. Retinal detachments were produced by subretinal injection of 0.25% solution of Healon through this micropipette. Twenty~four hours later, detached retinas were reattached by air-fluid exchange and subretinal fluid drainage with a glass micropipette. Then 30% perfluoropropane (C 3F 8) gas temponade was porfomed. Retinal detachment was successfully made in 93.6%(73/78), and 97.1%(33/34)of the detached retinas were successfully reattached. By light microscopy, histologic sections showed that seperated retina located between RPE cells and photoreceptors, and neural retina contacted to RPE cells closely in reattached retinas. In conclusion, by using micropuncture, we have established an animal model of retinal detachment and reattachment with minimum injury to the retina, and a high rate of retinal detachment and reattachment were obtained.

5.Cultured homologous dermal fibroblasts of rabbit in aPVR animal model

Objectives:To establish animal model of anterior proliferative vitreoretinopathy (aPVR) with cultured homologous dermal fibroblasts of rabbit, and to provide evidence why hypotony was caused by aPVR. Methods:Animal models of aPVR were established with cultured homologous dermal fibroblasts on pigmented rabbits. Rabbits were sacrificed on the 14th, 28th and 56th day after the operation to prepare naked eyes and to receive histological examinations. Results:Naked eye examination showed that the peripheral retina was detached by dragging in the experimental group 28 and 56 days postoperatively. Microscopic examination showed atrophy or absence of the non-pigmented ciliary epithelium on the 28th and 56th postoperative day in the experimental group. Conclusions:The epiciliary membrane in aPVR dragged the ciliary body, made atrophy of non-pigmented epithelium, which perhaps was the main cause of hypotony.
6.Investigation of the expression of proliferating cell nuclear antigen of retinal pigment epithelial cells after experimental retinal detachment and reattachment in cats

Objective To examine the expression of proliferating cell nuclear antigen (PCNA) of retinal pigment epithelial (RPE) cells, thus assessing the role of mechanism of contact inhibition playing in the process of experimental retinal detachment and reattachemnt. Methods Retinal detachment was produced in 72 cats by subretinal injection of 0.25% solution of healon through a micropipette three weeks after extracapsular lens extraction and vitrectomy. Some of the detached retinae were reattached 24 hours later. At different time, the cats were killed and eye globes were fixed and embeded in paraffin. Histologic sections were processed for immunohistochemistry examination using an antibody to detect PCNA protein. Labeled RPE cells were identified, and the proliferation was quantified in detached and un detached retinae of detachment group, and also in reattached retinae of reattachment group. The comparsion of PCNA labeled RPE cells in different groups were analyzed by ANOVA. Results In detached regions of detachment group, PCNA expression of RPE cells occured within 24 hours, and reached a maximum after 5 6 days, then gradually declined to barely detectable levels after 20 days. Similar tendency was found in reattached retinae, but the number of PCNA labeled RPE cells was obviously small. Fewer PCNA labeled RPE cells were found in regions of un detached retinae in detachment group. The difference of these three groups was significant. Conclusion Proliferation of RPE cells is induced when they lose contact with neural retina, but inhibited after neural retina reattached to RPE cells. It suggests that the mechanism of contact inhibition plays a role in the proliferative process after retinal detachment and reattachment.
7.Clinical application of artificial intelligence to lung nodules diagnosis in regional medical center

Objective To explore the efficacy of artificial intelligence (AI) detection on pulmonary nodule compared with multidisciplinary team (MDT) in regional medical center. Methods We retrospectively analyzed the clinical data of 102 patients with lung nodules in the Xiamen Fifth Hospital from April to December 2020. There were 57 males and 45 females at age of 36-90 (48.8±11.6) years. The preoperative chest CT was imported into AI system to record the detected lung nodules. The detection rate of pulmonary nodules by AI system was calculated, and the sensitivity, specificity of AI in the different diagnosis of benign and malignant pulmonary was calculated and compared with manual film reading by MDT. Results A total of 322 nodules were detected by AI software system, and 305 nodules were manually detected by physicians (P<0.05). Among them, 113 pulmonary nodules were diagnosed by pathologist. Thirty-eight of 40 lung cancer nodules were AI high-risk nodules, the sensitivity was 95.0%, and 25 of 73 benign nodules were AI high-risk nodules, the specificity was 65.8%. Lung cancer nodules were correctly diagnosed by MDT, but benign nodules were still considered as lung cancer at the first diagnosis in 10 patients. Conclusion AI assisted diagnosis system has strong performance in the detection of pulmonary nodules, but it can not content itself with clinical needs in the differentiation of benign and malignant pulmonary nodules. The artificial intelligence system can be used as an auxiliary tool for MDT to detect pulmonary nodules in regional medical center.
8.Effects of Augmented Renal Clearance on Blood Trough Concentration of Patients Receiving High-dose Regimen of Teicoplanin

OBJECTIVE:To study the effects of augmented renal clearance (ARC)on blood trough concentration of patients receiving high-dose regimen of teicoplanin. METHODS :Patients who received high-dose regimen of teicoplanin in the ICU were prospectively collected from the Affiliated Suzhou Hospital of Nanjing Medical University/Suzhou Municipal Hospital during Jul. 2018-Jun. 2020. They were divided into ARC group and normal renal function group according to corrected creatinine clearance. The dosage regimen of teicoplanin in the two groups were loading dose of 600 mg,q12 h×3 doses,maintenance dose of 6-10 mg/kg,qd,and the dosage was adjusted in combination with creatinine clearance rate and blood trough concentration. The trough concentration of blood samples which were collected 30 min before the 4th and 8th-10th dosage of teicoplanin were determined by HPLC. Trough concentration ,clinical efficacy ,Gram-positive bacterial clearance rate and the occurrence of ADR were compared between 2 groups. RESULTS :A total of 56 patients were included and divided into ARC group (18 cases)and normal renal function group (38 cases). ARC group had younger age (P<0.001)and lower serum albumin level (P=0.025)than normal renal function group. The trough concentrations before administration of the 4th and 8th-10th dosage in ARC group were lower than normal renal function group (P=0.034;P=0.035). The trough concentrations in the ARC group and normal renal function group before 8th-10th dosage were all higher than 30 min before the 4th dosage (P=0.003;P<0.001). The clinical efficacy rate and the clearance rate of Gram-positive bacteria in ARC group were 77.8% and 76.2%,which were lower than those of the normal renal function group ,but there was no statistical difference (P=0.195;P=0.223). There was no liver function damage ,hemocytopenia and allergic reaction in both groups ,but in the normal renal function group ,the causal relationship between acute renal damage and teicoplanin was assessed as “very likely ”in one patient. CONCLUSIONS :ARC patients are younger ,most of them have hypoproteinemia,and the blood trough concentrations of teicoplanin in high-dose regimen are significantly lower than those of normal renal function patients. For critical ill ARC patients ,it is advisable to increase the loading dose of teicoplanin to make the trough concentration reach the target concentration range quickly.
