1.HbA1C for diagnosing diabetes:past,present,and future

Diabetes mellitus is a chronic metabolic disease with complications associated with long-term damage,dysfunction,and failure of various organs,and has become one of the leading causes of impairment of human health.At present,the prevalence of diabetes mellitus reaches 9.7%in China,which is considered one of the countries in the world with heavy diabetes burden.Patients with established diabetic complications will be at higher risk of poor health outcomes and with higsh healthcare costs.These realities support the critical need to identify diabetes and its precursors more efficiently and earlier.The current diagnostic criteria,fasting plasma glucose(FPG),and oral glucose tolerance test(OGYT),that require fasting and multiple blood samplings,may not be acceptable to patients and may limit the clinical application so as to impede the effort to diagnose diabetes in timely fashion.HbA1C,which accurately reflects longer term glycaemia,Can be done at any time without fasting or other preparation of the patient and may not be affected by short term lifestyle changes.Given the recognized need,some panels have been considering the possible utility of HbA1C as a supplementary screening tool to make the diagnosis of diabetes efficiently.
2.Characteristics of insulin sensitivity and insulin secretion in different types of impaired glucose regulation

Impaired glucose regulation includes three types, isolated-impaired fasting glucose, isolated-impaired glucose tolerance and combined glucose intolerance.The epidemiologic studies and pathogenetic studies indicate that each type has different characteristics of insulin secretion and insulin sensitivity.The distinct metabolic features conduce to different early interventions in order to prevent or delay their progress to type 2 diabetes.
3.Addison's disease in pregnancy

Although Addison's disease is uncommon, it may contribute to maternal and fetal morbidity and mortality if not treated appropriately. Clinical features, laboratory tests and endocrine function tests which conduce to diagnosis are reviewed. The long-term corticosteroid replacement therapy plays an important role in the treatment. Close monitoring during labor and postpartum, appropriate arrangement of corticosteroid could optimize the maternal and fetal outcome.
4.An investigation on the relationship between quality of life of the convalescent psychopaths in community and social supports

Objective To investigate the relationship between quality of life of the convalescent psychopaths in community and their social supports.Methods A total of 51 psychiatric patients in the rehabilitation period were recruited and investigated from the rehabilitation center of the community rehabilitation center in Beitang district,Wuxi city by using the questionnaire on the quality of life (QLS),social support scale(SSS) and the perceived social support Scale(PSSS).And then,they were compared with 50 healthy subjects.Results The dimension scores of QLS,SSS and PSSS of the psychiatric patients were significantly lower than those of healthy persons (P<0.01).The dimension scores of QLS had significantly positive relationship with most of the dimension scores of SSS and PSSS (P<0.05).Economic status,participation in social activities and family support showed significantly positive prediction to body function(P<0.05).Financial situation showed significantly positive prediction to mental function(P<0.05).Conclusion The quality of life of the convalescent psychopaths is poor,and there is a positive correlation with social support system.Economic status,participation in social activities and family support are the main factors that affect their quality of life.
5.Curative effects of pure radiotherapy and concurrent chemotherapy and radiotherapy for patients with cerical cancer

Objective To investigate the curative effects and adverse reactions of pure radiotherapy and concurrent chemotherapy and radiotherapy for patients with cervical cancer.Methods One hundred and twenty-seven patients with cervical cancer who accepted treatment in the Affliated Hospital of Inner Mongolia Medical Unive.rsity from May 2010 to May 2012 were collected.All patients were divided into two groups:pure radiotherapy group (n =65) and concurrent chemotherapy and radiotherapy group (n =62).The curative effects,adverse reactions and survival of two groups were observed.Results All patients were completed treatment.The median follow-up time was 42 months.The rate of complete response in the pure radiotherapy group was 80.0% (52/65),and the rate in the concurrent chemotherapy and radiotherapy group was 82.26% (51/62),with no significant difference (x2 =1.22,P =0.352).The 1-year overall survival rates in the pure radiotherapy group and the concurrent chemotherapy and radiotherapy group were 95.38% and 95.16% respectively,with no significant difference (x2 =0.32,P =0.533),but the 3-year overall survival rates were 81.54% and 90.32% respectively,the 5-year overall survival rates were 72.31% and 83.87% respectively,with significant differences (x2 =5.09,P =0.015;x2=3.87,P =0.039).However,for the patients who were ≥ 60 years,the 1-year overall survival rates in the two groups were 94.62% and 93.91% respectively,the 3-year overall survival rates were 85.02% and 87.25% respectively,the 5-year overall survival rates were 70.06% and 73.58% respectively,with no significant differences (x2 =0.06,P =0.753;x2 =1.16,P =0.279;x2 =0.48,P =0.511).The adverse reactions were mainly in grades 1-2.There were significant differences in the rates of leucopenia (56.10% vs.72.20%),thrombocytopenia (58.82% vs.76.80%),nausea and vomiting (34.04% vs.56.90%) among the two treatment groups (x2 =11.23,P =0.003;x2 =11.82,P=0.002;x2 =12.77,P =0.000).Conclusion The curative effect of concurrent chemotherapy and radiotherapy is better than that with pure radiotherapy for patients with cervical cancer,which can improve the 3-year and 5-year overall survival.But at the same time,it should be noted that the rates of adverse reactions may be increased during the same period.For the age of 60 or more patients with cervical cancer,concurrent chemotherapy and radiotherapy does not achieve even greater survival benefit.

7.Reasearch on mechanism of neurotrophins in discogenic low back pain.

Discogenic low back pain is the common type of chronic low back pain. However,its mechanism has not been completely clarified. Considerable evidence shows that neurotrophins play an important role in discogenic low back pain. The paper summarizes the mechanism of neurotrophins on discogenic low back pain according to the pain transfer pathway of neurotrophins in intervertebral disc, dorsal horn ganglia and spinal trigeminal nucleus. Changing the pain transmission by regulating neurotrophins and its receptor will provide a new way for the treatment of discogenic low back pain.

8.Professor Dong Gui-rong's experiences in treatment of child cerebral palsy.

According to the characteristics of the etiology and pathogenesis of child cerebral palsy, on the basis of "regulating the mind in treatment of all kinds of diseases" and "regulating the functions of five zang organs with back-shu points", Professor DONG Gui-rong applied the penetrating needling technique on the scalp points and acupuncture at back-shu points of five zang organs in the treatment of child cerebral palsy. The valuable clinical experiences have been summarized as "regulating the mind with scalp needling technique, benefiting the brain and opening the orifice", "regulating five zang organs with back-shu points" and "integration of acupuncture and rehabilitation, and function reconstruction". Two effective cases were introduced.
