2.Research on the statusand influencing factors of compliance of inhalation therapy in patients with chronic airway diseases

Objective:To investigate the adherence of patients with chronic airway disease with inhalation therapy and to explore the influencing factors.Methods:A total of 180 outpatients with chronic airway disease were selected by convenient sampling. The general information, adherence of inhalation therapyquestionnaire, asthma knowledge questionnaire, chronic obstructive pulmonary disease (COPD) health literacy questionnaire (COPD-Q) were used to evaluate the general information, adherence of inhalation therapy and disease knowledge level of the patients. The severity of the disease was evaluated by asthma control test (ACT) score and COPD assessment test (CAT) score. ANOVA and t-test were used to analyze the adherence of patients with chronic airway disease, and multivariate linear regression was used to analyze the adherence. Results:The adherence score of asthma patients was 89.74 ± 7.27 and the adherence score of COPD patients was 86.80 ± 9.16, which were in the middle level. The risk factors of non-adherence of inhalation therapy were retirement, living alone and smoking. The effect of inhaled treatment time on the adherence of inhaled therapy is not linear, and the turning point of adherence decline occurs one year after the patients received inhaled therapy. The results of multiple linear regression showed that the effect of course and employment on the adherence of inhalation therapy was statistically significant.Conclusions:The adherence of chronic airway disease patients with inhalation therapy is not ideal, especially for the patients with long course, smoking, retirement and living alone, the medical staff should pay more attention, take appropriate intervention measures to improve the adherence of inhalation therapy.

4.Associated risk factors analysis of 232 cases of aortic dissection

Objective To analyze the risk factors associated with morbidity and mortality of aortic dissection through the retrospective analysis.Methods Two hundred and thirty-two patients with aortic dissection(AD) who were treated in the Central Hospital of Taian were select as cases group,and were divided into A group of dissection involved ascending aorta with 91 cases and B group of dissection not involved ascending aorta with 141 cases according to type of Stanford.And 232 cases patients with chest pain in the same period of hospitalization and exclusion of aortic dissection were randomly selected as the control group.Through the history data collection,the relationship between age,sex,smoking and drinking history,history of previous illness,family history of cardiovascular disease,predisposing factors and the incidence of AD,and the difference of risk factors between A type and B type were analized.Single factor and multi factor Logistic regression analysis were performed on part of the related factors in the case group.Results Smoking history,hypertension history,coronary atherosclerotic heart disease history,Marfan syndrome and hyperuricemia of cases group were higher than of control group (x2 =6.165,11.700,9.260,14.070,35.170;P< 0.05).Marfan syndrome,coronary atherosclerotic heart disease history and hyperlipidemia history of A group were higher than of B group,hypertension history of B group was higher than A group,and the differences were significant (x2 =3.998,4.534,7.308,7.827;P<0.05).In the correction of other factors,the risk of death in patients with type A was 3.27times that of the B type(P<0.001),the risk of death in patients with a history of hypertension were 1.48 times more than that without history of hypertension(P=0.014),the risk of death in patients with hypotension shock were 2.27 times more than that without hypotension shock (P < 0.001).Conclusion History of smoking,history of hypertension,history of coronary heart disease and hyperuricemia are the risk factors of aortic dissection.A aortic dissection type,a history of hypertension,and hypotension or cardiogenic shock are the independent risk factors causing the death of patients with aortic dissection.
5.Clinical study of sorafenib for patients with advanced renal cell carcinoma

Objective To evaluate the safety and efficacy of sorafenib for patients with advanced stage renal cell carcinoma.Methods The clinical data of 85 patients with advanced renal cell carcinoma were reviewed.These patients were treated by sorafenib 400 mg Bid,dose escalation of sorafenib(400 mg Bid 1-4 weeks;600 mg Bid 5-8 weeks;800 mg Bid since then) or sorafenib 400 mg Bid+NF-α,respectively,until intolerance or disease progression occurred.The primary end points were objective response,disease control rate and adverse effects rate.Results The data of 80 patients can be evaluated.The median follow-up duration was 72 weeks (4-108 weeks).One patient (1.2%) reached complete remission(CR),17 cases(21.2%) reached partial remission(PR),50 cases (62.5%) maintained stable disease (SD),and 12 cases (15%) progressed.The objective response (CR+PR) was 22.5%,disease control rate (CR+PR-SD)was 85.0%.By May 2009,only 18 patients died,progression free survival and overall survival were not available.The common side effects included hand-foot skin reaction (55.0%),mucosa hemorrhage (52.5%),diarrhea(40.0%),lassitude (35.0%),anorexia(22.5%),mucosa ulcer(20.0%),hypertension(15.0%) and baldness(15.0%)etc.Most of these side effects could be released by symptomatic treatment.Conclusion Sorafenib has good short term effect for patients with advanced renal cell carcinoma and is well tolerated.
6.Expression and clinical implications of RADIL gene in pancreatic cancer

Objective To investigate the RADIL mRNA expression in pancreatic carcinoma and to evaluate its clinical significance.Methods Fluoesecent quantitative PCR (FQ-PCR) was used to detect the RADIL mRNA expression in 40 patients with pancreatic carcinoma and adjacent tissue and in 5 healthy adult with normal pancreatic tissue and to observe its relationship with clinicopathologic parameters.Results RADIL mRNA was expressed in pancreatic carcinoma and adjacent tissue, as well as normal pancreatic tissue, and the relative expression was 2.263 ± 3.826, 5.425 ± 8.858 and 8.559 ± 4.214, respectively.There was statistically significant difference among the three groups (P <0.05 ).RADIL mRNA expression was closely related with the metastasis and differentiation grade ( r = -0.312 and -0.294, P < 0.05 ), however, it was not significantly related to tumor site, tumor size, CA19-9, TNM staging, sex and age.Conclusions RADIL gene may have an inhibitory effect on the pancreatic cancer.
7.Clinical study of sorafenib in the treatment of Chinese patients with metastatic renal cell carcinoma

Objective To evaluate the efficacy and safety of sorafenib in the treatment of Chi-nese patients with metastatic renal cell carcinoma. Methods This muhicenter phase Ⅱ clinical trial was performed from May 2006 to December 2006. Sixty-two patients with metastatic renal cell carci-noma not suitable for curative treatment were enrolled. All patients received oral sorafenib as single a-gent at the dose of 400 mg twice a day until disease progression or intolerable toxicities occurred. Re-salts Partial responses were recorded as best response in 11 patients, while complete remission was found in 1 patient and stable diseases were found in another 35 patients. According to the intents-to-treatment population, the overall response rate was 19.4% (12/62), and the disease control rate was 77.4%(48/62). The median progression free survival time was 9.6 months with 1-year progression-free survival rate of 41.9%. However, the median survival time had not reached due to the short fol-low-up. The most frequent adverse events included alopecia (66.1%), diarrhea (62.9%), hand-foot syndrome (58.1%), anorexia (40.3%), rash(37.1%), fatigue (37.1%), hypertension (35.5%), hoarseness(32.3%), joint pain (25.8%), hypophosphatemia (21.0%), fever (19.4%), nausea (19.4%), abnormal transeaminase( 11.3% ), elevated total bilirubicin( 16.1% ), leucopenia( 12.9% ), bleeding under nail(16.1%), and gum bleeding(11.3%). Grade 3 adverse events included hand-foot syndrome (16.1%), hypertension (12.9%), diarrhea(6.5%), hypophosphatemia (4.8%), joint pain (3.2%), and leucopenia(3.2%). Conclusions Sorafenib has prominent anti-tumor activity in Chi-nese metastatic renal cell cancer patients with most adverse events being grade 1 or 2. More attention should be paid to hypertension and cardio-cerebral vascular events during the application of sorafenib.
8.Epidemiological analysis of Hand-Foot-Mouth disease in Renqiu city from 2010 to 2012

Objective To analyze the condition and characteristics of hand-foot-mouth disease ( HFMD) from 2010 to 2012 in Renqiu city.Methods Surveillance and detecetion of HFMD was collected according to Renqiu city system for diseases control and prevention .The pathogen of HFMD severe case was deteceted .Results 12 293 cases including 735 severe cases were recorded in Renqiu city from 2010 to 2012,The highest of the resident population was in 2012 and the lowest one was in 2010(r=0.47,P<0.05).The total morbidity presented the obvious seasonal char-acteristic,which reached the summit in June ,July,August.The population morbidity was the clustered children .The average incidence rate of severe cases was 5.98%.The incidence rate in 2012 and 2011 was higher than that in 2010 (r=0.43,0.39,all P<0.05).There was significant difference of the pathogens types in severe cases among three years with the pathogen of CoxA 16 in 2010,2011 and humantero virus 71 viruses in 2012.Conclusion The inci-dence of HFMD presents the increasing and seasonal characteristics with the prevalence in the scattered children and the pathogens of CoxA16 in 2010,2011,humantero virus 71 in 2012.
9.Evaluation on Transdermal Absorption Properties and Efficacy of Huoxue Zhitong ;Cataplasm

Objective To study the effects of microemulsion/ethosomes on transdermal absorption properties and efficacy of Huoxue Zhitong Cataplasm. Methods The improved Franz diffusion cells were used for the in-vitro permeation experiment with rat skins as the barriers, which was used to evaluate the transdermal absorption properties. In the erxeriment, the contents of paeonol, eugenol and methyl salicylate were used as markers, and detected by ultra performance liquid chromatography to evaluate the transdermal absorption effects. The anti-inflammatory and analgesia activity were evaluated through the writhing plate experiments. Results The cumulative release rate of paeonol in Huoxue Zhitong Cataplasm, Microemulsion Huoxue Zhitong Cataplasm and Ethosomes Huoxue Zhitong Cataplasm were, in order, 65.30%, 61.30%and 60.20%in 24 h;eugenol were, in order, 51.08%, 54.71% and 55.66% in 24 h; methyl salicylate were, in order, 49.20%, 65.17% and 72.15% in 24 h. Furthermore, Microemulsion Huoxue Zhitong Cataplasm high-dose group and Ethosomes Huoxue Zhitong Cataplasm medium-dose group had good effects on reducing the inflammatory exudate of peritoneal capillary and capillary permeability (P<0.05) in animal models. Conclusion Huoxue Zhitong Cataplasm based on microemulsion/ethosomesnano-technology has good transdermal absorption properties and efficacy.
10.Paragonimus skrjabini Infection in Animal Reservoir Hosts and Questionnairing in Residents at a Village of Hubei Province

Freshwater crabs (Sinopotamon denticulatum) were examined for metacercariae. Cats and dogs were also examined for Paragonimus infection. Questionnairing was carried out on health knowledge and behaviors among local residents in a village of Baokang County, Hubei Province. Results showed that the infection rate of Paragonimus skrjabini metacercariae in Sinopotamon denticulatum was 20.5% (46/214), with 15.6% (20/128) in a mining area and 30.2%(26/86) for the non-mining area respectively ( ?2=6.5, P0.05). Questionnairing showed that dogs and cats were with the habit of foraging and defecating at streams and children had the habits of eating raw or under-cooked crabs. The natural and ecological environments are in favor of the life cycle of P. skrjabini.
