1.An Assessment of the Effectiveness of Practical in Parasitology for Undergraduate Students Using the Results of Practical Examination.

We assessed the effectiveness of practical instruction in parasitology for undergraduates at Jichi Medical School by examining grades on a practical examination. Two hundred six second-year medical students in 1997 and 1998 (103 students in each year) were enrolled in this study. The students took written and practical examinations at the end of the program. We found that grades on the practical examination were correlated with grades on the written examination (r=0.5664; p<0.001). The discrimination index ranged from 0.23 to 0.78. The percentage of correctly identified species was significantly higher when students studied live specimens than when they used other methods (p<0.0001 in both 1997 and 1998). The highest rates of correct identification (more than 90%) were for Anisakis species larvae and Enterobius vermicularis eggs in 1997 and for Anisakis species larvae, E. vermicularis eggs, and Anopheles mosquitoes in 1998. Results of neither written nor practical examinations differed significantly between students who chose biology at the entrance examination and those who did not. Our results suggest that undergraduates would gain a better understanding of parasitology by studying live specimens.

During baseball training, a commercially available sports drink was provided ad libitum, whereas during American football training, the effect of free water intake was compared between tap-water and sports drink. The sweat rate and the amount of water consumption were significantly correlated with WBGT under each experimental condition. Body weight loss was 0.2% body wt./h during baseball training, whereas during American football training, body weight loss was 0.5%/h with tap-water and 0.4%/h with sports drink on average, revealing a significantly higher value for tap-water.
These results indicate that both sweat loss and water intake during exercise increase with WBGT, and that body weight loss is maintained at a fairly constant level during exercise with free water intake, although the loss is significantly higher when tap-water is provided.
3.Relationship between drinking behavior and body temperature during exercise in heat.

Effects of drinking on body temperature during exercise and recovery in heat were studied in the field. Five healthy baseball players participated in baseball training with and without fluid re-placement on summer days, and rectal temperature, body weight loss, rating of thirst and the amount of water consumption were measured.
The rectal temperature was significantly lower throughout exercise when fluid replacements were allowed. The normalization of rectal temperature during rest periods was also significantly accelerated by fluid replacement. The rating of thirst was significantly correlated with the increase in rectal temperature and sweat loss, and also with the volume of fluid intake. Although full rehydration was not observed, body fluid replacement by drinking is suggested to be advantageous for body temperature regulation during exercise in heat.

5.Successful Treatment of an Infant Case of Apnea Caused by Rhinovirus Infection with High-flow Nasal Cannula

The case involved a one-month-old male infant. Around two weeks after birth, cold-like symptoms were observed within the family. Over the course of approximately one month, the infant gradually developed coughing, apnea, and cyanosis, leading to a visit to a nearby medical facility. The episodes of apnea were initiated by a wet-sounding, choking cough. Respiratory pathogen multiplex screening revealed rhinovirus infection. The infant was managed in the intensive care unit and treated with a high-flow nasal cannula (HFNC), which resulted in an improvement of the recurrent episodes of apnea. We discontinued HFNC on the 7th day of hospitalization; however, the patient had recurrence of apnea. After relieving abdominal distension through rectal enema, the infant was successfully weaned off HFNC and discharged on the 14th day of hospitalization. In addition to the immaturity of the respiratory center in infants, laryngeal chemoreflex triggered by chemical receptors in the larynx is known to contribute to apnea. In the present case, we considered that the reduction of respiratory effort through HFNC, the expectorant effect of warmed and humidified air, and the decrease in gastroesophageal reflux due to alleviation of abdominal distension played roles in the successful outcome.
6.Strengthened tuberculosis control programme and trend of multidrug resistant tuberculosis rate in Osaka City, Japan

Osaka City has the highest tuberculosis (TB) notification rates in Japan. In the period 1999–2003, the TB control programme was strengthened, and the Stop TB Strategy was implemented to reduce the number of notified cases. The objective of this study was to assess the effect of these control activities in Osaka City, including the implementation of directly observed treatment (DOT), by analysing TB surveillance and routinely collected data. We reviewed the surveillance data of all sputum smear-positive pulmonary tuberculosis (PTB) cases registered in the Osaka City Public Health Office from 2001 to 2008 and data collected from the routine TB programme. The DOT implementation rate increased from 0% in 2001 to 68% in 2008 for smear-positive PTB cases of the general public and to 61% for all PTB cases of the homeless. The proportion of smear-positive PTB cases that had treatment failure and default combined, declined from 8.0% (52 of 650) in 2001 to 3.6% (20 of 548) in 2006. The proportion of cases among the homeless with previous treatment declined from 28% in 2001 to 15% in 2008. The proportion of cases with multidrug resistant-TB (MDR-TB) among those without previous treatment declined from 1.7% in 2001 to 0.9% in 2008. It is logical that reduction in the failure and default rate would lead to the reduction of cases with previous treatment and TB transmission, including resistant TB, therefore to the reduction of MDR-TB rates.
7.Case Report of a Smooth Muscle Tumor with Low Malignant Potential (STUMP) Suspected Preoperatively to Be Leiomyosarcoma

We report a case of an epithelioid smooth muscle tumor of low grade malignancy, known as smooth muscle tumor of uncertain malignant potential (STUMP), in a 65-year-old woman. The patient was referred to our hospital because of an abnormal result on screening of the uterus. We initially suspected leiomyosarcoma based on the findings of preoperative medical examinations and tests, so she underwent a total abdominal hysterectomy with salpingo-oophorectomy. The final diagnosis was a tumor with a low malignant potential (STUMP).
8.PROPOSAL OF NEW GUIDELINES FOR PREVENTION OF HEAT DISORDERS DURING SPORTS AND DAILY ACTIVITIES BASED ON AGE, CLOTHING AND HEAT ACCLIMATIZATION

The guidelines for the prevention of heat disorders during sports activities were established 13 years ago in Japan. Since then, various studies on preventive measures against heat disorders have been done, yielding new knowledge about its prevention. It has been reported that the incidence of heat disorders is high in children and the elderly, and heat acclimatization and clothing are the factors involved in this disorder. We proposed to lower the WBGT (wet-bulb globe temperature) limit for warning (discontinuation of hard exercise) from “28°C or more” to “25°C or more” (corresponding to an ambient temperature of 28°C) for non-acclimatized persons, children, the elderly, and persons wearing clothes covering the entire body. We also indicated that heat disorders can occur due to unpredictable causes, because the mechanism is very complicated.
9.The Effects of Acupuncture on Shoulder-Hand Syndrome (SHS) as a Complication of Hemiplegia.

Hemiplegia after a cerebrovascular event is a complication that causes problems in daily life such as difficulty of walking and may lead to social disadvantages. Furthermore, various subsequent health issues that accompany hemiplegia tend to hinder rehabilitation. In particular, Shoulder-Hand Syndrome (SHS) is an intractably painful disease with primary symptoms of pain and swelling in the upper limbs of patients. We investigated the effect of the acupuncture treatment on those patients with hemiplegia after a cerebrovascular event who are suspected to have SHS because of such symptoms as pain, swelling, and paresthesia in the upper limb of the affected side by measuring the changes in the numerical scale (NS), Gibbons' RSD score, and range of motion (ROM) in upper extremities.
The present study consisted of 13 hemiplegic subjects (eight males, five females) with pain, swelling, and paresthesia in the upper limbs. Acupuncture treatment was applied twice a week for over two months. Specifically electrical acupuncture and/or the retaining needle technique was applied to the upper extremities for 20 minutes in each session. Of 13 participants, 10 showed a significant decrease in NS (reduced by five or more points), eight showed improvement of the swelling in the upper limbs, and nine showed reduction in paresthesia.
From these results, we concluded that the acupuncture treatment was effective for SHS that accompanies hemiplegia after a cerebrovascular event and that the improvement of the peripheral blood flow might play an important role in generating treatment effects.
10.A study on the physical work load of farmers and dust conditions in the harvesting process of the chrysanthemums.

The physical work load of farmers and dust conditions in the harvesting process of chrysanthemums were investigated for two households (seven workers for A and three for B household, respectively) at mid March in 1984.
The total working hours a day of each household were 3225 min (460 min as average per a worker) for A and 1954 min (652 min) for B, being allotted more times to males than females. However, all of the house keeping hours were carried out only by females.
The intensity of each working load of farmers was not so heavy, as “light” and “moderately heavy” by Christesen's criteria and energy expenditure a day was calculated 2895-3604 kcal for males and 2155-2295 kcal for females.
In this working process farmers were enforced to keep careful handling with chrysanthemums and fixed body posture for 4 to 6 hours a day. Particulary, the working posture of sorting and bundling chrysanthemums may cause over strained load to the muscle and joint of the lower back and legs.
The dust concentrations of the sorting room were not so high. However, the farmers were exposed a large quantity of cilia of chrysanthemum leaves, calculated 37-259 pieces/cm2 on the slide grass a day. The numbers of cilia varied with amounts of handling chrysathemums.
It was suggested that the cilia of chrysanthemums may be the most important allergen to the immediate type of allergy of the chrysanthemum growers.
