1.Efficacy of fluconazole mouthwash by means of oxygen-driven atomizing inhalation for treating the pediatric oral fungal infections

Objective To explore the efficacy of fluconazole mouthwash by means of oxygen-driven atomizing inhalation for treating the pediatric oral fungal infections. Methods A total of 60 cases of child patient from the department of Pediatrics of Jiaozhou People's Hospital have been selected and divided into control group and test group with 30 cases for each group by the method of random number table. The sodium bicarbonate solutions as well as the fluconazole capsules were used for the two groups of children patients. Everyday there were five doses of sodium bicarbonate solution as follows:one after getting up in the morning, one after breakfast/lunch/dinner, and one before bedtime by gargling for three minutes followed by spitting. Daily dose of fluconazole capsules was 3 mg/kg, with 50 ml of NS added for preparing a solution of 1-3 mg/ml. Differences between the two groups are as follows. The control group used the traditional method of gargling. The prepared fluconazole solution was divided into five doses for gargling followed by swallowing which were carried out two hours after the sodium bicarbonate solution. The test group used the oxygen-driven atomizing inhalation therapy. The intervals with the sodium bicarbonate solution were two hours. Before the oxygen-driven atomizing inhalation therapy, the children patients had gargled with water to remove the food debris inside the mouth. Next, the daily dose was divided into five doses to be added into the nebulizers for carrying out the oxygen-driven atomizing inhalation therapy focusing on the exposed ulcer areas. The difference in healing time of oral fungal infection of different position and degree between both groups of child patient has been observed. Results The healing time of oral mucositis at different position of both groups has been compared with each other, with the exception of gingival position, the healing time of child patients in test group at the position of cheek, palate, tongue, pharynx, lips and so on was respectively (6.67±1.68) d, (6.38±1.80) d, (6.36±1.96) d, (6.50±1.60) d and (5.00±0.82) d, which were all better than that of the control group, which was (8.13 ± 2.13) d,(8.00 ± 2.14) d,(8.23 ± 2.13) d, (8.67 ± 1.75) d and (7.20 ± 1.48) d, the difference between both groups was statistically significant(t=2.14-2.64, P<0.05);the healing time of child patients in test group with a degree ranging from I to IV was respectively (5.10±0.88) d, (7.92±1.32) d,(8.00±1.00 )d and (10.25±0.96)d,which were all better than that of the control group, which was(6.36±0.81)d,(9.00±1.29) d,(10.33 ± 0.58)d and(12.33 ± 0.58)d,the difference between both groups was statistically significant(t=2.10-3.50,P<0.05). Conclusions It's suitable for oral care of children patients if we replace the fluconazole mouthwash with the oxygen-driven atomizing inhalation therapy which can reduce the severity of oral mucositis and shorten the healing time.
2.Crude polysaccharides extracted from cultivated Artemisia rupestris L. in Xinjiang enhances the effi-cacy of OVA

Objective To investigate the efficacy of using crude polysaccharides extracted from cultivated Artemisia rupestris L. ( CARCP) in Xinjiang as an immunologic adjuvant for ovalbumin ( OVA) . Methods The mice were subcutaneously immunized twice with OVA vaccine formulated with CARCP. ELISA assay was performed to measure the levels of IgG, IgG1 and IgG2a antibodies. Flow cytometry analy-sis was performed to detect the percentages of different T lymphocyte subsets ( CD3+CD4+ and CD3+CD8+T cells, CD4+CD44+ and CD8+CD44+T cells) in splenocytes as well as the expression of intracellular cyto-kines ( CD4+IFN-γ, CD8+IFN-γ) and CD4+CD25+Foxp3+Treg cells. Results The levels of IgG, IgG1 and IgG2a antibodies in serum, the percentages of CD3+CD4+, CD3+CD8+, CD4+CD44+ and CD8+CD44+T cells as well as the ratio of CD4+/CD8+T lymphocytes increased significantly in mice immunized with OVA in combination with CARCP (P<0. 05). Moreover, CARCP enhanced the expression of CD4+IFN-γand CD8+IFN-γ( P<0. 05 ) , but inhibited the expression of CD 4+CD 2 5+Foxp 3+Treg cells ( P<0. 05 ) . Conclusion As an adjuvant for OVA, CARCP enhanced the humoral and cellular immune responses, especially the T cell immune responses.
3.Clinical significance and expression of serum chemerin in the acute myocardial infarction patients

Objective To investigate the levels of serum chemerin in the acute myocardial infarction (AMI) patients and explore the clinical significance.Methods A total of 76 AMI patients from January 2013 to December 2013 were inrolled and 30 healthy patients were included as normal control(NC) group.Chemerin, tumor necrosis factor α(TNF-α), adiponectin were asessed by enzyme linked immunosorbent assay(ELlSA).The clinical data of high sensitivity C reactive protein (hs-CRP), body mass index (BMI), blood lipids, underlying diseases et al were collected.Results Serum levels of chemerin in AMI patients was higher than the NC group significantly((12.97±4.17) μg/L vs.(30.96± 17.16) μg/L, t =5.642, P<0.001).The Serum levels of Chemerin in single vessel disease group, two vessel disease group and triple vessel disease group were (22.25 ± ±6.93) μg/L, (28.57 ± 15.92) μg/L, (37.95 ± 16.52) μg/L respectively and was linearly increasing relationship with increasing AMI severity (P<0.001, F =22.84, r2 =0.397).Serum levels of Chemerin positively correlated with TNF-α, hs-CRP level(r2=0.347,0.455 ,P<0.001) and negative correlated with adiponectin(r2 =0.396,P<0.001).Logistic regression analysis showed that chemerin was an independent risk factor for AMI (OR =4.822,95% CI 4.422-7.141, P =0.032).Conclusion The results showed that serum chemerin levels were significantly elevated in AMI patients,and chemerin may be involved in the development process of AMI.
4.The clinical and pathological study in moulding operation about breast cancer

Purpose:We shall provide the clinical and pathological bases in breast cancer operation in which the nipple and ring reserved.Methods:We combine clinical and pathological material , analyses the result of the nipple and ring that cut off wholly and the sample cut down in slice continuously. We analysis the DNA polyploid of carcinoma tissue in FCM technology . Sinonimously , we measure the PCNA expression in immunohistochemistory. We reserve the breast of patients that suffer from breast cancer and further discuss the possibility and safely .The study will provide practice evidence for breast cancer patients who would perform operation. Results:We measure the linear distance from edge of tumor to ring that are 19.6mm. and The patient suffer from breast cacinoma in stage and tumor bed under nipple and ring could not perform operation that reserved nipple and ring.Conclusions:If we stringent control the standard in reservation of breast, The patients suffering from breast cancer will be safely more and more.
5.The influence of salmeterol with fluticasone on airway inflammation of patients with acute exacerbation chronic obstructive pulmonary disease

Objective To investigate the influence of salmeterol with fluticasone on airway inflammation of patients with acute exacerbation chronic obstructive pulmonary disease (COPD).Methods 40 COPD patients in the stage of acute exacerbation were randomly divided into salmeterol with fluticasone group( A group) and controll group (B group).Bronchial alveolar lavage( BAL) was performed as usual.The concentrations of IL-8 and TNF-α in bronchial alveolar lavage fluid(BALF) were measured by ELISA.The results were compared with that of 18 healthy volunteers.Results The levels of IL-8 and TNF-a in BALF of patients in A group(10.60 ± 1.42) μg/L, (14.80 ± 2.05) μg/Land B group( 10.77 ± 1.98) μg/L, (14.70 ± 2.03) μg/L were significantly higher than that of C group (3.40 ±0.65)μg/L, (4.67 ± 1.01) μg/L( all P <0.01) ;Before treatment,the levels of IL-8 and INF-α in BALF of A group( 10.60 ± 1.42)μg/L,(14.80 ±2.05) μg/L and B group( 10.77 ± 1.98) μg/L,(14.70 ±2.03) μg/L had no significant differences (all P > 0.05) ,and the concentrations of IL-8 and TNF-a in BALF in group A (4.39 ± 0.92)μg/L,(5.84 ±1.26) μg/L were significantly lower than that in group B(9.69 ± 1.43) μg/L, (12.88 ± 2.51) μg/L after two weeks treatment ( all P < 0.01).Conclusion Salmeterol with fluticasone could inhibit airway inflammation of COPD patients in the stage of acute exacerbation.
6.Application of FIESTA to Abdominal Scanning of Patient Undergoing Uneffective Respiratory Triggering

Objective To investigate the value of FIESTA sequence for the abdominal scanning of the patient undergoing uneffective respiratory triggering. Methods 40 cases undergoing uneffective respiratory triggering were adopted as the subjects. GE SIGNA 1.5T and fat-suppressed technique got involved in. Two chief physicians and two associate chief technologists compared the image quality and the lesion detective rate. Results FIESTA sequence was better than FSE T2WI in the aspect of image quality, but worse in lesion detective rate. Conclusion FIESTA sequence is clinically valuable for the abdominal scanning of patient undergoing unffective respiratory triggering.
7.Pharmacokinetics of cisplatin during pelvic isolated perfusion with the balloon occlusion technique in dogs

Objective To demonstrate the pharmacokinetic characters of cisplatin in isolated prefusion of dog pelvic.Methods Pelvic isolated perfusion with the balloon occlusion was performed in 11 dogs.Blood samples of perfusion and non-perfusion regions were assayed for platinum using atomic absorption spectrophotometer.Pharmacokinetic characters of cisplatin were analized. Results C_(pel max) 30.3 ?g/ml,and C_(sys max) 4.8 ?g/ml,and AUC_(pel 0~25min) 330 ?g/ml?min were observed in perfusion area.AUC_(sys 0~25min) in non-perfusion area was 56 ?g/ml?min. Conclusions Balloon catheter-mediated pelvic I-P has favorable pharmacokinetic characters for chemotherapeutics and therefore should be an alternative method to treat pelvic neoplasma.
8.Influence of arterial data on Revolution CT perfusion parameters of renal cell carcinoma

Objective To explore the effect of whole renal perfusion imaging removal of arterial phase data on perfusion parameters of renal clear cell carcinoma using Revolution CT.Methods Perfusion imaging with Revolution CT was retrospectively analyzed in 10 patients with pathologically proven clear cell renal cell carcinoma.The z-direction coverage model was used in perfusion imaging,and the images were analyzed with CT Perfusion 4D software.All images were analyzed twice.All 23 phases data was included in group A and only 16 phases except arterial data (9-15 phases) were selected in group B.The abdominal aorta on the level of right hilus was chosen to be the input artery,and the perfusion parameter maps were obtained,including blood flow (BF),blood volume (BV),mean transit time (MTT),permeability of surface (PS).Every perfusion parameters of lesions and contralateral normal cortex,lesions and normal cortex in both groups were compared.Results The BF and PS of lesions were lower than those of normal cortex in both groups (both P<0.05).There was no statistical difference in BV and MTT between lesions and normal cortex in both groups (all P>0.05).There was no statistical difference in all perfusion parameters of renal clear cell carcinoma between group A and group B (all P>0.05).The difference of BF in normal cortex between the two groups was statistically significant (P 0.009),and the difference of the PS,BV,MTT had no statistically significant (all P>0.05).Conclusion When the duration time of renal CTP is 600 s,there is no difference between including and excluding arterial phase in all perfusion parameters of renal clear cell carcinoma.
9.Efficacy of using Xinjiang wild Artemisia rupestris L.crude polysaccharides as an immunologic adju-vant for influenza virus vaccine

Objective To investigate the efficacy of using Xinjiang wild Artemisia rupestris L. crude polysaccharides ( WARCP) as an immunologic adjuvant for influenza virus vaccine( IVV) .Methods ICR mice were subcutaneously immunized with 0.3 μg of IVV and 1.5 μg of IVV alone or co-administered with 200 μg of WARCP on 0 d and 14 d.Antibody levels in serum samples were detected by using indirect ELISA.MTT method was used to measure the proliferation of splenocytes.The growth conditions of mice were observed as well.Results No significant differences in the body weight were observed between mice from different groups (P>0.05).The levels of influenza virus-specific IgG, IgG1 and IgG2a were signifi-cantly increased in mice injected with WARCP adjuvant (P<0.05).The levels of IgG antibody in mice im-munized with low-dose of IVV and WARCP were significantly higher than those in mice immunized with high-dose of IVV alone (P<0.05), indicating at least 80% reduction in vaccine dosage by adding WARCP as adjuvant.Moreover, WARCP significantly promoted the proliferation of lymphocytes (P<0.05).Conclu-sion Adding WARCP to IVV enhanced the efficacy of IVV by boosting humoral and cellular immunity re-sponses with the advantages of high safety and dose-sparing.This study suggested the possibility of using WARCP as a novel immunologic adjuvant for influenza virus vaccine.
10.A review of competencies of community mental health service providers of different professional backgrounds

To promote the development of the domestic community mental health services, this paper introduced several foreign studies on the competencies of community mental health service providers of different professional backgrounds. The studies examined by this article inquire into the core competencies of community mental health service providers working with people of psychiatric disabilities, the core competencies of psychiatrists providing integrated care in the community setting, the competencies of psychologists in community mental health service, and the competency level of community mental health service providers in intervention activities. Drawing on foreign experience and strengthening cooperation among community mental health service providers of different professional backgrounds, and achieving complementary advantages, will help to promote service effectiveness.
