2.Association of Fms-like tyrosine kinase-3 mutation with the occurrence and prognosis of leukemia in patients with leukemia

BACKGROUND: Fms-like tyrosine kinase-3 (FLT3) gene is one of the receptors for growth factors in early hematogenesis. By binding to its ligand (FL), FLT3 regulates the proliferation and differentiation of hematopoietic stem/pregenitor cells via cell signal transduction pathway. FLT3 juxtamembrane internal tandem duplication (ITD)mutation is correlated with leukemia onset, development and act as an independent factor for poor prognosis.OBJECTIVE: To explore the relationship between Flt-3/ITD mutation and acute leukemia (AL), and analyze its clinical implication.DESIGN: A repetitive measurement.SETTING: Department of Biochemistry and Molecular Biology, Shenyang Medical College.PARTICIPANTS: Totally 147 samples of leukemia patients were diagnosed by morphological bone marrow biopsy in Shenyang Central Hospitals affiliated to Shenyang Medical College from January 1999 to October 2004. They were 95 males and 52 females with the mean age of 14.5 years old (5-23 years old). According to FAB standard, there were 62 patients with acute leukemia [including 33 cases of acute myelocytic leukemia (AMLs, M1, M2), 3 cases of acute myelo-monocytic leukemia (AMMOLs, M4), 18 cases of acute monocytic leukemia (AMOLs, M5), 8 cases of acute megakaryocytic leukemia (AMKLs, M7)], 43 patients with acute lymphoblastic leukemia (ALLs), 13 patietns with juvenile chronic myelocytic leukemia (JCMLs) and 29 patients with myelodysplastic syndrome (MDSs). Informed contents were obtained from all the participants.METHOD: Flt-3/ITD was detected with polymerase chain reaction (PCR) sequence in the 147 leukemia patients.MArN OUTCOME MEASURES: The clinical manifestations and prognosis were observed in the ALL patients with Flt-3/ITD;The Flt-3 gene expression, Flt-3/ITD positive rate, duplication region and length were detected in all kinds of leukemia.to Flt-3 gene products, including 3 cases of AMLs (M1, M2), 1 case of AMMOL (M4), and 1 case of AMOL (M5). DNA sequencing and blast alignment revealed that ITDs existed in all samples with unusual products within exon 11, in various region and length (33-72 bp). Expression of Flt-3 in different levels was found in 43 cases of ALL, 29 cases of Flt-3/ITD all died short after the diagnosis, and the mean life span of was only 10.8 months.
3.Protection of electroacupuncture and compound salviae in coordination on ischemia-reperfusion myocardium

BACKGROUND: Both electroacupuncture and salviae act on prevention and treatment ofis chemia-reperfusion injury, but, whether such action is achieved or not by coordination of both? OBJECTIVE: To probe into the effects of electroacupuncture and salviae on expression of myocardial cellular heat-shock protein 70mRNA that acts on protecting myocardium and on dopamine level that is the mark for myocardial injury after ischemia-repeifusion as well as the interaction between electroacupuncture and salviae. DESIGN: Factorial design of two factors. SETTLNG: Department of Anesthesia of Renji Hospital Affiliated to Shanghai Second Medical Scientific University. PARTICIPANTS: The experiment was performed in Animal Experimental Room of Renji Hospital from September 2001 to December 2002, in which 24 healthy New Zealand white rabbits were employed and randomized into 4 groups, 6 rabbits in each, named ① ischemia-reperfusion group (IR group), ② electroacupuncture group (EA group), ③ salviae group (SA group), ④ electroacupuncture + salviae group (ES group). METHODS: ① IR group: ischemia-reperfusion model was prepared by clipped the anterior descend branch of cardiac coronal artery for 30 minutes and released for 2 hours. ② EA group: modeling was done as the previous method. Twenty minutes before ischemia, electroacupuncture was done on Neiguan (PC 6), Yunmen (LU 2) and Lieque (LU 7), 0.8 V in voltage, 3.0-4.0 Hz in frequency. ③ SA group: modeling was done as the previous method. Before ischemia, salviae was injected intravenously once at 1.5 mg/kg, and was injected before and after reperfusion once respectively at 1.0 mg/kg. ④ ES group: modeling was done as the previous method. Both electroacupuncture and medication were applied in the group. The content of dopamine was determined in blood before ischemia,30 minutes after ischemia and 2 hours after reperfusion successively in rabbits of every group. Reverse transcription-polymerase chain reaction (RT-PCR) was used to determine myocardial heat-shock protein 70mRNA expression in ischemic and non-ischemic areas. RESULTS: After supplemented, 24 rabbits entered result analysis. ① Expression of myocardial heat-shock protein 70mRNA: that after ischemia and reperfusion was increased significantly compared that before ischemia in every group (P < 0.01). That in the rest 3 groups was all higher than IR group,and that in ES group was higher than EA and SA groups (F=4.48, P <0.05). CONCLUSION: ① Both electroacupuncture and salivae stimulate expression of heat-shock protein 70 mRNA after ischemia and reperfusion, enhance protein stability of heat-shock protein 70 and alleviate myocardial damage. ② Both electroacupuncture and salivae inhibit increased dopamine content in the body after ischemia and reperfusion, reduce dopamine mediated injury and protect myocardium. ③ Coordination is present between electroacupuncture and salivae.
4.Correlation of hemodynamic change of cerebral vessels and functional disturbance in cerebral infarction patient

Objective To observe and investigate the correlation of hemodynamic change of cerebral vessels and functional disturbance in cerebral infarction patient.Methods To investigate hemodynamic characters of cerebral vessels in every type and every group patients via comparing cerebral infarction group(observe group)with control group,comparing stage of recovery with early stage, sever, moderate, mild degree in observe group.Results Apparent hemodynamic change was observed in above 94.10% cerebral infarction patients. The more apparent the hemodynamic change,the severer the functional disturbance.Conclusion The hemodynamic change of cerebral vessels is the predictive index of state of illness and prognosis of cerebral infarction patient.Clinical doctor should pay enough attention to modify it in treatment.
5.Injection of bradykinin or/and cyclosporine A to hippocampus induces Alzheimer-like phosphorylation of tau and abnormal behavior in rats

Bradykinin (BK) is a calcium/calmodulin dependent protein kinase Ⅱ (CaMKⅡ) specific activator, and Cyclosporin A (CSA) is reported to suppress protein phosphotase (PP)-2B activity. In vitro studies have shown that CaMKⅡ and PP-2B play an important role in Alzheimer-like phosphorylation of microtube-associated protein tau. To reconstitute an animal model based on the imbalance of protein kinase (s) and protein phosphatase (s) seen in Alzheimer brain, we injected BK and/or CSA into rat hippocampus. The results from behavioral study showed that an obvious disturbance in learning and memory was seen with BK or BK plus CSA injected rats. Moreover, the behavior abnormality appeared earlier in aged rats than young adults of the same kind after the injection. On the other hand, no obvious dysfunction in living and behavior was observed with CSA alone injected rats. The results obtained by immunohistochemical assay indicated that the staining for M4\, 12E8\, PHF-1 and CaMKⅡ was stronger, and for Tau-1 was weaker in BK injected rats compared with Control group. It was also found that the binding of M4 and PHF-1 but not 12E8 to tau was significantly increased in CSA injected rats. As the same as BK injection, binding of Tau-1 to tau was decreased after CSA injection. The immunostaining for 12E8\,PHF-1 and CaMKⅡ was increased, whereas for Tau-1\, M4\, and GSK-3 was decreased after combination injection of BK and CSA. In addition, the staining of PP-2B decreased in all the three models. To our knowledge, this is the first data shown in vivo that the activation of CaMKⅡ induces both Alzheimer-like tau phosphorylation and behavioral disturbance.
6.Tne effect of nitric oxide and its inhibitor on secondary spinal cord injury

Objective To explore the effect of NO on secondary spinal cord injury. Method L - arginine{L - Arg} and N - nitric-Larginine methylester{L - NAME} were infused into subarachnoid space of rats 30 min before spinal cord injury(SCI) . Spinal cord blood flow (SCBF) at trauma sites was monitored. After SCI by a Laser - Doppler flowmetry and neurological function was evaluated. Result The result showed that L - arg increased SCBF at trauma sites, but exaggerated the SCI. The proper dose of L - NAME decreased early SCBF at trauma sites, but ameliorated neural function. Large dose of L - NAME restricted NO production for a long time and resulted in spinal cord isehemia, thus exacerbating SCI. Conclusion That over - production or over - restriction of NO worsen SCI.
7.Simultaneous Spectrophotometric Determination of Zinc, Cadmiumand Mercury with Hybrid Linear Analysis

A new method for simultaneous spectrophotometric determination of Zn, Cd and Hg using 2-(5-Br-2-pyridylazo)-5-diethylaminophenol as the color developing reagent was proposed. The absorption spectra of these three complexes have similar features with severe overlap in visible spectral range. For resolving these spectra, hybrid linear analysis was used, and the pure spectrum of each component was obtained from the calibration mixtures by least squares method. The effects of reaction condition, selection of wavelengths, determination of pure spectrum and additivity of absorbances etc. on the determination were discussed. The proposed method offers the advantages of simple, rapid, and accuracy. It has been successfully applied to the simultaneous determination of Zn, Cd and Hg in synthetic sample. A comparison was also made with the partial least squares method.
8.An Analytical Method for Oxygenated Nonhydrocarbonsin Sediments and Crudes

An analytical method for seperating nonhydrocarbons in sediments and crudes into an acidic fraction, a neutral fraction (Ⅰ), and a more polar neutral neutral fraction (Ⅱ) is reported. With this method, components only appear in individual fractions. Fatty acids and polyols and polyones are seperated into the acidic fraction; mono-ketones, esters and straight-chain alkanols fall into the neutral fraction Ⅰ; Streols are found in the neutral fraction Ⅱ. No evident alteration of ratios of the components through the separation procedures is observed. Erucic acid, 5α(H)-androstan-17-one, and 5α(H)-androstan-3β-ol are selected first time as internal standards for sterols in acidic fraction, neutral fraction Ⅰ and Ⅱ, respectively. They are good standards for quantitation of the components with gas chromatography, especially 5α(H)-androstan-3β-ol with in the first time an interal standard for quantitation of sterols. The analytical method has been proved to be effective by seperation of more than hundred sedimental samples
9.Clinical study of sunitinib as first line treatment in 40 patients with metastatic renal cell carcinoma

Objective To evaluate the efficacy and safety of sunitinib as first line treatment in patients with metastatic renal cell carcinoma (mRCC).Methods A total of 40 patients with mRCC were treated with sunitinib between January 2009 and December 2011,including 27 males and 13 females.The median age was 59 (31-75) years old.All patients received a pathologic diagnosis of renal clear cell carcinomas with radical nephrectomy.All cases were treated with first line therapy.Sunitinib monotherapy was administered in repeated 6-week cycles of daily oral therapy for 4 weeks,followed by 2 weeks off in 34 patients,while another 6 patients received 37.5 mg/d continuously until disease progression or unacceptable toxicities occurred.Overall response rate and safety were evaluated.Results The median follow up was 14 months (range 4-36 months).All patients could be evaluated the efficacy.17.5 % (7/40) patients achieved partial responses,77.5 % (31/40) atients were in stable and 2 (5.0 %) patients were with disease progression including 1 death.The objective response rate was 17.5 % (7/40),and the disease control rate was 95.0 % (38/40).The most common side effects included fatigue 12 cases (30.0 %),alopecia 3 cases (7.5 %),thrombocytopenia 23 cases (57.5 %),thyroid dysfunction 26 cases (65.0 %),neutropenia 20 cases (50.0 %),hypertension 18 cases (45.0 %),hand-foot syndrome 17 cases (42.5 %) and diarrhea 4 cases (10.0 %).The major grade 3 adverse events included thrombocytopenia 2 cases (5.0 %) and edema 1 case (2.5 %).Most adverse events were ameliorated by treatment interruption.Conclusion Sunitinib has definitive efficacy for patients with mRCC with slight side effects.
10.Analysis of risk factors and prognosis of pneumonia in elderly patients of cerebral stroke sequelae period

Objective To investigate the risk factors and prognosis of pneumonia in elderly patients of cerebral stroke sequelae period.Methods Eighty-eight cases of pneumonia in elderly patients with cerebral stroke sequelae period were selected as stroke group,with non-stroke patients of the same period suffering from pneumonia 1 ∶ 1 matched as control group.The risk factors and prognosis of pneumonia in elderly patients of cerebral stroke sequelae period were analyzed.Results Pneumonia in stroke group was related with hypertension,diabetes,conscious disturbance,bucking or nasogastric feeding,dyscinesia,incision of trachea,multiple organ disfunction syndrome (MODS),hospitalizations,hospital stay and abandon treatment or death (P < 0.05),and not related with smoking,coronary heart disease,kindey dysfunction,malignant tumor,phlegm bacterial culture,hypoxemia and mechanical ventilation was tested by conditioned Logistic regression quantitatively(P> 0.05).The independent risk factors for pneumonia in elderly patients of cerebral stroke sequelae period were bucking or nasogastric feeding,and MODS by two-way unconditional Logistic regression.Conclusions Hypertension,diabetes,conscious disturbance,bucking or nasogastric feeding,dyscinesia,incision of trachea,MODS,hospitalizations are risk factors for pneumonia in elderly patients of cerebral stroke sequelae period,and lead to relatively poor prognosis.Three-grade prevention and control of risk factors can improve prognosis.
