1.Toxicological assessment of Shanze weight-reducing food

Objective To evaluate the toxicological safety of Shanze weight-reducing food in order to provide some toxicological data for its application. Methods According to the national standard of safety evaluation, the following experiments of acute toxicity test through mouth in rats, the genetic toxicity test, thirty days feeding test and teratogenicity test in mice were performed. Results LD 50 was more than 20.0 g?kg -1, equivalent to 467 times of the recommended dosage, in acute toxicity test through mouth in both male and female rats, which fell into the scale of non-toxicity. Under the dosage range of 1.05 g?kg -1 to 4.20 g?kg -1, equivalent to 100 times of the recommended, the negative results were observed in the genetic toxicity test of micronucleus test of bone marrow cells, sperm shape abnormality test in mice and the Ames test. The negative impacts on the growth, development, hematology, biochemistry, histology and the ratio of organ to whole body were not found on the experimental rats of thirty days feeding. Under the same dosage range, no teratogenic effects were found in the experiments of teratogenicity test. Conclusions Shanze can be used as a kind of health weight-reducing food with toxicological safety under the experimental dosage range of 1.05 g?kg -1 to 4.20 g?kg -1.
2.Three-dimensional Video-assisted Thyroidectomy:a Report of 10 Cases

Objective To investigate the feasibility and safety of three-dimensional video-assisted thyroidectomy (3D VAT). Methods During January to April of 2014, 10 cases with unilateral or bilateral thyroid benign diseases , thyroid cancer, or parathyroid lesions were enrolled .The 3D VAT was carried out by using a 10-mm 3D stereoscopic endoscope .Local excision or lobectomy was performed via a 2-4 cm curve skin incision one finger above the sternal notch .In two patients , lobotomy combined with systematic lymphadenectomy was performed when papillary thyroid microcarcinoma was identified during operation . Results All the procedures were accomplished successfully under 3D video-assisted system, without intra-operative abnormal blood loss or conversion to conventional thyroidectomy .The operative time was 30-90 min (61.8 ±22.4 min);the intra-operative blood loss was 1.3-93.0 g (median, 5.2 g).All the patients were followed up for 1-4 months.Neither intra-nor post-operative complications about recurrent laryngeal nerve , superior laryngeal nerve , or parathyroid were observed . Conclusions Preliminary impression emerging from this study seems to suggest that 3D VAT is safe and effective.Compared with 2D surgery, a good perception of depth and easy recognition of anatomic structures are noticeable .

4.Clinical research of microneurosurgery for pituitary adenoma with superciliaryarch-keyhole approach

Objective To study clinical effects and complications of minimal invasive neurosurgery for pituitary adenoma through superciliaryarch-keyhole approach. Methods 127 cases pituitary adenoma in three hospitals in Shangdong province from May. 2006 to Sept. 2009 confirmed by surgery were analyzed. Patients were treated by different operative approaches, 53 cases using surgery through superciliaryarch-keyhole approach and 74 cases through conventional subfrontal approach. Clinical parameters were compared in patients undergoing surgery through different approaches. Results There were no differences in two groups, in aspects such as resection rate, improvment of endocrine hormone secretion and acuity of vision with campus visualis, electrolyte disturbances, hypothalamus damage (P >0.05). Significant differeces reside in average hospitalization days, subcutaneous fluidify, psychiatric symptom and circumscribed cerebral ischemia ( P < 0.05 ). Conclusion Advantages of minimal invasive neurosurgery through superciliary arch approach such as less trauma, better expose,shorter hospitalization days, less subcutaneous fluid accumulation and psychiatric symptoms in terms of this apporach are suitable for removing hypophyseal adenomas that grow on or around sella turcica.
5.Chronic pancreatitis affecting the basal levels of several pancreas-related endocrine hormones: a clinical trial

Objective to determine the basal levels of several pancreas-related endocrine hormones in patients with chronic pancreatitis.Methods according to inclusion and exclusion criteria,consecutive patients from February 2016 to August 2016 in Department of Gastroenterology,Changhai Hospital,Second Military Medical University and ten healthy control (matched for age and gender) were included.Basal levels of glucagon-like peptide 1,pancreatic polypeptide,Secretin,glucagon,somatostatin between groups of CP vs healthy control,CP with abnormal glycometabolism vs CP with normal glycometabolism and alcoholic CP vs non-alcoholic CP were compared.Results a total of 53 patients with chronic pancreatitis and 8 healthy subjects were included in this study.(1) CP vs healthy controls:the level of secretin in healthy control patients is significant lower than that in CP patients;(2) CP with abnormal glycometabolism vs CP with normal glycometabolism:the level of triglyceride and somatostatin is significant lower than that in CP patients;the prevalence of patients with chronic alcohol consumption and the level of glucagon-like peptide 1 in CP with abnormal glycometabolism is significant higher than that in CP with normal glycometabolism;(3) the prevalence of abnormal glycometabolism in alcoholic CP group is significant higher than that in non-alcoholic CP.The results above are all of statistical significance.Conclusions in addition to dysfunction of islets/3-cells,CP also easily affects the level of other pancreas-related hormones such as secretin,somatostatin and glucagon-like peptide 1.Otherwise,chronic alcohol consumption is also strongly related with abnormal glycometabolism,the mechanism deserves further researches.
6.Causes and countermeasures of complications of one-stage surgical management for spinal tuberculosis

Objective To analyze the causes of the complications of one-stage surgical management for spinal tuberculosis and its countermeasures.Methods From May 1997 to December 2008,116 patients with spinal tuberculosis underwent operations.Among them,6 were in cervical vertebra,3 in cervicothoracie vertebra,29 in thoracic vertebra,37 in thoracolumbar vertebra.29 in lumbar vertebra and 12 in hmbrosacrum vertebra.All cases underwent the anterior and/or radical debridement,decompression,autogenous bone grafting or radical debridement,decompression,autogenous bone grafting and internal fixation.The causes of the complications of surgical management and the countermeasures were analyzed.Results One hundred and ehven cases were followed upfor an average of(3.86±2.16)years(range 6 months to 12 years).The rate of complication was 13.5%(15/111).Recurrent spinal tuberculosis occarred in 5 cases,internal fixation loosened in 1 case,rupture of the external iliac vein occurred in 2 cases,lateral ventral syndrome in 1 case,venous embolism of the lower extremity in 1 csse,reactional psychosis in 2 cases,pneumatothorax in 2 cases and femoral nerve irritating syndrome in 1 case.Among the recurrent cages,2cases with anterior debridement and 2 cases with posterior debridement underwent anterior operation again andgot good results, 1 case with anterior debridement gave up the reoperafion.Other cases relieved after conservative therapy.Conclusions Standardized anti-tuberculosis thempy is fundamental for preventing the recurrence of tuberculosis.Correct approach for the operation and thorough radical debddement are the key points for preventing the recurrence of tuberculosis.Good and effective therapy surround the operation is helpful for preventing the recurrence of tuberculosis.
7.Comparison of pharmacokinetics and subacute toxicity for four arsenic species in rats

Objective Study on the absorption,distribution,transformation,excretion and toxicity of four forms of arsenic in rats.Methods Four thousand and fifty data were obtained from 28 days animal metabolism experiments of 81 rats exposed to the four As species.Distribution,metabolism,excretion,and subacute toxicity of 4 As species were compared by analyzing the arsenate [As(V)],arsenite [As(Ⅲ)],methyl arsenate (MMAV) and dimethyl arsenate (DMAV) content of rat feces (excrement and urine),blood,seven organs,and liver and kidney pathology.Results After oral administration of each As species,82.9% of As(Ⅲ),85.1% of As(V),95.0% of MMAV and 96.2% of DMAV were accumulatively secreted via feces and urine,while 16.2% of As(Ⅲ),14.1% of As(V),4.65% of MMAV and 0.120% of DMAV were detected in blood.The DMAV accumulated in blood and urine after dosing As(Ⅲ),As(V) and MMAV,and the content of DMAV in the blood was 8 times greater than urine.Four kinds of arsenic had effects on rat liver and kidney function,the influence on liver was acute phase effect while that on renal was cumulative effects.The effect of As(Ⅲ) was the most significant,followed by MMAV.Four kinds of arsenic could cause tissue deformation and inflammatory cells infiltration,but showed no difference between groups.Conclusion The distribution,metabolism,excretion,and toxicity of the four kinds of arsenic in rats were different in varying degrees.The research of health risk for arsenic should consider its different forms.
8.Operative procedures for thoraco-lumbar burst fractures coupled with neurological dysfunction

Objective To explore the indications of anterior and posterior approach operations for unstable thoraco-lumbar fractures coupled with neurologic deficits and prevention of intraoperative and postoperative complications. Methods We investigated 107 consecutive eases of thoraco-lumbar fractures coupled with spinal cord injury who had been operated on from January 2000 to December 2006.Their average age was 37.8(range.17 to 78) years old. They were 71 males and 36 females. Anterior approach was selected for 46 cases and posterior approach for 61 cases according to their McCormack grades. By the Frankle system for neurologie deficits, 7 cases were graded as A,16 as B,39 as C and 45 as D. Fracture height vecoveries before and after surgery were analyzed statistically. Kyphotic deformity was assessed on lateraJ radiographs using the Cobb method. Results The mean follow-up of 97 cases was 2.8(range,0.5 to 6) years. In both groups. All the cases gained at least 1 grade improvement except those with Frankle grade A. The mean kyphotic angles before operation, after operation and at the latest follow-up were 23.7°,10.6°and13.1°respectively, with significant differences(P<0.01),in the anterior approach group, while 16.3°,8.4°and 11.7°respectively, with significant differences(P<0.01),in the posterior approach group. Some complications, such as deficit of cutaneous nerve of thigh, appeared in the 2 groups. Conclusions In treatment of unstable thoraco-lumbar fractures coupled with neurologic deficits, the anterior approach may be better than the posterior approach. The posterior approach can be applied for the cases with less than 7 points in McCormack index. Correct selection of operative approach, careful operation and good rehabilitation after operation are necessary for the prevention of complications.
9.Clinical application of magnetic-controlled capsule endoscopy in screening and early diagnosis of stomach diseases

Gastric disease is a great burden in China; chronic gastritis, gastric ulcer, and gastric cancer incidences are the highest in the world.However,the screening rate of gastric disease in China is low because endoscopy and other screening methods are not accepted widely and they consume large amounts of manpower and material resources.The screening of gastric cancer has not been implemented,leading to high mortality because of gastric cancer. In recent years, the rapid development of magnetic-controlled capsule endoscopy provides a painless,highly accepted screening method for gastric cancer and other diseases.This article aims to introduce clinical applications of magnetic-controlled capsule endoscopy in screening and early diagnosis of stomach diseases.
10.Research progress on the use of BioEnterics intragastric balloon for treating obesity
