1.Clinical observation of different concentrations of ropivacayin combined with dezocine for Epidural analgesia

Objective To investigate the analgesic effect and motor block of ropivacayin combined dezocine in lower abodiminal operation.Methods Sixty patients of ASA Ⅰ - Ⅱ were divided into three groups randomly(n =20 for each group).They were treated with dezocine 5 mg +0.125% ropivacayin/100 ml( group DI ),dezocine 5 mg +0.175% ropivacayin/100 ml( group D2),dezocine 5 mg +0.225% ropivacayin/100 ml (group D3 ).The initial dose was 5 ml,and the maintenance dose was 2 ml/h,with lock-out time of 20 mins,and the duration of patient-controlled epidermal analgesia (PCEA) was 48 h.The analgesic effect was evaluted by VAS score at 2,4,12,24 and 48 h after surgery.Bromage score and the side effects such as motor ability,and vomiting after surgery were recorded.Results At 2,4,12 h after surgery,the VAS scores of group D2 and D3 were less than that of group D1 (2.54 ± 1.34,2.47 ± 1.05 and 3.62 ± 1.02 at 2 hrs,2.13 ± 1.30,2.15 ± 1.28 and 3.56 ± 1.10 at 4 hrs,1.03 ± 0.43,1.04 ± 0.51 and 1.92 ± 1.20 at 12 hrs,respectively) ( Ps < 0.05 ).As for the Bromage score among the 3 groups,there were no sighificant diffrences between group D1 and D2,wheras they were significantly lower than that in group D3 ( 1.21 ± 0.21,1.28 ±0.22 and 1.53 ± 0.31 at 2 hrs,1.15 ± 0.15,1.44 ± 0.25 and 1.61 ± 0.12 at 4 hrs,0.92 ± 0.14,0.99 ± 0.13 and 1.71 ± 0.22 at 12 hrs,0.85 ± 0.11,0.88±0.14 and (1.66±0.15) at 24 hrs,0.42 ± 0.10,0.55 ± 0.09 and 1.19±0.11 at 48 hrs,respectively,Ps <0.05).There were 2,2 and 3 patients affected with nausea and vomiting among the 3 groups,and there were no other side effects ( P > 0.05 ).Conclusion 0.175% ropivacayin combined with dezocine 5 mg via PCEA in lower abodiminal operation shows better analgesic effect and benefical to the restoration of lower limbs movements.

5.Preparation and quality standard of toothache drop pills

Objective To prepare toothache drop pills and establish its quality standard.Methods The drop pills were prepared by a routine method; HPLC was used for the determination of the EU isoimperatorin content.Results The drop pills were well- distributed in size, smooth and glossy in appearance, mild in hardness; TLC can identify isoimperatorin characteristic spots; Determination of the Indigo 0.50~520μg/ml linear relationship was good,r=0.999 8, the recovery of 98.38%.RSD values were 1.05%.Conclusion The preparation method of toothache drop pills is simple, and the drop pills are well-shaped with controllable quality.
6.Characteristics and Drug Resistance of Staphylococcus spp Nosocomial Infection in Suzhou Area 2004-2007

OBJECTIVE To investigate the infection and drug resistance of Staphylococcus spp from hospitalized cases in Suzhou area.METHODS The data from hospitalized cases of 32 hospitals in Suzhou(from 2004 to 2007) were analyzed retrospectively.RESULTS From 2004 to 2007,17 668 cases of nosocomial infection were collected from 32 hospitals in Suzhou area.The infection rate of Staphylococcus aureus was 5.78%,7.11%,8.39% and 7.50%,respectively;the number of meticillin-resistant S.aureus(MRSA) infection cases was 66(34.74%),107(33.86%),138(37.00%) and 219(53.16%) respectively and the total number was 530(41.05%).The nosocomial infection caused by S.epidermidis accounted for 5.99%,5.47%,5.35% and 5.25%,respectively from 2004 to 2007.The number of meticillin-resistant S.epidermidis(MRSE) infection cases were 118(59.90%),128(52.67%),119(50.00%) and 134(46.53%) and the total number was 499(51.66%).The main infection site of S.aureus and S.epidermidis was respiratory tract(74.28% and 71.81%,respectively).Antibiotic resistance strains of S.aureus and S.epidermidis was on rising,and most of them were multi-drug resistance strains.All of the strains were sensitive to vancomycin.CONCLUSIONS In Suzhou area,nosocomial infection and drug resistance of Staphylococcus is on the rise.Evevy hospital must take effective measures to control nosocomial infections of Staphylococcus and drug resistance.
7.Influence of resistive respiratory training for patients with chronic obstructive pulmonary disease on rehabili-tation effect

Objective To explore the influence of resistive respiratory training for patients with chronic obstructive pulmonary disease(COPD) on rehabilitation effect.Methods According to the digital table,68 COPD patients were randomly divided into the observation group and control group,34 cases each group.The observation group were taken respiratory muscle training under the supervision of rehabilitation nurses for 8 weeks,and the respir-atory muscle training including shrinkage lip diaphragm breathing training and abdominal muscle resistance training. Before and after the abdominal muscle resistance training and training were given 1 h double nasal oxygen tube contin-uous low flow oxygen inhalation.The control group were taken pursed lips abdominal breathing training.In the two groups before treatment and 8 weeks after, the blood gas analysis ( PaO2 , PaCO2 ) , pulmonary function test ( FEV1 , FEV1/FVC%) and respiratory muscle function( MIP,MEP) were detected.Results After treatment,blood gas anal-ysis in the two groups comparison(tPaO2 =2.95,tPaCO2 =2.07),pulmonary function(tFEV1 =2.38,tFEV1/FVC%=2.04) and respiratory muscle physiology index(tMIP =6.40,tMEP =4.04),the differences were statistically (all P<0.05).In the observed group before and after treatment,blood gas analysis(tPaO2 =19.23,tPaCO2 =3.05),pulmonary function(tFEV1 =2.67,tFEV1/FVC%=4.19)and respiratory muscle physiology index(tMIP=9.09,tMEP=9.67) were compared,the differences were statistically(all P<0.01).Conclusion Respiratory mus-cle resistance training can improve COPD patients with respiratory muscle strength and endurance,and it has the cura-tive effect to improve pulmonary function.
8.Building and put the emergency nursing program for burst group accidents into practice

Objective To find out the weakness in the emergency nursing for burst group accidents, and then building a normative nursing program which have included framework and 4 stages. Methods Using the method of Fish-Bone Drawing to analyzed the nursing courses in 22 burst accidents within the past 4 years retrospectively. Results After using the normative nursing program, nurses have known their own working targets and responsibility, which can apply the nursing care effectively and orderly. Conclusion The application of emergency nursing program is a kind of quick and proper nursing method, which can improve the patients' prognosis effectively.
9.Effects of simultaneous bilateral central venous intubation on chest drainage for bilateral malignant pleural effusion induced by primary non-small lung cancer

Objective To observe the effects of simultaneous bilateral central venous intubation on chest drainage for bilateral malignant pleural effusion induced by primary non-small lung cancer and summarize the nursing measures. Methods Totally 65 patients with bilateral malignant pleural effusion were randomized into experiment group and control group. In the former group, chest drainage was done using bilateral central venous intubation and in the latter, unilateral chest drainage was used. The two groups were compared in terms of effectiveness rate, time for intubation, remission duration and hospitalization. Results The time for remission duration, hospital stay and time for intubation were all significantly shorter than those of the control group (all P<0.05). There were no significances in curative effect and adverse reactions between the two groups. Conclusion Chest drainage using simultaneous bilateral intubation for bilateral malignant pleural effusion induced by primary non-small lung cancer can be effective in shortening the time for remission, intubation and hospitalization. But it causes no more adverse reaction.
10.The Analysis of the Cost and Clinical Efficacy of Antimicrobials in 160 Patients Undergoing Orthopaedic Operations

OBJECTIVE:To evaluate the rational use of antimicrobials in 160 cases undergoing orthopaedic operations.MET_HODS:To analyse and evaluate the use of antimicrobials in 160 cases undergoing orthopaedic operations in 1999 in respect to the cost of antimicrobials related cost ratio,usage,clinical efficacy,and adverse drug reactions.RESULTS:The cost of antimicrobials per person()was 1 676.55 yuans and average day of treatment()was (7.3?3.4)d.151 patients healed by first intension.9 patients recovered in delayed healing.The ratio of the total cost,cost of drugs and cost of antimicrobials was 1∶0.38∶0.14.CONCLUSION:The use of the antimicrobials in 160 cases undergoing orthopaedic operations was reasonable.
