Effects of microiontophoretically applied transmitter receptor antagonists on the evoked response of “pain” units of PVN in rats by sciatic stimulation were observed. The results showed: (l)The evoked response of 7 out of 38 PVN “pain” units could be blocked by atropine (7/38); 5 out of 27 by hexamethonium; 6 out of 31 by phentolamine (6/31); and 4 out of 25 by propranolol (4/25). Seventeen out of 52 were blocked by cyproheptadine (17/52), but 3 out of 52 were augmented by cyproheptadine (3/52). (2) The evoked response of the same “pain” unit could be blocked by two antagonists: the evoked response of 2 out of 27 PVN “pain” units could be blocked by atropine and cyprohepadine; 3 out of 20 by hexamethonium and cyproheptadine; 2 out of 25 by prepamolol and cyproheptadine. These results suggest that the noxious somatic input to PVN involves 5-HT, cholinergic and adrenergic transmitter receptor mechanisms and that the convergence of various transmitter systems on PVN “pain” unit is indicated.

Immunohistochemistry was used to detect the protein expression of Snail in 5 specimens jusxta-eancerous normal breast tissues, 35 specimens of cancerous tissues without metastasis and 20 specimens of breast carcinoma with lymphonode metastasis. Breast carcinoma cell line MDA-MB-231 was transfected with antisense Snail. Results showed that the expression of Snail protein was significantly higher in breast carcinomas than in their normal tissues. The mRNA and protein expressions of Snail in the breast carcinoma cells treated with antisense Snail was significantly decreased while the E-cadherin protein significantly increased (both P < 0.05). The number of invasive ceils treated with antisense Snail was (10.5±1.3)%, while in treated with EGFP was (68.2±2.1)% (P < 0.05). The abnormal expression of Snail contribute to the invasiveness of breast carcinoma. The antisense Snail could prevent the cells ability to invade in vitro, and the effect is related with the up-regulated E-cadherin protein.

Objective To study the dynamic changes of anatomy and the dosimeter distribution those changes influenced. Methods Initially simplified intensity modulated radiation therapy (sIMRT)were performed to twenty-nine patients with phase Ⅲ - Ⅳa esophageal carcinoma from January 2007 to March 2009. The target volumes and involving organs were contoured on the primary spiral CT pictures. After sIMRT planning being finished, secondary CT scan was acquired to rectify the treatment center. For eleven patients at every other week and eighteen patients at the fourth week, spiral CT images were acquired according to the same treatment center, and thereafter fused with the first CT images. Firstly, the law of change and the best time of replanning were searched:the changed gross tumor volume (GTV), gross node volume (GTVnd), plan target volume (PTV) and normal organs (lung, spinal cord, heart and outline) on the fusion interface were modified by a single physician, the changes for each structure throughout treatment were measured by system software. Secondly, dose distributions were computed and evaluated for replanning CT using the same beams arrangement as the initial plan. Cumulative dose was estimated using weighted average and compared with the original plan. Results For eleven patients, The law of change:the volume of outlines and GTV gradually decreased, and the change come to peak in the fourth week. The conformal index for PTV gradually decreased, whereas the heterogeneous index gradually increased. For twenty-nine patients on the fourth week, the dose of GTV were more than 60 Gy. The dose of PTV-D95 and CTV-D99 decreased ( t = 1.49, P = 0. 147 and t = 2. 07, P = 0. 048 respectively). The dose of CTV-D99 in two patients deceased to 54 Gy or less. The cord-Dmax and lung V30 increased significantly ( t = - 2. 42, P = 0. 022 and t = -2. 26,P =0. 032). Conclusions During the course of sIMRT for esophageal cancer, the volume of GTV decreased and the change come to peak in the fourth week. It is the best time for evaluating the change of dose of target volume using CT-CT fusion. For some patients, revise of the treatment plan is needed to ensure adequate target volume dosage and safety of normal tissues.

Gut and Liver 2020;14(6):685-698
The advent of biologics and biologic therapy has transformed the management of inflammatory bowel disease (IBD) with enhanced early and adequate responses to treatment, fewer hospitalizations, a reduced need for surgery, and unprecedented outcomes including complete mucosal and histologic healing. However, an important issue with the use of anti-tumor necrosis factor (anti-TNF) agents in IBD is the increased risk of tuberculosis (TB). This is compounded by the diagnostic dilemma when differentiating between Crohn’s disease and gastrointestinal TB, and the potentially serious consequences of initiating an incorrect treatment in the case of misdiagnosis. The interplay between IBD and TB is most relevant in Asia, where more than 60% of the 10.4million new TB cases in 2016 were reported. A number of studies have reported an increased risk of TB with anti-TNF agents, including in patients who had tested negative for TB prior to treatment initiation. The limited evidence currently available regarding adhesion molecule antagonists such as vedolizumab suggests a comparatively lower risk of TB, thus making them a promising option for IBD management in TBendemic regions. This comprehensive review examines the available literature on the risk of TB with the use of biologics in the TB-endemic regions of Asia, focusing on the diagnostic dilemma, the risk of reactivation, and the optimized management algorithms for latent and active disease.
