An 18-year-old woman was admitted to the hospital for swelling of the right lower limb and vulva with milky-white vesicles for 3 years.Skin examination revealed swollen and enlarged right lower limb and labium,numerous clustered or scattered pinhead-to millet-sized whitish thick-walled vesicles over the right labium majus,bilateral labium minus,vaginal orifice,and right thigh,with milky-white chylous fluid draining from the vagina.Magnetic resonance imaging revealed obvious lymphangiectasia in the lower abdominal cavity and right side of the pelvis,dilation and distortion of lymphatic vessels in the skin and subcutaneous tissue of the right inner thigh,the vaginal wall and at the medial side of the right labium majus,but no abnormality in the uterus.Histopathological examination of the milky-white vesicles showed cystic dilation of lymphatic vessels in the superficial dermis,and fibrous hyperplasia of the middle and lower dermis.A diagnosis of primary chylous reflux syndrome Ⅰ was made.

Objective To explore the musculoskeletal MR findings in dermatomyositis and to evaluate the role of MR in diagnosis of dermatomyositis. Methods Twenty patients with early dermatomyositis and 25 patients with active disease confirmed by pathology underwent T 1-weighted (SE 450/20), T 2-weighted (FSE 4000/100), and fat-suppressed MR images. MR findings in these 45 patients were analyzed. Results Within all the myositis, MR showed symmetrical slight high patchy signals on T 2-weighted and STIR images in the muscles of bilateral thighs, but the morphology of muscles appeared normal in 26 patients, while limited high signals only appeared on STIR image in 19 patients. There was correlation between serum muscle enzymes and the signal intensity as well as affected field of myositis. The myositis often affected gluteus and the external lateral muscles in the lower part of thighs. Inflammatory changes in the subcutaneous fat in 18 patients with active dermatomyositis were the severe appearance of the disease. Inflammatory fascia in 17 patients was found in both early and active cases. 5 patients had skin damage. Conclusion MR features of dermatomyositis are multiple myositis, fascia and subcutaneous inflammation, with the latter being an appearance of severe period of the disease. Early affected location of dermatomyositis is gluteus and the external lateral muscles in the lower part of thighs. MR is an efficient examination and STIR is a sensitive sequence in the diagnosis of dermatomyositis.

Objective To assess pathological features of muscles as well as microvascular changes between muscle fibers in patients with dermatomyositis (DM),and to analyze pathological differences in muscles between DM and polymyositis (PM).Methods Specimens were obtained from involved muscles of 16 patients with DM,5 patients with PM,and from normal muscles of 9 patients with bone trauma (controls).Routine histopathological examination and immunohistochemical staining for CD34 and CD61 were conducted.Results Of the 16 patients with DM,6 (37.5％) had perifascicular atrophy,3 ( 18.8 ％) had an obvious inflammatory cell infiltration around microvessels between muscle fibers.Perifascicular atrophy was absent in muscle specimens from patients with PM or bone trauma.The number of CD34-positive microvessels between muscle fibers was reduced in patients with DM,but normal in those with PM.CD61 was positive in perifascicular area of 10 patients (5 moderately positive and 5 weakly positive) with DM,with an expression rate of 62.5％,however,only 1 case of PM was weakly positive for CD61,and all the controls were negative.Conclusions There is a decrease in the number of microvessels but an enhancement of neovascularization between muscle fibers in involved muscles of patients with DM,which may serve as a pathological marker to distinguish DM from PM.

ObjectiveTo investigate the localization G protein couple receptor 30 (GPR30) and its mRNA in submaxillary gland, and to supply theoretic evidence for further studying functional significance of the GPR30 in submaxillary gland of rats. Methods Four male SD rats were sacrificed by cervical dislocation after the intraperitoneal anesthesia, and excised the submaxillary glands. The distribution of GPR30 and its mRNA were studied through immunohistochemistry and in situ hybridization in the experiment. After isolation of the total RNA from the submaxillary gland, RT-PCR was conducted to obtain GPR30 cDNA by using the specific primers. The products of PCR were analyzed by sequencing with Sanger's method. Results The serous acinus epithelial cells and granular convoluted epithelial cells in submaxillary gland of rats showed GPR30 immunoreactivity, which were located in cytoplasm with negative nuclei. GPR30 mRNA hybridized signals were also detected in cytoplasm in the above cells. The products of PCR is identical to that of the GPR30 sequence of rats. Conclusion The serous acinus and granular convoluted epithelial cells not only express GPR30 but also may be a target organ by rapid estrogen signaling pathway in submaxillary gland of rats. This may be involved in the functional regulation of submaxillary gland.

Objective To enrich the knowledge in large gastric folds. Methods We investigated 45 cases of large gastric folds in ten years (1990 - 2001) retrospectively. All cases were examined endo-scopically and histologically. Some of them were examined by endoscopic ultrasonography and upper gastrointestinal barium. Results Of 45 cases with large gastric folds there were 30 cases with gastritis (66. 7% ) , seven with malignant tumors( 15. 6% ) (five with gastric carcinoma and two with malignant lymphoma) , four with Menetrier disease(8. 9% ) , two with systemic lupus erythematosus, one each with Billroth II anastomosis and gastric ulcer. The diseases including tumors, Menetrier disease and connective tissue diseases were located in gastric fundus and/or body . Conclusion The diseases located in upper stomach have more clinical significance. All of endoscopic ultrasonography, upper gastrointestinal barium and endoscopic mucosal resection are helpful in differential diagnosis of the large gastric folds.

Objective To evaluate the therapeutic effect of traditional Chinese medicine and western medicine(TCM-WM) for the treatment of senile femoral neck fracture(SFNF) during perioperative period.Methods A retrospective analysis was carried out in 59 patients.Perioperative clinical data of SFNF patients admitted in orthopedics department of the second affiliated hospital,Guangzhou University of TCM were collected and analyzed.Postoperative complications,chief complaints,joint function and X-ray examination results were used to evaluate the effect of perioperative treatment.Results The patients received AO cannulated screw fixation,artificial femoral head replacement and total artificial hip joint replacement according to the classification of SFNF.Before theoperation,55 patients suffered from internal medical complications,in particular the cardiovascular diseases.After operation,one patient had dislocation of artificial hip joint and then died of deterioration of heart failure.Neither of the other patients suffered from infection of operative wound,vascular embolism of lower limbs,cerebrovascular accident,myocardial infarction,or pulmonary embolism.The 58 patients finally discharged successfully.Conclusion Active perioperative evaluation and treatment are the important measures to ensure the success of operation for the SFNF.

Based on melamine-Cu conjugate and subsequent interruption of formation of polythymine (Poly T)-templated copper nanoclusters (CuNCs), a novel fluorescent strategy was developed for detection of melamine.The method relied on the principle that the coordination of melamine to copper would be unfavorable for the reduction of Cu2+ in the poly T-CuNCs synthesis process, and thereby resulting in the decrease of fluorescence intensity of CuNCs.By this method, the fluorescence response of CuNCs displayed an linear relationship with melamine concentration in the range from 5 μmol/L to 120 μmol/L.The detection limit was 1.5 μmol/L.Furthermore, the assay was successfully applied in the detection of melamine in milk samples with good recoveries.

ObjectiveTo study the mechanisms and related factors and preventive managements of the stress ulcer complicating with acute cervical spinal cord injury. MethodsThe clinical findings of eight cases of the acute cervical spinal cord injury with stress ulcer were reviewed retrospectively.ResultsFive cases were cured by conservative treatment, one case was cured after operations, two cases died of breath and circulation failure.ConclusionStress ulcer is a critical complication of acute cervical spinal cord injury. It should be prevented actively.

Objective:To study the best opportunity for minimal invasive puncture in the treatment of hypertensive intracerebral hemorrhage.Methods:Forty-one patients with hypertensive intracerebral hemorrhage were randomly divided into four groups:<6 h,6-12 h,12-18 h and 18-24 h from the onset of symptoms.The comparison among groups in outcome measures were performed,including the rebleeding rate,short-term efficacy,long-term efficacy,mortality,and excellent and good rate.Results:The incidence of complication such as rebleeding etc had no significantly difference in the four groups.The excellent and good rates of clinical outcomes at 3 months were the<6 h group>6-12 h group,and 12-18 h group>18-24 h group(P<0.05).The 18-24 h group had the highest mortality(30%,3/10).Conclusions:Within 6 h after the onset of hypertensive intracerebral hemorrhage was the best opportunity for the minimal invasive puncture.
