1.The effect of vascular pediceled peripheral nerve tissue grafting in repair spinal cord injury

Objective:To observe the repairment effects of vascular pediceled nervous tissue on adult rat's spinal cord injury. Method:A 5mm cord defect of the left lateral column was made at Ti-3 vertebral level,and then the defect was grafted randomly using vascdarized peripheral nerve (Group VPN),embryonic day 14 fetal spinal cord(Group FSC) or vascular pediceled peripheral nerve and fetal spinal cord(Group V + F).8 weeks after surgery, the survival,differentiation of the grafts, and the ability of repaired host spinal cord were assessed. Result: In VPN-FSC co-graft group, peripheral nerve segment was extensively vascularized and well innervated,plenty of Schwann cells survived and proliferated. In this group, anti-NF, S-100 positives fibers were significantly greater than group VPN (P<0.01). The fetal implant was intimately fused with the host CNS parenchyma, the volume, nerve fibers and neurons density of the fetal implant were significantly higher than Group FSC(P<0.01). The majority of neurons in this group were well differentiated, well-developed and synapses were scattered throughout the neuropil. SEP assessments showed that the latgncy of P1 ,N1 wave was significantly decreased in this group (P<0.01).Conclusion:The vascular pediceled peripheral nerve could prompt the development of fetal implant and support the regeneration of central injured axons to certain degrees.
2.Effect of Pulsed Electromagnetic Fields Stimulation on the Repair of Peripheral Nerve

Aim: To study the effect of pulsed electromagnetic fields(PEMF) stimulation on the repair of rats grafted sciatic nerve. Methods: One hundred Wistar rats were classified randomly into two groups, a control group (group A, n = 50) and a PEMF group(group B, n = 50). The biological effect of the grafted nerve, the relative lumbar spinal cord and one of the ending organs - triceps muscle was studied at the same time. Results: The neuron's vitality in lumber spinal cord had been stimulated in the PEMF- treated rats. The Wallerian degeneration of the nerve distal to the grafted was accelerated after PEMF stimulation. The increased regeneration of the nerve was found throughout 16 weeks recovery period in PEMF group. Application of PEMF had decreased the atrophy of the triceps muscle caused by denervation and promoted its functional recovery. Conclusion: PEMF stimula- tion may enhance the regeneration of the injured peripheral nerve.
3.Medication Monitoring of Anti-infection Drugs in a Patient with Lupus Nephritis Complicated with Com-munity Acquired Pneumonia

Objective:To explore the value of clinical pharmacists in the anti-infection practice for a patient with lupus nephritis complicated with community-acquired pneumonia(severe). Methods:Clinical pharmacists carried out medication monitoring in the entire treatment process of a patient with lupus nephritis complicated with community-acquired pneumonia( severe)in the clinical front line. Depending on the pharmaceutical knowledge,clinical pharmacists assisted physicians to make out the optimal medication and ad-justed the medication according to the patient′s conditions. Results:Moxifloxacin and piperacillin sodium /tazobactam sodium could ef-fectively control the severe pulmonary infection of the patient with lupus nephritis and avoid the potential risk of recurrence or aggrava-tion of lupus nephritis. Conclusion:The choice of antimicrobial agents for the special populations should consider not only the antibac-terial spectrum and antibacterial activity,but also the individual characteristics of patients and pharmacokinetics of drugs in order to im-prove the safety and effectiveness of medication.
4.Effect of neck rotation and lateral bending on the blood flow of the vertebral arteries

Objective To study the effect of the neck rotation and lateral bending on the vertebral artery blood flow, and to analyze the relationship between the artery blood flow and cervical vertigo. Methods Thirty patients with cervical vertigo were studied with exclusion of auditory vertigo and severe cardio-cerebrovascular disease, which involved 18 males and 12 females, whose average age was 52.13 years with a range of 42-73 years. They all had cervical degenerative change in different degree and neck-shoulder-back pain history. The study also included 15 volunteers without vertigo, 8 males and 7 females, the average age was 49.47 years with range, 38-70 years, they also had some cervical degenerative change. They were selected to detect the blood flow of bilateral intracranial segments of the vertebral arteries using TCD with the neck in neutral, bilateral rotation 15?,30?,45?,60? and bilateral bending. 6.O-POSSUM and P-POSSUM for evaluation of operative risk of aged patients in orthopedic department

Objective To retrospectively review the mortality and morbidity after orthopedic surgery for aged patients with reference to orthopedic version physiological and operative severity score for the enumeration of mortality and morbidity (O-POSSUM) and Portsmouth POSSUM (P-POSSUM) and validate the score' s accuracy in operative risk prediction.Methods O-POSSUM and P-POSSUM for mortality and morbidity were applied to 839 aged patients who had undergone orthopedic surgery between January 2008 and December 2011.Observed mortality and morbidity were compared with the expected values.Results O-POSSUM predicted 230 morbidities,but the observed value was 182 (x2 =2.718,P > 0.05).O-POSSUM predicted 46 deaths,higher than the observed 3 deaths (x2 =38.870,P<0.01).P-POSSUM predicted 16 deaths,higher than the observed 3 deaths (x2 =8.997,P < 0.01).Conclusions O-POSSUM accurately predicts postoperative morbidity in aged orthopedic patients.However,O-POSSUM and P-POSSUM overpredicts the mortality and should be used with caution.
7.The Specific Ligand Screening for M_2-G_(i1α) Fusion Protein Expressed in Sf9 Cells

Objective Using M2-Gi1α fusion protein expressed by baculovirus-Sf9 cell system to find the specific ligand for M2 receptor and detect the interaction of the two parts of the fusion protein.Methods The fused M2-Gi1α cDNAs were generated in a two-step PCR and then expressed in Sf9 cells.[3H]QNB and[35S]GTPγS binding experiments were employed to study the function of M2-Gi1α fusion protein.Results The expression level of M2-Gi1α fusion protein was 8.44±0.39 nmol·g-1 protein.The affinity of GDP to the Gi1α part changed under the affection of different ligands.The IC50 value in the appearance of acetylcholine,oxotremorine,arecoline,atropine,fangchinoline,levitimide were 21.35 μmol·L-1,23.86 μmol·L-1,11.91 μmol·L-1,0.13 μmol·L-1,1.05 μmol·L-1,1.75 μmol·L-1,and 2.5 μmol·L-1 when there was no ligand.Conclusion The M2-Gi1α fusion protein expressed in baculovirus-Sf9 cell system has pharmacological specificity for M2 receptor and the efficient coupling function between the two parts.The M2-Gi1αfusion protein is a helpful tool for detecting the new specific ligands of the M2 receptor.
8.Protective effects and mechanism of insulin on impairment induced by glutamine in PC12 cells

BACKGROUND: More and more researches prove that cell apoptosis could be induced by glutamine, also there are more researches on studying the indirect and direct nervous-protective effects of insulin, but the nervous-protective effects of insulin on impairment induced by glutamine, as well as its mechanism still need further investigation.OBJECTIVE: To investigate the nervous-protective effects of insulin on impairment induced by glutamine in PC12 cells, and to explore its molecular mechanism.DESIGN: A prospective controlled study based on cells.SETTING: Department of Neurology, Zhejiang Hospital; Department of Neurology of Sun Yat-wen University Hospital.MATERIALS: The study was carried out at the Laboratory of the Third Affiliated Hospital and the Experimental Animal Center of Sun Yat-sen University from March 2002 to March 2003. PC12 cells were purchased from the same animal center.METHODS: Traumatic models were made in PC 12 cells by treated with 0.5 mmol/L glutamine for 20 minutes, and the insulin of different concentration were used for protection, after 24 hours, protective effects of insulin were assessed with MTT method, Hoechst33258 fluorescence staining, DNA agar gelatin electrophoresis, meanwhile the expression of PKB/Akt protein were also detected./Akt protein in experimental group.RESULTS: The A value of50 mU/L, 100 mU/L, 200 mU/L, 400 mU/L insulin groups were 0. 214 ±0. 062, 0. 234 ±0. 067, 0. 260 ±0. 076 and 0. 265 ± 0. 069, respectively, but the value of single glutamine group was 0. 201 ± 0. 079, statistical analysis indicated that compared with single glutamine group, there were no significant difference in 50 mU/L, 100 mU/L insulin groups( P > 0.05), but 200 mU/L, 400 mU/L insulin groups were found statistically different from single glutamine group(t=-2.398,-2. 716, P < 0.05); "DNA Ladder" could not be observed in 400 mU/L insulin group by electrophoresis;It was proved that Insulin could enhance the expression of PKB/Akt protein.CONCLUSION: Insulin has nervous-protective effects on impairment induced by glutamine in PC12 cells, furthermore it also has property of anti-apoptosis, and its protective mechanism might be associated with enhancement of the expression of PKB/Akt protein.
9.Establishment of an animal model of sacral nerve root avulsion in rats

Objective To establish an animal model of sacral nerve root avulsion in rats and e-valuate its efficiency. Methods A total of 20 adult SD rats (either sex) were chosen at random to es-tablish the sacral nerve root avulsion model by avulsing the fight L4-6 nerve roots out of intervertebral fo-ramina without laminectomy. The left side was set as control group. The models were evaluated in aspects of survival rate, Basso-Beattie-Bresnahan (BBB) scores, somatosensory evoked potential (SEP), horse radish peroxidase (HRP) tracing, bilateral weight and cross section area (CSA) of muscle biceps femo-ris, fiber of triceps surae and anterior tibial muscle. Results Of all, 19 rats were survived but one died, with survival rate of 95.0%. The BBB score was (10.78+3.15) points in experimental group and 21 points in control group. The success rate of establishing animal model was 89.5% ,for there detected no SEP in bilateral cerebral cortex of the wounded extremity of 17 rats. HRP showed positive reaction in the spinal segment of L4-6 in two rats but negative in 17, with success rate of 89.5%. There was statistical difference in aspects of weight and muscle fiber CSA of double biceps femoris, triceps surae and anterior fibial muscle between experimental group and control group. Electron microscope found denervation chan-ges including muscle fiber atrophy, nucleus shifting to (center and muscle satellite cell. Conclusion A-vulsion of L4-6 sacral nerve root out of vertebral canal is a feasible and ideal method to establish the avui-sion model of sacral nerve root injury in rats.
10.Selection of ideal tidal volume according to pressure-volume curve for instruction of mechanical ventilation nursing

Objective To study the influence of the different tidal volume on hemodynamics,pulmonary ventilation and pulmonary mechanics during mechanical ventilation in patients with acute lung injury.Methods To adopt contrast method,making use of the VIIP +0.196 kPa of P-V curve to decide PEEP,then according to the VUIP of P-V curve,obtaining the three different data of humidity 100%Vt,85% Vt and 70 Vt.At the same time,monitoring the change of static compliance,the changing of hemodynamics and P-V curve.Results 85%Vt group was better than 100%Vt group and 70%Vt group in the heart rates,CVP,PaO_2,the peak airway pressure(PIP) and mean pressure(Pm),Cst,PaO_2/FiO_2.Conclusions When insuring the PEEP data in low inflection point pressure (Pinf) of breathing system P-V curve,and 85% humidity in upper inflection point pressure,which is in accordance with individual protecting breathing strategy.The curative effect is the best for improving static compliance and decreasing the harm of lung injury.The change of pressure-volume curve should be paid attention to in nursing.
