1.Modulatory effects of the fruits of Tribulus terrestris L. on the function of atopic dermatitis-related calcium channels, Orai1 and TRPV3

Objective: To examine the effects of Tribulus terrestris L. (T. terrestris) extract on the modulation of calcium channels to evaluate its use in topical agents for treatment of atopic dermatitis. Methods: The 70% methanol extract of T. terrestris was prepared. Human HEK293T cells with over-expressed calcium release-activated calcium channel protein 1 (Orai1), transient receptor potential vanilloid 1, or transient receptor potential vanilloid 3 (TRPV3) were treated with T. terrestris extract. Modulation of ion channels was measured using a conventional whole-cell patch-clamp technique. Results: T. terrestris extract (100 mg/mL) significantly inhibited Orai1 activity in Orai1-stromal interaction molecule 1 co-overexpressed HEK293T cells. In addition, T. terrestris extract significantly increased the TRPV3 activity compared with 2-Aminoethyl diphe-nylborinate (100 mmol/L), which induces the full activation of TRPV3. Conclusions: Our results suggest that T. terrestris extract may have a therapeutic po-tential for recovery of abnormal skin barrier pathologies in atopic dermatitis through modulating the activities of calcium ion channels, Orai1 and TRPV3. This is the first study to report the modulatory effect of a medicinal plant on the function of ion channels in skin barrier.
2.A Case of Recurrent Tolosa-Hunt Syndrome.

Tolosa-Hunt syndrome (THS) is a clinical syndrome of painful ophthalmoplegia associated with an idiopathic inflammation in the superior orbital fissure or cavernous sinus. We report a 61-year-old woman with recurrent THS. She had been treated with steroids for the painful ophthalmoplegia. After a symptom-free period, she developed similar symptoms. The postcontrast brain MRI showed increased signal in the cavernous sinus, which was also seen in the previous MRI.

3.Pattern of circle of Willis between normal subject and patients with carotid atherosclerotic plaque

Objective: We investigated whether circle of Willis (COW) morphology on 3D time-of-flight (TOF)
MR angiography differs between young normal subjects, older normal subjects, and patients with
carotid atherosclerotic plaques. Methods: One hundred seventy-seven subjects were grouped according
to age into a young group (20 – 40 years; n = 91) and an older group (> 60 years; n = 86). Subjects
underwent brain MR examination as part of a health check-up. Fifty-three patients with carotid
atherosclerotic plaque were also included for evaluation in this study. COW morphology on 3D TOF
MR angiography was analyzed in terms of completeness or incompleteness of the anterior and posterior
components of the circle and completeness of the circle. Results: An incomplete pattern of anterior
circulation was significantly more common in carotid atherosclerotic plaque patients (20.7%) than
normal older subjects (5.5%) (p < 0.01). A complete posterior circulation pattern was more frequent
in normal young subjects (46.5%) than in normal older subjects (16.5%) or the patient group (18.9%)
(p < 0.01). Of patients with carotid artery stenosis, 18.9% had a bilateral incomplete connection and
were significantly more likely to have an incomplete pattern than normal young (2.3%) or normal
older subjects (2.2%) (p < 0.001).
Conclusion: Young, healthy subjects were significantly more likely to have a complete pattern of posterior
circulation than older subjects. Patients with carotid atherosclerotic plaques were significantly more
likely to have incomplete anterior circulation and an incomplete circle than young, normal subjects.

6.Image-Guided Percutaneous Biopsies with a Biopsy Gun.

PURPOSE:We report the results of image-guided percutaneous biopsies with a biopsy gun and evaluate the clinical usefullness. MATERIALS AND METHODS: One hundred and five biopsies under ultrasonographic or fiuoroscopic guidance were performed. Various anatomic sites were targeted (liver;50, chest;22, kidney;12, pancreas;8, intraperitoneum;7, retroperitoneum; ). RESULTS:Obtained tissue was diagnostic in 98 of the 105 biopsies(93%). In each instance, representative core tissue specimens were obtained. Evaluation of the core tissue by pathologist revealed consistent, uniform specimens that contained significant crush artifact in no case. Five biopsies yielded inadeguate tissues which were too small for histopathologic interpretation or were composed of necrotic debris. Two biopsies yielded adequate tissues, but tissues were not of the target. The diagnoses were malignancy in 77 biopsies and benign disease in 21 biopsies. No complications other than mild, localized discomfort were encountered except a transient hemoptysis and pneumothorax which was observed in two patients. CONCLUSION:Cutting biopsy with a biopsy gun provided sufficient amount of target tissue for an accurate diagnosis of malignant and benign disease. It was a safe and useful procedure for percutaneous biopsy.

7.Sonographic characterization of tenosynovitis.

Tenosynovitis of the extremities is not uncommon but its diagnosis is not easy owing to its non-specific clinical manifestation. Thus it was beyond the field of imaging diagnosis so far. Recently the development of high resolution ultrasonogram has aided preoperative imaging diagnosis of tenosynovitis. The authors performed a retrospective review of 27 patients who had ultrasonography due to tendon pathology(including 18 tenosynovites) by oserving sonographic finding and evaluation the diagnostic value of each finding. The overall diagnostic accuracy was 81.1% and common sonographic findings were focal swelling of the tendon. well-defined margin of the lesion, preserved fibrillar pattern, echo change of the lesion site and fluid collection. Above al,. fluid collection was the only statistically significant criterion for diagnosis of tenosynovitis (p<0.05). But its sensitivity was as low as 50%. In conclusion the ultrasonography is useful in diagnosis of tenosynovitis and fluid collection is of diagnostic value, but the differentiation between nodular tenosynovitis without fluid collection and other benign tumor is still beyond the scope of ultrasonographic diagnosis.

8.Role of IVIR in Limb Fracture.

PURPOSE: In evaluation of the limb fracture, MR scan has been used in limited roles as evaluating the associated soft tissue injury not the fracture itself. This study aims at understanding the possible role of MR in fracture. METHODS AND MATERIALS: thirty three sets of MR scans in twenty nine patients were retrospectively analyzed. They included twelve tibial plateau fractures, four pateliar fractures, four distal femur fractures, five epiphyseal fractures and four others. RESULTS: All MRs except two showed better image and more information about the extent of the injury (93.9%). Evaluation of cartilage (which was impossible on other modalities) was possible in 28 MR scans (84.8%). Surrounding marrow change associated with fracture was also able to be evaluated. Associated soft tissue injury or other abnormality could be evaluated-cruciate or collateral ligament injury, meniscus tear, chodromalacia or osteonecrosis. In epiphyseal injury, direct demonstrability of premature bony fusion on MR took a critical role in making a management plan. CONCLUSION: MR scan in fracture has its value in evaluating the extent and degree of the injury and it is especially advantageous in detecting cartilage injury and early complication of growth plate injury.

9.MR angiography of the renal arteries.

We reviewed MR angiograms to evaluate its efficacy for visualizing the renal arteries and detecting renovascular disease. 41 renal arteries in 19 patients were examined by MR angiography. 3-D time-of-flight technique was used as routine examination method for MR angiography and 2-D time-of-flight technique was added in some particular cases to visualize venous flow. Within two weeks after MR angiography was performed, 23 renal arteries in 10 patients were additionally examined by conventional angiography or intraarterial DSA. The success rates of vessel visualization on MR angiography in normal renal arteries were 100% in main 67.7% in segmental, and 11.8% in intrarenal arteries. As a result of comparative study in normal main renal arteries with MR angiograms and conventional angiograms, overall correspondence in the number and the shape was noted and the caliber discrepancy between two examination did not exceed 3.0 mm. one arteriovenous fistula with aneurysm, one stenotic artery and two occluded arteries were well evaluated One arteriovenous fistula with aneurysm, and two occluded arteries were well evaluated by MR angiography. However three stenotic lesions were misdiagnosed as occlusions on MR angiography. and the overall accuracy was 87%. We conclude that MR angiography has the potential to be a noninvasive and useful screening method for determining the number of renal arteries and for detection of abnormalities of main renal arteries.

10.Efficacy of Laparoscopic and Percutaneous Radiofrequency Ablation of Renal Tumor.

PURPOSE: We report here on the safety and efficacy of nephron-sparing radiofrequency ablation(RFA) for treating renal tumor. MATERIALS AND METHODS: Starting June 2004, a total of 14 patients underwent RFA for renal tumor during the following 3 years. Of these, 12 cases were followed up for at least 6 months postoperatively. Eight cases of combined computed tomography(CT) and ultrasonogram-guided percutaneous RFA, and four cases of intraoperative ultrasonography-guided laparoscopic RFA were performed with mean follow-up of 18.2 months(range: 4-27 months). The treatment indications were a localized, small(<4cm), solid renal mass in the elderly patients and those patients with co-morbid conditions. Physical examination, CBC, determining the serum creatinine levels and urine analysis were performed for the follow-up laboratory study and kidney CT was performed at day 1, 1 week, 1 month, 3 months, 6 months and 1 year after ablation and thereafter semi-annually. The mean follow-up duration was 18.2 months(range: 4-27 months). RESULTS: All the patients underwent successful RFA without any serious events. Four patients had mild perinephric hematoma on the follow-up CT scan and there was one case of mild gross hematuria postoperatively. With a mean follow-up of 18.2 months, two patients showed residual tumor at 3 months & 22 months, respectively, on the follow-up contrast- enhanced CT after the first tumor ablation. One patient underwent a second RFA and another patient underwent laparoscopic radical nephrectomy, and no residual tumor was seen on the follow-up CT. Distant metastasis was not found in any cases and all the patients are alive on serial follow-up. CONCLUSIONS: Percutaneous or laparoscopic RFA is considered a useful treatment for selected patients who have a small renal mass, and to spare the nephrons. The ultimate role of this modality will continue to evolve and this warrants further studies.
