Objective To explore the curative effect of bisoprolol fumarate combined with wenxin granule for patients with heart failure complicated with premature ventricular.Methods 132 patients with heart failure and premature ventricular complexes were enrolled in our hospital from January 2013 to January 2016,and which patients divided into two groups,bisoprolol fumarate group (Group BF,n =66) accepted bisoprolol fumarate treatment (5 mg/time,1 times/day),and bisoprolol fumarate combined with wenxin granule group (Group BW n =66) adopted wenxin granule (9 g/time,3 times/day) based on the patients in Group BF.After 4 weeks treatment,the curative effect of all patients were evaluated with electrocardiogram monitor and cardiac ultrasound examination,and the plasma NT-proBNP concentration and hemorheology indexes were analyzed compared with pre-treatment.Results The recovery of ventricular function in Group BW was better significantly than that patients in Group BF from the cardiac ultrasound examination (P ＜ 0.05);The total relieved effective of clinical symptom,premature ventricular complexes and heart failure in Group BW were more remarkable than those in Group BF,and which difference from them was significance (P ＜ 0.05);The blood viscosity,plasma viscosity,plasma fibrinogen and plasma NT-proBNP concentration in patients with Group BW were significantly lower than those Group BF (P ＜ 0.05).Conclusions The clinical curative effect of bisoprolol fumarate combined with Wenxin Granule for heart failure complicated with premature ventricular was more remarkable compare with bisoprolol fumarate single treatment,and which possessed the relative safety.

Objective To study the percentage and the distribution of nosocomial infection, and susceptibility of related pathogens to antibiotics in patients with systemic lupus erythematosus (SLE). Methods The clinical data of in patients with SLE in our hospital from June, 1990 to June, 1998 were analyzed. The susceptibility of the pathogens to antimicrobial agents was tested by K-B paper dilution method. Results The percentage of nosocomial infection in these cases was 16.1％ . The major pathogens were staphylococcas aureus, Escherichia coli and Candida. The most highly resistant microbes were E.coli, S.arueus and Pseudomonas aeruginosa. Conclusions The nosocomial infection is one of the most common complication of SLE in the in patients. It is necessary to examine the pathogens regularly and use antibacterial agents according to the susceptibility.

Objective To compare the efficacy of liposuction and open surgery for patients with accessory breast.MethodsA total of 73 patients with accessory breast,who were treated in our hospital from June 2000 to December 2007,were enrolled in this study and divided into open surgery(June 2000 to June 2004,43 patients with accessory breast at 80 sides)and liposuction(April 2004 to December 2007,30 cases with accessory breast at 58 sides)groups.The outcomes of the patients,operation time,and intraoperative blood loss in the two groups were compared.Results The curative rate of the both groups were 100%.However,the open surgery group had more blood loss [(49.6?13.3)ml vs(28.9?6.9)ml,t=10.836,P=0.000] and longer operation time[(57.4?11.1)min vs(27.3?4.3)min,t=19.597,P=0.000].Conclusions Compared with open surgery,liposuction leads to quicker recovery and less trauma without leaving scars,and thus is more acceptable to the patients.

Objective To compare the differences of three diagnostic criterions for metabolic syndrome (MS) proposed by the International Diabetes Federation (IDF) in 2005, the Third Report of National Cholesterol Education Program-Adult Treatment Panel Ⅲ (NCEP-ATPⅢ) in 2005 and Chinese Diabetes Society(CDS)in 2004 in population of Xilinguole city in inner mongolia. Methods 1 060 employees aged more than 20 yrs in enterprises taken health examination in Xilinguole hospital were investigated by epidemiological survey and the prevalence rates of MS and its components were calculated according to these three definitions respectively, and compared the agreement rates and difference between these definitions. Results The age-adjusted MS prevalence rates were 25.0%, 27.9% and 22.5% according to IDF (2005), ATPⅢ (2005) and CDS (2004) respectively. The agreement rate of diagnosis between IDF and ATPⅢ was the highest, and the Kappa value was 0.929. The kappa values between IDF and CDS,ATPⅢ and CDS were 0.726 and 0.763, respectively. There were 4.3% and 10.8% subjects respectively without MS diagnosed by IDF and CDS criterion presented at least 3 risk factors respectively. For Mongolia nationality people, when the waist circumference was 90cm in male and BMI was 25kg/m2 in female, the sensitivity and specificity were relative better, and the distance of ROC curve was the shortest, but for Han nationality, BMI≥25 kg / m2 in male and waist circumference ≥80 cm was better. Conclusion ATPⅢ definition can screen out the highest prevalence of MS and risk factors aggregation among three definitions, and it's best for screening for the high risk population of cardio-cerebrovascular diseases. The cutoff value of waist circumference (WC) in IDF and ATP Ⅲ seems better for Mongolia nationality men; while the cutoff value in CDS was better for Han nationality men. We should combine waist circumference and body mass index to judge the condition of obesity.

Objective To summarize 380 cases of the laser hair removal experience. Methods Using 800nm's wavelength of wide 30ms diode laser hair removal machine, different energy of selection and the time interval were different according to the different positions such as: the lowest energy 19J/cm 2 of hairline was selected and treated once at the interval of 3 - 4W with increasing 1-2J/cm 2 every time, as a result of 3 -5 times had been taken for cure. For legs, the lowest dose was 36 J/cm 2 . The highest energy achieved at 25J / cm 2 with interval 7 weeks, and it needed to increase 2-3J/cm 2 every times, and a total of 3 to 5 times were used for the treatment. After treatment each time, no special care was needed. Results The pati ents were followed up for more than half a year to 1 year. Cure rate was 95 %. Thereinto 20 persons appeared several spots of pigmentations after treatment, and those disappeared within 2 to 4 months. Only 2 persons with dark color of the skin still had slight pigmentation after 1 year. Conclusion Hairs treated many times by laser show regeneration delay and become thin in quality in the newborn hairs with lighting in color. The different energy is chosen for the treatment according to the different hair position that finally achieves permanent hair removal.

Objective To study the concentration alteration of serum vascular endothelial growth factor (VEGF) in patients with primary and secondary glomerular diseases (GD), the effect of immune inhibitors on the concentration, and analyze the relationship between VEGF concentration and some clinical indexes. Methods Serum VEGF concentration of GD patients was determined by sandwich ABC-ELISA. Results All of the GD patients had higher level of serum VEGF than the control group. Of the 5 groups divided according to the clinical classification, patients of chronic glomerulonephritis(CGN) group, latent glomerulonephritis(LGN) group, Henoch-Schonlein purpura nephritis(HSPN) group and lupus nephritis(LN) group had higher serum VEGF level than the control group, respectively, with no difference among the above groups. The patients without taking immune inhibitors had higher serum VEGF level than the control group,while no difference was observed between the patients taking immune inhibitors and the control group.Patients with glomerulonephritis had a significantly lower serum VEGF level after receiving immunosuppression aggressive therapy than before. The serum VEGF level of nephrotic syndrome in nephrotic phase was higher than in remission stage. In membranous glomerulonephritis serum VEGF correlated positively with 24-hour proteinuria excretion. The serum VEGF level correlated positively with anti-dsDNA Ab titer and the concentration of ? globulins in LN. Conclusion Cytokine VEGF is involved in the pathogenesis of glomerulonephritis, and in the occurrence and progression of proteinuria. Immunosuppression therapy can inhibit the expression of VEGF. In LN patients, the serum VEGF level is related to LN activity.

Objective To study the effects of cytokines Th1 and Th2 in Class V lupus nephritis (V-LN). Methods Serum concentration of Th1 cytokines (IFN-γ, IL-1, IL-2 and TNF-α) and Th2 cytokines (IL-4, IL-5, IL-6, IL-10 and IL-13) were simultaneously analyzed using fast quant human Th1/Th2 protein array, and Pearson analysis was used to evaluate the association between cytokines and clinical parameters. Results ① Cytokine profiling: Among the 9 cytokines detected simultaneously by fast quant human Th1/Th2 protein array, the expression of four cytokines was up-regulated obviously, namely, Th1 cytokines (IL-2) and Th2 cytokines (IL-4, IL-10 and IL-13); that of IL-10 was 10 times above the normal control. ② Pearson correlation analysis: There was a positive correlation between IL-10 and SLEDAI (r=0.877, P<0.01), but a negative correlation between IL-10 and hemoglobin concentration (r=-0.856, P<0.01). There was also a negative correlation between IL-4 and 24h urine protein (r=-0.754, P<0.05), between IL-13 and 24h urine protein (r=-0.769, P<0.05). Besides, IL-1 and creatinine were positively correlated (r=0.784, P<0.05). Conclusion There were extensively abnormal Th2 cytokines in V-LN patients, suggesting that anti-Th2 therapy may produce a marked effect on V-LN.

BACKGROUND: There is still recrudescent risk in the convalescence stage of the acute myocardial infarction and the living condition is relatively poor. The study proved that it has relation to that the patients haven't mastered the scientific conformable life style for the myocardial infarction recovery.OBJECTIVE: Take the rehabilitation education to help the patients set up conformable life style so as to improve the quality of life.DESIGN: The patients in the convalescence stage of the acute myocardial infarction were chosen for the control study.SETTING: Nursing Institute of Beijing Medical University and two university hospitals.PARTICIPANTS: The cases were taken from the Cardiology Departm ents of the Second and Third Affiliated Hospital of Beijing Medical University from April 1999 to November 1999. Inclusive standard: the first onset of the acute myocardial infarction and voluntary for this study. Exclusive standard: not the first onset of the acute myocardial infarction. The control group contained 35 patients hospitalized from April 1999 to June 1999 conforming to the including standard. The experimental group contained 40 patients hospitalized from August 1999 to November 1999 conforming to the including standard. After eliminating the special unmatched cases and missed follow-up cases in the experimental group, the final 30 cases were respectively involved in the both groups. There were 23 male cases and 7 female cases in the control group with the mean age of(60 ± 14), and 22 male cases and 8 female cases in the experimental group with the mean age of(62 ± 10) . There was no significant difference in the age and sex in these two groups.METHODS: The Neuman health care system pattern was taken as the theoretical frame for the rehabilitation education of the patients in the convalescent stage of the acute myocardial infarction. That is the conver genceteaching and personal illustration accompanied the rehabilitation education hand books were adopted for the rehabilitation education in the experimental group before their leaving. Then the telephone education was taken for further direction and consultation during the first three months after being discharged. The evaluation information was obtained through the follow-up after the first three months.MAIN OUTCOME MEASURES: The quality of life and dimensions in the two groups.RESULTS: Compared with the control group, the scores of the quality of life, the dimensions of the body, the psychology, the duty role and the health consciousness in the experimental group were all significantly higher(t= 11.03,3.26,3.62,2.05,15.48, P < 0. 05), while there was no significant difference in the social degree( P > 0.05) .CONCLUSION: The rehabilitation education can significantly improve the quality of life of the patients in the convalescence stage of the acute myocardial infarction.

ObjectiveTo explore the role of parameters of organ function during heat stroke (HS) on the prognosis, and to form the treatment strategy through an analysis of parameters of organ function during HS. Methods A retrospective study was conducted. Thirty-seven patients with HS (HS group) and 54 patients with mild-to-moderate stroke (stroke group) admitted to Zhejiang Xiaoshan Hospital from 2011 to 2014 were enrolled. The experimental results of organs function indicators for patients were recorded including:① cardiac markers:troponin I (TnI);② myocardium zymogram: creatine kinase (CK), MB isoenzyme of creatine kinase (CK-MB), lactate dehydrogenase (LDH), and aspartate aminotransferase (AST);③ renal function indexes: blood urea nitrogen (BUN), uric acid (UA), and serum creatinine (SCr);④ electrolyte: serum K+, Na+, and Cl-;⑤coagulation function: prothrombin time (PT), international normalized ratio (INR), activated partial thromboplastin time (APTT), thrombin time (TT), fibrinogen (FIB), and D-dimer;⑥ blood gas analysis: pH value, arterial partial pressure of carbon dioxide (PaCO2), base excess (BE), standard bicarbonate (SB), and actual bicarbonate (AB);⑦ routine blood test: blood platelet count (PLT);⑧ hepatic function: alanine aminotransferase (ALT). Abnormal rates of laboratory parameters of 37 HS patients were statistically analyzed. Various laboratory parameters of organs function as well as the initial value and extreme value (maximum or minimum value) during treatment of CK and PLT in HS patients were compared between two groups.Results The abnormal rates of 37 HS patients were more than 70%, including incipient value of TnI, CK, LDH, AST, serum Na+, ALT, D-dimer, PaCO2, AB, maximum value of CK, and minimum value of PLT, the abnormal rates being 73.0%, 70.3%, 81.1%, 78.4%, 78.4%, 70.3%, 70.3%, 70.3%, 75.7%, 81.1%, 75.7%, respectively. The abnormal rates of other parameters were less than 70%. There were significant differences in incipient value of TnI, CK, LDH, AST, serum K+, serum Na+, D-dimer, and PLT between HS group and mild-to-moderate stroke group [TnI (μg/L): 0.087 (0.026, 0.306) vs. 0.007 (0.004, 0.110),Z = -7.017,P = 0.000;CK (U/L): 392.30 (287.60, 524.10) vs. 137.10 (106.33, 607.80),Z = -7.930,P = 0.000; LDH (U/L): 317.98±122.74 vs. 207.85±57.71, t = 1.678,P = 0.000; AST (U/L): 94.90 (52.80, 155.80) vs. 26.10 (18.13, 317.40),Z = -6.157, P = 0.000; serum K+ (mmol/L): 3.46±0.65 vs. 3.86±0.57,t = 1.662,P = 0.001; serum Na+ (mmol/L): 129.75±7.34 vs. 138.79±4.26,t = 1.674,P = 0.000; D-dimer (mg/L): 2.53 (0.63, 6.00) vs. 0.30 (0.21, 9.71),Z = -5.084, P = 0.000; PLT (×109/L): 144.62±86.14 vs. 219.48±64.76,t = 1.669,P = 0.000]. There were also statistically significant differences in the initial value and extreme value of CK and PLT between HS group and mild-to-moderate stroke group [CK (U/L): 392.30 (287.60, 524.10) vs. 721.50 (546.30, 964.10),Z = -6.351,P = 0.000; PLT (×109/L):132.40±82.55 vs. 68.24±44.62,t = 1.688,P = 0.000].Conclusions HS can impair several organs and systems, having complications, and it is a heavy insult for body. Increasing of CK and decreasing of PLT has some value to assess illness changes. It is helpful of laboratory results for doctors to estimate complications on time.

Objective To investigate the influence of telephone follow-ups on the compliance of syphilis patients.Methods The 120 patients diagnosed with syphilis during February 2010 to December 2011 were randomly divided into experiment group and control group with 60 cases in each group.The control group was given disease-related education during outpatient visits,while the experimental group received nursing intervention by phone calls.The two groups were compared in respect of curative compliance. Result The curative compliance of the experiment group after follow-ups was significantly better than that of the control group (P<0.01).Conclusion The telephone follow-ups can significantly improve the curative compliance of syphilis patients,which can significantly prevent the spread of syphilis.
