1.Relations between human papilloma virus infection and head and neck cancers
China Oncology 2000;0(06):-
Human papillomavirus is an oncogenic DNA virus and has more than 90 subtypes. High risk types (HPV 16, 18, 33) are associated with many kinds of malignant neoplasms of head and neck. The HPV DNA can induce mutations of p53, RB genes and increase cell transformation and over proliferation. The frequences of HPV positive are different in malignant neoplasms from different head and neck sites. HPV was more often detected in squamous cell carcinoma of oral cavity, oropharynx and larynx. In nasopharyngeal carcinoma, adenocarcinoma or lymphoma, the expression of HPV was quite low. The detection of HPV in cervical metastatic lymph nodes can improve diagnosis of primary sites.

Objective Tongue cancer shows an increasing frequency in young individuals. There is controversy concerning the clinical course and outcome for oral tongue cancer in young patients. Methods A retrospective review of 23 patients under 40 years of age with squamous cell carcinoma of tongue was performed. These patients were matched to an older patients group. The 5-year disease-free survival, rates of local, regional and distant recurrence were determined for both groups. Results The 5-year disease-free survival for the young patients was 68 % versus 74 % in the older group (P>0.05). 7/23 (30 %) of young patients recurred locally versus 3/23 (14 %) of the older patients (P>0.05). 6/23 (26 %) of young patients recurred regionally versus 4/23 (18 %) patients in the older group (P>0.05). The metastatic rates were similar in both groups. Conclusion In this series, young patients with sguamous cell carcinoma of the oral tongue had a higher locolregional recurrence rate than did older patients. This dose not mean that there is a survival difference.

Purpose:To discuss the functional neck dissection preserving cervical plexus in the treatment of differentiated thyroid cancer.Methods:15 cases of papillary thyroid cancer underwent functional neck dissection while preserving the cervical plexus. Results:Localization of positive lymph nodes were Ⅲ, Ⅳ, Ⅵ. The rate of neck node involvement of Ⅵ (+) is 57. 1%. No impairment of sensation of ears and lower necks and upper shoulders were found in the 15 patients. Conclusions:Indications:1.N 0 patients with papillary thyroid cancers. 2.N+ patients with papillary thyroid cancers whose metastatic lymph nodes are limited to level Ⅳ, or lymph nodes of parajugular are small (

Purpose:To analyze the patterns of cervical lymph node metastasized from different primary sites of head and neck cancers based on computed tomography (CT) imaging study. Methods:We studied the CT showing the positional relationship between 160 patients with different primary sites of head and neck squamous cell carcinoma and their cervical lymph node metastases using our new modified classification retrospectively. Results:The metastatic rates of level Ⅱa, Ⅱc and Ⅴ were significantly different between nasopharyngeal and non nasopharyngeal carcinoma ( P
