1.Laparoscopic cholecystectomy for stones straddling between the cystic duct and the common bile duct: Report of 19 cases

Objective To investigate the surgical techniques of laparoscopic procedures in the management of stones straddling between the cystic duct and the common bile duct. Methods Clinical data of 19 cases of stones straddling between the cystic duct and the common bile duct treated by laparoscopic cholecystectomy (LC) from January 2001 to June 2003 were retrospectively reviewed. Results The operations were completed by opening the gallbladder ampulla and/or the cystic duct in all the patients. The stones were 0 4~0 6 cm in diameter (mean,0 5 cm).The drainage was removed 48 hours ofter the surgery.The patients were discharged from hospital 3~5 days postoperatively.Follow-up checkups in the 19 cases for 2~24 months (mean,10 months) found no residual stones. Conclusions Thorough exposure of the union of the cystic duct with the common bile duct and the opening of the cystic duct to expel the stones is the best way to manage stones straddling between the cystic duct and the common bile duct under laparoscope, and also the most effective method to prevent postoperative residual stones.
2.THE STUDY ON THE EFFECTS OF HMBA ALONE OR COMBINED WITH HMBPA ON MELDS19 CELL GROWTH AND DIFFERENTIATION

Objective To investigate the effects of hexamethylene bisacetamide(HMBA),3-(2-hydroxy-3-methyl-butyrylamino-)-propionic acid(HMBPA) and HMBA plus HMBPA on MELDS19 cell growth and differentiation.Methods To make the MELDS19 cell growth curve,count the MELDS19 cell mitosis index,numbers of colony forming and benziding staining positivity percentage when MELDS19 cells exposed to HMBA,HMBPA and HMBA plus HMBPA.Results The numbers of the MELDS19 cells exposed to 2mmol/L HMBA was 5.0?10~5/ml,to 1?mol/L HMBPA was 6.0?10~5/ml and to 1?mol/L HMBPA+2mmol/L HMBA was 3.3?10~5/ml respectively,mitosis index and colong forming decreased.Benzidine staining positivity percentage exposed to 2mmol/L HMBA was 38%,to 1?mol/L HMBPA was 9% and to 1?mol/L HMBPA+2mmol/L HMBA was 64% respectively.Conclusion HMBA and HMBPA can inhibit MELDS19 cell proliferation and induce differentiation much effectively than HMBA or HMBPA used alone.
3.Clinical characteristics and prognosis of Omicron epidemic in Guang’an

Objective To analyze the clinical characteristics of the Guang’an Omicron epidemic and summarize the management experiences and practices in pandemic prevention and control of major infectious diseases. Methods Retrospective analysis was performed on patients infected with coronavirus disease (COVID-19), afterwards treated and observed in the isolation ward of Guang’an People’s Hospital and the shelter of Guang’an City from May 9 to June 26, 2022. The characteristics of patients at different age stages and the related factors affecting the severity, re-positive and negative conversion was analyzed. Results Finally 1 278 patients were collected, including 508 males and 770 females, with an average age of 41.3±22.6 years. Among them, 1 054 patients were asymptomatic carriers. The overall severe rate was 0.86%, the severe rate of the high-risk group was 3.06%. The median negative conversion time was 10.0 days and re-positive rate was 7.36%. Patients aged>60 years were 2.589 times more likely to have a longer negative conversion time than those aged≤60 years (95%CI 1.921-3.489, P<0.001). Conclusion The clinical characteristics of Guang’an COVID-19 epidemic are mainly that the elderly with high risk factors are more likely to develop severe cases, have longer clearance time, and re-positve is more likely to occur.
