1.Progress on target therapy in colorectal cancer

Target therapy is a major progress in treatment of advanced colorectal cancer. Cetuximab and bevacizumab are the most widely used target agents in colorectal cancer, which target against EGFR and VEGF respectively. The addition of target agent to chemotherapy improve efficacy and prolong survival in the first line setting, as well as in the second line setting. The increased respectability of liver metastases from colorectal cancer is of great importance, k-ras gene mutation is a definite predictor of cetuximab, however, there is no clear predictor for bevacizumab.
2.Observation of blood flow in verruca vulgaris using laser speckle contrast imaging before and after pulsed dye laser treatment

Objective To visualize blood flow changes in verruca vulgaris noninvasively with laser speckle contrast analysis (LSCA), and to evaluate the relationship of size and location of warts with efficacy of laser treatment. Methods Pulsed dye laser(PDL)was used to treat 30 verruca vulgaris lesions in 17 patients. In order to evaluate therapeutic effect of PDL, LSCA was performed to observe blood flow in warts and their surrounding normal skin, and to calculate speckle flow index (SFI)values before laser treatment, and at 10 minutes as well as on 3 weeks after laser treatment. Results Before PDL treatment, SFI values were significantly higher in warts than in their surrounding normal skin (11.600 ± 1.190 vs. 5.280 ± 0.481, t = 8.169, P < 0.01). Compared with those before the treatment, SFI values in warts significantly decreased at 10 minutes(3.112 ± 0.484, t = 4.407, P < 0.01)and on week 3(7.315 ± 1.083, t = 3.294, P < 0.01)after the treatment, and were significantly higher on week 3 than at 10 minutes (t = 4.646, P < 0.01). SFI values in surrounding normal skin significantly increased at 10 minutes after the treatment compared with those before the treatment (20.260 ± 2.063 vs. 5.296 ± 0.708, t = 6.770, P < 0.01), but were significantly lower on week 3 than at 10 minutes (4.941 ± 0.616, t = 6.964, P < 0.01). No significant difference was observed between SFI values in surrounding normal skin on week 3 after the treatment and those before the treatment (t = 0.378, P = 0.707). The efficacy of laser treatment was associated wart size and location. Changes of blood flow(|ΔSFI|)were significantly higher in warts measuring less than 0.5 cm2 in size than in those equal to or more than 0.5 cm2 (t = 2.287, P < 0.05), and significantly differed among warts at different sites (F =15.71, P < 0.01). The greatest changes of blood flow in warts were observed on fingers, followed by the dorsum of feet, toes, palms and soles and periungual areas. Conclusions Blood flow in verruca vulgaris is markedly increased compared with that in normal skin. PDL can clear verruca vulgaris by solidifying and gasifying capillaries. LSCA may be used to evaluate the efficacy of laser on verruca vulgaris more quantitively by monitoring regional blood flow.
3.A way for medical students to get out of the difficulty in finding a job

5.Determination of the content of berberine hydrochloride tablets by potassium dichromate colormetric method

OBJECTIVE To develop an assay for the determination of berberine hydrochloride in berberine hydrochloride tablets,and to compare the results measured by colormetric method with the assay described in Chinese Pharmacopoeia 1995 for assessing the correlation of the two methods.METHODS Potassium dichromate colormetric method was selected to determine the content, and the 0.2 mg*mL-1Potassium dichromate was used as the reference solution. The absorbency was measured at 421 nm.RESULTS The calibration curve for berberine hydrochloride was linear within the concentration range of 4.0~28.0 μg*mL-1(r=0.9999).The average recovery was 100.05%(n=5),and the RSD was 0.43%.The analytical results for berberine hydrochloride between the two methods was found to be correlated.CONCLUSION The method was proved to be simple, precise and reproduciable. It can be used for the quantitative determination of berberine hydrochloride. It is especially practicable for rapid analysis in production.
6.Effects of classical intrafascial supracervical hysterectomy on immune functions-A randomized controlled trial

Objective To study effects of classical intrafascial supracervical hysterectomy(CISH) on immune functions.Methods A total of 60 women with benign gynecopathy requiring a hysterectomy were randomly divided into two groups with 30 patients in each group,receiving either CISH or abdominal hysterectomy(AH).The white blood cell(WBC) count and neutrophil granulocyte percentage before and 1 day after the surgery,and peripheral T-lymphocyte subgroup(CD_3,CD_4,and CD_8),(interleukin-2)(IL-2),and interleukin-10(IL-10) 1 day before and 1 and 4 days after the surgery were measured,respectively. Results Postoperative levels of CD_3 and CD_4 in both groups decreased significantly at 1 day after operation(CISH Group: q_(CD3)=6.033,(q_CD4=4.763);AH Group: q_(CD3)=11.043,q_(CD4)=9.202;P0.05),and were significantly higher(q_(CD3)=6.531,q_(CD4)=5.269,P0.05) in the CISH Group.The IL-2 levels were significantly lower at 1 and 4 days after operation than before the operation in the AH Group((q_(1-2)=11.187),q_(1-3)=5.404,P
7.Gaseous signal molecule hydrogen sulfide and penile erection

Endogenous hydrogen sulfide is the third gaseous signal molecule after nitric oxide and carbon monoxide;it is synthesized through cystathionine-?-synthase and cystathionine-?-cleavage pathway.Hydrogen sulfide can directly react with the KATP channel,reduce extracellular Ca2+ influx,and work together with testosterone,nitric oxide,carbon monoxide,relaxing corpus cavernosum smooth muscle and improving diastolic function of blood vessels.It can also inhibit vascular smooth muscle cell proliferation,promote apoptosis,and improve vascular remodeling.Therefore,in-depth study of H2S in the regulating penile erection can provide a new way for the treatment of erectile dysfunction.
8.Study on Nosocomial Infection and Susceptibility of Related Pathogens to Antibiotics in Patients With the Systemic Lupus Erythematosus

Objective To study the percentage and the distribution of nosocomial infection, and susceptibility of related pathogens to antibiotics in patients with systemic lupus erythematosus (SLE). Methods The clinical data of in patients with SLE in our hospital from June, 1990 to June, 1998 were analyzed. The susceptibility of the pathogens to antimicrobial agents was tested by K-B paper dilution method. Results The percentage of nosocomial infection in these cases was 16.1% . The major pathogens were staphylococcas aureus, Escherichia coli and Candida. The most highly resistant microbes were E.coli, S.arueus and Pseudomonas aeruginosa. Conclusions The nosocomial infection is one of the most common complication of SLE in the in patients. It is necessary to examine the pathogens regularly and use antibacterial agents according to the susceptibility.
9.Alteration and significance of vascular endothelial growth factor in the serum of patients with glomerular diseases

Objective To study the concentration alteration of serum vascular endothelial growth factor (VEGF) in patients with primary and secondary glomerular diseases (GD), the effect of immune inhibitors on the concentration, and analyze the relationship between VEGF concentration and some clinical indexes. Methods Serum VEGF concentration of GD patients was determined by sandwich ABC-ELISA. Results All of the GD patients had higher level of serum VEGF than the control group. Of the 5 groups divided according to the clinical classification, patients of chronic glomerulonephritis(CGN) group, latent glomerulonephritis(LGN) group, Henoch-Schonlein purpura nephritis(HSPN) group and lupus nephritis(LN) group had higher serum VEGF level than the control group, respectively, with no difference among the above groups. The patients without taking immune inhibitors had higher serum VEGF level than the control group,while no difference was observed between the patients taking immune inhibitors and the control group.Patients with glomerulonephritis had a significantly lower serum VEGF level after receiving immunosuppression aggressive therapy than before. The serum VEGF level of nephrotic syndrome in nephrotic phase was higher than in remission stage. In membranous glomerulonephritis serum VEGF correlated positively with 24-hour proteinuria excretion. The serum VEGF level correlated positively with anti-dsDNA Ab titer and the concentration of ? globulins in LN. Conclusion Cytokine VEGF is involved in the pathogenesis of glomerulonephritis, and in the occurrence and progression of proteinuria. Immunosuppression therapy can inhibit the expression of VEGF. In LN patients, the serum VEGF level is related to LN activity.
10.Expression of Candida albicans Secreted Aspartyl Proteinase in Acute Vaginal Candidiasis

Objective To analyze the expression of secreted aspartyl proteinases (Sap) in human vaginal infection in vivo. Methods Vaginal swabs were collected from 9 healthy volunteers and 20 patients with vaginal candidiasis, and the expression of Sap1-6 was evaluated by reverse-transcriptase polymerase chain reaction using specific primer sets. Results The Sap2 and Sap5 were the most common genes expressed in the patients with vaginal candidiasis. The expression Sap3 and Sap4 was detected in all subjects. All six Sap genes were simultaneously expressed in some patients with vaginal candidiasis. Conclusion The data shows that the Sap genes may be involved in the pathogenesis of vaginal candidiasis.
