1.Meta-analysis on diagnostic value of monoclonal gastric cancer 7 antigen for gastric cancer

Objective To explore the clinical diagnostic value of monoclonal gastric cancer 7 antigen(MG7-Ag) in gastric cancer through a meta-analysis .Methods The PubMed ,Embase ,ISI Web of Knowledge ,China National Knowledge Infrastructure (CNKI) ,Wanfang databases were retrieved from their establishment to April 5 ,2013 .The qualities of the included literatures were assessed by the QUADAS scale .The heterogeneity test and meta-analysis were conducted by the Stata12 and the Meta-DiSc soft-wares .Results 610 articles were retrieved from databases .7 articles containing 4 516 cases conformed with the included standard . The sensitivity of MG7-Ag in diagnosing gastric cancer was 0 .72(95% CI:0 .68 -0 .76) ,its specificity was 0 .94(95% CI:0 .93 -0 .94) ,the positive likelihood ratio was 6 .09(95% CI:3 .36-11 .07) ,the negative likelihood ratio was 0 .34 (95% CI:0 .21-0 .56) and the AUC was 0 .893 4 .Conclusion The comprehensive research results indicate that MG7-Ag has the sensitivity of 0 .72 for di-agnosing gastric cancer ,which is higher than that of currently used diagnostic markers such as carcino-embryonic antigen(CEA) , CA50 and CA19-9 ,which has the high value for excluding other diseases and can be used as a marker for diagnosing gastric cancer .
2.The differences between Monte Carlo calculated dose-to-medium and dose-to-water for lung cancer IMRT

Objective To investigate the differences between Monte Carlo (MC) calculated doseto-water (Dw) and dose-to-medium (Dm) for lung cancers treated with intensity-modulated radiotherapy (IMRT).Methods A total of 10 lung carcinoma patients with 5-field IMRT treatment plans were stratified sampling randomly selected for this study,which were performed on Monaco treatment planning system (TPS) with MC algorithm.Using the patients' own CT images as quality assurance (QA) phantoms,two kinds of QA plan were calculated,one was the Dm,and another was the Dw plan.Dose volume histogram (DVH) parameters and the subtraction of two plans were used to evaluate the spatial distribution of the difference between the Dm and Dw.Results Differences between dose-volume indices computed with Dm and Dw for the PTV65 and PTV50 doses (D50%,D98% and D2%) were-0.3%,-0.2%,0.3% and 0.1%,-0.6%,0.4%,respectively,of which the D50% of PTV65 and D98% of PTV50 had statistical difference (t =-2.536,-3.776,P < 0.05).For normal tissues,spinal cord,heart,lung and esophagus,the D50% differences between Dm and Dw were 0.3%,1.1%,-0.2% and -0.1%,of which the Dm of spinal cord and heart were slightly lower than the Dw (t =2.535,3.254,P < 0.05).For the D2% of the normal tissues,the differences were 0.3%,-0.6%,-0.7% and 0.6%,the differences were statistically significant (t =2.311,-4.105,-3.878,6.214,P<0.05).All the differences were within 2%.Meanwhile planned subtraction analysis showed the differences between the Dm and Dw varied very much with the other body parts of the patient,especially for bone tissues,and the two doses were significant difference (> 5%).Conclusion In the course of clinical application,the relative differences between Dm and Dw for lung cancers MC calculations should be noted when considering the dose limitations of bone tissue.
3.A primary research of intensity-modulated dose verification based on anatomic structure of three-dimensional images

Objective To verify IMRT plans in point,planar and 3D dose,and to concretely analyze the dose differences of 3D anatomic structure based on Gamma passing rate.Methods Thimble ion-chamber,Matrixx and ArcCheck were separately used to measure six nasopharyngeal carcinoma treatment plans and six lung cancer treatment plans.The dose measurement deviation of the center point was compared as well as the Gamma passing rate of dose verification under the criteria of both 3%/3 mm and 2%/2 mm,the group t-test and one-way ANOVA were also proceeded.3DVH system was used to analyze the dose measurement deviation of target volume (TV) and organ at risk (OAR) through DVH.Results For IMRT and VMAT treatment plans,the mean deviation of point dose was (0.59 ± 1.31) % and (-1.00 ± 1.03)% respectively,and the maximum deviation was less than 3%.Under the criterion of 3%/3 mm,the Gamma passing rate measured by Matrixx,ArcCheck and 3DVH for IMRT plans was 96.28%,97.55% and 99.02% respectively,and for VMAT plans,the corresponding results of three different detectors were 97.24%,99.67% and 98.48%.The results analyzed and compared by 3DVH showed that even under the condition of high Gamma pass rate (more than 95% for a Gamma criterion of 3%/3 mm),the DVH metrics of both TV and OAR in two cases (account for 16.7% of the total plan) were significantly different on the clinical parameters,including GTV,spinal cord and brain stem etc.Conclusions The analysis of dose difference of the measurement results based on Gamma pass rate and on anatomic structure of 3D images can more effectively evaluate the influence of dose error to the implementing of clinical plan and the impact to the clinical treatment.
4.Quality Standard of Baoshenling Capsules

OBJECTIVE:To establish the quality standard of Baoshenling capsules.METHODS:Radix Astragali,Radix et Rhizoma Rhei,Ligusticum chuanxiong and Hirudo were identified qualitatively by TLC,and the content of diphenyl ethylene in Baoshenling capsules was determined by HPLC.RESULTS:The characteristic TLC spots of Radix Astragali,Radix et Rhizoma Rhei,Ligusticum chuanxiong and Hirudo were identified accurately.The linear range of diphenyl ethylene was 0.176~0.880?g(r = 0.999 8) with an average recovery rate of 98.9%(RSD = 1.3%,n =6).CONCLUSION:The established standard is applicable for the quality control of Baoshenling capsules.
5.Effect and mechanism of extracorporeal photopheresis on preventing acute graft versus host disease in ice

【Objective】 To investigate the preventive effects of early apoptotic splenic mononuclear cells induced by extracorporeal photopheresis (ECP) on acute graft versus host disease (aGVHD) in mice and explore the underlying mechanisms. 【Methods】 1) Splenic mononuclear cells were extracted from C57BL/6 mice and treated with different concentrations of 8-MOP (50 ng/mL, 100 ng/mL, 200 ng/mL, 300 ng/mL, 600 ng/mL). After treatment, irradiate the cells with 2 J/cm2 of ultraviolet light. Then, use the Annexin V-FITC/PI apoptosis detection kit to assess the early apoptosis rate of the cells and determine the optimal concentration of 8-MOP for the experiment.2) There were 35 SPF-grade female BALB/C mice (H-2Kd) aged 6-8 weeks. After whole-body irradiation with 8Gy X-rays, the mice were divided into five groups: sham irradiation group received intravenous infusion of 0.2 mL of normal saline, the syngeneic bone marrow transplantation group received intravenous infusion of 0.2 mL of BALB/C mouse bone marrow nucleated cell suspension (including a cell count of 1×107), the allogeneic bone marrow transplantation group received intravenous infusion of 0.2 mL of C57BL/6 mouse bone marrow nucleated cell suspension (including a cell count of 1×107), the aGVHD group received intravenous injection of a mixture of C57BL/6 mouse bone marrow nucleated cells (including a cell count of 1×107) and splenic mononuclear cells (including a cell count of 1×107) in 0.2 mL, the ECP prevention group received pre-transplant intravenous infusion of 0.2 mL of ECP-treated splenic mononuclear cells of C57BL/6 mice (including a cell count of 1×107 ) 48 hours before transplantation, and on the day of transplantation, intravenous injection of a mixture of C57BL/6 mouse bone marrow nucleated cells (including a cell count of 1×107) and splenic mononuclear cells (including a cell count of 1×107 ) in 0.2 mL.The preventive effects of ECP on aGVHD were observed, and the concentrations of IFN-γ, IL-2, TNF, IL-4 and IL-6 in mouse serum were measured using CBA. Th1 cell counts were determined by flow cytometry. 【Results】 Different concentrations of 8-MOP (50 ng/mL,100 ng/mL, 200 ng/mL, 300 ng/mL, 600 ng/mL) were used to treat mouse splenic mononuclear cells. The early apoptosis rates (%), observed after treatment were as follows: (14.18±0.865) vs (16.76±0.407) vs (18.83±0.404) vs (19.27±0.404) vs (14.5±0.529). The appropriate concentration of 8-MOP was determined to be 200 ng/mL.In vivo experiment, the results showed that the aGVHD group had decreased survival rate, reduced body weight, and increased clinical scores compared to the syngeneic and allogeneic bone marrow transplantation groups (P<0.01), and the chimerism of bone marrow cells in mice after transplantation was over 90%. ECP significantly improved the survival rate of mice after transplantation, reduced clinical scores (P<0.05), and decreased the concentrations of Th1 cell cytokines in serum (P<0.05) and the counts of Th1 cells in the spleen (P<0.05). 【Conclusion】 ECP-induced early apoptotic single nuclear cells from the spleen can prevent the occurrence of aGVHD by reducing the Th1 response in mouse.
