1.The Current Situation of Hospice Care and Influencing Factors in 235 Cases

Objective:To survey the level of hospice care and the influencing factors in Shaanxi province. Methods:Interview and return visit method was used to investigate. Results:In total 235 patients, 37. 02% patients received hos-pice care. There is not significant difference between hospice users and no hospice users in sex and geographic differ-ences. There was significant difference between hospice users and no hospice users in personal ages, and in gap between urban and rural areas. and in medical payment, and in basic lesions. Malignant tumor patients receive hospice care most-ly. No patient received hospice care patients with acute trauma. The effect of ages on hospice is maximum ( OR =2. 877). Basic lesions is an important cause of influence whether patients receiving hospice care services(OR=1. 569). Conclusion:The level of hospice care is low in Shaanxi. The age and basic lesions is influencing factors of hospice care. The geographic differences has no effect on hospice care.
2.AN ANALYSIS OF THE EFFECT OF TRANSSPHENOIDAL PITUITARY MICROSURGERY FOR CUSHING DISEASE

From the beginning of 1980 to the end of 1992, 125 patients with Cushing disease were undergone transsphenoidal microsurgery. 41 patients were operated by selective adenoma resection and 62 patients by radical surgery. 103 cases were available to be analyzed. The results of a long-term follow-up showed: (1) The cure rates were 63. 1% and 48.1% with positive and negative pathological study, respectively. (2) The cure rates were 72.6% and 41. 5% with radical and selective surgery respectively. In cured patients the plasma cortisol level dropped to normal or subnormal within 48 hours after the surgery. However, in uncured patients, it was still above the normal. After operation, 1. 6% of cases (1/60) had secondary hypothyroidism and 62.5 % of cases (30/48) who had abnormal menstruation preoperatively restored normal menstrual cycle. The recurrent rate was 11.4%. Conclusion: (1) The outcome of radical surgery is better than the selective adenoma resection; (2) The pathological study and the analysis of the plasma cortisol level within 48 hours after the surgery may have predictive value for efficacy of the surgery.

4.Self-choice behavior under stress

5.Clinical study of different forms of hystero scopic surgery for endometrial polyps

Objective To investigate the curative effects of different types of hysteroscopic surgery for endometrial polyps. Methods A total of 327 cases by different ways of hysteroscopic surgery for endometrial polyps from Nov 1999 to Nov 2004 were followed up. The mean age was (40?6) yea rs. The mean follow-up was( 3.0?0.6) years. Among 228 polyps patients in s exual maturity without desire of maintaining fertility, 53 (group A) underwent p olypectomy with electrosurgical vaporization, and 175 cases (group B) did polype ctomy with endometrial resection. Fifty-four (group C) cases (19 cases of infe rtility), who desired future childbearing, did polypectomy with endometrial rese ction of superficial layer near the polyps. Forty-five postmenopausal patients(group D) did polypectomy with endometrial coagulation. Results The time of operation: group A (15.1?0.8) second,group B (19.7?0.7) s econd,group C (20.9?0.7) second,and group D (22.1?0.8) second. None of polyps recurred for the patients of groups A and D after operation, and the recu rrent rate of groups B and C was 1.7% and 7.4%. There were no cases with amen orrhea in group C, who hoped to keep the function of fertility, but the recurren t rate of polyps was higher than other three groups. Of 19 cases of infertility , 14 cases became pregnant after the surgery. Conclusion It is feasible to select different hysteroscop ic surgery for endometrial polyps,according to different ages and the desire of childbearing of the patients.
6.Consideration on the Selection and Revision of National Essential Drug List

OBJECTIVE:To put forward policy suggestions on the selection and revision of National Essential Drug List (NEDL) in China. METHODS: The selection principles, criteria and procedures of WHO Model List of Essential Medicines and Australia’s Pharmaceutical Benefits Scheme Directory which was commonly regarded as Essential Drug List of Australia were analyzed to provide reference for the selection and revision of NEDL in China. RESULTS & CONCLUSION: The selection and revision of NEDL which were strongly supported by related policies demand active participation of all social sectors, open and transparent working procedures, scientific and operational methods for selection and revision, authoritative monitoring and aftereffect evaluation system.
7.Pathological morphological characteristics of experimental rats with acute light and heavy pancreatitis

Objective The aim of this study is to demonstrate the pathological characteristics about two types of pancreati‐tis ,providing new thinking about the mechanism of severe acute pancreatitis .Methods Thirty male Sprague‐Dawley rats were ran‐domly divided into three equal groups :sham‐operated (SO ,n=10) group ,mild acute pancreatitis (MAP ,n=10)) group and severe acute pancreatitis (SAP ,n=10) group ,all the rats were killed after 12 h of building model .Under the microscope ,we detected the pathological changes of pancreas ,liver ,lung and small intestine .The ultrastructure of pancreas ,liver ,lung and small intestine tissue were observed by transmission electron microscope .Results In SAP group ,congestion ,edema ,inflammatory cell infiltration ,lea‐king of blood componedts ,vascular endothelial injury and thrombosis of microcirculation were obviously observed .There is no ap‐parent pathological changes in the MAP group except the edema of pancreas .Conclusion Hemorrhage and necrosis are the main pathological characteristics in SAP rats ,has essential difference with MAP .These pathological characteristics provides us a new thinking for further study about the mechanism of SAP .
8.Physiological and biochemical reaction of anxiety among individuals with different special characteristics under military stress

Objective To understand the difference between psychological characteristics and stress response as a result of military stress and to evaluate the rate of stress response among individuals with different psychological characteristics.Methods The State-Trait Anxiety Inventory,blood pressure,breath,pulse wave,and serum levels of lactate acid and cortisol were measured at 8 to 9 am for 372 servicemen following 15 days'training.ResultsIn 97 subjects ,the value of special characteristic anxiety Was 2-standard deviation higher than normal model(i.e.high special characteristic anxiety).and low special characteristic anxiety was found in 79.The condition anxiety of 2 standard deviation higher than normal model wag found in 172 participants.A total of 132 subjects showed significant changes in condition anxiety,serum cortisol and lactic acid,and systolic blood pressure(P<0.01).Sixty-one or 71 subjects with high or low special characteristic anxiety experienced condition anxiety,changed serum cortisol and lactic acid,and systolic blood pressure,respectively.Conclusion Psychological characteristics may be considered as a predicting factor of stress response.
9.Involvement of pharmacists and dietitians in contract service of family doctor

Community health service centers provide basic medical and public health service for the residents in community with the contract service mode.In the last five years our center started the construction of primary care team.The family physicians,nurses and public health workers formed the basis of health care team,and pharmacists and nutritionists actively joined the team work.The pharmacists provided health education of rational medication,participated in the ward-rounds of home beds and comprehensive management of chronic disease patients,and also conducted counseling clinic of drug use.The clinical nutritionists provided special nutrition guidance for patients,delivered nutrition education and conducted nutrition clinic.The involvement of pharmacists and nutritionist in contract service has greatly ungraded the primary care team,improved the quality of comprehensive care and provided better service for the people in the community.
10.Determination of four alkaloids in Corydalis decumbens by HPLC.

OBJECTIVETo establish a quantitative HPLC method for determination of protopine, palmatine hydrochloride, bicuculline and tetrahydropalmatine, in Corydalis decumbens.
METHODThe separation was performed on a ZORBAX Eclipse XDB-C18 column (4.6 mm x 150 mm, 5 microm) at a flow rate of 1.0 mL x min(-1) using mixtures of two solvents [A(20 mmol x L(-1) ammonium acetate)-B(acetonitrile)]: with a gradient elution. The column oven temperature was 30 degrees C and the detection wavelength was set at 280 nm.
RESULTThe 4 alkaloids were well separated by this HPLC method. Linearifies of protopine, palmatine hydrochloride, bicuculline and tetrahydropalmatine were good in the ranges of 1.44-46.0 (r = 0.999 4), 1.2640.2 (r = 0.999 8), 1.37-44.0 (r = 0.999 9), and 1.3643.6 mg x L(-1) (r = 0.999 9), respectively. The average recoveries were 98.2% with RSD 2.7% for protopine, 101.9% with RSD 2.5% for palmatine hydrochloride, 102.8% with RSD 3.5% for tetrahydropalmatine, and 98.8% with RSD 3.1% for tetrahydropalmatine.
CONCLUSIONThis method is proved to be convenient, reliable and accurate., and it can be used for quality control of C. decumbens.
