1.Influence of case management on short-term intensive insulin therapy effect in newly diagnosed type 2 diabetic patients in outpatient clinic

Objective To explore the influence of case management on short-term intensive insulin therapy effect,including treatment compliance,safety and glucose metabolic index in newly diagnosed type 2 diabetic patients in outpatient clinic. Methods A total of 100 newly diagnosed type 2 diabetic patients in outpatient clinic were enrolled in this study and randomly divided into 2 groups:intervention group (n=52)and control group (Con,n = 48 ). The intervention group received standard insulin injection case management provided by a team composed of doctors and nurse case manager. The control group received routine insulin injection education in outpatient. Attitudes and compliance of insulin treatment,incidence of hypoglycemia,fasting blood glucose (FBG) and glycated haemoglobin A1c (HbA1 c ) were compared between the two groups at baseline and after 3 months follow up. Results After intervention,the total score and scores by dimension of insulin treatment attitude were higher in intervention group than in Con group[(76.30 ± 4.06 )vs (60.53 ± 3.18 )score,P < 0.01 ]. Insulin treatment compliance was better in intervention group than in Con group (82.7% vs 64.6%,P < 0.05 ). The hypoglycemia incidence was lower in intervention group than in Con group (19.2% vs 31.3%,P >0.05),although without statistical difference. FBG and HbA1 c were all lower in intervention group than in Con group [(6.79 ± 1.41 )vs (7.51±1.15)mmol/L,(6.62±0.69)% vs (7.15±0.75)%,P <0.05]. Conclusion Case management could effectively improve the insulin treatment attitude and compliance,and then optimize glycemic control innewly diagnosed type 2 diabetic patients treated with short-term intensive insulin therapy in outpatient clinic.

Triple negative breast cancer (TNBC) as a molecular subtypes of breast cancer, its biological behavior is special and existing multiple molecular targets, these molecular targets inlfuence the response of ionization radiation through different mechanisms and regulate radiotherapy sensitivity of TNBC This article reviewed the recent advances in radiotherapy and radiosensitivity related molecular targets of TNBC.
3.Determination of Content and Related Substances of Amlodipine Besylate Tablets by HPLC

Objective:To establish an HPLC method compatible with the mass spectrometry for the determination of the content and related substances in amlodipine besylate tablets. Methods:The sample was separated on a CAPCELL PAK C18 column (250 mm × 4. 6 mm, 5 μm) with the mobile phase consisting of methanol and 20 mmol·L-1 ammonium acetate (60∶40) at a flow rate of 1. 0 ml·min-1 . The detection wavelength was set at 237 nm. The column temperature was 35℃ and the injection volume was 20 μl. Be-sides, an HPLC-QTOF MS method was used to identify the molecular structure of the related substances of amlodipine. Results: The related substances were completely separated from amlodipine. The linear range of amlodipine besylate was 10-100 μg · ml-1 ( r =0. 999 8), and the mean recovery was 99. 2%(RSD=1. 0%,n=9). The main related substances could be detected by HPLC-QTOF MS. Conclusion:The established method is accurate, reliable and reproducible, which can be used in the quality control of amlodip-ine besylate tablets.
4.Study of the self life of three kinds of experimental dentin self-etching primers

Objective:To observe the effect of storage period of three experimental dentin self-etching primers on the shear bond strength.Methods:Three experimental primers with 2-hydroxyethyl methacrylate (HEMA)or N-methacryloyl glycine (NMGly)as a hydrophilic monomer and 1 0-methacryloxydecyl dihydrogen phosphate(MDP)or N-methacryloyl-2-aminoethylphosphonic acid (NMEP) as an acid monomer,were designed.Immediately after preparation of the samples of MDP-HEMA,MDP-NMGly and NMEP-NMGly, the primers were stored at 40 ℃for 0,6 and 1 4 weeks.At the end of each storage period,the shear bond strength of resin to dentin(n=1 4)and the pH value of the primers were measured.Results:After storage at 40 ℃ for 1 4 weeks,the shear bond strength of MDP-HEMA decreased from (1 7.61 ±1 .56)MPa to (7.53 ±1 .76)MPa;however the bond strenth of MDP-NMGly and NMEP-NMGly did not decrease;most of the specimens of MDP-HEMA and MDP-NMGly exhibited an interfacial failure of the resin from the dentin sur-face;however NMEP-NMGly exhibited a cohesional failure of the dentin and/or resin.The SEMimages showed a separation of resin and dentin of MDP-HEMA and MDP-NMGly had storaged at 40 ℃for 1 4 weeks;while the interface of NMEP-NMGly did not change.Con-clusion:The NMEP-NMGly primer has noticeably higher hydrolytic stability than the MDP-HEMA and MDP-NMGly primers.
5.Advances in the treatment of Eisenmenger syndrome

Eisenmenger syndrome (ES) was considered as a surgical contraindication in the past time. Only a few patients got lung transplantations or heart-lung transplantations due to lack of donors. Most of the patients had to choose conservative treatment to prolong their survival time. Recently, some clinical trials indicated targeted therapies in pulmonary arterial hypertension (PAH) were effective for the treatment of ES. This article reviewed the recent advances in this area.
6.Lymphangiogenesis and lymphatic vessel remodelling in cancer

It is well known that the tumor mainly transfers through the lymphatic vessels of the tumor .The current studies main-ly focus on the mechanism of metastasis , hoping to uncover the lymph node metastasis through investigating generation of lymphatic ves -sels and remodeling of tumor lymphatic vessels .Lymph node metastasis is directly related to and prognosis and the quality of life of cancer patients , and how to block the corresponding signaling pathways to prevent tumor lymphangiogenesis becomes the research fo -cus.This paper reviews the lymphangiogenesis and lymphatic vessel remodeling .
7.Relationship between Pain from Malnutrition and Multiple Myeloma(MM)

Objective] To investigate the relationship between pain from malnutrition and MM, looking for a cure for MM.[Methods]To analyze the relationship between pain from malnutrition and MM in clinical TCM based on the study of Neijing and the textual works of famous Chinese physicians. According to the etiology and pathogenesis to find the treatment method, and validation from clinical cases. [Result]We find that the pain from malnutrition caused by deficiency of kidney and spleen is a major cause of MM by summarizing of the ancient physicians. And we tally up the invigorating kidney is an important method for the treatment of MM.[Conclusion]Pain from malnutrition is an important pathogenesis of MM. To a method of treating MM, invigorating kidney and spleen is obvious and this method for the treatment of MM will bring more help.
8.Effects of Sec-O-Glucosylhamaudol on expressions of nitric oxide and interleukin-6 in Lipopolysaccharide stimulated murine macrophage

Objective:To investigate the effect of Sec-O-Glucosylhamaudol on inflammatory mediator and cytokine( NO and IL-6)production in Lipopolysaccharide(LPS) stimulated murine macrophage(RAW264.7).Methods: Macrophages were induced with LPS,and incubated with different concentrations of Sec-O-Glucosylhamaudol(20,40,80 μg/ml),the quantity of NO production was measured by Griess reagent;the IL-6 production were measured by enzyme linked immunosorbent assay(ELISA),the expression of nitric oxide synthase( iNOS) in cells were detected by Western blot;the expression of iNOS and IL-6 mRNA were analyzed by real-time PCR.Results:Each concentrations of Sec-O-Glucosylhamaudol(20,40,80 μg/ml) inhibited the production of NO and IL-6 in LPS-stimulated RAW264.7 cells(P<0.01).This compound also reduced the mRNA expression of iNOS and IL-6.Conclusion:Sec-O-Glu-cosylhamaudol exhibited anti-inflammatory activity by inhibited the NO and IL-6 production in LPS stimulated RAW264.7 cells.
9.Clinical significance of CD4+CD25+Foxp3+ regulatory T cells in patients with aplastic anemia

Acquired aplastic anemia(AA) is a disease of bone marrow failure mediated by autoimmune T cells. CD4+CD25+ regulatory T cells, which have immunosuppressive and anergic, belong to a subpopulation of T cells specialized for immune regulation and play important roles in the development of autoimmune T cells. Foxp3 plays a significant role in the development and function of regulatory T cells which have been confirmed by many investigations. In recent years, the relationship between CD4 +CD25 +Foxp3 + regulatory T cells and AA has become a hot spot of research. What’s more, its clinical significance is becoming more and more obvious in AA.
10.IL-10 gene polymorphism with ulcerative colitis susceptibility and its influence on clinical outcomes

Objective:To study the correllation of genetic polymorphisms of IL-10 polymorphic loci with ulcerative colitis sus-ceptibility and its influence on clinical outcomes.Methods:A total of 80 patients with ulcerative colitis were selected as case group and the others by sex and age matched healthy subjects as control group according to the case-control study design.Peripheral venous blood samples were drawn and DNA was extracted from all subjects prior to treatment.PCR primers of -819T/C(rs1800871),-592A/C (rs1800872),-1082G/A(rs1800896) were designed for PCR amplification.The fragments produced from human genomic DNA were performed by restriction enzyme digestion of amplified PCR products,and further separated using agarose gel electrophoresis.Relative risk(OR) and 95%confidence interval(95%CI) were calculated by the Logistic regression,in order to evaluate the correlation of IL-10 gene polymorphism with susceptibility of ulcerative colitis and clinical outcomes.Results:(1)The distribution frequency of genotype AA,GG and AG of polymorphic loci rs1800896 in cases patients were significantly different from that in control group(P<0.01).(2) Compared to rs1800896 genotype AA,genotype GG were significantly associated with increased risk for ulcerative colitis(P<0.01)and the decreased clinical remission rate ( P<0.01 ).( 3 ) The distribution frequency of genotype CC, CT and TT of polymorphic loci rs1800871 were not significantly different between groups(P>0.05).(4)The distribution frequency of genotype AA,AC and CC of polymorphic loci rs1800872 were not significantly different between groups(P>0.05).Conclusion:IL-10 polymorphic loci rs1800896 genotype GG would be associated with increased susceptibility to ulcerative colitis,and poor prognosis of patients.
