OBJECTIVES:
To analyze the clinical characteristics and prognosis of children with anti-N-methyl--aspartate receptor (NMDAR) encephalitis and to provide a basis for early clinical identification of this disease.
METHODS:
The clinical data of 42 cases of anti-NMDAR encephalitis at Department of Pediatrics, Second Xiangya Hospital, Central South University from January 2015 to March 2018 were collected. The clinical features and followed-up outcomes were analyzed retrospectively.
RESULTS:
There were 15 cases (35.7%) of males and 27 cases (64.3%) of females in 42 children, with a ratio of 1꞉1.8. They were aged from 4 months to 17 years, with an average of (9.20±4.66) years. The most common initial symptoms were seizures (47.6%, 20/42) and mental behavior disorder (35.7%, 15/42). During the course of the disease, 85.7% patients(36/42) had mental and behavior disorder, 85.7% patients (36/42) had epilepsy, 76.2% (32/42) had speech disorder, 66.7% patients (28/42) had dyskinesia, 66.7% patients (28/42) had the decreased level of consciousness, 61.9% patients (26/42) had autonomic instability, and 57.1% (24/42) patients had sleep disorder. All the children had positive antibody against NMDA receptor resistance encephalitis in cerebrospinal fluid. Head MRI showed the abnormal incidence was 50.0% (21/42), and the lesions involved in parietal lobe, frontal lobe, temporal lobe, occipital lobe, midbrain, thalamus, basal ganglia and optic nerve. There was a patient with optic nerve damage combined with myelin oligodendrocyte glycoprotein (MOG) antibody positive. Forty cases were examined by electroencephalogram (EEG), 92.5% cases (37/40) were abnormal, mainly showing diffuse slow waves, and δ brushes could be seen in severe cases. And there was 1 patient (2.4%) complicated with mesenteric teratoma. The mRS score (2.14±1.46) at discharge was significantly lower than the highest mRS score (3.88±1.38) during hospitalization (<0.05). After 3-39 months of follow-up, mRS score at 3 months after discharge was only 0.81±1.29, which was still improved compared with that at discharge, 76.2% cases (32/42) experienced complete or near-complete recovery (mRS score≤2), and 4.8% (2/42) cases relapsed. There was no mortality; the initial time of immunotherapy and the highest mRS score in the course of the disease were the factors affecting the prognosis. The earlier the starting time for immunotherapy and the lower mRS score in the course of the disease were, the better the prognosis was.
CONCLUSIONS
Seizures, mental and behavior disorder, dyskinesias, speech disorder and autonomic instability are common clinical manifestations of anti-NMDAR encephalitis in children. The effect of immunotherapy is significant, and the time to start immunotherapy and the severity of the disease are important factors affecting the prognosis. Anti-NMDAR encephalitis can be combined with other autoantibodies, but its clinical significance and mechanism need further study.

OBJECTIVE: To investigate the distribution, drug supply and quantity of licensed pharmacists in retail pharmacies around Third Grade Class A Hospitals, and to provide reference for decision-making of related management departments. METHODS: Field investigation and questionnaire survey were used to investigate the distribution, type, operating area, annual turnover, drug type, ranking of consumption sum, daily prescription amount and pharmacist allocation of 5 Third Grade Class A Hospitals in Beijing and general information of 100 surveyed pharmacists in 2017. RESULTS: There were 43 retail pharmacies in total within 1 km of 5 Third Grade Class A Hospitals, with an average of 8.60 retail pharmacies for each hospital. The number of retail pharmacies with distance between 200-400 m and 800-1 000 m was the largest (11), of which 21 (48.84%) were chain drugstores. The operating area of 38 pharmacies (88.37%) was in the range of 100-175 m2, mainly ranging 100-125 m2 (16 pharmacies, 37.21%). The annual turnover of 40 pharmacies were counted and ranged 50-10 000 ten thousand yuan, among which the pharmacies 0-200 m away from the hospital had the highest annual turnover. The average variety of drugs (prescription drugs and over-the-counter drugs) was 2 511 kinds. 34 pharmacies (79.07%) had more varieties of over-the-counter drugs than prescription drugs, and the consumption sum of over-the-counter drugs was higher than that of prescription drugs in 26 pharmacies (60.47%). Top 3 drugs in the list of consumption sum were antineoplastic drugs, cardiovascular drugs and anti-infective drugs. Average daily prescriptions of 28 drugstores (65.11%) were less than 10 pieces; average daily prescription were more than 20 pieces in only 7 pharmacies (16.28%). On average, each pharmacy had 7.4 staff members and 1.37 licensed pharmacists. 58 staff members’ first degree was junior college (58%), 38 staff members’ first degree was technical secondary school or high school (38%), and only 3 (3%) staff members’ first degree was undergraduates or above. CONCLUSIONS: With the gradual promotion of “health care policy” and the outflow of more hospital prescriptions, the retail pharmacies around hospitals should increase their numbers, expand the operating area, accelerate the construction of licensed pharmacists and improve the education level of pharmacists so as to improve the pharmaceutical care ability.

[Abstract] Objective: To investigate the expression of galectin-3 in bladder cancer tissues and its correlation with clinicopathological characteristics of bladder cancer patients, as well as to explore its effect on the proliferation, invasion and apoptosis of bladder cancer T24 cells. Methods:Atotal of 104 cases of pathologically confirmed bladder cancer tissues and the corresponding adjacent tissues were collected from the patients treated at the Affiliated Hospital of North Sichuan Medical College from May 2014 to June 2016. Immunohistochemistry staining was used to determine the galectin-3 protein expression in both cancer and adjacent tissues, and the correlations between galectin-3 expression and clinical pathological features were analyzed. siRNA-Gal3 and siRNA-Control were transfected into T24 cell, respectively. The expression of galectin-3 protein was detected by Western blotting, the proliferation of cells was detected by MTT assay; the invasion of cells was detected by Transwell assay; and the cell apoptosis was determined by Flow cytometry. Results: The positive rate of galectin-3 in bladder cancer tissues was significantly higher than that in adjacent tissues (73.1% vs 9.6%, P<0.05). The expression of galectin-3 in bladder cancer was correlated with histological grade, depth of invasion, lymph node metastasis and TNM stage (all P <0.05), but not with sex and age (P>0.05). The expression of galectin-3 was down-regulated significantly by siRNAGal3 (P<0.05). After interference with galectin-3, the proliferation and invasion of T24 cells was significantly decreased (all P<0.05) but the apoptosis was significantly increased (P<0.05). Conclusion: Galectin-3 is over-expressed in bladder cancer and is closely related to the clinicopathological features of bladder cancer patients. Interference of galectin-3 protein expression can inhibit proliferation and invasion and promote cell apoptosis of bladder cancer T24 cells.

Objective To investigate the frailty of elderly inpatients with cardiovascular disease and analyze its influencing factors. Methods From March 2017 to January 2018, we selected 698 elderly patients aged 65 or over of cardiovascular department at a class Ⅲ grade A hospital in Beijing by convenience sampling. The self-designed questionnaire, short physical performance battery (SPPB), Mini-Cog and phenotype of frailty (FP) were used to investigate patients' general information and some health indicators, the activity function of daily life, cognitive function and frailty respectively. Logistic regression was used to analyze the influencing factors of frailty. Results The incidences of frailty, prophase of frailty and non-frailty of elderly inpatients with cardiovascular disease were 37.5% (262/698), 50.3% (351/698) and 12.2% (85/698) respectively. There were statistical differences in elderly inpatients with cardiovascular disease among different ages, sexes, education levels, Mini-Cog, whether they fell in the past year, SPPB, ADL, coexistence of diseases and types of oral medicine (P< 0.05). There were also statistical differences in hemoglobin (Hb), neutrophils ratio (NEUT%), C reactive protein (CRP), serum albumin (ALB), prealbumin (PA), urea nitrogen (BUN), serum creatinine (SCr), homocysteine (Hcy), interleukin-6 (IL-6), free triiodothyronine (FT3) and left ventricular ejection fraction (LVEF) among elderly cardiovascular disease inpatients with different levels of frailty (P< 0.05). Logistic regression analysis showed that the influencing factors of frailty and pre-frailty included SPPB, ADL, Hb, BUN (P<0.05). The number of chronic diseases was the influencing factor of frailty (P<0.05). Conclusions Elderly inpatients with cardiovascular disease are a high-risk group for frailty. The main influencing factors of frailty include Hb, BUN, SPPB, ADL and the number of chronic diseases. We should screen the frailty of elderly inpatients with cardiovascular disease and provide comprehensive intervention for frailty patients so as to improve prognosis.
