2.Clinical value of combined detection of serum human epididymal secretory protein E4 and CA125 in the diagnosis of endometrial carcinoma

Objective To explore the clinical value of combined detection of serum human epididymal secretory protein E4 (HF4) and CA125 in the diagnosis of endometrial carcinoma.Methods From Jan 2010 to Apr 2011,the serum specimens were collected from124 cases of endometrial carcinoma,97 cases of benign disease of uterus and 109 cases of healthy women.HE4 levels in the serum were detected by ELISA,and CA125 levels in the serum were detected by the electro- ehemiluminescent immunoassay.Those results were shown with median level.Accuracy of the diagnosis was evaluated by the area under the receiver operating characteristic curve ( ROC-AUC ).ResultsThe median levels of HE4 and CA125 were 78.09 pmol/L and 33.43 kU/L in serum of endometrial carcinoma group.The median levels of HE4 and CA125 were 46.37 pmol/L and 18.26 kU/L in serum of benign disease of uterus group.The median levels of HE4 and CA125 were 31.75 pmol/L and 12.64 kU/L in serum of healthy women group.The HE4 and CA125 levels in serum of endometrial carcinoma group were significantly higher than those of benign disease of uterus group or healthy women group ( all P < 0.05 ).Compared with that benign disease of uterus group,the ROC-AUC of HE4 and CA125 in endometrial carcinoma group were 0.913 and 0.801,respectively.When the specificity was 95.0%,the sensitivities of HE4,CA125,and combined detection of HF4 and CA125 in endometrial carcinoma group were 41.1%,22.6% and 46.0%,respectively.The positive rates of HE4 and CA125 were 31% (27/86) and 12% (10/86) in stage Ⅰ - Ⅱ of endometrial carcinoma,while the positive rates were 63% (24/38) and 47% (18/38) in stage Ⅲ -Ⅳ of endometrial carcinoma,in which there were significant difference between patients in stage Ⅲ - Ⅳ and stage Ⅰ - Ⅱ (P < 0.01 ).ConclusionsThe combined detection of serum HE4 and CA125 is helpful to the diagnosis of endometrial carcinoma.The sensitivity,specificity and early diagnosis of HE4 are better than that of CA125.The positive rates of HF4 and CA125 in endometrial carcinoma are related to the clinical staging.
3.Clinical value of risk of ovarian malignancy algorithm in risk stratification of ovarian cancer

The serum specimens were collected from ovarian cancer (n =100),benign ovarian disease (n =69) and healthy women (n =95).The serum levels of HE4 and CA125 were detected.Risk of ovarian maliqnancy algorithm (ROMA) was calculated.Accuracy of prediction was evaluated by the area under receiver operating characteristic curve (ROC-AUC).And validity of prediction was evaluated by sensitivity and specificity.The results showed that the median level of ROMA algorithm was 83.0%,8.9% and 8.7% in ovarian cancer,benign ovarian disease and healthy women groups respectively.The difference were statistically significant (all P < 0.01).Compared with benign ovarian disease group,the ROC-AUC of ROMA algorithm was 0.900 in ovarian cancer group.The sensitivity and specificity were 81.0% and 92.8% in ovarian cancer group respectively.Thus ROMA algorithm is a useful parameter in risk stratification for ovarian cancer.The diagnostic accuracy of ROMA algorithm is better than that of HE4 and CA125 in ovarian cancer.
4.Relationship of peritoneal transport patterns and the nutritional status in continuous ambulatory peritoneal dialysis patients

Objective To explore the relationship of peritoneal transport patterns and the nutritional status in continuous ambu-latory peritoneal dialysis(CAPD) patients .Methods 89 patients who took CAPD in 2010 January to December 2012 in the perito-neal dialysis center of our hospital were selected in this study .According to the ratio of creatinine in peritoneal dialysis solution and in blood(D/Pcr)of the peritoneal equilibrium test(PET) ,all CAPD patients were divided into HA group(high average transport) and LA group(low average transport) .Nutritional status including serum albumin(ALB) ,prealbumin(PA) ,transferrin(TF) ,hemo-globin(Hb) ,Lean body mass(LBM) ,subjective global assessment(SGA)and normalized Protein Equivalent of Nitrogen Appearance (nPNA) were compared in both groups .The relationship of D/Pcr and the nutritional status were analyzed .Results D/Pcr in HA group were significantly higher than in LA group(P<0 .01) .ALB ,PA ,TF ,Hb and SGA in HA group were significantly higher than in LA group(P<0 .01) ,but nPNA in two groups has no statistical differences (P>0 .05) .D/Pcr was negative related to ALB , TF ,Hb and SGA(P<0 .01) ,but not related to nPNA ,PA and LBM (P>0 .05) .Conclusion The nutritional status is related to peritoneal transport patterns in peritoneal HA patients ,it is necessary to strengthen nutritional guidance ,as the nutritional status is worse compared to LA ones .
5.Analysis of the proliferation ability and the antineoplastic activity of CIK cells against original hepatocellullar cancer cells between healthy adults and tumor patients

Objective To explore the differences of the proliferation ability and the antineoplastic activity of CIK cells against origenal hepatocellullar cancer cells between healthy adults and tumor patients in vitro. Methods Peripheral blood mononuclear cells (PBMC) from healthy donors and tumor patients were incubated to induce CIK cells in the presence of interferon gamroa(IFN-γ) ,IL-2 and anti-CD3 monoclonal antibody (mAb). The changes in the proliferation activity and phenotypes of the CIK cells were identified by flow cytometric analysis. Single cell suspension was prepared bom the fresh hepatocellular carcinoma tissue by using mechanical trituration method. MTT assays were used to determine the cytotoxicity of CIK cells against origenal hepatocellullar cancer cells. Results The CIK cells from both healthy donors and tumor patients were significantly increased with the extension of time. The expression rate of CD3+/CD56+ cells from healthy donors rose from 1.053% ±0.22% on 1st day ,25.36% ±2.19% on the 7th day to 55.12% ±1.99% on the 14th day ,which was significantly higher than that of tumor patients ( P < 0.05). MTT assays showed that the cytotoxicity of CIK cells from both enhanced obviously with the addition of Effect/Target rate and extension of time(P <0.05). After the origenal hepatocellullar cancer cells were treated by CIK cells 24 hours, the cytotoxicity of CIK cells from healthy donors at the effector:target ratio of 10 ∶ 1,20 ∶1 and 40 ∶1 was significantly higher than that of tumor patients(P<0.05) ,respectively. After treated 48 hours, compared with tumor patients, the cytotoxicity of healthy donors? CIK at the three effector:target ratio was alsosignificantly higher(P <0.05), respectively. Conclusion CIK cells from both have amplification ability and cytotoxic activity in vitro,and the proliferation ability and killing activity of healthy adults CIK is stronger than that of tumor patients,which provides an experimental basis for CIK to clinical application as an adoptive immunotherapy.
6.Development of Medical Box for Debriding and Change Dressings in Sanitary Train with Application Module Theory

Objective To establish a kind of medical box for wound debriding and change dressing in sanitary train,which could be used to satisfy the requirements of natural and manmade disasters for medical service.Methods With tri-grade modularization theory,the medical box were divided into four functional sections,including disinfection cleaning solution section,debriding and change dressings common section,contaminated wound change dressings section and infected wound change dressing section.Through the four sections,the medical box could be provided 5 wound debirding and 50 change dressings.Results The medical box can satisfy all requirements for wound treatment with useful and convenient function.Conclusions The invented medical urgent box has so many advantages of modularization,complete functions and expansionary,which could satisfy all requirements for sanitary train.

The individual variability should be considered in precision medicine-prevention and treatment strategies.Medical research using genomics, proteomics, metabolomics, systems analyses, and other modern tools has made big progress.In 2002, the members of the Complex-Trait Consortium proposed to develop a new mouse genetics resource called the Collaborative Cross (CC).The CC is a genetic reference panel of recombinant inbred lines of mice, designed for the dissection of complex traits and gene networks.It will provide a powerful measure for functional studies of biological networks, which will be essential to understand the intricacies of disease processes.
8.Immunophonotype of acute myeloid leukemia and mixed leukemia detected by flow cytometry

Objective To detect by flow cytometry (FCM) acute myeloid leukemia. Methods Fourcolor staining was used for analysis of immunphonotypes of 52 cases with acute myeloid leukemia and 7 cases of mixed leukaemia. Results 52 cases with series of patients with AML expression, with the main expression CD13 (94.2 %), CD117 (90.4 %), cMPO (90.4 %) CD33 (86.5 %), CD34 (57.7 %), HLA-DR (53.8 %). In 7 cases of mixed cell leukemia 3 cases of myeloid/B (M/B), are expressing CD13, CD34, CD117, CD10, CD19, cMPO, cCD79a.Myeloid/T (M/T) 2 cases with expression of CD13, CD117, CD33, CD34, CD5,CD7, cMPO; B department/T (B/T) 2cases, with expression of CD5, CD7, CD10, CD19, CD20, CD34. And with the morphological and organizations with high accuracy of chemical diagnosis. Conclusion FCM can improve the diagnose rate for AML and mixed leukemia. And has played an important clinical significance.
9.Immunophenotype of double acute leukemia detected by flow cytometry

Objective To detect double immuno-phenotype of acute leukemia by the application of flow cytometry (FCM). Methods Four-color FCM was used to analyse 23 cases of acute leukemia of double immune phenotype. Results Among 23 cases, cCD3 was expressed in 10 cases (43.4 %), cCD79a in 16 cases (69.6 %), cMPO in 20 cases (87.0 %), TdT in 14 cases (60.9 %), CD34 in 19 cases (82.6 %) and CD117 in 20 cases (87.0 %). 13 cases (56.5 %) expressed both myeloid and B lymphocyte antigens, in which cCD79a and cMPO were positively expressed, 7 cases (30.4 %) were found to express both myeloid and T lymphocyte antigens of cCD3 and cMPO 3 cases (13.04 %) were positive for both cCD3 and cCD79a. Conclusion cCD3,cCD79a, and cMPO were lineage specific antigen markers, which could be used for diagnosis and differential double phenotype leukemias. FCM is a useful and reliable method for diagnosis of acute leukemia, and guidance of treatment and prognosis of leukemia in clinical.
10.Comparison of clinical features between acute disseminated encephalomyelitis and classical multiple sclerosis

Objective To improve differential diagnosis between acute disseminated encephalomyelitis ( ADEM) and classical multiple sclerosis ( CMS).Methods All 20 cases of ADEM and 24 cases of CMS were examined.Their epidemiological and clinical findings,laboratory features and magnetic resonance imaging ( MRI) data were analyzed using x2 test for categorical variables,Wilcoxon Rank-Sum tests for continuous variables.Results ADEM and CMS showed no sex predominance.Patients with ADEM ((27 ±15) years) were younger than CMS ((37 ±13) years,Z= -2.218,P =0.027).The following findings were more commonly seen in ADEM compared with CMS:predemyelinating infectious disease (75% vs 4%,x2 =23.652,P = 0.000),fever (65% vs 4%,x2 =18.609,P = 0.000),meningeal irritation sign (40% vs 0,x2 = 9.189,P =0.002),seizure (25% vs 0,x2 =4.514,P = 0.034),and encephalopathy.ADEM patients were more likely to present with blood leucocytosis ( (11.9 ± 5.8) ×109/L vs (8.0±3.2) ×109/L,Z= -2.030,P=0.042),high C-reactive protein (2.74 mg/L vs 0.49 mg/L,Z = - 3.028,P = 0.002),increased erythrocyte sedimentation rate (11.00 mm/h vs 7.00 mm/h,Z= -2.406,P =0.016),and cerebrospinal fluid leucocytosis (9 × 106/L vs 2×106/L,Z =- 2.781,P = 0.005).There were no differences in cerebrospinal fluid protein and oligoclonal band between the two groups.The following MRI lesions were more commonly seen in ADEM patients:cortical gray matter lesions (14/20,x2=15.213,P=0.000),basal ganglia gray matter lesions (14/20,x2 =8.910,P = 0.003),and brainstem lesions ( 14/20,x2 = 5.867,P = 0.015).In contrast,lesions in subcortical white matter (21/24,x2 = 17.628,P =0.000),periventricular area (21/24,x2 =15.213,P=0.000) and corpus callosum ( 14/24,x2 = 8.640,P = 0.003 ) were more common in the MRI image of CMS patients.The lesions in spinal cord were usually centrally distributed in ADEM (83% ),while peripherally in CMS (85%,x2 = 11.542,P = 0.001).The lesions had poorly defined margins in ADEM (95%),but well defined margins in CMS (75%,x2 =21.787,P = 0.000).Conclusion There are differences in epidemiological and clinical findings,laboratory features and MRI appearances between ADEM and CMS.
