1.Analysis of influencing factors of early neurological deterioration in acute cerebral infarction patients with middle cerebral artery occlusion

Objective To analyze the influencing factors of early neurological deterioration in acute cerebral infarction patients with middle cerebral artery (MCA) occlusion.Methods One hundred and thirty-four acute cerebral infarction patients with MCA occlusion in the department of neurology of our hospital from January 2017 to December 2019 were enrolled retrospectively.The definition of early neurological deterioration (END) was the national institutes of health stroke scale (NIHSS) score increased by ≥ 2 compared with the baseline NIHSS score of admission within 72 hours.Forty-eight cases in END group and 86 cases in non-END group.Baseline and clinical data at admission were compared between the two groups.Binary multivariate logistic regression analysis were used to assess the influencing factors of END in acute cerebral infarction patients with MCA occlusion.Results (1)Gender,age,time of onset,admission NIHSS score,collateral circulation and parameters of blood pressure variability (SBPsd,SBPcv,SBPmax-min,DBPsd,DBPcv,DBPmax-min) were statistically significant between non-END group and END group.(2)Multivariate logistic regression analysis suggested that poor collateral circulation(OR=8.330,95%CI 1.629~42.587,P=0.011) and SBPmax-min (OR=1.139,95%CI 1.032~1.258,P=0.01) were independent risk factors for END in acute cerebral infarction patients with MCA occlusion.SBPmax-min predicted effectively END with best cutoff value 35.5,sensitivity 93.8%,specificity 64% and AUC0.85 (95%CI 0.788~0.912,P<0.001).Conclusion There were many factors affecting END in patients with acute cerebral infarction with MCA occlusion.Poor collateral circulation and Blood pressure variability (BPV) were independent risk factors for END.
2.Clinical efficacy of vericiguat in the treatment of acute decompensated heart failure and its effects on LVEF of patients with different LVDd

OBJECTIVE To observe the efficacy of vericiguat in the treatment of acute decompensated heart failure (HF) and its effect on left ventricular ejection fraction (LVEF) in patients with different left ventricular end-diastolic internal diameters (LVDd). METHODS A total of 52 patients with acute decompensated HF who were hospitalized in Anqing Municipal Hospital from September 2022 to May 2023 and were stabilized by intravenous injection of diuretics or vasodilators and then given vericiguat orally were selected. Clinical baseline data were collected, and blood creatinine (Scr), estimated glomerular filtration rate (eGFR), N-terminal pro-B-type natriuretic peptide (NT-proBNP), echocardiographic indexes(LVEF, LVDd), and Kansas City cardiomyopathy questionnaire (KCCQ) scores and so on were recorded and analyzed at 1 and 6 months after treatment. The patients were divided into two subgroups according to whether they had dilated cardiomyopathy or not, the baseline data were compared between the two subgroups, and the changes in the above follow-up indexes for the entire population and two subgroups of patients were analyzed by using mixed effect model. RESULTS Patients in the dilated cardiomyopathy group were significantly younger than those in the nondilated cardiomyopathy group, while the former had a lower proportion of patients with combined coronary artery disease, longer LVDd, lower LVEF levels, and more HF patients with reduced ejection fraction (P<0.05). Compared with baseline values, KCCQ scores were significantly higher in patients in the whole population, non-dilated cardiomyopathy group and dilated cardiomyopathy group at 1 month after treatment (P<0.001). The difference between the whole population in terms of elevated KCCQ scores and decreased lg(NT-proBNP) levels was statistically significant at 6 months after treatment (P<0.05); the levels of lg(NT-proBNP)of two subgroups were significantly decreased and KCCQ scores were significantly increased (P<0.05), but the difference was not statistically significant (P>0.05); LVEF levels of the patients E-mail:guangf4-508@163.com in both subgroups were significantly elevated (P<0.05) and the difference between the subgroups amounted to 7.52% (P=0.030). Mixed effect model result showed that whether patients had coronary artery disease and different baseline levels of LVDd were likely to affect follow-up LVEF levels, with coronary artery disease contributing to follow-up LVEF elevation (P=0.043), but the coronary artery disease subgroup × time interaction was not significant (P>0.05);compared with patients with baseline LVDd ≥62 mm, patients with baseline LVDd≤61 mm had a faster LVEF improvement (P<0.05). CONCLUSIONS Vericiguat is able to improve cardiac function and quality of life in patients with acute decompensated HF without negatively affecting their renal function. The drug is able to significantly improve LVEF levels in patients with baseline LVDd ≤61 mm.

5. X-ray texture analysis in differential diagnosis of phyllodes tumor and fibroadenoma in breast

Objective: To investigate the value of X-ray texture analysis in differential diagnosis of breast phyllodes tumors (BPTs) and breast fibroadenoma (BF). Methods Preoperative X-ray data of 32 patients with BPTs and 32 patients with BF were retrospectively analyzed. MaZda 4.6 software was used to manually draw ROIs on the maximum level of mammography images. The optimized texture parameters of texture analysis were selected with Fisher coefficient, probability of classification error and average correction coefficient (POE+ACC), mutual information (MI) as well as combination of the above three methods (MI+PA+F), respectively. The diagnostic results obtained by radiologist or using texture analysis techniques were contrasted with pathology. The misclassification rates of BPTs and BF were compared between texture analysis and subjective diagnosis by radiologist. Results: The misclassification rate of subjective diagnosis, Fishers, POE-ACC, MI and MI+PA+F was 20.31%(13/64), 14.06% (9/64), 23.44% (15/64), 25.00% (16/64) and 3.12% (2/64), respectively. The misclassification rate of MI+PA+F was lower than that of subjective diagnosis, POE-ACC and MI, respectively (all P<0.05). Conclusion: It is feasible to use texture analysis on X-ray films for differentiation of BPTs and BF.
6. Application of cystatin C and β2-microglobulin in the assessment of condition and short-term efficacy of multiple myeloma

Objective:
To explore the application value of serum cystatin C (Cys-C) and β2-microglobulin (β2-MG) in the diagnosis and short-term efficacy evaluation of multiple myeloma (MM).
Methods:
A total of 450 patients with MM admitted to Anqing Hospital of Chinese People′s Liberation Army Navy and Jinhua Central Hospital of Zhejiang Province from October 2016 to October 2018 were selected as subjects (MM group), according to the Durie-Salmon staging criteria, including 150 patients in stage Ⅰ, Ⅱ and Ⅲ. A total of 150 healthy subjects were selected as the control group. The levels of Cys-C and β2-MG in the serum of the subjects were determined. The differences of Cys-C and β2-MG levels between the two groups and the MM patients with different Durie-Salmon stages were compared. The differences of Cys-C and β2-MG levels between the patients with different short-term efficacy were compared. The receiver operating characteristic (ROC) curve was used to analyze the value of the two indicators in the evaluation of MM efficacy, and the correlation between Cys-C and β2-GM was analyzed. Logistic regression analysis was used to analyze the multiple factors affecting the clinical efficacy of MM patients.
Results:
The levels of Cys-C and β2-MG in the serum of the patients with MM were (2.11±0.78) mg/L and (6.07±3.08) g/L respectively, and those in the control group were (0.75±0.20) mg/L and (1.78±0.59) g/L, with significant differences (
7.Relationship between childhood abuse and dating violence perpetration and victimization among medical college students

Objective:
The purpose of the current study was to determinate the prevalence of dating violence perpetration and victimization and to examine its relationship with childhood abuse among medical college students.
Methods:
Cluster sampling survey was carried out among 3 727 freshmen and sophomores in two medical colleges from Anhui Province. Information with general demographic variables, childhood abuse and dating violence was obtained. Logistic regression was used to identify the relationship of different types of childhood abuse and dating violence after controlled for confounding factors.
Results:
The rate of dating violence perpetration and victimization was 37.8% and 27.8%, respectively. Female students had higher childhood emotional abuse experience (61.9%) than male students (51.6%), while male students had higher childhood physical and sexual abuse (28.2% and 19.4%) than females (24.3% and 10.5%) (χ2=33.84,6.33,52.97,P<0.01). Results showed that increased dating violence perpetration [OR(95%CI)=1.71(1.48-1.98),1.55(1.33-1.82),1.75(1.43-2.15)]and victimization [OR(95%CI)=1.84(1.43-2.16),1.59(1.34-1.87),1.62(1.32-2.00)](P<0.01) among individuals with physical, emotional and sexual abuse after controlling for confounding variables. No gender difference was found within other groups in the relationship between childhood abuse and dating violence(P>0.05).
Conclusion
Childhood abuse experiences are associated with higher risk of dating violence perpetration and victimization among college students. Reducing childhood abuse is beneficial to the prevention and control of dating violence among medical college students.
8.Prevalence and plasmid characterization of the qnrD determinant in Morganella morganii isolates

Objective To investigate the prevalence and plasmid size of qnrD determinant in Morganella morganii (M.morganii) isolates.Methods A total of 100 non-duplicated M.morganii clinical isolates were collected from inpatients.Standard ager dilution method was used to determine the minimum inhibitory concentrations (MICs) of fluoroquinolones against M.morganii isolates.PCR were performed to detect plasmid-mediated quinolone resistance determinants (PMQRs) in M.morganii isolates and the prevalence of extended-spectrum β-lactamase (ESBL) genes and AmpC β-lactamase genes in PMQRs-positive M.morganii strains.The homology analysis among qnrD-positive M.morganii strains were conducted by using pulsed-field gel electrophoresis (PFGE).The location of qnrD gene and the size of plasmid carrying it were determined by southern hybridization.The transferability of qnrD gene was determined by conjugation experiment.Results Thirty out of 100 M.morganii isolates (30%) were found carrying PMQRs including 17 qnrD-positive strains,14 aac (6')-Ib-cr-positive strains and 5 qepA-positive strains.PCR and sequencing confirmed that thirty PMQRs-positive isolates carried blaDHA-1.Among them,six isolates were positive for ESBLs genes (four for blaCTX-M-14,one for blaCTX-M-3 and one for blaCTX-M-24) and four isolates were positive for blaTEM-1.Almost all PMQRs-positive M.morganii isolates showed reduced susceptibility to fluoroquinolones.Moreover,seventeen qnrD-positive M.morganii isolates harbored blaDHA-1 including five (29.4%) harboring aac(6')-Ib-cr gene,four (23.5%) harboring blaCTX-M-14,two (11.8%) harboring blaTEM-1 and one harboring aac(6')-Ib-cr gene,blaCTX-M-14 and blaDHA-1.PFGE analysis showed that the 17 qnrD-positive M.morganii isolates were divergent from each other and not clone-related.Southern hybridization analysis showed that qnrD genes of all M.morganiiis isolates were mainly located in a 2.7 kb plasmid,but only a few of them were located in a size of 5.1 kb plasmid.M.morganiiis isolates failed to transfer qnrD gene to E.coli EC600 through conjugation.Conclusion PMQRs were widely distributed in M.morganiiis isolates.qnrD gene was the predominant determinants with a high prevalence rate of 17.0%,followed by aac(6')-Ib-cr gene.qnrD gene was located on a non-conjugative plasmid of approximately 2.7 kb or 5.1 kb.One qnrD-positive M.morganii isolate carrying aac(6')-Ib-cr gene,blaCTX-M-14 and blaDHA-1 was detected.
9.Expression and clinical significance of Wip1 in thyroid carcinoma and biological effect of siRNA targeting Wip1 on its cell line

Objective:To investigate the expression and clinical significance of wild-type p53-induced phosphatase 1 (Wip1) in thyroid carcinoma and biological effect of siRNA-targeting Wip1 on the thyroid carcinoma cell line. Methods:Immunohistochemistry and reverse transcription polymerase chain reaction (RT-PCR) were performed to detect the expression of Wip1 in 73 specimens of thy-roid carcinoma tissues and normal thyroid tissues (5 cm away from the margin of thyroid carcinoma), respectively. Wip1 siRNA was transiently transfected into the papillary thyroid carcinoma cell by using a liposome-mediated method and then detected by RT-PCR and Western blot. Methyl thiazolyl tetrazolium (MTT) assay and flow cytometry (FCM) were also conducted to observe cell proliferation, cell apoptosis, and cell cycle. Results:The positive rates of Wip1 protein were 80.8%in thyroid carcinoma tissues and 9.6%in the nor-mal tissues (χ2=47.036, P<0.05). The relative mRNA contents of Wip1 were 0.665 ± 0.046 and 0.225 ± 0.039 in carcinoma and normal tissues, respectively;these results significantly differed between the two types (t=12.637, P<0.05). Significant correlation was not ob-served between Wip1 expression and other factors, such as patient's gender, age, and tumor size (P>0.05). However, significant correla-tions among Wip1 expression, lymph node metastasis, clinical stages and tumor differentiation (P<0.05) were observed. RT-PCR and Western blot results showed that K1 cell-transfected Wip1 siRNA exhibited a relatively lower expression than normal cells (t=17.039, t=14.637, P<0.05). MTT assay results showed that the K1 cells transfected with Wip1 siRNA showed a lower survival fraction, higher cell apoptosis, higher percentage of G0/G1 phases, and lower cell concentration in G2/M and S phases (P<0.05). Conclusion:Wip1 pro-tein and mRNA were increased in thyroid carcinoma and are correlated with lymph node metastasis, clinical stages and tumor differenti-ation. Wip1 may be involved in proliferation, apoptosis, and cycle of thyroid cancer cells.
10.Study on the resistant genes and virulence gene of vancomycin-resistant Enterococci

Objective To study the vancomycin-resistant genes and the virulence factors genes in vancomycin-resistant Enterococci (VRE),and to analyze the drug-resistance character and epidemic characteristics of VRE strains and provide the basis for clincal selection of drugs and infection control.Methods VRE were screened by agar dilution sieving plate (ADSP) containing 6 μg/ml of vancomycin,drug resistance of VRE to other common antibiotics were detected by VITEK-60 automatic microbial analyzer.The gene types and virulence factor genes of VRE were determined by PCR.And the genetic relationships among VRE were determined by multilocus sequence typing.Results Seven vancomycin-resistant Enterococcus faecium strains were found in 360 enterococcus strains.All the VRE strains exhibited high-level vancomycin resistance ; some of them were medium or senstive to teicoplanin.They all carried vanA gene and esp gene and one of them carried 4 kinds of virulence factor genes.The ST type of the 7 VRE strains were diffused distribution.Conclusion We found vanB phenotype vanA genotype vancomycin-resistant Enterococcus faecium isolates in Wenzhou; these VRE strains were multidrug resistance and carried various virulence factor genes.Linezolid could be used as a recommend drug for treatment of VRE infection.The protection of antibiotics sensitivity should be strengthened.
