Aiming at the shortcomings of traditional medical device tracking and supervision mode, such as delayed feedback of medical device positioning information, poor visual tracking effect of medical device, and missing early warning of medical device working state supervision, a medical device visual tracking and supervision system based on wireless Local Area Network (LAN) real-time positioning system is developed. Introduction of wireless LAN real-time positioning system, using standard coding algorithm generating device object ID tags, with the only attribute based on RFID read-write terminal complete physical mapping of medical equipment and material ID label, we realize a real-time positioning and traceability of medical equipment, with the help of active warning algorithm based on depth of learning medical equipment working state supervision for early warning, with the help of indoor map generation and trajectory playback medical equipment to realize visualization back regulation. Choose equipment of hospital of Linyi Central Hospital for performance evaluation of the carrier, the evaluation model of engineering practice and clinical statistical analysis, the results show that the system has a complete real-time positioning, visible trace, abnormal warning function. In medical equipment real-time positioning information visual tracing feedback, abnormal state warning etc, medical equipment has obvious advantages.

Niigata Prefecture is suffering from a chronic shortage of physicians. The 2000 statistics shows the number of physicians in active service is 162.5 for 100,000 population. This ranks the prefecture 40th among all the 47 administrative divisions of Japan (Average : 191.6 physicians). In the area including Ojiya, where our hospital is located, the number of physicians per 100,000 population works out at 124.5. A lack of physicians poses a serious problem. It is extremely difficult to maintain the quality of medical treatment and meet emergencies. As one of the countermeasures, the Niigata Prefectural Federation of Agricultural Cooperatives for Health and Welfare has made a network of seven hospitals in the Chuetsu area. The network is functioning well between our hospital and the surrounding four hospitals-Nagaoka Chuo General Hospital (Nagaoka), Nakajo Hospital and 2nd Nakajo Hospital (Tokamachi) and Kariwa-gun General Hospital (Kashiwazaki) because of favorable geographic conditions. Our hospital accepts psychiatric patients from 2nd Nakajo Hospital who need surgical treatment. Although our hospital does not have a psychiatric department, we accept those patients and have obtained good results.

BACKGROUND: Facial involvement of psoriasis is known to be one of the clinical manifestations that indicate the severity of the psoriasis and thought to be more closely associated with certain distribution. Centrofacial (CF) psoriasis has been suggested to be related with severity of systemic disease while peripherofacial (PF) psoriasis has been thought to have connection with scalp psoriasis. OBJECTIVE: To analyze the epidemiologic characteristics, clinical features and subjective feelings of patients with facial psoriasis and to find out relationship between scalp psoriasis and facial involvement according to the facial types. METHODS: One hundred nineteen facial psoriasis patients were categorized into 3 types according to the distribution: PF type, CF type and mixed facial (MF) type. Onset and duration of facial and scalp psoriasis, and their relationship were questioned. Severity and extent of psoriasis on whole body, face, and scalp were rated by clinicians. RESULTS: There was no significant difference of whole body psoriasis area and severity index (PASI) and body surface area (BSA) score but scalp PASI and BSA was much higher in PF psoriasis compared to CF psoriasis (scalp PASI, 17.9 vs. 10.1; p=0.005) (scalp BSA, 40.9 vs. 22.2; p=0.002). According to the questionnaire, patient's objective feeling about the spreading of scalp lesion to facial area was markedly more prominent in the patients with peripheral involvement (PF+MF, 90.1%; CF, 54.2%; p<0.0001). CONCLUSION: Among subtypes of facial psoriasis, PF psoriasis is closely associated with spreading of scalp lesion into the face rather than reflecting the disease severity.

This tutorial examines the relationship between CL, F, and hepatic blood flow (Q(H)) quantitatively at oral and i.v. administration as an answer to the quiz set for KSCPT members. In case of oral dosing, when hepatic blood flow increases, the hepatic clearance (CL) and bioavailability (F) increases in high-extraction ratio drugs according to the well-stirred model equations for hepatic clearance: CL(H) = Q(H)·ER = Q(H)·f(u)·CL(int)/(Q(H)+f(u)·CL(int)) and F = 1 - ER Despite such a clear relationship, many students may feel it rather paradoxical that the increased CL (thus decreasing the AUC) causes increased F and thus the AUC (F·Dose/CLH) remains unchanged. This tutorial clarifies that the degree to which CL increase fails to match that of the Q(H) increase, and thus the decreased ER (= CL/Q(H)) that results in the increased F. Contemplating this simple, but seemingly paradoxical phenomenon may help students gain a deeper understanding of the first-pass effect.

Background: Plasma lipids are affected not only by dietary intake and life style but also by genetic factor. Objective: To discover the relationship between plasma lipids and the presence of LPL S447X and PAF-AH V279F in the bodies of rural Vietnamese population. Subject and Method: One hundred and ninety three subjects were randomly selected from rural areas in Ho Chi Minh city. The presence of LPL S447X and PAF-AHV279F polymorphism was determined using an Invader assay, and the plasma lipid levels were measured by an enzymatic method. A profile regarding the dietary intake was also made for each subject. Result and Conclusion: The frequency of LPL 447SX/XX (12.2%) polymorphism, PAF-AH Val279Phe mutation allele frequency (5.4%) in the Vietnamese was closely similar to that of other Asian populations. LPL447X polymorphism was found to be significantly associated with a higher HDL-C level in comparison to LPL 447S. LPL S447X affects plasma HDL-C and TG concentration in Vietnamese rural population.

We investigated the system of emergency medical service in the Chuno area in Gifu prefecture.It was found that about 20,000 emergency cases were carried into the Emergency Medical Center (EMC) in Chuno Kosei Hospital annually. About 90% were patitents with mild disease or injury. During the past four years an increasing number of severely ill patients such as those acute myocardial infarction and cerebral apoplexy were transfered to our EMC from other hospitals in the Chuno area.We found that many emergency patients came to our EMC, which was not staffed with so many emergency care specialists nor equiped with so many beds for emergency patients. Therefore, we requested residents, family doctors, primary care clinics, common hospitals and administrators in the Chuno area, to contribute their share to emergency medical care together with EMC.In conclusion, we thought it necessary to build a better system of emergency medical care in this area promptly.
