1.Effect of Herbal Decoction for Invigorating the Spleen to Dissolve Damp on Infantile Rotavirus Enteritis and on Myocardial Enzymes

Objective To observe the effect of herbal decoction for invigorating the spleen to dissolve damp on infantile rotavirus enteritis (IRVE) and myocardial enzymes.Methods Eighty patients were randomized into a treated group and a control group.The former was treated with the herbal decoction which could invigorate the functions of spleen to dissolve damp,the latter treated with Smecta.The course of treatment for both groups was 3 days.The changes in the symptoms of diarrhea,vomiting,and fever were observed and the myocardial enzymes including aspartate aminotransferase (AST),lactic dehydrogenase (LDH),creatine phosphokinase (CK) and CK-MB were determined.Results The effect of the treated group in stopping diarrhea,relieving fever,and stopping vomiting was all superior to that of the contro1 group (P0.05).Conclusion The herbal decoction for invigorating the spleen to dissolve damp in the treatment of IRVE functions to shorten the course of disease,improve the symptoms,and reduce the damage of cardiac muscles.
2.Analysis on main biochemical indexes in patients with type 2 diabetes mellitus and complications

Objective To analyze the main biochemical indexes in the patients with type 2 diabetes mellitus (T2DM) and complications.Methods A total of 112 cases of patients with T2DM in the Zigong Municipal Third People's Hospital were selected from March 2015 to March 2016 and divided into the T2DM without complications group (61 cases) and T2DM with complications group (51 cases) according to whether having complications.Contemporaneous 55 volunteers undergoing physical examination were selected as the control group.The levels of insulin,fasting blood glucose (FBG),D-dimer (D-D) and serum lipids (HDL,LDL,TC and TG) were compared among three groups.Results The levels of FBG,D-D,TC,TG and LDL in the T2DM without complications group and T2DM with complications group were higher than those in the control group,while the levels of insulin and HDL were lower than those in the control group,the differences were statistically significant (P<0.05);moreover,except HDL,the levels of insulin,FBG,D-D,LDL,TC and TG had statistical difference between the T2DM with complications group and T2DM without complications group (P<0.05).Conclusion Regularly detecting the biochemical indexes of blood glucose and lipids in diabetic patients is an important mean to preventing and treating diabetes mellitus and its complications.
3.Determination the Contents of Paracetamol and Caffeine in999Ganmaoling Granules With TLC

OBJECTIVE:To simultaneously determine the contents of paracetamol and caffeine in999Ganmaoling granules with TLC.METHODS:Chloroform-ethylacetate-methanol-annmonia water(15∶4∶3∶0.3)was taken as developing a?gent.The spots were scanned with CS-930TLC scanner at? S =245nm,? R =315nm for paracetamol and at? S =270nm,? R =350nm for caffeine.RESULTS:Average recovery of paracetamol was99.33%,RSD=1.28%;Average recovery of caffeine was100.1%,RSD=1.71%;The labelling contents of paracetamol and caffeine in three batches of samples were99.02%,98.48%,99.23%and99.74%,98.57%,99.24%respectively.CONCLUSION:The method is simple,quick and accurate.It can be used for the quality control of999Ganmaoling.
4.Analysis of influencing factors of platelet transfusion during bone marrow empty window period on patients with allogeneic peripheral blood stem cell transplantation

Objective To analyze the influence of different factors and their relating correlation results on platelet transfusion during the bone marrow empty window period on the patients who have undergone allogeneic peripheral blood stem cell transplantation (allo-HSCT) with retrospective analysis of case-control data.Methods Clinical data of 153 cases were collected by the clinical blood management and evaluation information system with discharge diagnosis of allo-HSCT in the hematology department of The First Affiliated Hospital of Nanchang University within a time frame from January 2014 to December 2016.A total of 90 cases were considered valid for retrospective analysis according to the case exclusion criteria.The average transfusion dose for patients with allo-HSCT during the bone marrow empty window period was defined as the threshold value which divided the 90 cases into the observation group of 38 cases receiving more than 6 Units of platelet transfusion and the control group of 52 cases with less than 6Units of platelet transfusion.The amount of platelets transfused during the bone marrow empty window period,clinical indexes include Hb,ANC,Plt,SF before pretreatment,platelet engraftment time and the number of mononuclear cells implanted were compared and analyzed by Logistic regression.Results (1) There was no significant difference between the two groups in gender,age,primary diagnosis,HLA matching,Hb before pretreatment and the number of mononuclear cells implanted (P>0.05).The ANC(×109/L) (1.24±0.57 vs 3.36±1.33) and Plt(×109/L) (43.55±68.29 vs 126.62±84.73) counts before pretreatment in the observation group were significantly lower than those in the control group(P<0.05).SF(μg/L) (2351.05 ± 1 587.96 vs 1 000.96± 362.97)before pretreatment and P LT recovery time (d) (16.84± 2.47 vs 12.73 ± 1.65)was significantly higher than that in the control group(P<0.05).Donor-recipient ABO blood group typing incompatibility (15 vs 10) was significantly higher than the control group (P<0.05);(2) Single factor Logistic regression analysis showed that ABO blood group matching,clinical indexes include ANC,Plt,SF before pretreatment,PLT recovery time were statistically significant,Only ANC before pretreatment and PLT recovery time had significant effect on the platelet transfusion during bone marrow empty window period in patients with allo-HSCT in multivariate Logistic regression analysis(P<0.05).Condusion The ANC before pretreatment and PLT recovery time are independent factors for platelet transfusion of the bone marrow empty window period in patients with allo-HSCT.The PLT recovery time is an independent risk factor,which indicates that the longer the duration of PLT implantation,the greater the amount of platelet transfusion will be needed.Besides,the ANC before pretreatment is the independent protective factor,which indicates that the greater the ANC,the smaller the amount of platelet transfusion is required.
6.Retrospective Analysis of Clinical Pharmacists Participating in the Consultation for 187 Infection Cases in Paediatrics

Objective:To summarize the experience and effect of clinical pharmacists participating in the consultation for infection cases in paediatrics.Methods:All of medical records involving the anti-infective consultation of clinical pharmacists from July 2014 to June 2017 in our hospital were retrospectively analyzed,and the data including gender,age,primary diseases,infection sites,disposal opinions,suggestion adoption and treatment outcome were statistically analyzed. Results: During July 2014 to June 2017, clinical pharmacists participated in the consultation for 187 infection cases. Among them,69 cases were newborns. The consultation reasons mainly included the selection and adjustment of antimicrobial agents which accounted for 83.42% (156 cases).There were 105 cases of positive etiological examination. According to the outcome of diseases, the total effective rate of consultation performed by clinical pharmacists was 92.31%. Conclusion:Due to the specificity of children's infective diseases,participaton in the treatment by clinical pharmacists is important for the diagnosis and treatment of children patients and the improvement of rational drug use in paediatrics.
7.Systemic chemotherapy for patients with advanced biliary tract cancer: a single-center retrospective study

Objective:To study the efficacy of different systemic chemotherapy regimens as first-line and second-line therapy and to determine the prognostic factors for patients with advanced biliary tract cancer.Methods:The clinical data of patients with advanced biliary tract cancer who underwent systemic chemotherapy in Cancer Hospital of Chinese Academy of Medical Sciences from January 2011 to December 2018 were studied. The efficacy of chemotherapy on objective response rate (ORR) and disease control rate (DCR) were evaluated. Potential prognostic factors for survival were studied using the Cox proportional hazards models.Results:Of 151 patients enrolled into this study, there were 75 males and 76 females, with ages ranging from 31 to 77 years (median 58 years). Two treatment protocols were used: (1) 104 patients received a gemcitabine-based regimen (combined with platinums or fluorouracils) or a combination of platinums and fluorouracils, while (2) 47 patients received a combination of albumin-bound paclitaxel and S-1. The corresponding ORR for each group were 15.4%(16/104) and 27.6%(13/47), respectively, and the DCR were 65.4%(68/104) and 72.3%(34/47), respectively. Of 58 evaluable patients who received chemotherapy as a second-line therapy, 31 patients received the regimen containing gemcitabine, platinums or fluorouracils with an ORR of 3.2% (1/31) and a DCR of 35.5%(11/31); a total of 18 patients received the taxanes-based regimen with an ORR of 11.1%(2/18) and a DCR of 38.9%(7/18); 9 patients received the irinotecan-based regimen with an ORR of 22.2%(2/9) and a DCR of 44.4%(4/9). Univariate analysis showed positive liver metastasis and elevated carbohydrate antigen (CA)19-9 level to be significantly correlated with worse survival outcomes ( HR=1.540, 95% CI: 1.019-2.328, P=0.040 and HR=1.892, 95% CI: 1.123-3.188, P=0.017). Conclusion:For patients with advanced biliary tract cancer, in addition to the conventional regimens containing gemcitabine, platinums and fluorouracils, the combination of albumin-bound paclitaxel and S-1 was shown to be an effective chemotherapeutic regimen for these patients. Second-line chemotherapy was insufficient and ineffective, and an irinotecan-based regimen deserves to be further investigated. Liver metastasis and elevated CA19-9 level were worse prognosis after chemotherapy for patients with advanced biliary tract cancer.

Gastric carcinoma is a heterogeneous malignant disease involving genetic factors.To identify predictive markers for gastric cancer treatment in Chinese patients,we evaluated the association between polymorphisms of the gene encoding cytochrome P450 2A6 (CYP2A6) and outcomes of S-1 plus oxaliplatin (SOX) chemotherapy treatment.Clinical data on 60 consecutive gastric cancer patients receiving SOX regimen were collected prospectively.We sequenced all exons of CYP2A6 and a total of 22 different polymorphisms were detected in the present study.Comprehensive analyses of these genetic polymorphisms were performed to determine their association with both safety and efficacy of SOX regimen.Our results showed that polymorphisms of CYP2A6 were associated with the safety and efficacy of SOX treatment.Among them,missense mutations CYP2A6 rs60823196 and rs138978736 could be possible risk factors (P < 0.05) for severe diarrhea induced by SOX,whereas CYP2A6 rs138978736 could be a conceivable predictor for overall survival of patients treated with SOX adjuvant chemotherapy.Further large-scale randomized prospective studies are warranted to confirm these findings.
9.Application of Carotid Ultrasonography in Detecting Carotid Artery Lesions of Patients with Ischemic Cerebro-vascular Disease

Objective:To explore the application of carotid ultrasonography in detecting carotid artery lesions of patients with ischemic cerebrovascular disease(ICVD).Methods:Carotid ultrasonography and vascular reactivity(VR)were performed on 82 patients with ICVD and 40 health controls with Color Doppler Ultrasound.The ultrasonic indexes were statistically analyzed. The intima-media thickness(IMT)and the plaque score,the plaque total area and the cerebral blood flow volume of two groups were compared with each other.Results:Vessel diameter (VD),IMT,peak systolic velocity (PSV),end-diastolic velocity (EDV),mean flow velocity(MFV)in patients with severe stenosis of internal carotid artery and common carotid artery in ICVD group were significantly higher than those in control group,and the differences were statistically significant(P <0.01).VR in patients without stenosis and with mild,moderate and severe stenosis was lower than that in the control group(P <0.05).In ICVD group,IMT,carotid plaque score and plaque total area were significantly higher than those in the control group(P <0.01).Conclusions:The detection on degree of carotid lesions in patients with ICVD via carotid ultrasonography was conducive to understanding the status of ICVD,so as to determine the course and the prognosis.It was significant to clinical treatment of ICVD.
10.Quality analysis of the primary semen samples from 512 donors.

OBJECTIVETo evaluate the semen quality of the donors.
METHODSThe semen parameters of the primary samples of 512 donors were examined following the World Health Organization (WHO) guide and computer-aided sperm analysis (CASA, product by Harmilton Thorne) system.
RESULTSOf the 246 (246/512, 48%) donors with potential fertility, only 146 (146/512, 28.5%) came up to the semen standard set by the Chinese Ministry of Health, while 266 (266/512, 52%) were below the WHO reference values of semen parameters.
CONCLUSIONRepeat semen analyses may increase the success rate in screening semen donors. The semen quality of the donors suggests that it is necessary to pay more attention to male reproductive health.
