6.Airway obstruction from intralaryngeal extension of thyroglossal duct cyst in an elderly man: A case report

To report an unusual presentation of thyroglossal duct cyst causing airway obstruction in an elderly man.

Results:
A 71-year-old man with an anterior neck mass was brought to the Emergency Room due to progressive difficulty of breathing. A smooth, non-ulcerating right supraglottic mass obstructed the airway. Following an emergency high tracheotomy, contrast computed tomography scan of the neck revealed a hypodense mass with peripheral rim enhancement in the right supraglottis and an extralaryngeal component. Intra-operatively, a dumbbell- shaped cystic mass with a tract connected to the hyoid bone led to a Sistrunk procedure. Final histopathology findings were consistent with thyroglossal duct cyst.
Conclusion
It is possible for an elderly patient with impending upper airway obstruction, dysphonia, and neck mass to still have a benign and congenital thyroglossal duct cyst with intralaryngeal extension.

7.Larygospasm secondary to a multinodular goitre

Laryngospasm is a serious event that can result in partial or complete upper airway obstruction. It is a common complication during recovery phase of anaesthesia, resulting from acute irritation of the vocal folds. We report a rare case of laryngospasm secondary to a multinodular goitre that settled after treatment. We postulate that the enlarged thyroid gland can lead to direct irritation of the laryngeal nerve as possible cause of repeated episodes of laryngospasm in this patient. To our knowledge, this is the first reported case of a multinodular goitre as the underlying cause of laryngospasm.

8.Treatment of nasolacrimal duct obstruction and lacrimal stenosid or obstruction by dacryorhinostomy

The study was done on 82 eyes having nasolacrimal duct obstruction and lacrimal stenosis or stenosis-obstruction. All were treated with dacryorhinostomy (DCR) and silicone intubation. The successful prevalence is 94.4%. Among them, the prevalence of functional success is 49.9%. The successful prevalence is increasing during the follow-up time.
