1.Biomechanical analysis in patients with knee osteoarthritis after Chinese massage combined with functional exercise

BACKGROUND:There are less reports on the evaluation of plantar pressure distribution in knee osteoarthritis patients using modern gait analysis, and moreover, the database of characteristic plantar pressure has not been established in patients with osteoarthritis. OBJECTIVE: To evaluate the therapeutic effect of Chinese massage and functional exercise on knee osteoarthritis. METHODS:Forty patients with left knee osteoarthritis who were eligible for the inclusive criteria were randomized into two groups, with 20 in each group. Control group was given conventional treatment, and treatment group was given Chinese massage combined with functional exercise. Japanese Orthopaedic Association score and gait parameters were measured and compared between two groups before and at 3 months after treatment. RESULTS AND CONCLUSION:After treatment, the two groups both had evident efficacy in pain relief during walking, and there was no statisticaly difference between the two groups. There was a significant difference in the knee range of motion in the patients in the treatment group when going upstairs and downstairs as wel as during joint flexion before and after treatment, but no changes occurred in the control group. After treatment, the range of motion during joint flexion was better in the treatment group than the control group, but there was no difference in the sweling reduction between the two groups. In addition, the treatment group had a higher Japanese Orthopaedic Association score after treatment than before treatment (P < 0.05), and there was no statistical difference in the control group. Gait analysis showed that there were improvements in the percentage of contact time, parameters during stance phase and peak plantar pressure in the two groups, but there was no significant difference before and after treatment. Foot axis angle in the treatment group was improved significantly, which was significantly better than that in the control group. Al the indexes in the treatment group were improved a lot, but did not reach the normal.
2.Psychological distress status and related factors of patients with bladder tumor

Objective To investigate the prevalence of psychological distress and analyze the relevant factors among patients with bladder tumor so as to provide evidence for future clinical practice. Methods Totally 128 patients were recruited from a urological surgery ward of a comprehensive hospital in Beijing in the study by using self-design questionnaire and the psychological distress thermometer (DT) recommended by the U.S. national comprehensive cancer network (NCCN). The acquired data were analyzed by SPSS17.0. Results The average score of the patients was 4.00(1.00~5.00). The identification rate of psychological stress was 55.47%( 71/128 ) , higher than the Chinese normal ( U = 8 . 28 , P < 0 . 05 ) . The relevant factors of psychological stress based on the rank from high to low scores included emotion problems ( 1 . 63 ± 0 . 67 ) , practical problems ( 1 . 42 ± 0 . 64 ) , communication problems (1.29 ± 0.65), physical problems (1.28 ± 0.33) and religion problems (1.00 ± 0.08). Conclusions The prevalence of psychological distress is higher among patients with bladder tumor and the influence factors mainly include emotional problems , practical problems and communication problems. Nurses should pay attention to the psychological distress of patients with bladder tumors and develop targeted interventions so as to relieve their distress.
3.Current diagnosis and treatment of insomnia in children and adolescents and study progress

Insomnia is one of the most common sleep disorders among children and adolescents.Poor sleep affects cognitive performance and is associated with poor emotional and physical health in children and adolescents.Early identification and treatment can have positive short-and long-term effects on the children,the parents,and the family as a whole.Pediatric sleep medicine had sustained development in recent years,however,in China,the researches on the epidemiology,diagnosis and treatment of insomnia in children and adolescents are in lack.In order to capture the majority of pediatric medical workers and parents attention,and to improve the clinical diagnosis and treatment level,the focuses on current diagnosis,therapy and related study progress of insomnia in children and adolescents were reviewed.
4.Application and evaluation of clinical path in clinical teaching in the department of gynecology and obstetrics

Objective To evaluate the effects of applying clinical pathway (CP) in clinical teaching of gynecology and obstetrics. Methods Totally 200 clinical specialist interns selected for the study were randomly divided into two groups (each 100), respectively, using CP teaching and tradi-tional teaching. After the internship, professional assessment (clinical skills assessment, theory test, the respondent)were conducted to two groups of students and their professional examination scores were compared, while questionnaires were conducted in the CP teaching group. All date were analyzed by SPSS 17.0 software. Chi-square test was used for enumeration date while t test was used for mea-surement data. Results CP teaching team of professional test results [(80.01 ± 9.34) vs. (72.37 ± 10.79)], the passing rate(96%vs. 87%) were superior to the traditional teaching group(P=0.000 and 0.022). The questionnaire showed that: applying CP in clinical teaching of obstetrics and gynecology can improve interns' learning enthusiasm and initiative, improve their comprehensive analysis ability, clinical thinking ability, practical ability and many other capabilities and it got about 90% of interns' recognition. Conclusions The application of the CP in clinical teaching of obstetrics and gynecology can help enhance the teaching effectiveness, standardize the teaching process, and improve the qual-ity of teachers teaching gynecology and obstetrics.
5.Research in preventive effect of auricular therapy in schizophrenia patients with constipation

Objective To evaluate the preventive effect of auricular therapy in schizophrenia pa-tients with constipation. Methods 200 psychotic inpatients with anti-psychotic medication were random-ly divided into the intervention group and the control group with 100 cases in each group. The intervention group received conventional therapy combined with auricular therapy while the control group adopted only conventional therapy. The incidence of constipation was observed in the two groups. Results The rate of constipation was 11.34% and 36.73% in the intervention group and the control group, respectively. Auricu-lar therapy showed various effect in constipation caused by different drugs, especially for constipation caused by clozapine, chlorpromazine and pentiapine. Conclusions Auricular therapy showed good effect in preventing constipation induced by antipsychotic drags with less pain and adverse effect, easy and safe to operate, besides, it can improve the medication compliance of patients and promote their rehabilitation.
6.A case of lower limbs infection caused by Helcococcus Kunzii and literature analysis

Objective To describe a case of lower limbs infection of Helcococcus Kunzii in North China University of Science and Technology Affiliated Hospital in October 2017 and to analyze the etiology and drug susceptibility??Methods The bacteria were identified by French Meriere mass spectrometer and BD Phoenix?100 automatic bacterial identification/drug sensitivity system??The drug resistance of the bacteria was detected by disk diffusion method,and collected the clinical information and related literature information of the Helcococcus Kunzi to analyze??Results The Gram?positive cocci was isolated from the foot secretion of a patient with venous thrombosis of lower limbs complicated with infection??The result of smear was Gram?positive cocci in double,agglomerate and varied in size??On the blood plate,there was a small colony of α hemolysis??Thixozyme negative,β galactosidase negative??The results of mass spectrometry identification and two kinds of fully automatic bacterial identification / drug sensitivity system were identified as Helcococcus Kunzii,the isolates was resistant to erythromycin and clindamycin among the 10 antibiotics tested??By searching,sorting out and analyzing the literature information, it is revealed that the infection of this bacterium is not related to sex??Middle?aged and elderly people are mostly infected??And the cocci easily infected patients with lower immunity??The infection of lower limbs is 47??37%,the infection of blood flow is 15??79% and the other ways of infection are 42??11%( lung, mammary gland, etc??)??Conclusion Helcococcus Kunzii is a conditional pathogen, and patients with lower immunity are more common??Erythromycin resistance could identify it from the green balloon bacteria??Moreover, the bacteria could cause a wide spectrum of diseases,and easily cause sepsis and lower limbs infections,which reminds us that we should be vigilant in clinical practice??
