Objective To study the influence of postural intervention on ambulation of patients after video-assisted thoracoscope surgery? Methods One hundred and forty eight patients having undergone chest cardiac surgery were randomly divided into the control group(n = 75)and the observation group(n = 74)? The former received routine nursing intervention and the latter postural intervention besides routine nursing? The two groups were compared in terms of the time for ambulation and the ability in ambulation? Result The ambulation in the observation group was earlier than the control group and the ability in ambulation was better compared to the control group(both P < 0?05)? Conclusion The postural intervention after thoracic surgery is effective in pushing patient’s ambulation and improve their ability?

Objective To explore the methods for early detection of cognitive impairment in type 2 diabetic patients. Methods With the results from the mini-mental state examination (MMSE) ,48 patients of type 2 diabetes mellitus with cognitive impairment (group B) and 52 without cognitive impairment (group A) were enrolled. And 40 healthy subjects served as control. The clinical, laboratory, and imaging data of these subjects were compared. Results No differences in the prevalence of carotid atherosclerosis, serum HbA_1C, triglycerides, high-density lipoprotein-cholesterol, low-density lipoprotein-cholesteroi, body mass index, and waist-to-hip ratio were found between groups A and B. Atrophy of temporal lobe and hippocampus was more marked in group A than that in group B by MRI. The demyelinating changes were found both in group B (35%)and A (12%). Among 3 groups, group B had the longest latency of mismatch negativity (MMN) and P300 and the lowest amplitude of MMN ; the order was followed by group A and control group (all P<0.05). Logistic regression analysis showed that the findings in MRI, P300, MMN were negatively correlated with cognitive impairment in type 2 diabetes mellitus. Conclusion MRI, MMN, and P300 are helpful in detecting cognitive impairment in type 2 diabetic patients. MMN seems to be more sensitive in early detection of the impairment.

A direct current glow discharge mass spectrometry ( GD-MS) for the determination of Li, Be, Na, Mg, Si, P, K, Ca, Ti, V, Cr, Mn, Co, Ga, Sr, Mo, Ag, Cd, Sn, Sb, W and Pb in nickel-zinc ferrites was developed. Copper powder as the conductive medium was evenly mixed with nickel-zinc ferrites powder for tableting, and then the impurity elements in nickel-zinc ferrites were determined by GD-MS. The influence of sample preparation and the glow discharge conditions including discharge current, discharge voltage, discharge gas flow rate and pre-sputter time on discharge stability and analysis sensitivity were investigated. The mass spectrometry interferences of analytes were investigated, as well as appropriate isotopes were selected. Most of the mass spectrometry interferences were avoided by measuring in medium resolution mode and high resolution mode. Total signal of Fe, Ni, Zn, O and Cu normalization was adopted, and the impurity elements in nickel-zinc ferrites were calculate in subtraction. The results showed that the detection limits for the investigated elements were in the range of 0. 001-0. 29 μg/g, the relative standard deviations of each element were within 3 . 5%-18 . 6%. The analysis results were consistent with those obtained by inductively coupled plasma mass spectrometry.

Objective To explore the effect of health belief model on compliance behavior of patients with PICC(peripherally inserted central catheter). Methods According to admission order,75 cases of PICC were divided into control group(n=36)and observation group(n=39).Patients in the control group received conventional health education and regular visits after discharge and besides the regular methods, patients in the observation group were intervened by health belief model for the compliance behavior. All patients of two groups were investigated with self-designed scale for compliance behavior.Result The compliance behavior of the observation group was better than that of control group with significant difference(P<0.05).Conclusion The health belief model of education can improve the compliance behavior of PICC patients after discharge from hospital.

Objective To explore the effect of treating posterolateral tibial plateau fractures via the fibular osteotomy approach. Methods From August 2009 to August 2011,17 patients with posterolateral tibial plateau fractures, including 12 males and 5 females, aged from 24 to 76 years (average,37.8 years),were treated via the fibular osteotomy approach in our hospital.According to the Schatzker classification,8 cases were type Ⅱ,3 cases were type Ⅲ,6 cases were type V.After operation,X-rays were taken in all patients, and Rasmussen's radiological and functional gradings were used to evaluate radiological and functional outcomes of knees. Results All patients obtained follow-up,ranged from 9 to 35 months (average,18months).The healing time of the fracture ranged from 10.0 to 18.0 weeks (average,13.5 weeks).During the period of followed-up,there was no loss of reduction; one case presented with symptoms of common peroneal nerve injury, such as local hypesthesia in distal lateral lower leg and dorsi pedis, which recovered two weeks postoperatively.According to Rasmussen's radiological grading, the mean score of the knee joint was 17.5(range,14.0 to 18.0).The range of motion of the knee joint ranged from -5°to 135°(average,123.5o).According to Rasmussen's functional grading, the mean score of the knee joint was 26.9 (range,22 to 30).Conclusion Treating posterolateral tibial plateau fractures via the fibular osteotomy approach can obtain sufficient exposure, good reduction and fixation, and avoid flexion contracture of the knee and peripheral vascular nerve injury. Moreover, postoperative function and stability of the knee joint recover well.
