Two techniques are available for endovascular treatment of post-SAH vasospasm: mechanical angioplasty and phamacologic infusion(papaverine or other vasodilator). The advantages of papaverine infusion are simplicity, safety, and the ability to reach small distal vessels. The disadvantages are that the treatment results may be temporary and inconsistant. The advantages of balloon angioplasty are rapid, dramatic result and consistantly successful. It is important to treat vasospasm as soon as possible for two reasons: vascular response is better and permanant ischemic damage is minimized. If ischemic symptom have developed, it may already to be late. So, the treatment of vasospasm should be proreactive rather than reactive.

We report here on the use of the Venture(TM) catheter to facilitate successful advancement of a guidewire across difficult, complex lesions after the prior attempts at guide wire passage were unsuccessful with using standard wires. This Venture(TM) catheter may increase the success rate and reduce the procedural time for such a challenging interventional procedure.

We report our experience in coronary angioplasty and intravascular ultrasonography (IVUS) on a heavy, encircling, calcified lesion that was not dilated with the use of a cutting balloon and a non-compliant balloon. The angioplasty was successfully performed with a simple and inexpensive hugging balloon technique.

Coronary pseudo-lesion is an artificial lesion that occurs during percutaneous transluminal coronary angioplasty by an angioplasty guide wire and/or a balloon as a result of a straightening of the vessel curvature. A specific treatment is not required and the condition is completely resolved after removing the angioplasty wire. There are few reports about a pseudo-lesion, particularly in the left circumflex artery. We report two cases of a coronary pseudo-lesion induced by an angioplasty guide wire; one case affecting the left circumflex artery and the other affecting the right coronary artery.

The complications associated with the use of a guide wire, used during angioplasy, are rare and often go unrecognized. However, occasionally the guide wire itself may cause serious complications such as perforation or dissection of the distal coronary artery. A guide wire fracture during angioplasty is a rare complication, however entrapment and uncoiling of the guide wire can cause fracture. We report a case of guide wire fracture that developed by entrapment of the distal bending portion during stenting for bifurcation lesion. The broken free end of the guide wire remained within the stent strut, and urgent surgical intervention was necessary for its retrieval. We experienced a case of entrapment and fracture of the guide wire during stenting that was successfully surgically removed.
